AIM:To evaluate the economic effectiveness in different pharmacotherapeutic schemes for tuberculosis.METHO_DS:Using pharmacoeconomic cost-effect analysis,three chemotherapy schemes,A,B and C,for bacillary tuberculosis were compared.RESULTS:Among three schemes,C was the best one.CONCLUSION:The results show that pharmacoeconomics plays an important role in optimizing therapeutic scheme,guiding rational drug use and increasing economic effectiveness.

Objective To investigate the influence of Xiyanping injections therapy on inflammation factors and lung function for old patients with acute exacerbation chronic obstructive pulmonary disease (AECOPD).Methods Ninety-two patients with AECOPD were randomly divided into the control group (n =46) and observation group(n =46).Patients in control group were given conventional treatment,in observation group were given XiYanPing injections for 2 weeks treatment on the basis of treatment of control group.The C-reactive protein(CRP),blood sedimentation rate (ESR),interleukin 10 (IL-10),1 s forced expiratory volume (FEV1),forced vital capacity (FVC),FEV1/FVC change and clinical effect of the two groups were recorded.Results There was no statistically significant difference in terms of CRP,ESR and IL-10 between the two groups before treatment (P > 0.05).After treatment,CRP and ESR in control group were reduced from (35.6 ±9.8) mg/L and (56.2 ± 16.4) mm/h to (14.2 ± 8.1) mg/L and (22.4 ± 11.6) mm/1 h and the difference was significant (t =-14.515,-12.008;P < 0.05),while in observation group decreased from (36.4 ± 8.2) mg/L,(54.6 ± 15.4) to (8.2 ± 6.2) mg/L,(16.8 ± 9.6) mm/h and the difference was significant (t =-18.468,-15.249;P < 0.05).Meanwhile,the level of IL-10 in the two groups significantly increased and the observation group showed more.IL-10 in control group increased from (168.8 ± 62.4) ng/L to (212.4 ± 85.6) ng/L and the difference was significant(t =5.468,P < 0.05),and in observation group increased from (172.8 ± 65.1) ng/L to (266.4 ± 92.6) ng/L and the difference was significant (t =6.368,P < 0.05).The FEV1 and FVC were markedly increased after treatment and the observation group showed increase more.FEV1and FVC in control group were increased from (1.41 ±0.15) L and (2.62 ±0.88) L to (1.62 ± 0.18) L and (2.89 ± 0.92) L,and the differences were significant (t =2.186,2.415 ; P < 0.05).While in observation group,FEV1 and FVC were increased from (1.42 ± 0.16) L and (2.59 ± 0.84) L to (1.84 ±0.21) L and (3.12 ± 0.94) L,and the differences were significant (t =4.366,4.585 ; P < 0.05).Total efficacy of observation group was 91.3% (42/46),obviously higher than that of control group(78.3% (36/46) ; x2 =4.658 ;P < 0.05).Conclusion Xiyanping injections can reduce the inflammatory response in patients with AECOPD,improve lung function and the effect of treatment.
4.Clinical Observation of E_2 Transdermal Patch in the Treatment of Menopaus-al Syndrome

Objective:To evaluate the security, efficacy and side effects of E_2 transdermal patch (Fem-7) in the treatment of menopausal syndrome. Methods:A total of 60 cases with menopausal syndrome (blood serum FSH >30 U/L,E_2 <109.8 pmol/L) were randomly divided into treatment group ( Fem-7 group) and control group (estrodiol valerate (E_2V) group) for 12 weeks treatment. Fem-7 group changed patch every 7 days and took the 2 mg medroxyprogesterone (MPA) every day. While the E_2V group took the 2 mg MPA and 1.5 mg E_2 V every day. Serum E_2, FSH, lipoprotein and coagulation function were evaluated at baseline and at the 12th week of the treatment. In addition, menopausal symptoms and some other index of security and side effects were also observed. Results:Kupperman indix in both groups was decreased and the mean value of E_2 was increased obviously. Hepatic function, renal function, coagulation function and blood glucose were no obvious change. Lipoproteina was descended markedly in Fem-7 group. Other index of security in both groups was no obvious change. Conclusions:Fem-7 patch is secure and valid for relieving symptom of menopause, especially for the women who is not sutable for oral medication.
5.Atorvastatin Reducing Contrast-induced Nephropathy Rate in Patietns With Emergent Percutaneous Coronary Intervention via Improving Heat Shock Protein-90 Expression

