1.Total en bloc spondylectomy for 9 patients with primary tumor of the lumbar spine

Objective To evaluate the possibility and curative effect of total en bloc spondylectomy (TES) for the treatment of primary tumors of the lumbar spine. MethodsBetween June 2005 and July 2009,nine cases of primary tumor of the lumbar spine were treated with total en bloc spondylectomy through a single posterior approach performed in seven cases(L1 giant cell tumor, L1 osteosarcoma, L2 giant cell tumor,L2 solitary plasmacytoma, L2 chondrosarcoma, L2 chordoma, and L3 histiocytic sarcoma), and two cases through a single stage anterior and posterior combined approach (L4 giant cell tumor with paraspinal mass and L4 fibrosarcoma). They were performed a single stage reconstruction after removing tumors of vertebra. Neurofunction was evaluated perioperatively and recurrence of tumor was tracked for all cases.ResultsThe follow up was obtained for 3-49 months (averaging 19 months). All patients attained significant clinical improvement after surgery without any severe complications. There was no local recurrence and distal metastasis in shortterm follow-up. ConclusionThe treatment by TES to remove tumors and reconstruction of spine are reliable for primary tumors of the lumbar spine. The curative effect of short-term follow-up is satisfactory. However, the long-term curative effect remain to be further follow-up.
2.Clinical features of unilateral oculomotor nerve palsy related to multiple myeloma:a case report and literature review

Objective To analyze the clinical features of unilateral oculomotor nerve palsy caused by multiple myeloma(MM).Methods A 79-year-old female diagnosed MM for 3 years was admitted due to the disease relapse.The patient presented left oculomotor nerve palsy,bone marrow biopsy showed 4.5% of myeloma cells,the serum level of λ light chain was up to 18 g/L,and brain CT/MRI scan revealed left parasphenoidsinus lesion.A diagnosis of relapsing intramedullary and extramedullary MM,IgGλ type,stage ⅢA was made.Ten cases were retrieved from literature since 1990,the clinical features of 11 cases were analyzed as follows.Results Among 11 cases of unilateral oculomotor nerve palsy caused by MM,7 were males and 4 females with a mean age of (60 ± 12) years (40-79 years).The sub-types of MM were IgG type (7 cases),IgA type (2 cases),biclonal of IgG and IgA type (1 case),and IgD type (1 case).Oculomotor nerve palsy was presented as the initial manifestation of MM in 9 cases,as a sign of relapse of MM in 1 case,and during MM treatment in 1 case.In 7 cases,oculomotor nerve was the only cranial nerve involved,while in other 4 cases,the Ⅳ,Ⅴ,Ⅵ cranial nerves were also involved.Neuroimaging revealed parasphenoid sinus lesions in 9 cases,and myeloma meningitis in 2 cases.Most of the reviewed cases achieved significant clinical improvement after chemotherapy and/or radiotherapy,except 2 cases with myeloma meningitis.The case in our department was improved significantly and rapidly after receiving chemotherapy with bortezomib.Conclusion MM should be considered when unilateral oculomotor nerve palsy is presented with or without the history of MM,the disease is sensitive to chemotherapy in most circumstances.
3.Tri-dimensional omics analysis on effect of zhuanggu zhitong capsule against experimental postmenopausal osteoporosis.

To propose the new concept of multidimensional omics, and define that the multidimensional omics is a proper method for studying the material base and mechanism of traditional Chinese medicine (TCM) compounds. Zhuanggu Zhitong capsule was taken for example to study its effect against experimental postmenopausal osteoporosis. From the perspective of chemi-omics, genomics and proteomics of TCM, it systematically interpreted the efficacious materials and mechanisms of Zhuanggu Zhitong capsule in preventing and treating experimental postmenopausal osteoporosis, while taking the lead in designing a three dimensional form to intuitively exhibit the results of the multidimensional omics study. This study provides a new idea and solution for studies on the efficacious materials and mechanisms of TCM compounds.

