2.Observation of improved extra panretinal photocoagulation for high risk proliferative diabetic retinopathy

AIM: To observe the therapeutic effect and safety of improved extra panretinal photocoagulation ( E - PRP) in the treatment of high risk proliferative diabetic retinopathy (hsPDR).
METHODS: A total of 88 consecutive cases (102 eyes) with hsPDR were diagnosed by fundus fluorescein angiography(FFA) from February 2011 to December 2014 in our hospital . Fifty two eyes had been treated by improved E - PRP with 532nm frequency - doubled laser. Fifty eyes had been treated by standard PRP. All cases were checked by FFA and fundus photocoagulation every 3mo. Patients with persisting neovascularization or non perfusion area were treated with laser again. All cases were followed up 6-36mo.
RESULTS: The postoperative visual acuity had no statistical difference between two groups ( P > 0. 05). In improved E - PRP group, retinal non perfusion area and neovascularization disappeared in 35 eyes ( 67%). Effective rate was 88%. Six eyes (12%) underwent pars plana vitrectomy because of vitreous hemorrhage, fiberosis and stretched retinal detachment. In standard PRP group, retinal nonperfusion area and neovascularization disappeared in 23 eyes ( 46%). Effective rate was 66%. Seventeen eyes(34%) underwent pars plana vitrectomy because of anterior retina bleeding or vitreous hemorrhage. The rate of neovascularization disappeared and effective rate had statistical difference between two groups (P<0. 05).
CONCLUSION: It is a safe and effective methods to treat hsPDR by improved E-PRP and it was more effective than traditional PRP.
3.Isolation,Identification and Characterization of a Methanotrophic Strain

Strain QJ16,one of some methanotrophic bacteria isolated from soil samples utilizing the separate device designed by our laboratory;was studied in detail and identified as one of the Genus methylomonas through 16S rDNA sequencing and comparison. The culturing and reaction conditions for methane utilization of this strain were investigated. The results indicated that the optimal conditions for the growth of strain QJ16 were methane gas as carbon source and the coexistence of NH4Cl and KNO3 as nitrogen source,the cultivation temperature of 30℃ and medium initial pH 6~7,and the optimal Cu2+ concentration of 15 ?mol/L.
5.Optimum dose of dexmedetomidine for lumbar plexus combined with sciatic nerve block when mixed with ropivacaine

Objective To determine the optimum dose of dexmedetomidine for lumbar plexus combined with sciatic nerve block when mixed with ropivacaine.Methods Eighty ASA physical status Ⅰ or Ⅱ patients of both sexes,aged 18-63 yr,weighing 47-83 kg,scheduled for elective ankle joint surgery,were randomly divided into 4 groups (n =20 each) using a random number table:ropivacaine group (group R) and different doses of dexmedetomidine mixed with ropivacaine groups (RD1-3 groups).Lumbar plexus block was performed by using psoas-compartment approach guided by a nerve stimulator.0.5% ropivacaine 20 ml was injected in group R.0.5% ropivacaine 20 ml containing dexmedetomidine 1.0,1.5 and 2.0 μg/kg was injected in RD1 3 groups,respectively.Labat's sciatic nerve block was performed,and 10 ml of the corresponding drug was injected in each group.The onset time and duration of sensory and motor blockade,and side effects such as cardiovascular events and excessive sedation were recorded.Results There was no significant difference in the onset time and duration of sensory and motor blockade between the four groups.The duration of sensory and motor blockade was significantly longer in RD1-3 groups than in group R,in RD2 and RD3 groups than in group RD1,and in RD3 group than in RD2 group.The incidence of over-sedation and bradycardia was significantly higher in RD3 group than in RD1.2 groups.Conclusion The optimum dose of dexmedetomidine is 1.5 μg/kg for lumbar plexus combined with sciatic nerve block when mixed with ropivacaine.
6.Relation between expression of BCSCs associated genes from swelling axillary lymph nodes of breast cancer with recurrence and metastasis of tumor

Objective To explore the relation between the expression of breast cancer stem cells(BCSCs) related gene CD44 , OCT4 and Twist1 in breast cancer tissues and swelling axillary lymph nodes with the recurrence and metastasis of breast cancer . Methods The genes and antigen expressions of BCSCs relative factors CD44 ,CD24 ,OCT4 and Twist1 in th breast cancer tissue and axillary lymph nodes were detected by the immunohistochemical method and RT‐PCR .Results Totally 14 cases of preopera‐tively and intraoperatively diagnosed breast cancer had axillary lymph nodes metastasis of cancer cells .The over expression of BC‐SCs relative factors CD44 ,OCT4 and Twist1 antigen positive in the breast cancer tissue and axillary lymph nodes were detected by the immunohistochemical method ,while CD24 was lowly expressed;the gene expressions of BCSCs relative factors CD44 ,OCT4 and Twist1 in the breast cancer tissue and axillary lymph nodes were enhanced by the RT‐PCR detection (P<0 .05) ,while CD24 gene was lowly expressed(P>0 .05) .Conclusion The genes and antigen expression of BCSCs related factors CD44 ,OCT4 and Twist1 in the patients with breast cancer complicating swelling axillary lymph nodes are significantly increased ,which might indicate that the BCSCs related factors are related with easily early metastasis and recurrence of breast cancer cells .
7.Clinical Efficacy Observation of Zhishanghan Zhufeng Formula in Treatment of Knee Osteoarthritis

