1.Analysis of the usage of antibacterial drugs in 75 hernior-rhaphy inpatients during perioperative period

Objective: To evaluate the rationality of the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Methods: Seventy-five cases of herniorrhaphy discharged in March, June and September of 2009 were collected. Parameters including indications, the types of antibacterial drugs, time of administration, solvent choice, usage and dosage, combination, etc, were assessed for the rationality according to the "the guiding principles of clinical use of antibacterial drugs"and other relevant documents. Results: The antibacterial drugs application rate was 100%, while the rationali-ty of the drug selection, the drug administration route, the single dose and the number of dosing, was 70.7%, 100% , 100% and 93.3% respectively. Sixty-two patients(82.7%)were treated with antibacterial drugs at 0.5~2 hours before surgery, and the average time course of the prophylactic use of antibiotics after surgery was 2.3 days. Conclusions: Some problems existed for the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Surgeon should ensure the correct use of the antibacterial drugs to make the safety, efficiency, rationality, and economy for the patients.
2.Survey in related factors of nurse' compassion fatigue and compassion satisfaction in a certain tertiary hospital

Objective To describe the status and analyze the influencing factors of nurses' compassion fatigue and compassion satisfaction in a certain tertiary hospital,which may provide evidences for the emotional management of nurses by nursing managers.Methods A cross-sectional survey was used in this study and 246 nurses were recruited totally.They investigated about compassion fatigue and compassion satisfaction by questionnaires.Results The average score of nurses' compassion fatigue was (30.76± 5.61)points.The average score of nurses' compassion satisfaction was (25.68±4.12)points.There was a significant difference for nurses' compassion fatigue in various wards,and ICU nurses had the top score.There were no significant differences for nurses' compassion satisfaction.Conclusions The nurses' compassion fatigue and compassion satisfaction in this hospital was relatively moderate.ICU nurses' compassion fatigue score was higher than other wards.Education background,professional title,relationship with colleagues,extra work,support from leaders and family members,wards,education background and relationship with colleagues were the influencing factors of nurses' compassion satisfaction score.
3.Research status on regulation of Chinese herbal compound on intestinal microecology.

The ralationship between traditional Chinese medicine (TCM) and intestinal microecology is increasingly being given more and more attention. Combined with the devolopment of intestinal microecology disciplines, effects of TCM on regulation of intestinal microecology have been gradually explained. Both clinical studies and animal experiments have confirmed that TCM can maintain the balance of intestinal microecology and regulate the intestinal flora. The author arrangemented the documents related to Chinese herbal compound adjusting intestinal flora in the recent ten years, summarized that the Chinese herbal compound which can strength spleen and replenish Qi, relax bowels and regulate Qi, dissipate dampness and check diarrhea, clear away heat and toxic materials, promote digestion and relieve stasis had certain regulation effects on intestinal microecology, providing basis for revealing the TCM essence of intestinal microecology.

4.Study on mechanism of sleep disorders associated with Parkinson's disease

Sleep disorders are commonly occurred among patients with Parkinson's disease,such as difficulties in the initiation of sleep,fragmented sleep,sleep behavior disorder and excessive daytime sleepiness.The mechanism of sleep disorders associated with Parkinson's disease is not clear,which may be associated with the injury of brain stem,nuclei of median raphe,nuclei fasciculi solitarii,thalamencephalon and the changes of neurotransmitters as dopamine,hypocretin(orexin) and melatonin.This article gives an overview of the mechanism of sleep disorders associated with Parkinson's disease.
5.Late side reactions of long-term survivors after radiotherapy for nasopharyngeal carcinoma

15-20 years accounted for 11%,12%,14% and 10% , respectively. 260 patients had grade 3 and 4 other injuries, including 55 skin, 79 muscles/soft tissue, 36 mucosa, 79 xerostomia and 11 trismus. Among 320 patients who suffered from caries of teeth, it involved all teeth in 192 patients. Two patients had mandible necrosis. 225 patients had decline of hearing. Among 672 ears in 336 patients,194 ears had severe deafness. The overall side reaction occurrence rate was 84.5%. Conclusions Different late side reactions do exist in nasopharyngeal carcinoma patients(84.5%) with long-term survival after radiotherapy, and some patients suffer side reactions so serious as to affect their quality of life. Cranial nerve injuries are not rare, and it may occur anytime after radiation, but it dose not stop as the patients survive along.
6.Suppression of chondrocyte apoptosis in the osteoarthritis by interleukin-1Ra ex vivo gene therapy

