1.Adjustments and countermeasures of China's medical foreign investment policy
Chinese Journal of Health Policy 2017;10(9):75-80
Since the reform and opening up, the government of China has continuously adjusted its foreign medical policy and vigorously guided foreign investment to set up medical institutions in pilot provinces and cities. Based on healthy China 2030 plan, the paper summed up the foreign medical policy adjustment's process from its establishment, stagnation, and development to reform in China. The government of China liberalizes the market for foreign medical gradually, but the foreign medical institution is facing a series of bottlenecks, such as the lack of le-gal regulations, the cumbersome of approval procedures, and the limit of development and so on. The development of foreign medical care should be under the guidance of the country's legal and policy. Foreign medical institution can carry out some special medical care to improve the health of the residents.

Objective To obsevre the clinical efficacy of acupuncture at bilateral sphenopalatine ganglions in treating allergic rhinitis. Methods Patients with allergic rhinitis were selected, and then were randomly divided into bilateral group, unilateral group, and control group, with 35 cases in each group. By the end of the study, 5 cases of bilateral group, 3 cases of unilateral group, and 2 cases of control group were removed. The bilateral sphenopalatine ganglions were acupunctured simultaneously in bilateral group, and the unilateral sphenopalatine ganglion was treated by acupuncture in unilateral group, once a week, for 4 weeks. The control group received routine acupuncture on Yingxiang (LI20), Bitong (EX-HN8), Yintang (EX-HN3), and Hegu (LI4), twice a week, for 4 weeks. Rhinitis symptom scale and life quality of nasal conjunctival scale score before and after treatment in the three groups were observed. The clinical efficacy was evaluated and the adverse reactions were recorded. Results The total effective rates of bilateral group, unilateral group and control group were 93.33% (28/30), 90.63% (29/32) and 72.73% (24/33), respectively, and the bilateral group and unilateral group were better than the control group (χ2=19.507, P=0.001), without statistical significance between bilateral group and unilateral group (P>0.05). Rhinitis symptoms and life quality were improved in the three groups (P<0.05), and the bilateral group and unilateral group were better than the control group (P<0.05), without statistical significance between bilateral group and unilateral group (P>0.05). Only 1 case of subcutaneous hematoma showed in unilateral group. Conclusion Acupuncture at sphenopalatine ganglions has confirmed efficacy, and there is no difference in the efficacy between acupuncture on bilateral sphenopalatine ganglions and unilateral sphenopalatine ganglion.

Objective To investigate the natural head position and cervical posture in the patients with different vertical facial patterns with skeletal class Ⅰ relationship, and to clarify the relationships between vertical facial patterns and natural head position, cervical posture in the patients with skeletal class Ⅰ relationship. Methods 94 patients aged 8- 15 years old with skeletal class Ⅰ relationship were selected as the subjects. The subjects were classified into high angle group, average angle group, and low angle group according to their mandibular plane angles. The cephalometric radiographs in natural head position were taken, and the variables representing craniofacial morphology, head position, and craniocervical posture were measured. The intergroup differences in variables were compared. Results The inclinations of mandible to the true vertical and cervical column (ML/VER,ML/OPT,ML/CVT)were smallest in high angle group, and they were largest in low angle group (P <0.01);other variables representing head posture and craniocervical posture (NSL/VER,FH/VER,NSL/OPT,NL/OPT,NSL/CVT,FH/CVT)were largest in high angle group, and they were smallest in low angle group (P <0.05).The inclinations of ramus to cervical column (RL/OPT,RL/CVT)were largest in high angle group and they were smallest in low angle group (P <0.01).There were no significant differences in the inclinations of cervical column and cervical lordosis (OPT/HOR,CVT/HOR,OPT/CVT)among three groups (P >0.05). Conclusion The significant differences exist in natural head position and cervical posture among the patients with skeletal class Ⅰ relationship with different vertical facial patterns (high angle, average angle, low angle);the subjects with high angle show extended head position and cervical posture, while the subjects with low angle exhibit the opposite tendency.

Objective To systematically assess the diagnostic performance of CTA for lower extremity peripheral arterial disease (PAD) using a Meta analysis method. Methods Studies were located through electronic searching of the PubMed, EBSCO, Springer, Ovid, CNKI, Cochrane library (from the date of establishment of the databases to October 2009 ). Bibliographies of the retrieved articles were also checked. All the studies concerning the diagnosis of PAD using CTA had been searched and reviewed, and the studies with the DSA as the gold standard were adopted as eligible. Subsequently, the characteristics of the included articles were appraised and extracted. Data on accuracy of included studies were extracted for further heterogeneity exploring, statistical pooling and SROC ( summary receiver operating characteristics)analyzing using the Meta Disc 1.4 software. Results Totally 24 studies met the inclusion criteria with a total of 1096 patients. The heterogeneity was found in these studies. The pooled accuracy indicators like sensitivity, specificity, and diagnostic odds ratio (DOR) were 0.95 ( 95% CI:0.94-0.95 ), 0.96 ( 95% CI:0.95-0.96), and 471.13 (95% CI:242. 92-913.71 ), respectively. The area under of SROC curve was 0.9888 and the Q index was 0.9555. Subgroup analysis demonstrated significant difference on diagnostic performance for various CT slices (P < 0.05 ). Conclusion CTA can be regarded as an effective and feasible method for PAD diagnosis and screening, based on the results of this systematic review. However, more rigorous evaluations of CTA in patients with critical limb ischemia are needed.

Chinese Journal of Thoracic and Cardiovascular Surgery 2014;30(4):242-246
Objective To investigate whether Th17/Treg imbalance existed, and whether VEGF165 attenuated the imbalance in allogeneic skeletal myoblasts transplantation(allo-SMT) for acute myocardial infarction (AMI).Methods There were 60 C57BL/6 male mice,3 to 4 weeks old, weighting 0.016-0.020 kg.On the day 1,2,4,and 7 after allo-SMT,the percentages of Th17 and Treg cells, and the ratios of Th17/Treg cells were analyzed by FCM in AMI group,AMI-S group(alloSMT) and AMI-Ⅴ group(with VEGF165 treatment).Subsequently, related proinflammatory and regulatory cytokines and key transcription factors ROR-γt mRNA and Foxp3 mRNA expression were examined by Bio-plex and RT-PCR respectively. Results On the day 1,2,4,and 7 after allo-SMT,the percentage of Treg, related cytokines concentrations and transcript factor Foxp3 mRNA in AMI-S group were lower than those in AMI group, while the AMI-Ⅴ group were higher than those in AMI groups. However, the percentage of Th17 cells, related cytokines concentrations, and ROR-γt mRNA in AMI-S group were higher than those in AMI group; but the AMI-Ⅴ group were lower than those in AMI group.Apparently, compared with AMI group, the ratios of Th17/Treg significantly ascended in AMI-S group and decended in AMI-Ⅴ group. Conclusion Th17/Treg imbalance participated in the formation and development of inflammation and immune response after allo-SMT.However,transfected VEGF165 could relieve the severity of Th17/Treg imbalance.
