1.Clinical outcomes of tibial plateau fractures (SchatzkerⅠ,Ⅱ) treated with arthroscopy-assisted operative management and locked by LCP

[Objective]To discuss the clinical outcomes of low-energy tibial plateau fractures treated with arthroscopy-assisted operative management and locked by LCP.[Method]From January 2006 to March 2007,15 patients with tibial plateau fractures(SchatzkerⅠ,Ⅱ)were reduced with arthroscopy-assisted and treated by LCP and bone defects were filled with homogeneous allograft bone.[Result]With follow-up visits for 12~26 months,all cases were healed.According to creiteria of Rasmussen,excellent were in 10 cases,good in 4 cases,fair in 1 case,with the rate of being excellent and good added up to 93%.[Conclusion]LCP fixation is an effective technique for the low-energy tibial plateau fractures treatment with the advantage of less invasion,steadier fixation and lower complication rate.Arthroscopy-assisted operative management can help to reduce reduction loss from intra-articular more precisely to reach a higher union rate.It is also a preferable method for low-energy tibial plateau fracutures treatment(SchatzkerⅠ、Ⅱ).

4.Efficacy analysis for adolescent idiopathic scoliosis treated with Milwaukee or Boston orthosis

ObjectiveTo evaluate the efficacy about the different wearing manner with Milwaukee and Boston for the treatment of adolescent idiopathic scoliosis (AIS).MethodsRetrospectively summarization and analysis was performed in 85 adolescent patients with idiopathic scoliosis who were treated from February 2004 to March 2009.The skeletal growth of them had not completed.There were 57 cases who received brace treatment.In them,28 were treated with (CTLSO) Milwaukee brace,and the rest were treated with (TLSO) Boston brace and orthopedic gymnastics.The 30 cases wear regularly used for 21 to 23hours per day,and 27 cases could consist on wearing only 6 to 15 hours per day.The other 28 cases of 85AIS cases were only treated with orthopedic gymnastics instead of orthosis treatment.All patients were periodically observed with lateral side (X)-ray photograph at standing position and photograph,and Cobb angle and Risser sign were measured every 3 to 6 months.Since 2008 all adolescent idiopathic scoliosis patients treated with orthosis were requested to fill with simplified Chinese SRS-22.Results73 cases adolescent idiopathic scoliosis patients were followed up for 2 ~5 years [ mean(26.3 ± 33.7)months ].Milwaukee orthosis group showed the regular wearing group had 91.67 % ( 11/12) effective rate and the intermittent wearing group had 56.25% (9/16) effective rate and the group without wearing orthosis only had 20% (4/20)effective rate.The group regularly wearing Milwaukee brace had superior effect than the other two groups (P < 0.05 ).Boston orthosis group showed the regular wearing group had 88.89% (16/18) effective rate and the intermittent wearing group had 54.55% (6/11 ) effective rate and the group without wearing brace with 25% (2/8) effective rate.The group regularly wearing Boston brace had better effect than the other two groups ( P < 0.05 ).Due to the different choice of AIS patients and orthosis,the effective rate of the Milwaukee and Boston orthosis was not compared.ConclusionsThe adolescent idiopathic scoliosis patients should insist on regularly wearing brace regardless of the Milwaukee or Boston orthosis ( this article suggest that the wearing time should not less than 21 ~ 23 h/d).The group regularly wearing with the Milwaukee or Boston orthosis had better effect than the intermittent group or the group without wearing brace.It's a good treatment for the AIS patients who have with the indication of orthosis treatment.
5.Epidemiological study on disabilities in 24128 newly detected leprosy patients in China

