Chinese Journal of Organ Transplantation 2000;21(4):245-247
Objective To evaluate the efficacy and safety of CsA substituted by tacrolimus(FK506) combined with MMF and prednisone in prevention of rejection in renal transplant recipents with hepatic dysfunction. Methods Eight patients with hepatic dysfunction received the treatment of FK506 substituting for CsA. FK506 was administeted at least 24 h after the last dose of cyclosporine. The initial dose of FK506 was based on body weight, the degree of hepatic dysfunetion and the period after operation of the patients. After administration of one week.the dose of FK506 was subsequently adjusted to maintain its whole blood trough levels between 5~15μg/L. Results Substitute of tacrolimus for cycbsporin resulted in a markedly reduced blood index,direct-bilirubin from(22.66±17.19)μmol/L to(7.05±2.32)μmol/L(P<0.05), indirect-bilirubin from(42.15±34.15)μmol/L to(14.54±2.59)μmol/L(P<0.05), sAL T from (83.00±93,14)IU/L to(29,50±15.41)IU/L(P>0.05),Cr from (177.91±86.41)μmol/L to(135.92±34.05)μmol/L(P>0.05)in one month. The ascites of 3 patients disappeared in one month. Astriction, anorexia, upper limbs trembling appeared only in one patient. Conclusion It is effective and safe to substitute CsA by FK506 combined with MMF and prednisone in prevention of rejection in renal transplant recipients with hepatic dvsfunction.

The severity of carotid atherosclerosis and the levels of serum matrix metalloproteinase-9(MMP-9)in patients with obstructive sleep apnea syndrome(OSAS)increase with the severity of apnea, and that MMP-9 plays an important role in the progression of carotid atherosclerosis. This article reviews the relationship between carotid atherosclerosis and serum MMP-9 so as to analyze the possible mechanism of developing carotid atherosclerosis in patients with OSAS.
5.Two regimes of transient intensive insulin therapy in newly diagnosed type 2 diabetic patients
Chinese Journal of General Practitioners 2008;7(2):86-89
Objective To evaluate two regimes of transient intensive insulin therapy in newly diagnosed type 2 diabetic patients with severe hyperglycemia. Methods Sixty-eight newly diagnosed type 2 diabetic patients with fasting plasma glucose(FPG)≥10.0 mmol/L and glycated hemoglobin A1c (HbA1c)≥8.0%were randomized to either cantinuous subcutaneous insulin infusion(CSII)group or multiple daily injection(MDI)group.Glycemic control, daily dose of insulin, body mass index(BMI),the mean area under the insulin curve(AUC),β-cell function(HOMA-β)and insulin resistence(HOMA-IR) were compared before and after treatment. Results There were no significant differences between two groups in days reaching glycemic control and BMI.The maximal insulin dose for glycemic control in CSII group was(0.53±0.06)U·kg-1·d-1 and that in MDI group was(0.71±0.04)U·kg-1·d-1(t= 11.100,P<0.01).After CSII and MDI treatment,HOMA-β and the mean AUC of insulin were significantly increased(P<0.01),but there was no significant difference between two groups(P>0.05).No significant changes in HOMA-IR were observed before and after treatment or between two groups. There were no severe hypoglycemic episodes in two groups and mild hypoglycemic episodes were 0.34%(13/3806)in CSII group and 1.09%(44/4045)in MDI group. Conclusions The excellent glycemic control can be reached by short.terra CSII and MDI treatment in newly diagnosed type 2 diabetic patients with severe hyperglycemia.Lower insulin dose and less mild hypoglycemic episodes seem to be the advantage of CSII regime than MDI regime.β-cell function can be dramatically improved in both therapy regimes.
6.The correlation analysis among physical knowledge, attitude and self-efficacy of university students
Chinese Journal of Behavioral Medicine and Brain Science 2001;10(2):133-134
Objective:To probe into the relation among the exerise behaviour, physical knowledge, attitute and self-efficacy, and to provide basis for helping to bring about students' lifelong physical exercises and formation of their health life manner, and NCD prevention. Method We randomly sample in cluster 1243 students(male 654 and female 589)studied in different specialities in six colleges or universities in Kunming, which filled in the question and answer paper about students'sports cognition and behaviour.Result The proportion of the male sports population is 51.22% and that of the female is 41.77%. The scores of the self-efficacy, attitute and behaviour exist obvious sex difference. Scores of self- efficacy and behaviour of male students are 13.87 and 13.62,respectively,which are significantly higher than that of the femal are 12.35 and 12.17 (P<0.01), respectively. The exercise attitude, self- efficacy and behaviour of the physical population group have an advantage over that of the case group(P<0.01), meanwhile the difference among groups of students about sports knowledge and the sex difference are not viewed. The exercise behaviour and attitude, the exercise behaviour and self- efficacy of students are positively related to middle extent(r=0.41 and 0.47, resp.),but the exercise behaviour and the knowledge dimension is related to little extent (r=0.12). Conclusion There is a larger difference between students sports population and the aim of physical education in colleges or universities. The sports attitude and self-efficacy of students play an important role which have an effect on the exercise behaviour. The group of the female students is emphasis of intervention.

