2.Regulatory effect of triptolide on angiogenesis in synovium of collagen-induced arthritis rats

Objective To study the regulatory effect of triptolide(TP)on the angiogenesis of coll-agen-induced arthritis(CIA) rats.The effect of TP on arthritis is also explored.Methods After the model of CIA was established,the articular volume was measured and the synovium was examined with regular HE stainand the inflammation and pathological changes were evaluated.In addition,the vascular endothelial growth factor (VEGF),basic fibroblast growth factor (bFGF)and endostatin protein expressions in synovium and serum were tested.The micro-vessel density (MVD) of synovium was also measured by caulating CD34 level.Results The expressions of VEGF,bFGF and MVD in CIA rats'synovium and serum were evidently higher than the control group(X2=65.3,31.6,q=9.2,P<O.01,respectively),while the expression of endostatin showed no statistical difference with controls (X2=0.8,P>0.05).After treated with TP,the expressions of VEGF,bFGFand MVD decreased markedly(X2=19.7,6.0,q=6.5,P<O.01,respectively),but the pmtein expression of endostatin significantly increased (X2=3.9,P<O.05).However,only the expression of endostatin increased significantly after treated with MTX (X2=17.9,P<0.01).Conclusion Imbalance in growth factors prnduction may play an important role in the process of arthritis development.Re-establishing the balance of growth factors maybe one of the mechanisms of TP in the treatment of arthritis.
3.The application of risk management in the nursing of elderly patients in CCU

Objective This study is aimed to reduce the incidence of nursing risk and avoid the occurrence of defects of nursing in order to improve the quality of nursing of elderly patients in Coronary Care Unit(CCU).Methods Between December 2007 and December 2008,risk management has been applied to the nursing of elderly patients in CCU.The risk of nursing was assessed and relative protocols of prevention of nursing risk werle established and brought into routine nursing practice.The effectiveness of this protocol has also been evaluated.30 patients were selected from CCU every quarter(totally 120patients were selected).These patients fulfilled the indacing format made by PLA General Hospital.The rate of satisfactory.complaints and defect were compared before and after the implement of management.Results After application of risk management,the satisfaction rate of patients raised from 87.50%to 98.33%(P<0.01).Compared to pieapplication,the rate of complaint of nursing(10.83%,3.33%;P<0.05)and defects(14.17%,5%;P<0.01)have both dropped significandy.Conclusions The application of risk management play an important role to prevent the occurrence of nursing defects,enhance the ability of nursing staff recognizing and copping with risks,reduce the risk rate and improve the nursing safety in CCU.
4.Investigation of psychological status of long term professional nursing company and related strategies

Objective To investigate the psychological status of long term professional company and to seek effective ways to improve their psychological status, so as to improve the nursing quality of professional company. Methods The baseline data, nursing company related knowledge and basic nursing skills of long term group ( n = 40, company time > 6 months) and short term group ( n = 40, company time < 6 months) were collected, while the self-rating anxious scale and self-rating depressive scale were applied to assess their psychological status. Results The basic nursing skills scales and depression scales of long term group were significantly higher than that of short term group ( P < 0. 05 ). Conclusions Depression is associated with nursing company carrying long term company, yet their nursing skills are much better. Nurse should make full use of predominance of long term nursing company to help nurse to accomplish basic nursing care. Meanwhile, nurse should pay attention to their psychological health and carry psychological nursing on them and provide simple but effective ways to relieve the stress so that company skills can be improved in general, which will serve patients more effectively.
5.Mutations in hepatitis B virus genome involved in immunoprophylaxis failure against vertical transmission

