OBJECTIVETo seek for a good therapy for childhood hyperkinetic syndrome (CHS).

METHODSTwo hundred and ten patients of CHS were randomly divided into three groups, the Yizhi mixture (YZM) group, the Ritalin group and the combined treated group, 70 in each group. They were treated with YZM alone, Ritalin alone and combined YZM and Ritalin respectively for 12 weeks. The effect was assessed by Conners' questionnaire for hyperkinetic scoring, Achenbach's questionnaire for behavior estimation, clinical general image questionnaire and Tess's questionnaire before and after treatment.

RESULTSThe total effective rate in the YZM group, Ritalin group and combined treated group was 75.7%, 74.2% and 82.8% respectively, and the markedly effective rate in the three groups was 44.3%, 45.7% and 80.0% respectively. By chi 2 test, comparison among the three groups showed that the difference of the total effective rate was insignificant, while that of the markedly effective rate was significant (chi 2 = 23.31, P < 0.01). Further analysis conducted by partitioning method to make comparison between each two groups showed that the markedly effective rate was significantly higher in the combined treated group than that in the YZM group (chi 2 = 18.98, P < 0.01) and in the Ritalin group (chi 2 = 17.62, P < 0.01). The low hyperkinetic score, behavior score, achievement of Chinese and mathematics in the CHS patients were improved after treatment, the improvements were significantly higher in the combined treated group than those in the other two groups (P < 0.05, by q-test). Moreover, the soft nerve signs and abnormal encephalogram were significantly improved in the combined treated and the YZM groups, and the side-effect occurred in the two groups was less than that in the Ritalin group (P < 0.01).

CONCLUSIONThe therapeutic effect of combined treatment of YZM and Ritalin was better than that of YZM or Ritalin alone.

OBJECTIVETo investigate the clinical outcomes of cannulated screw combined with buttress plate in treating transarticular shear fractures of the distal humerus.

METHODSFrom July 2008 to December 2011, 17 patients with shear fractures of the distal humerus were treated with cannulated screw combined with buttress plate. Among them, included 10 males and 7 females aged from 20 to 59 years old (mean 36.5). All the fractures were classified into type I (8 cases), type II (1 case), type III (3 cases) and type IV (5 cases) according to Bryan-Morrey and McKee classification. According to AO/ASIF classification, 10 cases were type 13B3.1, 7 cases were type 13B3.3; according to Dubberley classification, 5 cases with type 1A, 2 cases with type 1B, 2 cases with type 2A, 2 cases with type 2B, 4 cases with type 3A, 2 cases with type 3B. There were 12 cases with fresh injuries, 4 cases with old injuries. All fractures were closed injuries. Complications and range of motion of elbow were observed, and clinical efficacy were evaluated by Broberg-Morrey standard.

RESULTSAll patients were followed up with a mean time of 21.3 months (ranged, 18 to 24). All fractures obtained bone healing from 10 to 15 weeks with a mean of 12.5 weeks. No malunion or delayed healing occurred. Two cases occurred ossification in lateral collateral ligament attachment. 2 cases occurred heterotopic ossification in the capitellum in front of capsule joint. The range of elbow extension was (16 +/- 7) degrees, flexion was (115 +/- 9) degrees, the average are of pronation and supination was (65 +/- 5) degrees and (60 +/- 5) degrees respectively. According to Broberg-Morrey standard, there were excellent in 7 cases, good in 8 cases, fair in 2 cases, and average score was 92.44 +/- 4.64.

CONCLUSIONFor transarticular shear fractures of the distal humerus, classification should be performed according to preoperative X-ray and CT, cannulated screw and buttress plate combined and early function rehabilitation could recover elbow function.

OBJECTIVETo evaluate the association between left coronary artery stenosis degree and myocardial perfusion by 64 multi-slice CT.

METHODSA total of 223 patients underwent 64 multi-slice CT coronary artery images (CTA) were included and divided into normal group (91 cases), mild stenosis group (72 cases), moderate stenosis group (36 cases) and severe stenosis group (24 cases). Myocardial density was measured at apical, septal and lateral segments. Myocardial density in infarcted segments was compared to non-infarct segments in 11 patients with old myocardial infarction (all from severe stenosis group).

RESULTSMyocardial density was significantly lower at apical segments [(55.8 ± 21.4) HU vs. (75.3 ± 7.5) HU], at septal segment [(87.8 ± 3.3) HU vs. (98.2 ± 5.2) HU] and at lateral segment [(86.8 ± 7.9) HU vs. (95.6 ± 11.6) HU] in severe stenosis group than in normal group (all P < 0.05). Myocardial density of patients with old myocardial infarction was significantly reduced in non-infarct segment [(70.9 ± 8.3) HU vs.(98.7 ± 7.3) HU, P < 0.01] and increased in infarct segment [(42.5 ± 15.7) HU vs. (17.8 ± 4.1) HU, P < 0.01] post contrast enhancement.