ObjectiveTo approach the status of leprosy disabilities in newly detected cases in recent 11 years in China.MethodsBased upon the records from the National Leprosy Recording and Reporting System in National Center for STD and Leprosy Control, 24128 leprosy cases detected during 1989-1999 in China were analyzed in terms of leprosy disbility.ResultsThe proportion of disabilies in newly detected leprosy cases in 1989 in whole country was 46.49% and decreased to 32.7% in 1999, and the proportions of cases with grade Ⅱ disabilities were 25.55% (1989) and 22.06% (1999). There were 19 provinces where disability rate was more than 40%. Out of patients with disabilities, those with grades Ⅰ and Ⅱ disabilities and with deformities (loss of eyebrow, facial paralysis or saddle nose) accounted for 37.54%, 61.04% and 1.42%, respectively. There were 20 provinces where grade Ⅱ disabilities accounted for more than 50% of all patients with disabilities. The disability rates in patients aged under 15 years, 15-65 years and over 65 years were 24.74%, 39.3% and 53.33%. The patients with a delay in detection of 2 years had a disability rate of 28.95%, and those with a delay of more than 2 years and 5 years had the rates of 48.06% and 60.95%, respectively. The disability rate was 53.76% in patients with leprosy reactions. The grade Ⅱ disability rate in paucibacillary patients (28.53%) was significantly higher than that in multibacillary ones (22.03%). Conclusions Disability rate of leprosy in newly detected cases is still high although it has decreased in the recent 11 years. The rate is associated with delay in detection, leprosy reaction and leprosy type. It suggests that early detection of leprosy patients, regular treatment with multidrug therapy, and management of leprosy reactions will be the effective measures to prevent disabilities of leprosy.
6.Expression and significance of PCNA and p27 in benign prostate hypertrophy and prostate carcinoma.

OBJECTIVETo investigate the expression of PCNA and p27 in human benign prostate hypertrophy (BPH) and prostate carcinoma (PCa) and their effect on the genesis and progression of the tumor.
METHODSThe paraffin-embedded sections of 30 cases with BPH and 37 cases with PCa were collected. The expression of p27 and PCNA protein were examined by S-P immunohistochemical method. Comparative analysis for BPH and pathological grade and clinical stage of PCa was performed.
RESULTSThe expression of PCNA in BPH (3.3%) was significantly lower than that in Pca (83.8%, P < 0.01). The expression of p27 in BPH (70.0%) was significantly higher than that in Pca (27.0%, P < 0.05). The expression of p27 was not correlated with histological grade and clinical stage in Pca (P > 0.05). An inverse correlation was found between p27 and PCNA expression in BPH (P < 0.01), while no correlation was found in Pca (P > 0.05).
CONCLUSIONThe loss or decreased expression of p27 protein may be related to the genesis of benign prostate hypertrophy, but not to the development of prostate carcinoma; the overexpression of PCNA may play an important role in the malignant behavior and progression of prostate carcinoma.

7.RAPD analysis for genetic diversity of Chrysanthemum morifolium.

OBJECTIVETo investigate the genetic diversity of Chrysanthemum morifolium at the level of molecular biology.
METHODThe total genomic DNA was extracted from medicinal chrysanthemums by 2% CTAB method. And the genetic diversity of 22 C. morifolium accessions was tested by RAPD marks. The NTSYS software was used to analyze the marks.
RESULT26 10-mer arbitrary primers were found to acquire polymophic results. A total of 233 bands were amplified, of which 89.7% bands were found to be polymophic. 8.04 polymophic bands were amplified by each primer on the average. The results of cluster analysis by using UPGMA method showed that all the tested accessions could be differentiated by RAPD marks.
CONCLUSIONThere actually existed much genetic diversity at the molecular level among the germplasm resources of C. morifolium. RAPD marks could be effective tools to construct DNA fingerprintings of C. morifolium. The differences between the tested chrysanthemums are related to the environments. However, it was affected by genetic facters more significantly.

8.Pharmacokinetics of Maxing Shigan decoction in normal rats and RSV pneumonia model rats by HPLC-MS/MS.