Objective To determine the factors responsible for failed postnatal immunoprophylaxis for hepatitis B virus(HBV) in Qidong, China. Methods Eleven children who developed into chronic HBV infection after receiving HBIG and HBV recombinant vaccines were recruited into the study. Eleven paired mothers with chronic hepatitis and other 6 mothers whose children successfully generated anti-HBs after im-munoprophylaxis were included as the control in the study. Full-length HBV DNA was amplified through ser-um sample by PCR method and underwent cloning and sequencing. HBV DNA level was quantified by real-time PCR. Results The mean levels of HBV DNA in mothers who had HBV DNA positive children and healthy children were ( 1.2 ×107± 3.1 × 106 ) copies/ml and ( 1.6× 107±8.8×106 ) copies/ml, respec-tively. There was no significant difference between the groups (P >0.05). Meanwhile, viral load in chil-dren was unrelated to that in their mothers (r2 =0.2429). In 11 HBV DNA positive children, 4(36.4% ) demonstrated amino acid substitutions in HBsAg "a" determinant region with 6 different types, I.e. T125A, I126T, Q129H, M133V, D144V and G145A. All of the mothers showed the wild-type sequence in "a" epitope, indicating surface escape mutants were not acquired from the initial infection, but developed under the immune pressure. The mutation rates after immunoprophylaxis for preS1, preS2, S, X, preC/C and P genes were 0.38%, 0. 22%, 0.27%, 0.17%, 0.11%, and 0.11%, respectively, nt2999-3157 in preS1, nt529-677 in S, nt1955-2016 in C, nt923-1001 and nt2489-2602 in P genes were among the hottest muta-tional spots throughout the HBV genome. Conclusion HBV mutation may occur in all the open readingframes after passive and active immunoprophylaxis. In addition to S gene, HBV preS and P genes could alsoassociate with the escape mutants.
6.Determination of national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases

Objective: To determine the national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases (CQQC) according patients with hypertension, coronary artery disease(CAD), and congestive heart failure(CHF)and to analyze the influence factors. Methods: The quality of life was estimated by CQQC to evaluate 7937 persons normal and with cardiovascular diseases(contain with hypertension, CAD, and CHF ) in 28 hospitals of 19 cities across China. The data were compared in gender, age, exercise, psychologic condition, education, number of family member and medical cost. Results: 1、The mean scores(national norms)of normal persons were (95.74±23.21); 2、The scores of hypertension patients were (73.75±23.62), the scores of hypertension stage 3 group (69.06±21.78)were least compared with other stages(P<0.01); 3、The scores of CAD patients were(64.67±20.59), the scores of unstable angina group(55.37±18.62)were least compared with other types in CAD patients(P<0.01); 4、The scores of CHF patients were (40.36±17.24), it was least compared with those of hypertension and CAD patients (P<0.01); 5、Influence factor: (1)Sex, age: The mean scores were (71.41±27.8)for 7937 persons, the scores of male persons were significantly higher than those of female (P=0.000). In normal persons, along with age increased, the scores decreased (P<0.05) ; except ≥70 ages group, the scores of male persons were higher than those of female(P<0.05) ; (2)Exercise: The scores of patients participating exercise were significantly higher than those of no participating exercise [(72.64±21.87)vs.(66.91±23.13), P=0.000]; (3)Psychological condition: The scores of patients with healthy psychological condition were significantly higher than those of with psychologic disorder [(70.13±25.66)vs.(68.91±20.44), P=0.001]; (4)Other: patients with better education, more than 1 family members and medical insurance or public expense had more scores (P<0.05~0.001). Conclusions: The sample size is large and the datum is reliable for Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases. It can reflect influence of age, sex, exercise, psychologic condition, family, education background, medical condition and disease, its item is brief, clear, and operation is easy, so it may well be spread.
7.Health investigation of ACS patients and related nursing strategies

