1.Mechanism of Gemcitabine Resistance in Pancreatic Cancer Chemotherapy

Objective To investigate the possible mechanism of gemcitabine resistance in pancreatic cancer chemotherapy. Methods Recent literatures about the genes and signal pathways those play key roles in mediating gemcitabine chemotherapy resistance of pancreatic cancer were collected and reviewed.Results Oncogenes like c-Src and bcl-X-L, inflammation pathway of NF-?B, cytokines like IL-1? and NO are closely related with the chemoresistance; the relationship between multiple drug resistance relevant genes like MDR1/P-gP and the resistance to gemcitabine remains to be clarified. Conclusion Genes and pathways like c-Src, bcl-X-L, NF-?B, etc. might become new targets to increase the chemotherapeutic sensitivity of pancreatic cancer, however, the mechanism of pancreatic cancer chemotherapy resistance still needs further to be studied.
2.Color Doppler sonographic evaluation in 32 patients with venous leakage in venous incompetence in erectile dysfunction

Objective To investigate the hemodynamic change of venous leakage in venogenic impotence. Methods Thirty-two patients with vasculogenic impotence were evaluated with conventional penile duplex sonography with spectral analysis and color Doppler imaging after intracavernosal injection of PGE1 to induce an erection.The color Doppler appearance of deep dorsal vein of penis,cavernous veins and bulbourethral vein wen observed and the correlativity with resistance index(RI)of cavernous artery were analyzed. Results After five minutes following intracavernous injection,the flow and diameter of the penis vein were continuous increased.The coefficient correlation r between the discharge of deep dorsal vein of penis,cavernous veins and bulbourethral vein with RI of cavernous artery were-0.55,-0.53,-0.24(P<0.05).Considering the existence of mixed venous leakage,r between the discharge of vein of penis with RI of cavernous artery was-0.88(P<0.001).Conclusions Higher qualitative ultrasound imaging could demonstrate venous leakage sensitively and assess the location and degree of venous leakage in venogenic impotence initially.

Objective To measure the pressure-volume changes and uroflow rate,and assess the function of upper urinary tract in sigmoidorcctal pouch patients.Methods Between 2007 and 2011,a to-tal of 10 patients who had a radical cystectomy because of muscle-invasive bladder tumor underwent a Mainz pouch Ⅱ procedure were evaluated.The ureters were implanted into the post walls of the detubularized sigmoid segment at least 30 cm in length.In all cases,sigmoidoscopy was done and the anal pressure was measured preoperatively,and video urodynamic study of sigmoidorectal pouch as well as the intravenous pyelography was conducted after 3 to 6 months postoperatively.Results Video urodynamic study of sigmoidorectal pouch revealed that no reflux up to an average volume of 360 ml (270-532 ml) of the descending colon.The sigmoidal colon pressure was 26 cm H2O during the 3 to 6 months post-operative follow-up.The anal sphincter pressure was 90 cm H2O on average preoperatively and did not change postoperatively.The renal function and upper urinary tract were preserved well.The Qmax was 30 ml/s and the average uroflow rate was 8 ml/s under abdominal strain.All the patients suffered slight incontinence in the first two months and became continent since the 3 month postoperatively with nocturnal voiding one to four times.Conclusions The sigmoidorectal pouch provides a reservoir with a higher capacity,lower pressure without reflux to the upper urinary tract and descending colon and lower metabolic acidosis problem.It is also a good alternative diversion procedure that would preserve upper urinary tract and good quality of life.

6.Comparative study of the parameters related to type I thyroplasty measured by laryngeal specimens and computed tomography.