Objective To investigate the expression and clinicopathological significance of matrix metallo-proteinase(MMP-9) and matrix metalloproteinase tissue inhibiting factor-1 (TIMP-1) in colorectal carcinomas. Methods Immunohistochemistry (PowerVision method) in the paraffin embedded tissue samples was used to deter-mine MMP-9 and TIMP-1 in 42 colorectal carcinomas and its adjacent normal colorectal mucosa. The relationship of their expression with some clinicopathological characteristics was analyzed. Results MMP-9 and TIMP-1 were both significantly overexpressed in colorectal carcinomas compared with its adjacent normal colorectal mucosa(69.1% and 61.9% vs.45.2% and 40.5% ,P =0.009 and P =0.004) ,and MMP-9 was positively associated with the re-gional lymph node metastasis, bowel wall invasion and Dukes stage(rs=0.372,P =0.015;rs =0.372,P =0.015;rs = 0.429,P = 0.005).TIMP-1 was positively associated with Dukes stage, bowel wall invasion and regional lymph node metastasis (rs = 0.394, P = 0.010;rs= 0.382,P = 0.013rs = 0.382, P = 0.013 ).There was a positive corre-lafion between the expression of MMP-9 and TIMP-1 (rs=0.641, P <0.001).Conclusion MMP-9 and TIMP-1 may play a key role in colorectal carcinogenesis.Examination of MMP-9 and TIMP-1 expression may have an impor-tant significance for evaluating prognosis, predicting of invasion and metastasis and comprehensive therapy.
10.Effects of morphine and tramadol pre-emptive employment on expressions of substance P and calcitonin gene-related peptide mRNA in dorsal root ganglia following acute myocardial ischemia in rats
Chinese Journal of Emergency Medicine 2011;20(1):35-38
Objective To investigate the effects of morphine and tramadol pre-emptive use on the expressions of substance P mRNA (SPmRNA) and calcitonin gene-related peptide mRNA (CGRPmRNA) in dorsal root ganglia (DRG) following acute myocardial ischemia in the rats. Method Twenty-four adult male SD rats weighing 270 to 300 g were randomly (random number) divided into four groups (n = 6, in each): group Ⅰ(sham operation), group Ⅱ (myocardial ischemia), group Ⅲ (morphine pre-emptive use) and group Ⅳ (tramadol pre-emptive use). The left anterior descending branch of coronary artery was occluded (CAO) for 3 hours in rats of group Ⅱ and Ⅳ.In group Ⅲ morphine 1.25 mg·kg-1 was injected through caudal vein 15 minutes before CAO.In group Ⅳ,tramadol 12.5 mg·kg-1 was daministered via caudal vein 15 minutes before CAO.In 3 hours after myocardial ischemia, the tissue of DRG (T1-5) were taken for detecting the expressions of SPmRNA and CGRPmRNA by using RT-PCR. One-way ANOVA was used for statistical analysis. Results In the tissue of DRG, the expressions of SPmRNA(0.93±0.02) ,α-CGRP mRNA(0.98±0.02) and β-CGRP mRNA(0.83 ± 0.02)were up-regulated in group Ⅱ compared with those in group Ⅰ (0.84±0.04),(0.86±0.01),(0.45±0.03) (P <0.05),and decreased markedly in group Ⅲ (0.88 ± 0.03) ,(0.90 ± 0.02), (0.67 ± 0.02) (P < 0.05) and group Ⅳ (0.88±0.04) ,(0.90 ± 0.01),(0.66±0.01) (P < 0.05), but showed no difference between group Ⅲ and Ⅳ (P > 0.05). Conclusions Morphine and tramadol pre-emptive use can significantly inhibit the expressions of SPmRNA and CGRPmRNA in rat's dorsal root ganglia after CAO.