Objective To investigate the clinical features of ACS (Acute coronary syndrome)patients and promote related nursing strategies so as to improve ACS patient'S life quality.Methods A total of 220 consecutive patients with chest pain.Who were admitted in our institution between 0ctober 2006 and October 2007,were divided into a normal control group,a SAP(Stable angina pectofis)group,and ACS group.Baseline clinical characteristics were coilected and evaluated by a health investigation and analysis.Results Compared with 66 controls,ACS patients were older(P<0.01),prominently in male(P<0,05),more cases having smoking habits(P<0.01)and drinking habit(P<0.01);ACS patients had higher BMI values(P<0.01), higher WHR valUes(P<0.01)and were more overweight(P<0.01)and abese(P<0.05)than controls. When compared with controls,SAP patients and ACS patients had higher TG and LDL-c level(SAP VS.Controls, P<0.05;ACS VS.Controls,P<0.01).When comparing SAP patients with ACS ones,no significant differences were found in LDL-C levels (P>0.05).However,ACS patients did have lowest HDL-c levels among the three groups.Condusioas We,as nurses,should promote the timely and accurate nursing strategies based on the clinical features of ACS patients,in order to improve the nursing quality and the patients'life quality.
8.Nursing requirements and status of anxiety and depression incidence among senile patients with acute coronary syndrome

Objective To study the influencing tactors of senile acute coronary syndrome ( ACS) patients with anxiety and depression and related nursing requiements.Methods Totals of 256 senile ACS patients were investigated with Zung self-rating anxious scale (SAS),Zung self-rating depressive scale (SDS) and self-designed observation scale to understand the incidence of anxiety and depression.Related influencing factors,nursing requirements were analysed.Results Different degrees of anxiety and depression were found in all the patients.121 cases (47.26%) presented with anxious symptoms,47 cases( 18.36% ) presented with depressiive symptoms,and 33 cases ( 12.89% ) presented with both anxious and depressive symptoms.The incidence of anxiety and depression were 78.51%.Logistic regression analysis showed factors such as knowledge related disease,behaviors,hospital environment,and social supporting systems were closely related to the incidence of anxiety and depression ( P < 0.05 ).Patients with anxiety or (and) depression had more problems such as hypochondriacal idea,discomfort complaint,inattention and so on compared with that no symptom patients( P < 0.05 ).Patients with anxiety or(and) depression needed more nursing requirements such as the time of oxygen,back paddling,massage and psychological intervention than patients without those mental symptoms( P < 0.05 ).Conclusions It is important to strengthen psychological nursing and targeted health education in order to reduce the incidence of anxiety and depression among senile ACS patients,improve their disease and life quality.

10.Establishment and practice of the teaching mode of multidisciplinary collaboration on nursing training among elderly patients in ICU

Objective To investigate the on-job training mode suitable for ICU nursing staff for elderly patients,in order to improve the comprehensive treatment and nursing capabihties of ICU nursing staffs in critically ill elder patients with a variety of diseases.Methods Nurses from coronary care unit (CCU),respiratory intensive care unit (RICU),and surgery intensive care unit(SICU) were recruited to participated in the training.Every three months were considered as a training cycle,each ICU took responsibility for one-month training program for the participants in turns.Satisfaction rate and examination scores concerning the first aid theory and first aid skills of nurses were evaluate and compared before and after training.Results After receiving the training program,satisfaction rates for the first aid theory and the first aid skills of nurses in patients and their families were 96.6% and 95.2%,and higher than that were 83.7% and 86.3% before the training program; 95.5% and 93.7% among doctors after the training were higher than 80.9% and 84.6% before the training program; 98.7% and 97.6% among nursing staffs after the training were higher than 80.9% and 84.6% before the training program; and these differences were statistically significant between before and after the training (x2 =8.900,4.395,5.224,4.940,12.890,7.680,respectively; P <0.05).The examination score of the first aid theory before the training was (59.2 ± 4.2) and lower than that was (89.7 ± 3.7) after the training,the examination score of the first aid skills of nurses before the training was (63.8 ± 4.6) and lower than that was (90.3 ± 5.3) after the training,these differences were statistically significant between before and after the training (t =-59.12,-41.06,respectively; P < 0.01).Conclusions The establishment of the teaching mode of multidisciplinary collaboration on nursing training among elderly patients in ICU,which can develop the comprehensive treatment and nursing capabilities of ICU nursing staffs and improve the satisfaction rate for nursing job and the rate of successfully rescuing critically ill patients,can be used to achieve the sharing of human resources,medical instruments and information resources,and is feasible in nursing practice in ICU and more worthy of clinical promotion and application.
