2.Observation of two types of foldable scleral - fixated posterior chamber intraocular lens to treat cataract dislocation

AlM:To observe the clinical efficacy of two types of scleral-fixated posterior chamber intraocular lens ( lOL ) combined intra-capsule lens extraction to treat cataract dislocation.
METHODS: After intra - capsule lens extraction of dislocated cataract, two types of lOL were used as scleral-fixated posterior chamber lOL . First group was with four seal loops, the second group was with two open loops. All 21 patients (23 eyes) took the examination of the best corrected visual acuity, intraocular pressure, fundus, and lOL decentration using camera system attached to slit - lamp ophthalmoscope and lOL tilted using ultrasound biomicroscopy (UBM) at 6mo after surgery.RESULTS: After 6mo, all patients had improved eye sight. There was significant difference in the mean lOL decentration between the two groups ( 0. 57mm vs 0. 79mm, P<0. 05). There was significant difference in the mean lOL tilted degree between the two groups (6° vs 11°, P<0. 05) .
CONCLUSlON: lOL with four loops are more appropriate as scleral-fixated posterior chamber lOL with less tiltness and dicentration.
3.Clinical observation of different surgical methods on dry eyes in patients with pterygium excision combined transplantation

?AlM: To observe the situations of different surgical methods on dry eyes in patients with pterygium excision combined transplantation.
?METHODS: Seventy-eight cases ofpterygium patients (81 eyes ) were randomly divided into three groups. Group A underwent pterygium excision combined large autologous conjunctival flap transplantation; group B underwent pterygium excision combined with small conjunctival flap;group C underwent pterygium excision combined with small conjunctival flap with autologous limbal stem cell. Repair of postoperative corneal epithelium, 1, 3mo preoperation and postoperation tear film break up time ( BUT ) and questionnaire of ocular surface disease index ( OSDl ) were observed among three groups, which caused the situation of dry eyes by pterygium and pterygium excision were evaluated.
?RESULTS: BUT: Group A was shorter than that in groups B and C at 15d postoperation (P<0. 05); at 1mo postoperation, group A was no statistical difference with groups B and C (P>0. 05). Postoperative dry eye ratio of group A was higher than that in groups B and C at 1mo postoperation ( P < 0. 05 ). There was no statistical difference between group B and group C ( P>0. 05 ) , but group C showed lower postoperative dry eye ratio. Corneal epithelium recover time of group A was longer than that in groups B and C (P<0. 05). There was no statistical difference between group B and group C ( P>0. 05), but group C showed a tendency to be shorter recover time.
? CONCLUSlON: Pterygium excision combined with small conjunctival flap and autologous limbal stem cell shows guickly corneal epithelium recover and low dry eye ratio and deserve to recommended.
4.Method of bubble under water capsulorhexis in small incision used in mature cataract operation

AIM: To observe application of underwater bubble method capsulorhexis overmature period to improve the small incision cataract surgery, so as to explore the clinical value of the surgical method.
● METHODS: From Jul. 2012 to Mar. 2016 at the grassroots of blindness 58 people fail in the 66 eyes overmature period of cataract were randomly divided into underwent capsulorhexis by underwater bubble method to improve the small incision cataract surgery group ( 36 eyes of 30 cases ) and conventional viscoelastic agent underwent capsulorhexis small incision cataract surgery group (30 eyes of 28 cases).
● RESULTS: A total of 66 eyes in success rate of continuous circular capsulorhexis: 92% ( 33/36 eyes ) of underwater bubble method, method of viscoelastic agent only 40% ( 12/30 eyes ) . Two groups of cases of postoperative corneal endothelial cell density are compared with preoperative significantly reduced, no significant statistical difference between the two groups(P>0. 05).
● CONCLUSION: Underwater bubble method capsulorhexis difficult to overmature period of cataract surgery capsulorhexis solution is a better way.
5.Optimization of formulas of Quban gel by uniform design.

OBJECTIVETo optimize formulas of Quban gel.
METHODThe U6 (6(2) x 3) uniform design was adopted to optimize gel formulas, with rheological parameters, such as viscosity and yield value in room temperature, viscosity and yield value in average temperature of skin, thixlotropy.
RESULTThe optimum proportion of matrix was made of 1.0 g carbomer 940, 5 mL glycerin and pH value 5-6.
CONCLUSIONThe regression model for gel matrix quality and gel rheological parameters was established to directly reflect the impacting effect of various factors, and provide certain preference basis for the screening of gel matrix formulas. Quban gel prepared by the method was evenly distributed, moderately viscous and highly thixotropic

6.Efficacy observation on multiple wave length laser for diabetic retinopathy and central retinal vein occlusion

AlM:To observe the efficacy of the multiple wave length laser in treating diabetic retinopathy combined with central retinal vein occlusion.
METHODS:Totally 95 cases ( 100 eyes ) with diabetic retinopathy combined with central retinal vein occlusion were treated by multiple wave length laser. Krypton yellow laser was used for macular edema in focal photocoagulation and diffuse photocoagulation. For peripheral retina, krypton green or krypton red laser were used. Visual acuity, slit - lamp biomicroscopy, ophthalmoscopy and fundus fluorescein angiography were performed preoperatively and postoperatively. The patients were followed up for 12 to 48wk. ln this study, change in visual acuity and macular edema were observed in both groups, and statistical analysis was performed. RESULTS: The effective rate was 61. 2% in diffuse macular edema group and 86. 3% in focal macular edema group. The general effective rate of later was higher than the former, while the treatment effect had significant statistical difference (P<0. 05).
CONCLUSlON:Multiple wave length laser is an effective and safe way to treat diabetic macular edema of diabetic retinopathy combined with central retinal vein occlusion, which is worth widely applying in clinical practice.
7.Effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes

Objective To explore the effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes. Methods A total of 202 elderly patients with type 2 diabetes in our hospital were selected,and were randomly divided into observation group and control group according to the random number table, with 101 cases in each group. The control group were given healthy related education during hospitalization , and post-discharge telephone follow-up every month, lasting 10~15min each time.The observation group were additionally given timeliness motivation theory-based continuing care by telephone follow-up each month , including emotional arousal , need motivation , examples stimulating and benefit motivation, lasting 6 months. Their self-care ability and healthy behavior in the two groups were compared. Result The self-care ability in the observation group was significantly significantly better than that of the control group (P<0.001), and had significantly much more healthy behaviors than the latter (P < 0.001). Conclusion Timeliness motivation theory-based continuing care can improve the self-care ability and healthy behavior in elderly patients with type 2 diabetes.
8.Dynamic changes and their clinical significance of serum insulin-like growth factor-1 in patients with acute traumatic brain injury

Objective To investigate the dynamic changes of serum insulin-like growth factor-1 (IGF-1) in acute traumatic brain injury (TBI) as well as their correlations with the initial severity of TBI and prognosis.Methods A total of 229 patients with acute TBI admitted from September 2014 to June 2016 were retrospectively studied.Patients were further classified as mild TBI group (GCS 13-15 points),moderate TBI group (GCS 9-12 points) and severe TBI group (GCS 3-8 points) according to Glasgow coma score (GCS).The control group consisted of 30 healthy subjects.The prognosis was evaluated by using Glasgow outcome scale (GOS) at 6 months after TBI.The IGF-1 levels were further tested at days 1,3,5,7 and 14 and their correlations with the initial GCS and GOS at 6 months after injury were evaluated.Results (1) The serum IGF-1 levels of mild,moderate and severe TBI group at all time points were significantly lower than those of the control group (P < 0.05);the serum IGF-1 levels of severe and moderate TBI groups at all time points after injury were significantly lower than those of the mild TBI group (P <0.05);the serum IGF-1 levels of the severe group at days 1,3,5 and 7 d after injury were lower than those of the moderate TBI group (P<0.05).(2) IGF-1 levels were significantly different between the two groups (P < 0.05) at different time points during the follow-up of 6 months.(3)IGF-1 levels were positively correlated with both GCS and GOS at the acute stage of TBI and sub-acute stage following TBI (P < 0.05).Conclusion The dynamic changes of serum IGF-1 levels in patients with acute TBI are related to both initial severity of TBI and the neurological outcomes and can be used as a reliable biomarker for early severity assessment and prognostic prediction of TBI.
9.Effect of manganese on apoptosis in striatum neurons of rats..

OBJECTIVETo explore the mechanism of neurotoxicity induced by manganese, and observe the effects on the apoptosis of neurons in rat striatum.
METHODSSD rats were divided into four groups, six rats each group. Three dose groups were exposed to high, middle, and low level of MnCl(2). At the end of experiment, all rats of the exposed groups and control group were decapitated, their striatums were removed and the Mn content of striatum, the apoptotic morphology, ratio and ultrastructural organization were analyzed.
RESULTSThe Mn content of striatum and apoptosis index of the three dose groups exposed to high, middle, and low level of Mn were significantly higher than control group (P < 0.05). The Mn content of striatum of the three dose groups exposed to high, middle, low level of MnCl(2) and control group were 2.98 +/- 0.52, 2.75 +/- 0.37, 2.61 +/- 0.73, 0.60 +/- 0.20 respectively. The apoptosis index of striatum of the three dose groups exposed to high, middle, low level of MnCl(2) and control group were 24.83 +/- 5.98, 17.00 +/- 5.33, 15.33 +/- 2.58, 2.83 +/- 0.41 respectively, and following higher level dose, the apoptosis index increased. The nucleus of neurons in striatum become smaller, condensed, etc, and these character showed apoptosis of neurons.
CONCLUSIONMn can result in apoptotic morphology and increase level of apoptosis in striatum. The level of apoptos varies with Mn concentration.

10.Effect of manganese on cytosolic free calcium concentration in cortical neurons.

OBJECTIVETo investigate the change of free Ca(2+) in cytoplasma in the neurotoxicity of the manganese (Mn).
METHODSThe cortical neurons were separated from the neonatal Wistar rats and cultured in vitro. The neurons were grouped as the Mn-treated groups and the untreated group. The neurons in the Mn-added groups were incubated in the culture media containing lower, medium and high dosage manganese chloride (MnCl(2 x 4) H2O) with the concentration at 0.2, 0.6, 1.0 mmol/L respectively. Meanwhile, neurons in control were cultured in the normal culture media. All treatments stopped 24 h later. Neurons were labeled Ca(2+) sensitive prober, Fluo-3/AM. The fluorescence intensity of Fluo-3 combined with Ca(2+) was examined by LSCM (Laser scanning confocal microscope) and was treated by the picture analysis technique. The intensity was equal to the free Ca(2+) concentrations in cytoplasma of neurons.
RESULTSMnCl(2) can induce free Ca(2+) overloaded in cytoplasma of neurons, but the increasing degree varied in MnCl(2) dosage. Cytoplasma Ca(2+) concentration in the moderate dosage The moderate dosage MnCl(2) group and the high dosage MnCl(2) group were significantly higher than that in the lower dosage MnCl(2) group and the control group (P < 0.05).
CONCLUSIONThe Ca(2+) overload is involved in the neurotoxicity of manganese, and a dosage response relationship is found between the manganese chloride dose and Ca(2+) overload in cortical neurons.