To establish a LC-MS/MS method to determine the concentrations of liquiritin, glycyrrhizin, glycyrrhetinic acid, amygdalin, amygdalin prunasin, ephedrine, pseudoephedrine and methylephedrine of Maxing Shigan decoction in rat plasma, and study the differences on their pharmacokinetic process in normal rats and RSV pneumonia model rats. After normal rats and RSV pneumonia model rats were orally administered with Maxing Shigan decoction, the blood was collected from retinal vein plexus of different time points. Specifically, tetrahydropalmatine was taken as internal standard for determining ephedrine, while chloramphenicol was taken as internal standard for determining other components. After plasma samples were pre-treated as the above, the supernatant was dried with nitrogen blowing concentrator and then redissolved with methylalcohol. The chromatography was eluted with mobile phase consisted of acetonitrile and 0.1% formic acid solution in a gradient manner. ESI sources were adopted to scan ingredients in ephedra in a positive ion scanning mode and other ingredientsin a negative ion scanning mode. The multiple-reaction monitoring (MRM) method was developed the plasma concentration of each active component. The pharmacokinetic parameters of each group were calculated by using Win-Nonlin 4.1 software and put into the statistical analysis. The result showed the plasma concentration of the eight active ingredients, i.e., liquiritin, glycyrrhizin, glycyrrhetinic acid, amygdalin, amygdalin prunasin, ephedrine, pseudoephedrine and methylephedrine within the ranges of 1.04-1040, 1.04-1040, 0.89-445, 1.05-4200, 1.25-2490, 0.3-480, 0.3-480, 0.3-480 microg x L(-1), with a good linearity and satisfactory precision, recovery and stability in the above ingredients. After modeling, except for glycyrrhetinic acid whose pharmacokinetic parameters were lacked due to the data missing, all of the rest components showed significant higher Cmax, AUC(0-1) and lower clearance rate (CL) than that of the normal group, indicating the increase in absorption in rats in the pathological state by reducing the clearance rate. The method is accurate and sensitive and so can be used to determine the plasma concentrations of the eight active ingredients in Maxing Shigan decoction. RSV pneumonia-infected rats absorbed more ingredients in Maxing Shigan decoction.

9.Relationship between serum estradiol levels in the early postnatal period and the occurrence of hyaline membrane disease and bronchopulmonary dysplasia in neonates.

OBJECTIVEThis study examined the changes of serum levels of estradiol during the early postnatal period in neonates in order to investigate the possible relationship between the serum estradiol levels and the occurrence of pulmonary hyaline membrane disease (HMD) and bronchopulmonary dysplasia (BPD).
METHODSFifty-nine premature infants with the gestational age between 26 and 32 weeks and 61 full-term infants with the gestational ages between 37 and 42 weeks were enrolled. Serum levels of estradiol were measured on postnatal days 1, 3 and 7.
RESULTSSerum levels of estradiol decreased rapidly after birth in both premature and term infants and there were significant differences among different postnatal ages groups. However, there were no significant differences in the serum estradiol levels between the premature and term groups on postnatal days 1, 3 and 7. Serum estradiol levels measured in premature infants with HMD were not statistically different from those in premature infants without HMD on all time points. Serum estradiol levels in premature infants with BPD were higher than those in premature infants without BPD on postnatal day 3, but there were no noticeable differences on postnatal days 1 and 7.
CONCLUSIONSSerum estradiol levels decline rapidly within 7 days after birth in both premature and term infants. Serum estradiol levels in the early postnatal period are not associated with the occurrence of HMD and BPD, suggesting that serum estradiol in the early postnatal period can not be used as a marker for predicting the development of HMD and BPD.

METHODBased on the random blocks field experiments design, the main character of leaf and capitulum of medicinal C. morifolium were observed, it was studied on that the botanical character of different cultivated types medicinal C. mortfolium in China were comparatively study with statistical means.
RESULT AND CONCLUSIONThere are some different of botanical character among different cultivated types medicinal C. morifolium in China, which can be divided into 7 colonies according as its diversity of the botanical and morphological character. The sawtooth number of lamina, leafstalk length and leaf division number are the important laminae morphologic character of different types medicinal C. morifolium, which are relative independent characters.
