1.Essence of defensive qi and its medical significance

In traditional Chinese medicine,medical practice is combined with qi,which forms special qi-theory.Body fluid which includes blood and Jin-ye is the fluid structure of human being.Nutrient qi and defensive qi depend on these two kinds of body fluid.Defensive qi not only maintains physiological functions of Jin-ye,but also participates pathomechanism such as Jin-ye consumption,retention,etc.The identity of defensive qi and Jin-ye plays an important role in theory and clinic.
3.The short-term prognostic evaluation in first-drug therapy outcome in children with absence epilepsy

Objective To observe the initial therapy outcomes of Valproic acid for childhood absence epilepsy (CAE),and to assess its therapeutic reaction and short-term prognosis,and to investigate the risk factors for initial therapy failure.Methods From January 2010 to December 2015,absence seizures as key words were used to search CAE in the video-electroencephalogram(VEEG) database of Children's Hospital Affiliated to Nanjing Medical University.Sixty-seven children fulfilled the CAE diagnosis criteria of International League Against Epilepsy in 1989.These patients were separated into 2 groups based on the therapy outcome after 6 months,including seizure control failure group and seizure control group.The patients' clinical characteristics and VEEG characteristics were reviewed.The gender,age of seizure onset,a family history of epilepsy or febrile seizures (FS),consistent with 2005 Panayiotopoulos diagnostic criteria,and VEEG findings were analyzed to evaluate the predictive ability of independent variables and the relationship between these features and treatment outcomes by using a stepwise multivariate Logistic regression model.Results The age at seizure onset was (5.89 ± 2.91) years old,and follow up duration was 6 months.Approximately 23 cases (34.3%) of CAE patients had poor response.No statistical correlation was made for gender,age at onset,the occurrence of generalized tonic clonic seizures,and family history of FS or epilepsy between 2 groups (all P >0.05).Compared with seizure control group,patients of the seizure control failure group had significantly higher rates of focal epileptic discharge (87.0% vs.6.8%),higher rates of intermittent photic stimulation (IPS) induced seizures (52.2% vs.6.8 %),fewer rates of occipital intermittent delta activity (8.7 % vs.77.3 %),and fewer rates of patients met the new diagnostic criteria proposed by Panayiotopoulos in 2005 (8.7% vs.88.6%) (all P < 0.001).The presence of occipital intermittent rhythmic delta activity during wake stage and the interictal focal epileptiform discharges on VEEG during sleep stage were significantly associated with the therapy outcomes in a multivariable Logistic regression analysis (OR =133.714,P < 0.05;OR =0.068,P < 0.05).Conclusions The presence of focal epileptiform discharges,and a typical absence induced by IPS are important factors for first-drug treatment failures with CAE,and conversely the occipital intermittent rhythmic delta activities have a good efficacy.There is no statistical correlation between clinical characteristics and treatment outcomes.
4.A novel fear conditioning memory model formation and erasing by foot-shock in tree shrew

Objective To investigate the way of fear conditioning memory model evoked and erased by foot-shock in tree shrew. Methods First, detect the tree shrew activities regularly in light/dark box. Second, test a suitable voltage degree of foot shock on tree shrew. Third, investigate the memory formation and erasing of fear conditioning on tree shrew of trial group. Results The duration of tree shrew (n=4) stay in the dark-box was significantly lon-ger than that of in the light box (P<0. 01) in normal condition. In the same environment of two light boxes, given different voltage degrees, the durations of tree shrew (n=6) stay in the stimulating chamber gradually reduced and the durations of tree shrew stay had significant difference between stimulatus chamber and no stimulatus chamber when the stimulus voltage up to 12 V ( P<0. 05 ) , 16 V ( P<0. 01 ) and 20 V ( P<0. 01 ) . The animal of trial group ( n=4 ) could build up the fear conditioning memory of the dark box with the stimulus of 16 V foot-shock in the dark box ( P<0. 001 ) . After formation of the fear conditioning memory, the same stimulus in light box ap-peared for 4 days. The durations of tree shrew stay in trial group (n=4) decreased in light box, and there was no significant difference between the trial group and the control group. Conclusion Tree shrew prefers to stay in the dark box. The suitable voltage for foot-shock on tree shrew is 16 V. The fear conditioning memory can be evoked and erased by foot-shock.
5.Hexokinase-Ⅱ role in tumor growth and targeting therapy

The energy for tumor cells mainly derives from the aerobic glycolysis,that is,the Warburg effect,which also provides a large amount of precursor substances for the growth of tumor cells.Hexokinase-Ⅱ (HK-Ⅱ),highly expressed in tumor tissue,is the rate-limiting enzyme of glycolysis and closely related to the energy metabolism of tumor.Recent studies have showed that HK-Ⅱ not only mediates Warburg effect,but also promotes tumor proliferation by inhibiting tumor cell apoptosis and regulating autophagy.It has been confirmed that blocking HK-Ⅱ gene expression and inhibiting HK-Ⅱ with small molecule inhibitor can kill tumor cells in many kinds of cancer.Agent targeting HK-Ⅱ may become a new generation of targeted drugs.
6.The homing of transplanted bone marrow mesenchymal stem cells to the thymus

