Chinese Journal of Geriatrics 2017;36(8):864-867
Objective To study the risk factors of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) induced by community-acquired Pseudomonas aeruginosa(PA) infection which provides the basis for early clinical intervention in high-risk patients. Methods Retrospective analysis of clinical data from 236 patients with acute exacerbation of COPD was conducted in Department of Respiratory Medicine of our hospital from January 2015 to December 2015.The patients were divided into PA group(n=44)and the non-PA group(n=192)according to respiratory tract pathogen infection. The univariate analysis and multivariate logistic regression analysis were performed between the two groups to analyze risk factors for AECOPD induced by PA infection. Results Univariate analysis showed that hypoalbuminemia,AECOPD-induced hospitalization in the recent one year, application of broad spectrum antibiotic within recent 90 days, intermittent use of systemic corticosteroids within recent 90 days, lung function grading of GOLD3/4 level in COPD,acute exacerbation frequency> 1 time in last year, the BODE index, walking distance during six minutes,mMRC grading, and CAT scores were significantly correlated with occurrence of community acquired PA-induced AECOPD(all P< 0.05).Logistic regression analysis showed that higher BODE index,AECOPD-induced hospitalization in the last year, the use of broad spectrum antibiotic within 90 days and hypoalbuminemia were independent risk factors for community acquired PA induced-AECOPD(OR =14.573、18.958、4.170、5.112,all P<0.05).Conclusions We suggest that the COPD patients having a higher BODE index scores should be clinically intervened early to reduce the occurrence of AECOPD-induced hospitalization, should receive active etiology examination, and should receive therapies for community-acquired Pseudomonas aeruginosa (PA)infection with specific narrowspectrum antibiotics according to the drug susceptibility and multiple comprehensive measurements, and for correcting hypoalbuminemia.
3.Influence of exercise training and angiotensin-converting enzyme inhibitor on blood pressure, aortic compliance and cardiovascular tissue in spontaneously hypertensive rats
Chinese Journal of cardiovascular Rehabilitation Medicine 2012;21(5):456-462
Objective: To explore influence of exercise training and angiotensin-converting enzyme inhibitor (perindopril) on blood pressure, aortic compliance and cardiovascular tissue structure in spontaneously hypertensive rats (SHR). Methods: A total of 38 SHR aged seven weeks were divided into control group (C group, n=9), exercise group (Exe group, n=9), perindopril group (Per group, n=10) and perindopril + exercise group (Per+Exe group, n=10). SHRs were observed for eight weeks, and tail-cuff method was used to measure systolic blood pressure(SBP)once a week. When rats were 15 weeks old, regression line(once linear function)was obtained by single regression analyzing the relation between blood pressure and pulse wave velocity(PWV). The b of this once linear function was used as index of aortic compliance(there was negative relationship between b and aortic compliance). The left ventricular mass (LVM)/body weight(BW)was used to evaluate ventricular thickening, and aortic weight (AO)/ BW was regard as index of vascular thickening. Results: Compared with C group, SBP of Exe group significantly decreased on 12 weeks old (P<0.01) and 13 weeks old (P<0.05), but decrease of SBP was no significant on 14 weeks old(P>0.05). SBP of Per group and Per+ Exe group on eight weeks old immediately decreased to level of seven weeks old (P<0.0001),but there was no significant difference between the two group(P>0.05). On 15 weeks old, the LVM/BW [(2.19±0.36) mg/g, (2.16±0.32) mg/g ] and AO/BW [(0.20±0.01) mg/g, (0.19±0.01) mg/g] of Per group and Per+ Exe group significantly decreased compared with C group [(2.95±0.58) mg/g, (0.26±0.01) mg/g], P<0.05 all, but there was no significant difference in these indexes between Per group and Per+ Exe group and between C group and Exe group(P>0.05 both). The b (index of aortic compliance)were [(C group :(21.2±1.2), Exe group:(20.5±1.4), Per group:(14.8±1.6),Per+Exe group:(15.0±1.3)] respectively, there was no significant difference between Per group and Per+ Exe group and between C group and Exe group(P>0.05 both). Conclusion: The exercise training of eight weeks possesses certain lowering blood pressure tendency, but no significant influence yet for aortic compliance and cardiovascular tissue structure in spontaneously hypertensive rats.