CONCLUSIONCTA could be used to evaluate the severity of the left coronary artery stenosis based on myocardial density measurement. Myocardial delayed enhancement derived from CTA could be used to identify infarct segments.

OBJECTIVETo investigate level and its influential factors of occupational stressors and strains in workers of a steel plant and explore the feasibility of occupational stress and mental health as indicators of effect evaluation of health promotion at workplace.

METHODS1039 workers were investigated by using cross-sectional method. The questionnaires included job stressors, skill discretion, job control latitude, mental health and sleep disorders.

RESULTS54.5% of respondents reported "job doesn't allow mistakes" as stressor, while 47.6% of respondents reported "too job responsibility" as stressor. The scores of job stressors were significantly higher than female workers (P<0.01). Workers with high education level scored more skill discretion, job control latitude and lower mental health than ones with low educational level (P<0.05 or P<0.01). Elder workers had more skill discretion and job control latitude scores than younger. There were significant differences in scores of skill discretion among different length of service groups (P<0.05). There were also significant differences in scores of skill discretion among different job title groups (P<0.01). Occupational stressors were correlated significantly positively with sleep disorder (r=0.37, P<0.01), but job control latitude negatively to mental health (r=-0.19, P<0.01).

CONCLUSIONThe workers in the steel plant report occupational stressors, which is related to poor self-reported health symptoms. The occupational stressors and mental health should be considered as one of effect evaluation indicators of health promotion at workplace.

To study the ecological distribution and diversity of endophytic fungi associated with Ferula of medicinal plants in Xinjiang. The endophytic fungi were isolated from roots, stems and leaves of Ferula by microbiology research methods and technology. The endophytic fungi were identified using ITS rDNA sequence analysis and morphology analysis. The composition, diversity and preference of endophytic fungal community were analyzed with Shannon-Wiener biodiversity index (H') and Sorensen coefficient (Cs). A total of 337 strains endophytic fungi were isolated and classified into 38 genera, Alternaria, Aureobasidium and Fusarium were the dominant genera. Among the 337 isolates, the endophytic fungi of F. sinkiangensis were the most, The Shannon-Wiener biodiversity index (H') associated with roots of F. fukanensis was the highest, reached 1.85. The highest Sorensen coefficient ( Cs) was between leaf of F. sinkiangensis and leaf of F. ovina, reached 0.75. From the result, endophytic fungi were widely distributed in six Ferula, there are some notable differences between distribution and composition of the endophytic fungi isolated from different issues and different species of Ferula, show a certain degree of species and tissue preference. The results obtained in this study will provide realistic basis and theoretical basis for further study the secondary metabolites of endophytic fungi associated with Ferula, and the relationship between endophytic fungi and their host plants.

OBJECTIVEAn RP-HPLC procedure was established for the determination of danshensu and protocatechuic aldehyde in Guanxinning injection powder.

METHODAn RP-HPLC analytical procedure was developed using Hypersil ODS2 C18 column (4.6 mm x 250 mm, 5 microm) with methanol-0.5% glaceal acetic acid (4.5:95.5) as mobile phase, and a wavelength of 280 nm for UV detection.

RESULTThe linear range was 3.004-45.06 microg x m(L-1) (r = 0.999 5, n = 6) for danshensu, and 0.300-4.509 microg x mL(-1) (r = 0.999 3, n = 6) for protocatechuic aldehyde. The average recoveries were 99.1% and 97.9%, respectively.

CONCLUSIONThe method was stable, accurate, and reproducible, and can be used for the quality control of Guanxinning injection powder.

Objective To observe changes of hepatic hemodynamics and hepatic functional reserve after splenectomy and periesophagogastric devascularization. Methods From July 2006 to August 2007, thirty patients with portal hypertension caused by hepatitis B cirrhosis underwent splenectomy and periesophagogastric devascuiarization. The PVPG (portal venous pressure gradient) was measured by inductor continually during operation. The HAF (hepatic artery flow), PVF (portal venours flow) and hepatic arterial RI(resistant index) were measured with Doppler sonography. The EHBF(effective hepatic blood flow) and ICGR15 (indocyaninegreen retention rate at 15 minutes) were obtained respectively by indocyaninegreen clearance test before and after the operation. Results PVPG after laparotomy (19±4) mm Hg, ligating the splenic artery(14±4) mm Hg, splenectomy(14±3)mm Hg and periesophagogastric devascularization (12±4) mm Hg showed a tendency to decrease progressively. The PVF decreased [from (42±14) ml/s to (16±8) ml/s] and HAF increased in compensation after operation. The EHBF increased [from (0.48±0.10) L/min to (0.56±0.10) L/min], and the ICGR15 decreased (from 23%±8% to 18%±4%) postoperatively. Conclusion After splenectomy and periesophagogastric devascularization, the hepatic functional reserve improves at least in a short term notwithstanding the decrease of PVPG and PVF.