5.Heat-related illness in Jinshan District of Shanghai:A retrospective analysis of 70 patients

BACKGROUND: This study aimed to investigate the epidemiological and clinical characteristics of patients with heat-related il ness, and guide the prevention, diagnosis and treatment of heat-related il ness. METHODS: From June 2013 to August 2013, seventy patients with heat-related illness were treated at Jinshan Hospital of Fudan University, and their epidemiological characteristics, laboratory results, treatment and prognosis were retrospectively analyzed. RESULTS: In the 70 patients, 18 patients suffered from heat stroke and 52 patients from non-heat stroke. When the environmnent temperature was above 35 °C, the body temperature of the patients began to increase markedly. The patients with heat stroke were significantly older than those with non-heat stroke (P<0.05). The body temperature, heart rate, blood glucose, blood lactate dehydrogenase and blood creatine kinase in the patients with heat stroke were higher than those in the patients with non-heat stroke (P<0.05). Blood lactate dehydrogenase and blood creatine kinase were positively correlated with body temperature (r=0.801). CONCLUSION: When the environmental temperature goes above 35 °C, heat-related illness should be prevented, especially in the elderly. The body temperature, heart rate, blood glucose, blood lactate dehydrogenase and blood creatine kinase in patients with heat stroke are higher than those in patients with non-heat stroke. Blood lactate dehydrogenase and blood creatine kinase are positively correlated with body temperature, but their relationship with heat-related illness awaits further study.
6.Effects of heterochromatin in colorectal cancer stem cells on radiosensitivity

Background and Objective:Radiotherapy(RT)is a major nonsurgical modality in the comprehensive treatment for colorectal adenocarcinoma.The radioresistance of cancer stem cells(CSCs)is a key factor that influences therapeutic effectiveness.This study was to investigate the effects of specific chromosome structure and histone modification in CSCs in colorectal adenocarcinoma radioresistance.Methods:Samples were collected from resected human colorectaI adenocarcinomas.Subcutaneous colorectal cancer model was established in nude mice.Immunohistochemistry showed that xenografts generated from bulk colorectaI cancer cells resembled the original tumor specimen.Flow cytometry was performed to sort CSCs (CD133~+)and non-CSCs(CD133~-)from both resected samples of colorectal adenocarcinoma and xenograft before and after high single-dose radiation.The markers labeling heterochromatin(H3K9me3,HP1-α,and H3K4me1)and euchromatin(H3K4me3)in CD133~+ and CD133-nucleus were detected by immunoflurescence.Results:There was distinct difference in chromatin structure between colorectaI CSCs(CD133~+)and non-CSCs (CD133~-).The chromatin displayed compact patches in CD133~+ nucleus,but loosely latticed structure in CD133-nucleus:immunoflurescence verified that the compact patches existing in CSCs was generated from heterochromatin construction.In addition.the vacuole-like defect in heterochromatin regions of CSCs was observed within 24 h after exposure to 10 gray(Gy)single-dose RT.Interestingly.this phenomenon was repaired from 96 h.and recovered to dense plaque structure in heterochromatin regions of CSCs after 144 h.However,no significant difference in non-CSCs was observed after RT exception for a loose chromatin structure.Conclusions:CSCs play a role in radiosensitivity in colorectal cancer.The mechanism may be related to heterchromatin formation and histone methylation.
7.Proteomics of epilepsy induced by focal disorder of cortical development

