Objectives To analyse the clinical feature,drug and surgical treatment and prognosis of rheumatic diseases associat- ed corneal melting.Design Retrospective case series.Participants 14 patients (22 eyes) with rheumatic diseases associated corneal melting.Methods The clinical data of patients were analysed,including history,visual acuity,slit-lamp microscopy examination,drug and surgical treatment and follow-up results.Main Outcome Measures The clinical feature,therapeutic effects and recurrence. Results The follow-up was 6～24 months.Rheumatic diseases associated corneal melting always firstly occurred from the upper corneal limbus.The marginal lesions of cornea included melting of the stroma,resulting in corneal thinning or even perforation,with neovascu- larization.The corneal lesions in 4 eyes were within 1 quadrant,10 eyes within 2 quadrants,6 eyes within 3 quadrants,and 2 eyes in 4 quadrants.Eight eyes of 5 cases,whose corneal lesions were thinner than 2/3 of normal corneal thickness,received 1% cyclosporine A and 0.5% heparin eyedrops.Among those,the lesions in 3 eyes of 2 cases progressed worse.Fourteen eyes of 9 cases,whose corneal lesions were thicker than 2/3 of normal corneal thickness,received marginal lamellar keratoplasty.Among those,recurrent signs were noted in 10 eyes of 6 cases.Conclusions Rheumatic diseases associated corneal melting has typical clinical features.Local cy- closporine A and heparin treatment are effective for patients in early stage.A high recurrent rate is found for patients in late stage re- ceived marginal lamellar keratoplasty.

OBJECTIVE:To facilitate the automatic drug dispensing in hospital pharmacy.METHODS:Full automatic single dose pastill dispensing machine was introduced to hospital central drug dispensing.The running cost was analyzed from two aspects and the feasibility of cutting cost is demonstrated.RESULTS & CONCLUSIONS:The key to achieve automatic drug dispensing in hospital pharmacy is to reduce the running cost to a lowest degree,and the key to lower running cost lie in the omnidirectional controlling of cost,enhancing of performance and reinforcing of the consciousness on cost etc.

Objective To evaluate the application value of dermoscopy in the diagnosis of genital condyloma acuminatum (CA) in men.Methods A total of 122 warts from 66 male patients with suspected genital CA were firstly diagnosed by experienced dermatologists according to macroscopic morphological characteristics of lesions and clinical history of patients.Afterwards,the warts were examined and diagnosed by dermoscopy,and the two diagnosis results were compared with histopathological findings.Results Of 122 suspected warts,100 were diagnosed as CA by naked-eye examination,112 as CA by dermoscopy and 114 as CA by pathological examination.When histopathological findings served as the gold standard,dermoscopy had a sensitivity of 97.4％ and a specificity of 87.5％,and naked-eye examination had a sensitivity of 87.7％ and a specificity of 100％.Under dermoscopy,most male genital warts were pink (98/114,86.0％) and papillary (98/114,86.0％),and had distinct vascular characteristics (107/114,95.5％),which mainly included pleomorphic vessels (102/114,89.5％) and punctate vessels (98/114,86.0％).Conclusion CA has characteristic structures under dermoscopy,so dermoscopy can be used for the diagnosis of genital CA in men.

Objective To analyze the characteristics of gastroesophageal junction(GEJ) in gastroesophageal reflux disease (GERD) using high resolution manometry (HRM),and the features in different GERD subtypes,reflux esophagitis (RE) and non-erosive gastrooesophageal reflux disease (NERD).Methods Sixty GERD patients were divided into two subtypes,RE (n =33) and NERD(n =27),based on the results of endoscopy.All patients received HRM,and were divided into three subtypes,type Ⅰ,type Ⅱ and type Ⅲ according to the GEJ pressure morphology.HRM results among different subtypes were investigated.Results In 60 GERD patients,the GEJ was divided into three subtypes,including 26 cases of type Ⅰ (RE =14,NERD =12),29 cases of type Ⅱ (RE =14,NERD =15) and 5 cases of type Ⅲ (RE =5).The characteristics of GEJ morphologic subtypes were analyzed.The total length of lower esophageal sphincter(LES) of three GEJ subtypes(type Ⅰ,type Ⅱ and type Ⅲ) were (3.35 ± 0.56) cm,(3.29 ± 0.87) cm and(2.43 ± 0.22)cm,respectively.The LES intra-abdominal length of three GEJ subtypes were(2.41 ± 0.68) cm,(3.07 ± 4.59) cm and (0.00 ± 0.00) cm,respectively.The LES rest pressure of three GEJ subtypes were (22.00 ± 8.00) mmHg,(16.46 ± 8.60) mmHg and (11.07 ± 5.80) mmHg,respectively.The total length of LES were significantly different among three subtypes (P ＜ 0.05).The LES rest pressure were not significantly different among three subtypes (P ＞ 0.05).Among type Ⅰ and type Ⅱ patients,there were no significant difference in LES-CD distance,total length of LES,LES intra-abdominal length and LES rest pressure between RE group and NERD group.Conclusion HRM can clearly distinguish the GEJ subtypes.To some extent,esophageal motility characteristics of the patients with different subtypes GEJ is various.However,it still needs further studies with larger sample size to confirm.