4.The explanation and analysis of 2009-hemophagocytic lymphohistiocytosis
International Journal of Pediatrics 2012;39(3):264-266,270
Hemophagocytic lymphohistiocytosis (HLH) is characterized by hemophagocytosis and a reactive process resulting from prolonged and excessive activation of antigen presenting cells (macrophages, histiocytes) and CD8+ T cells. The majority of genetic causes identified to date affect the cytotoxic function of NK and T cells, crippling immunologic mechanisms that mediate natural immune contraction. The predominant clinical findings of HLH are fevers (often hectic and persistent),cytopenias, hepatitis and splenomegaly.Due to the life-threatening implications of the diagnosis of genetically determined HLH,antiinflammatory therapy, often consisting of steroids, etoposide or antithymocyte globulin, should be instituted promptly, followed by curative hematopoietic cell transplantation.Secondary HLH,associated with autoimmune disorders or viral infections in teens and adults, should control the primary inflammatory.

8.Research on Solid Pressure Sensor for High-pressureMeasurement Based on Fiber Bragg Grating
Chinese Journal of Analytical Chemistry 2017;45(7):980-986
Solid internal pressure is one of the important parameters in traffic bridge and defense field. However, the existing testing equipments have the defects of serious electromagnetic interference, poor reliability, and poor accuracy. To overcome the above problems, a fiber bragg grating (FBG) high pressure solid pressure sensor which used thin plates as elastic bearing diaphragm and applied the deflection under pressure to pull the pressure sensitive grating to realize pressure sensing axial displacement was designed.By calculating on the structure according to the measurement range and finite element simulation, its feasibility was confirmed. In the laboratory with constant temperature, the pressure calibration experiment was carried out, and the temperature compensation was performed by the temperature compensation grating in the same temperature field to correct the temperature drift of strain grating.Experimental results showed that the newly devised FBG pressure sensor could sense the pressure as high as 50 MPa, with a linearity of 99.2%.
9.Probable Sporadic Creutzfeldt-Jakob Disease:A Study of Glucose Metabolism byStatistical Parameter Mapping
Chinese Journal of Medical Imaging 2015;(12):884-887,891
PurposeTo analyze fluorodeoxyglucose (FDG) PET/CT images in patients with probable sporadic Creutzfeldt-Jakob disease (sCJD) and the affected encephalic regions by statistical parameter mapping (SPM) so that image information can be provided for early clinical diagnosis.Materials and MethodsSeven patients with probable sCJD diagnosed according to the WHO criteria and 2009 clinical diagnosis criteria and 7 controls at the matched age were enrolled in the study. Both groups underwent FDG PET/CT scan and the images were analyzed visually and by SPM.ResultsThe visual analysis showed that the patients with probable sCJD had a hypometabolism in the wide pallium and basal ganglia region and that a portion of patients mainly had a lateral decrease. The SPM analysis exhibited that patients in sCJD group had a pattern of hypometabolism that affected bilateral parietal, frontal, occipital cortices and head of caudate (P<0.05) and indicated that the patients with lateral hypometabolism existed crossed cerebella diaschisis (P<0.05).ConclusionFDG PET/CT image features of sCJD patients present a hypometabolism in the wide pallium and basal ganglia region, which may be helpful in the diagnosis of sCJD in certain clinical situations.
10.Effect of obesity on overall survival in lymph node negative breast cancer patients
Chinese Journal of Endocrine Surgery 2016;10(2):152-155,158
Objective To examine the association between baseline body mass index (BMI) and overall survival (OS) in lymph node negative breast cancer patients. Methods From Jan. 1st, 1998 to Dec. 30th, 2008, 662 patients with lymph node negative breast cancer were treated at the Breast Center of Peking University Peo-ple’s Hospital. The clinical factors such as menopause status, tumor stage, body mass index (BMI), total triglyc-eride (TG), total cholesterol (TC) and glucose (GLU) were collected. The chi-square test was used to assess the association between obese and other factors. Log-Rank test and Cox regression analysis were performed to make univariate and multivariate analysis. Results The median follow up was 82 months (ranging from 2 to 209 months). The 5-year and 10-year survival was 93% and 82%. Obesity was associated with GLU and tumor stage (χ2=5.224, P=0.031; χ2=6.007, P=0.14), whereas there was no relationship between obesity and other factors. The Log-Rank test revealed statistically significant better survival for non obesity (χ2=4.693, P=0.030), normal GLU (χ2=4.678, P=0.031) and HR positive patients (χ2=7.837, P=0.005). COX regression analysis showed that BMI (χ2=3.874, P=0.049) and HR status (χ2=6.782, P=0.030) had independent influence on overall survival of node negative breast cancer patients. Conclusion Obesity is an adverse prognostic factor for lymph node negative breast cancer patients, which may be associated with obesity-related disorders such as high blood GLU.