Objective To explore suppression of chondrocyte apoptosis by IL-1Ra gene transfer to the knees of experimental osteoarthritis (OA).Method ①Thirty New Zealand white rabbits were divided into 3 groups,partial synovectomy of the left knee had been done.Synovial fibroblasts (SF) were isolated and cultured.②Fibroblasts from each rabbit were transduced by using the retrovirus with either IL-1Ra gene (group 3) or marker gene to (group2) in vitro.③ OA was induced in right knee of rabbits by anterior cruciate ligament transection (ACLT).④ The autologous cells transduced with IL-1Ra gene or marker gene were injected into the right knee of group 3 or 2.⑤At the second week and at the forth week,IL-1Ra level in SF was determined by ELISA.⑥Cartilage section were stained for checking histologic changes and chondrocyte apoptosis were determined using TUNEL method.Results The IL-1Ra level in the SF of group 3 were (20.6?1.8)ng/ml and (4.82?0.52)ng/ml after 2 and 4 weeks,after gene delivered respectively,and the IL-1Ra level in the group 2 and group 1 were very lowat both 2 and 4 weeks.Approximately 40% of chondrocytes underwent apoptosis in group 2,whereas apoptosis were inhibited in group 3,only 18% of chondrocytes underwent apoptosis,and 2%~5% of chondrocytes underwent apoptosis in normal joint.Conclusion This study showes that a local increase IL-1Ra level in OA keen joints by intraartilcular injection of IL-1Ra gene transduced synovial cells can suppress articular cartilage chondrocyte apoptosis.
7.Radiation-induced cranial nerve palsy and its causative factors in nasopharyngeal carcinoma

7000?cGy). The cumulative incidence of cranial nerve Ⅸ-Ⅻ palsies increased in patients with advanced N stage. Patients in the first group of treatment field had the highest risk to progress cranial nerve Ⅸ-Ⅻ palsies, followed by the second group, and the third group had the lowest risk. Only 1 in 34 patients with brachytherapy developed cranial nerve palsy. Conclusions Our retrospective data show that the radiation-induced cranial nerve palsy is a common complication in NPC patients after radiotherapy and the incidence seems to increase with the duration of the follow-up. Factors that potentially affect the injury of cranial nerve Ⅰ-Ⅶ and Ⅸ-Ⅻ are different, because of the different anatomy. Controlling the doses to the nasopharyngeal region and using faciocervical fields may help to lower the incidence of cranial nerve palsy.
8.Relationship between carotid artery risk assessment and postoperative cognitive dysfunction with off-pump coronary artery bypass grafting

Objective To analyze the correlation of carotid artery stenosis(CAS)with postoperative cognitive dysfunc?tion(POCD)in patients who underwent off-pump coronary artery bypass grafting(OPCABG). Methods Between March 2014 and October 2014, a total of 157 patients who underwent OPCABG in our hospital were enrolled and divided into POCD group and non-POCD group according to their performance respondent to MMSE questionnaire survey conducted 5 days before operation. LOTCA questionnaire survey was conducted at the first day and 7th days pre-and post-operation re?spectively in order to investigate the influence of CAS on POCD in patients. Results The incidence of POCD was 30.6%(48 out of 157). Advanced age, diabetes,cultural level,carotid artery stenosis, Hb level after operation, score of LOTCA question?naire survey conducted before surgery,time of respirator and the time spend in ICU were significant predictive factors for POCD. However, multivariate logistic regression analysis showed that age, CAS and the time spend in ICU were independent risk factors for POCD, especially CAS(OR=5.078,95%CI:2.211-11.662, P<0.001). Patients with severe CAS suffer from a significant decline in cognitive function on space, time, action and ideation. Conclusion CAS is a predominant risk factor for POCD in patients undergoing OPCABG.
9.Protective immunity induced by DNA vaccine of Cysticercus cellulosae antigen

Objective: To investigate the immune response induced by Cysticercus cellulosae protective antigen cC1 DNA vaccine in mice and the protective immunity induced by immunized newborn pigs. Methods: Recombined plasmid p3 cC1 was constructed by inserting full length cC1 cDNA into an eukaryotic expression vector pcDNA3. Mice were injected intramuscularly with the recombined construct. Anti cC1 antibody and IgG 2a in serum were screened by ELISA. Then the protective immunization in pigs was done. Results: The immune response of specific antibody was induced during the 3r week. The highest level was during the 8th week. IgG 2a response was detected during the 2nd week. The higher duration of IgG 2a response induced by DNA vaccine was longlived. The protective rate induced in immunized newborn pigs was 73%. Conclusion: The cC1 DNA vaccine can effectively induce protective immunity in newborn pigs.
10.Computer-assisted design of TIMP-1 ribozyme

Objective: To design the ribozymes to cleave human TIMP 1 mRNA, and embed them into U 6snRNA to make them stable. Methods: Ribozymes were designed according to the “hammerhead structure” described by Symons.Computer was used to analyze the possible cleavage sites. Results: Three ribozymes targeting the nt123, nt299 and nt353 on TIMP 1 mRNA were designed. Embedding ribozyme in U 6snRNA had little effect on its binding with the substrate. Conclusion: Computer assisted design is indispensable in studying ribozyme. Embedding ribozymes in U 6snRNA may be a good way to solve the problems existing in ribozyme study. [