Objective: To explore the high dose atorvastatin reducing contrast induced nephropathy (CIN) rate in patiens with emergent percutaneous coronary intervention (PCI) via improveing heat shock protein-90 (HSP90) expression with its possible mechanism. Methods: A total of 158 STEMI patients with emergent PCI in our hospital were studied. The patients were randomly divided into 2 groups: High dose atorvastatin group, the patients received pre-operative atorvastatin 40 mg,n=80 and Control group, the patients received pre-operative placebo,n=78. The serum creatinin (Scr), creatinine clearance rate (Ccr), blood urea nitrogen (BUN), superoxide dismutase (SOD), malondialdehyde (MDA), nitrogen monoxide (NO), HSP90 mRNA expression and protein concentration and urine α1-microglobulin were examined in all patients and the incidence rates of CIN were compared between 2 groups. Results: Compared with Control group, High dose atorvastatin group had drcreased Scr (68.92 ± 8.80) μmol/L vs (77.25 ± 13.36) μmol/L, MDA (3.88 ± 0.53) nmol/L vs (4.08 ± 0.52) nmol/L and urine α1-micrglobulin (1.38 ± 0.36) mg/dl vs (1.89 ± 1.13 ) mg/dl; increased Ccr (89.71 ± 9.85) ml/min vs (77.28 ± 13.78) ml/ min, SOD (129.52 ± 30.63) U/ml vs (117.66 ± 27.98) U/ml, NO (66.23 ± 29.26) μmol?gprot vs (55.12±27.43) μmol?gprot, allP<0.05. Compared with Control group, High dose atorvastatin group presented higher post-operative HSP90 mRNA expression (0.466 ± 0.158) vs (0.224 ± 0.278 ) and protein concentration (1259.83 ± 121.17) pg/ml vs (1195.0 ± 127.65) pg/ml, allP<0.05. The incidence rate of CIN was lower in High dose atorvastatin group (2.5%) than Control group (10.3%),P<0.05. Conclusion: A high dose atorvastatin administration before emergent PCI may decrease CIN occurrence rate. Atorvastatin may promote HSP90 expression, increase NO produciton, then improve the vascular endothelial function and anti-oxidative ability to protect the renal function in STEMI patients.

Objective To investigate the efficacy of amplitude-integrated electroencephalographic (aEEG)in evaluating the maturation of cerebral function in preterm infants.Methods Ninety one neurologically normal and clinically stable preterm infants admitted to neonatal intensive care unit were studied prospectively by using aEEG.Background of aEEG activity patterns,presence of sleep-wake cycles,and the lower and higher border were analyzed.Results 112 aEEG were analyzed.The sleep-wake cycle and the continuity became more mature with PMA increased.The lower border were increased and the higher border were decreased with PMA increased(r =0.982,P < 0.01 ; r =-0.964,P<0.01).Conclusion In preterm infants,aEEG is a useful tool to evaluate the maturation of cerebral function.
7.The clinical analysis of 6374 patients with hyperthyroidism by radiopharmaceuticals 131I therapy

Objective To probe into optimal delivered dose of 131I therapy in hyperthyroidism and influencing factors.Methods Data of 6374 patients with hyperthyroidism receiving 131I therapy were summarized and analyzed.The patients were differentiated into Graves group (GD) and multinodular toxic goiter group (NGD) after 131I therapy.To compare incidence of hypothyroidism between two groups after 131I therapy,partial correlations analysis was performed between delivered dose and other variables influencing factors among cured patients.Results After 131I therapy incidence of permanently hypothyroidismin patients with Graves' group was much higher than that in patients with multinodular toxic goiter group(17.01%,0.39%,Pearson x2 value =704.8,P < 0.01).Delivered dose of 131I therapy related closely to thyroid size,with or without nodules and thyroid rigidity(r =0.469,0.484,0.553).Disposable cured rate of 131I therapy was higher than 63.99%.Conclusion 131I therapy is optimal to multinodular toxic goiter.Delivered dose of 131I therapy will be increased for patients with bigger and stiff mass for therapeutic effects.

9.Review of hip joint center in total hip replacement

[Objective]To explore the effect of hip joint center on the stability of hip joint,the radiographic measurements were compared between preoperation and postoperation.[Method]A total of 155 hips from 120 patients with primary total hip replacement from resent years were studied,the hip joint center difference through radiographic measurements between preoperation and postoperation were compared.[Result]The coincident rate and the incoincident rate were 63.23%(group A,98 hips) and 36.77%(group B,57 hips) respectively.For loose,dislocation or low back pain,the rate of revision in group A and group B were 6.12% and 29.82% respectively.[Conclusion]The comeback of hip joint center was essential for stability of hip joint.

Objective The purpose of this study was to investigate the expression of basic fibroblast growth factor (bFGF) and its effect on tumor interstitial angiogenesis and the biological behavior of gastric carcinoma. Methods CD34 was used to explore the microvessel density (MVD) as the marker of endothelial cell. bFGF was qualitatively and quantitatively detected with immunohistochemistry on 74 cases of gastric carcinoma and 17 cases of adjacent normal gastric tissue. Results Measurement of CD34 expression could be used to describe endothelial cell proliferation and vascularity. Strong expression of bFGF was localized in the cytoplasm of tumor cell and interstitial new microvessel endothelial cells.