OBJECTIVETo determine the consistence of the parameters related to type I thyroplasty measured by laryngeal specimens and CT scan.
METHODSThe related parameters of 50 laryngeal specimens (unilateral) obtained following total laryngectomy were measured postoperative immediately, and compared with those measured by spiral CT scan with multiple plain reconstructive (MPR) technique preoperatively. Comparative results were analyzed to evaluate the statistical significance between these two methods.
RESULTSThere were no significant statistical differences among the 6 parameters between two methods (P > 0.05), and the results (x +/- s) measured by CT scan and laryngeal specimens showed that the length of the thyroid notch to the inferior thyroid border were (20.7 +/- 1.7) mm and (20.6 +/- 1.7) mm; the length of the vocal cord were (17.3 +/- 1.8) mm and (17.3 +/- 1.8) mm; the length of the oblique line were (28.6 +/- 3.2) mm and (29.1 +/- 2.7) mm; the length of the presumptive horizontal line were (26.2 +/- 2.0) mm and (26.2 +/- 2.0) mm; the endolaryngeal vertical length of the anterior of the vocal cord to the presumptive horizontal line were (4.5 +/- 0.6) mm and (4.5 +/- 0.7) mm; the endolaryngeal vertical length of the vocal process to the presumptive horizontal line were (10.8 +/- 1.1) mm and (10.9 +/- 1.1) mm, respectively. As a result, the endolaryngeal anterior and posterior width of the wedge inserted in the thyroid cartilage were 4 - 5 mm and 8 - 9 mm respectively.
CONCLUSIONSMPR technique of spiral CT scan is able to design the size of the window and the prosthesis of type I thyroplasty preoperatively, which was testified to be a precise and reliable method to measure the larynx.

7.Benign infantile convulsions associated with mild gastroenteritis: a clinical analysis and follow-up study.

OBJECTIVETo study the clinical features and prognosis of benign infantile convulsions associated with mild gastroenteritis (BICE).
METHODSA retrospective analysis was performed for the clinical data of 436 children with BICE, and among these children, 206 were followed up for 1.5 to 7 years. Some parents were invited to complete the Weiss Functional Defect Scale to evaluate the long-term social function.
RESULTSThe peak age of onset of BICE was 13-24 months, and BICE had a higher prevalence rate in September to February of the following year. Convulsions mainly manifested as generalized tonic-clonic seizures, which often occurred within 24 hours after disease onset and lasted for less than 5 minutes each time. Sometimes they occurred in clusters. During the follow-up of 206 children, only one had epileptiform discharge, and the other children had normal electroencephalographic results. The parents of all the 206 children thought their children had normal intelligence and had no marked changes in character. Based on the Weiss Functional Defect Scale completed by the parents of some BICE children, there was no significant difference in the long-term social function between BICE children and healthy children matched by age and sex.
CONCLUSIONSBICE mainly occurs in children aged 1-2 years, with the manifestation of transient generalized seizures in most children and cluster seizures in some children. BICE seldom progresses to epilepsy and has good prognosis.

9.Analysis of application effect of cardiac remote real -time monitoring system

Objective:To explore clinical application effect of cardiac remote real -time monitoring system.Methods:Cardiac remote real -time monitoring warning instrument(iHolter)was applied in 210 patients.The iHolter gave a-lerts automatically in case of ECG abnormalities for corresponding treatment in time,and it offered corresponding reports on monitoring results.According to age,patients were divided into youth group(<45 years,n=59),mid-dle-aged group(45160 years,n=56)and aged group(≥60 years,n=95).Characteristics of arrhythmias among different age and sex were analyzed.Results:ECG abnormalities were found in 191 cases(90.95%)out of 210 pa-tients,including eight cases with malignant arrhythmias,in which five cases were treated and survived after timely intervention via alerts.Compared with youth group and middle -aged group,there were significant rise in percenta-ges of paroxysmal atrial tachycardia(16.9%,26.8% vs.54.7%)and atrial fibrillation(0%,0% vs.8.4%)in aged group,P< 0.05 or <0.01;abnormal percentages of heart rate variability of middle -aged group and aged group were significantly higher than that of youth group(25.0%,36.8% vs.5.1%,P<0.01 both).Male percent-age of paroxysmal ventricular tachycardia was significantly higher than that of female(81.3% vs.18.8%,P=0.013).Conclusion:Cardiac remote real-time monitoring system can offer timely alerts for cardiovascular events such as arrhythmia and myocardial ischemia etc.,fight for more time for rescuing patients,maximally protect cardi-ac function and improve accuracy of clinical diagnosis.