Objective:To detect the effect of pulse radiofrequency (PRF) treatment on the neuropathic pain established by L5-spinal nerve ligation (SNL) on rats,and to investigate if PRF treatment would affect the expression of autophagy related protein LC3 and autophagy related receptor P62 at the dorsal horn.Methods:A total of 36 male Sprague-Dawley rats were randomly divided into 3 groups:a Sham group,a SNL group,and a SNL+PRF group.The 50％ paw withdrawal mechanical threshold (PWMT) was detected at 1 day before and 1,3,7,14 and 28 days post-operation by using Von-Frey filaments.The autophagy related protein LC3 and autophagy related receptor P62 were investigated by Western blot.Results:Compared with the Sham group,the PWMT significantly decreased in the SNL group at each time points (P＜0.05);in SNL+PRF group,PRF treatment could elevate the PWMT at the 1st day post-operation and lasted for 28 days (P＜0.05).What's more,SNL could elevate the LC3-Ⅱ and P62 levels at the 7th day post-operation (P＜0.05),which were decreased by the PRF treatment (P＜0.05).Conclusion:PRF treatment could improve SNL-induced the neuropathic pain,which might be partly due to the regulatory effects on the autophagy levels at the spinal dorsal horn.

Objective To learn the current capacity building of emergency response teams in Tianjin for the purpose of goals setting.Methods Health Emergency Capacity Questionnaire was issued to 89 secondary and above medical institutions and 19 CDCs in Tianjin.The form covered such items as basic institutional information, workforce makeup, emergency preparations, detection and early warning, emergency response, and summary/assessment.Data collected in the questionnaire were subject to descriptive and correlation analysis.Results Tianjin has scored an initial success in emergency medicine as evidenced in its emergency response mechanisms in place, elevated capacity in emergency medical rescue and disposal, and enhanced competence of emergency teams.Rooms of improvement however include insufficient professionalism and independence of health emergency, inadequate emergency commanding and decision making system functions, insufficient laboratory test capacity at district/county levels, and insufficient social involvement in health emergency.Conclusions Top-down design should be emphasized, health emergency response should be enhanced in terms of management and response planning system, while capacity building of the teams and long-term primary care emergency mechanism deserve higher attention.

Objective: To investigate the inhibitory effect of Undaria pinnatifida polysaccharides (UPPS) on hepatocellular carcinoma HepG-2 cells and its possible mechanism. Method: The effect of inhibiting proliferation and inducting apoptosis of UPPS were determined by means of MTT and FCM. The expression of Bcl-2 and Bax proteins was immunohis to chemcally evaluated after treatment of UPPS. Results: UPPS inhibited HepG-2 cells growth in vitro , significantly higher than the negative control group (P

γ-glutamyltranspeptidase was detected from the cultured mycelia of Cordyceps sinensis (CSGT). Km and Vmax of CSGT was 2.54×10-4 mol/L and 0.1808 mol/L·min respectively when L-glutamic acid 5-(4-nitroanilide) (GpNA) and glycyglycine was used as its substrate. CSGT was stable from pH 8.0 to 11.0 and at or below 20℃. It was optimally active at pH 9.0～10.0 and 30℃. A series of reducing reagents could activate CSGT, and metal cations such as Zn2+, Cu2+, Hg2+ , Mn2+ inhibited strongly activity of the enzyme, but K+, Ca2+, Mg2+ and Na+ at high concentrations had no effect on its activity, indicating that its active center could contain -SH.

ObjectiveTo explore the possibility and significance that psychotic patients treated in open wards in general hospital,a new managing model for psychosis cases.Methods1 200 psychotic cases were treated in the open ward,that was a new managing model for psychotic patients.ResultsNew managing model had following advantages: a. free for patients to communicate with outsides and keep their secret and human rights; b. having better effective,shorter housing days,and more economy compared with controlled cases.ConclusionSetting up open managing psychosis wards in general hospital can help treatment of psychotic patients,and it is the tendency of modern mental health.

Objective:To investigate the aberrant methylation of CpG island in 5′promoter region of p16 gene in the pancreatic juice and its value in diagnosis of patients with pancreatic cancer.Methods:Pure pancreatic juice(PPJ)was collected from the pancreatic duct by a nasopancreatic tube put under endoscopic retrograde cholangiopancreatography(ERCP). Cytological examination was performed by H-E staining in pure pancreatic juice.Aberrant p16 methylation was detected using the methylation specific PCR(MSP)in the PPJ.Results:The sensitivity,specificity,positive predictive value,negative predictive value and accuracy cytological examination in diagnosis of pancreatic cancer were 40%,100%,100%,45.4% and 60.0%,respectively.The DNA sequences were successfully extracted from the PPJ of 30 patients with pancreatic diseases and were subjected to MSP.Seven(35%)of the 20 cases with pancreatic cancer showed aberrant methylation of p16 gene.No aberrant methylation was detected in the pancreatic juice samples of patients with chronic pancreatitis and mucinous cystoadenocarcinoma of pancreas.When cytological examination combined with p16 methylation detection,the sensitivity, specificity,positive predictive value,negative predictive value and accuracy for diagnosis of pancreatic cancer were 55%,100%, 100%,52.6% and 70%,respectively.Conclusion:Pancreatic juice collected by nasopancreatic drainage during ERCP can be used for molecular analysis.Detection of aberrant methylation of p16 gene in pancreatic juice combined with cytological examination is a better method for diagnosis of pancreatic cancer.