Objective To observe the clinical efficacy of Zhishanghan Zhufeng Formula in treating knee osteoarthritis.Methods Totally 90 patients were randomly divided into treatment group and control group, 45 cases in each group. The treatment group was treated with Zhishanghan Zhufeng Formula for 30 days. The control group was treated with routine medicine (diclofenac sodium sustained release tablets and glucosamine sulfate tablets) for 30 days. The clinical efficacy and the changes of VAS, WOMAC score and knee temperature before and after treatment of two groups were observed.Results TCM clinical symptoms were significantly improved, and the effects in the treatment group were more evident compared with the control group (P<0.05). After treatment, VAS and WOMAC score of the two groups decreased obviously, while the knee temperature increased, and the treatment group was superior to the control group, with significant significance (P<0.05).Conclusion Zhishanghan Zhufeng Formula has effective clinical efficacy for knee osteoarthritis.
8.Assessing the nutritional status of patients with gastric cancer by patient-generated subjective global assessment and its relationship with postoperative results

Objective To evaluate the preoperative nutritional status of patients with gastric cancer by using patient-generated subjective global assessment (PG-SGA) its relationship with postoperative results.Methods Make a preoperative nutrition assessment by using PG-SGA for 87 patients with gastric carcinoma who can be treated with operation.Analyze the effects of the postoperative complication,survival rate and the hospitalization time on patients.Results The number of patients of this group who can process the preoperative nutrition assessment by using PG-SGA accounts for 100% of the total.There are 37 patients with moderate or severe malnutrition before operation (account for 39.1%).The complication incidence of patients with moderate or severe malnutrition before operation and the patients with no or mild malnutrition are 52.9% and 5.7% separately (P < 0.01).The sensitivity and specificity of PG-SGA score for predicting complications are 85.7% and 75.8% respectively.And we should implement the analysis of regression to verify that the PG-SGA grade is the independent risk factor of postoperative complication by applying multiple Logistic(P < 0.01).The average hospital stays of patients with moderate or severe malnutrition before operation and the patients with no or mild malnutrition are (21 ± 6) d and (16 ± 4) d separately (P < 0.01).Conclusion According to the PC-SCA results,patients with moderate or severe malnutrition before operation have increased susceptibility to complication and the extension of hospital stays after the operatiou.Therefore,it is important to supply the proper nutrition support to these patients.
9.The Carotid Atherosclerosis in Hemodialysis Patients

Objective To investigate the carotid atherosclerosis(AS) in hemodialysis(HD) patients.Methods Clinical index was measured and intma-media thickness(IMT) of extracranial common carotid artery and presence of atherosclerotic plaques were determined by high-resolution B-mode ultrasonography.Results The age,disease duration,HD duration,triglyceride(TG),low density lipoprotein(LDL),serum C-reactive protein(CRP),mean carotid artery IMT in HD with atherosclerotic plaque were significantly higher than HD without atherosclerotic plaque patients,however,the serum albumin(Alb) was significantly decreased.Mean carotid artery IMT is related to the disease duration,Alb,TG,LDL,CRP in HD patients.Conclusion The CRP,mean carotid artery IMT were increased in HD with atherosclerotic plaque.Mean carotid artery IMT is related to the disease duration,Alb,TG,LDL,CRP in HD patients.To improve malnutrition and decrease micro-inflammation,serum lipid might reduce the rate of AS in HD.
10.Clinicopathologic characteristics and surgical treatment of obsolete Bennett fractures

Objective To investigate clinicopathologic characteristics of obsolete Bennett fractures and their surgical treatment and prognosis. Methods From August 2008 to April 2010,13 patients with obsolete Bennett fracture were admitted to our department.They were 12 men and one woman,with an average age of 34.3 years (from 17 to 42 years).They had a fracture history of over 4 weeks to as long as 12 weeks (average,7.9 weeks).In all patients,after X-ray examinations,the radio-dorsal ligament complex of the first carpometacarpal joint was properly released,fracture gaps were cleared up,fracture fragments were anatomically reduced and fixed with a Kirschner wire,and at last the first metacarpal joint was fixed with the Kirschner wire crossing the joint Postoperatively all patients were immobilized with cast plaster for 4 weeks before the Kirschner wires acrross the joint were removed.Gradual rehabilitation was conducted.The X-rays were taken every month. Results The patients were followed for 6 to 24 months(average,14 months).The fractures healed in all the patients,with an average healing time of 3 months.No dislocation of the joints occurred.Injury to articular cartilage of the trapezium bone occurred in one patient,and no apparent pain was experienced in the other 12 patients.According to the Total Active Motion assessment,the thumb function was rated as excellent in 9 cases,good in 2,moderate in one and poor in one,with a good to excellent rate of 84.6%.Conclusions In obsolete Bennett fractures,the radio-dorsal ligament complex of the first carpometacarpal joint will have cicatricial contracture,preventing reduction of the fragments of the radio-dorsal joint.Therefore it is important to properly release the radio-dorsal ligament complex to achieve anatomical reduction of the radio-dorsal joint.An injury history of over 3 months and complicated injury to the trapezium bone cartilage may be significant factors affecting the prognosis of obsolete Bennett factures.