Objective To explore the proteomics of epilepsy induced by focal disorder of cortical development (DCD) in revealing the molecular mechanisms of epilepsy caused by DCD and looking for the candidate targets and new therapeutic approaches in clinical practice. Methods Animal models of DCD were established and induced by liquid nitrogen in healthy Wistar newborn rats. Animal model of DCD were divided into epilepsy group and control group according to Racine classification. The proteomics maps of the frontal cortex were obtained in the epilepsy group and the control group by two-dimensional electrophoresis and both Coomassie brilliant blue G250 and silver dying. The proteomics profiles of frontal cortex were preliminary analyzed with PD Quest 7.3 analysis package. The differentially expressed protein spots were excised from gel and digested with trypsin under optimal conditions. The masses of tryptic-digested peptides were acquired with a Voyager DA-STR matrix assisted laser desorption ionization time of flight (MALDI-TOF) mass spectrometer. The acquired monoisotopic masses of analyzed proteins were matched in silico with theoretical peptide masses of human protein in the swiss-prot database with a mass tolerance of less than 50 ppm. Results One hundred and three proteins of differential expression were observed in the frontal cortex tissues of epilepsy associated with disorder of cortical development in rats, in which 64 were detected to be up-regulated and 39 were down-regulated. Finally, 12 proteins were identified as Lissencephaly-1 protein, synaptotagmin Ⅳ, Glial fibrillary acidic protein, HSP70, growth associated protein 43, neuronal enolase, tubulin beta chain, glutamine synthetase, neuron cytoplasmic protein, voltage-dependent anion channel proteins 1, pyruvate kinase and neurofilament light polypeptide. Conclusion These proteins may play pivotal roles in the pathogenic mechanisms of epilepsy caused by disorder of cortical development and may provide new therapeutic targets for refractory epilepsy in the future.
8.The application of three dimensional analysis system in measuring rotation angles of thumb carpometacarpal joints.

OBJECTIVETo measure the axial rotation angles of the carpometacarpal joints during the digital opposition of thumb-index finger, thumb-medial finger, thumb-ring finger, thumb-little finger and the thumb's maximal opposition, then the application of these parameters were studied.
METHODSTwenty neutrality-occupation volunteers (female 10, male 10) with no history of hand injuries or related diseases were involved in the study. First, all the markers' 3-D coordinates were obstained using the 3D motion analysis system (EVaRT4.1) during the digital opposition movements of thumb. Then, the axial rotation angles were calculated.
RESULTSThe average rotation angles of carpometacarpal joints during all kinds of digital oppositions were 29.1 degrees +/- 9.4 degrees (male), 24.8 degrees +/- 10.2 degrees (female), while the maximal rotation angles are: 35.3 degrees (male), 28.8 degrees (female).
CONCLUSIONSThe combination of video-based 3-D analysis system and mathematics make it possible to measure the axial rotation angles of thumb in vivo, as a result, the rotation angles of thumb carpometacarpal joints are measured precisely for the first time. These results can provide a few parameters for treatment and rehabilitation of carpometacarpal arthrositis.

9.EGFR gene copy number, ERCC1 and BRCA1 protein expression and their relationship in non-small cell lung cancer.

OBJECTIVETo evaluate the expression of epidermal growth factor receptor (EGFR) gene copy number and the expression of ERCC1 and BRCA1 proteins in patients with non-small-cell lung cancer (NSCLC) and the correlation between them.
METHODSThe status of EGFR gene copy number was determined by in situ hybridization (FISH), and the expression of ERCC1 and BRCC1 proteins was examined by immunohistochemistry (IHC). The relationship of EGFR gene copy number with the expression of ERCC1 and BRCA1 and the clinical pathologic features were analyzed.
RESULTSFISH-positive EGFR expression was identified in 40 of 166 samples (24.1%). More FISH-positive EGFR in the female than male patients (31.9% vs. 18.6%, P = 0.048), and non-smoker than smoker (32.8% vs. 16.7%, P = 0.045). FISH-positive EGFR was not associated with age, pathological type, clinical stage and metestasis status (P > 0.05). The expression of ERCC1 protein was identified in 60 of 132 samples (45.5%). The expression of ERCC1 protein varied significantly in tumors of different pathological types (P = 0.046), but not associated with age, gender, clinical stage, metestatic status and smoking status (P > 0.05). The expression of BRCA1 protein was identified in 46 of 131 samples (35.1%). The expression of BRCA1 was not associated with age gender, pathological type, clinical stage, metestatic ststus and smoking status (P > 0.05). There was a moderate correlation between the expressions of ERCC1 and BRCA1 (r = 0.449, P < 0.001), but EGFR gene copy number was not correlated with the expression of ERCC1 or BRCA1 protein.
CONCLUSIONSFISH-positive EGFR expression is associated with gender and smoking status, but not correlated with the expression of ERCC1 and BRCA1 proteins. There is a moderate correlation between the expressions of ERCC1 and BRCA1.