BACKGROUND:Bone marrow mesenchymal stem cels have low immunogenicity and can induce immune tolerance. At present, the mechanism of immune regulation of bone marrow mesenchymal stem cels is not completely understood. It has been rarely reported whether the bone marrow mesenchymal stem cels can migrate to the thymus after transplantation.
OBJECTIVE:To observe the distribution and survival of bone marrow mesenchymal stem cels in the thymus of aging rats after transplantation.
METHODS: Bone marrow mesenchymal stem cels cultured in vitrowere transfected by adenovirus vectors expressing green fluorescent protein. Transfected bone marrow mesenchymal stem cels were injected into the portal vein of aging rats. At days 3, 7, 14, 21 after transplantation, the survival of bone marrow mesenchymal stem cels homing to the thymus was observed under fluorescence microscope. At day 3 after transplantation, thymus tissues were taken and stained with hematoxylin-eosin for pathological observation.
RESULTS AND CONCLUSION:Green fluorescent protein-labeled bone marrow mesenchymal stem cels had a strong green fluorescence at days 3 and 7 after transplantation, and the cel contour was clear. There was no significant difference in the mean absorbance values at days 3 and 7 (P> 0.05). Expression of green fluorescent protein was weakened significantly at days 14 and 21 compared with that at day 3 (P < 0.05). At 3 days after transplantation, the transplanted bone marrow mesenchymal stem cels were clearly visible in the thymus, and acute rejection was not observed. The results show that bone marrow mesenchymal stem cels can migrate to the damaged thymus tissue through the blood circulation, and can survive at least 1 week.
7.Analysis of 120 cases of reduction of postpartum bleeding in cesarean section by misoprostol

Objective To observe the effects of misoprostol on the reduction of postpartum bleeding in cesarean section.Methods With oxytocin as control,the amount of bleeding during and two hours after cesarean section was observed.Results The mean amount of bleeding was (232.0±46.6) ml in misoprostol group and (385.0±74.7) ml in oxytocin group respectively.The difference between the two groups was significant (P<0.01).Conclusion Misoprostol is more effective to improve uterin contraction than oxytocin and is better in the reduction of postpartum bleeding with safety and convenience.

[Objective]To investigate the relationship between degeneration of cartilage and the expressions of Matrix Metalloproteinase 7(MMP-7),Matrix Metalloproteinase 9(MMP-9),Matrix Metalloproteinase 13(MMP-13)and Tissue Inhibitor of Matrix Metalloproteinase 1(TIMP-1)in osteoarthritis.[Method]The histological changes of cartilages by hematoxyllin-eosin staining and immunohistochemical expression of Matrix Metalloproteinase 7(MMP-7),Matrix Metalloproteinase 9(MMP-9),Matrix Metalloproteinase 13(MMP-13)and Tissue Inhibitor of Matrix Metalloproteinase 1(TIMP-1)in osteoarthritis.were studied in 20 osteoarthritis cases and 2 normal controls.All data were statistically analyzed by Mann-Whitney U test and correlation analysis.[Result]The osteoarthritis cartilage underwent fibroplasias and tearing.The quantity of chondrocyte increased and the clustered and hypertrophic cells came into being.Little immunostaining of MMP-7 and MMP-13 was observed in normal cartilage,while their expressions increased in degenerated cartilage(P0.05),superficial layer of the moderate and end-stage osteoarthritis.However,it significantly increased in deep layer(P
9.Related factors and clinical applications of therapeutic alliance in psychotherapy: A review

Therapeutic alliance is one of the most popular topics in psychological counseling and psychotherapy research,but the concept of it is not unified.Despite of its various versions,most scholars regard it as an important relationship variable,emphasizing the agreement between therapist and patient as to the goals and tasks of treatment,and the affective bond established in the therapeutic dyad.Related research show that the characteristics of therapist and patient,and the interaction in this therapeutic dyad are the related factors on the formation and development of therapeutic alliance,which constitutes a major clinical variable in explaining and predicting the course and outcome of treatments.Future researches should further clarify the related factors of therapeutic alliance,expand the clinical applications of therapeutic alliance and strengthen the exploration of indigenization.
10.New insights into pediatric acute respiratory distress syndrome

Pediatric acute respiratory distress syndrome (PARDS) is one of the most severe disease in pediatric critical care medicine with high mortality.Pediatric practitioners have recognized that ARDS in children is different from ARDS in adults.In the absence of identification of these differences,however,children have been characterized as having ARDS based on the adult definitions.Therefore,the managements for PARDS were conducted without specific considerations of children,and have limitations when applied to patients.With the purpose to highlight the gaps of ARDS between children and adults,the new insights into PARDS on the epidemiology,pathophysiology,diagnosis,treatment and prognosis in the recent years were summarized.
