1.Reliability and validity of the Chinese version of the Nutritional Form for the Elderly in elderly patients

Objective The study aimed to test the reliability and validity of the Chinese version of Nutritional Form for the Elderly (NUFFE-CHI) to provide tools for elderly malnutrition screening.Methods The Nutrition Form for the Elderly was translated into Chinese and the research was carried out among 208 old patients ≥60 years old in the First Hospital Affiliated to Liaoning Medical University in Jinzhou.The rehability and validity were analyzed by SPSS 17.0 and AMOS 17.0 software.Results The Cronbach's α coefficient was 0.74,the intra-class correlation coefficients (ICC) of each item were 0.84-0.96 and the scale ICC was 0.96.Item-to-total correlation analysis showed that all of the item-to-total correlations were statistically significant.The item content validity index (I-CVI) were 0.60-1.00,the scale content validity index S-CVI/UA was 0.93 and S-CVI/Ave was 0.93.Five common factors from exploratory factor analysis explained 64.80% of the total variance,and the communalities of the items were 0.460-0.793.Confirmatory factor analysis showed the goodness of fit index (GFI),the comparative fit index (CFI) and the incremental fit index (IFI) were all≥ 0.900,the mean root square error approximation (RMSEA)<0.08,the NUFFE-CHI five-factor model regression coefficients were 0.12-0.82.The best critical point was 13,≥ 13 indicated malnutrition,the sensitivity was 59.6%,the specificity was 91.5%,and area under the curve was 0.823.Conclusions NUFFE-CHI is a reliable and valid instrument which could be used for elderly malnutrition screening in elderly patients.
2.Development of the Chinese version stroke self-efficacy questionnaire

Objective To translate the English version of the stroke self-efficacy questionnaire (SSEQ) into Chinese,and to evaluate the reliability and validity of the Chinese version of SSEQ.Methods The reliability and validity of the Chinese version of SSEQ were tested among 300 stroke patients in Jinzhou city.Data were analyzed by SPSS 17.0 software and AMOS 17.0 software.Results The Cronbach's α was 0.972,the test-retest reliability coefficient was 0.806,the correlations between items and total scale ranged from 0.729 to 0.831 (P<0.01).A two-factor model was got by exploratory factor analysis,the communalities of the items was 0.672 to 0.929,and the cunulative explained variance was 82.687 percent.The confirmatory factor analysis revealed a good model fit,and the fit indices were x2/df =1.740,GFI =0.918,AGFI =0.853,RMSEA =0.070,NFI =0.965,CFI =0.985,standardized regression weights was 0.701 to 0.984.Conclusion The Chinese version of the SSEQ is proved to be reliable and valid.It can be used to assess the self-efficacy of stroke patients in china.
3.Reliability and validity of Chinese version of the Osteoporosis Knowledge Assessment Tool in the elderly

Objective To test the reliability and validity of the Chinese version of Osteoporosis Knowledge Assessment Tool (OKAT).Methods The English version of OKAT was introduced and the Chinese version was confirmed by translation,revision,retroversion and expert discussion.A sample of 400 community-living aged people were recruited to complete the OKAT (Chinese version).Internal consistency test,split-half reliability,correlation analysis,items analysis,content validity,exploratory factor analysis,confirmatory factor analysis and ROC curve were used for examination.Results The Cronbach's α of the total scale was 0.821.The split half reliability coefficient was 0.934,test-retest reliability was 0.866,inter rater reliability coefficient was 0.786,the correlations between items and total scale ranged from 0.163 to 0.542.A two-factor model containing 20 items was got by exploratory factor analysis,the cumulative contribution rate of the items was 75.528 percent.The confirmatory factor analysis revealed a good model fit.Conclusions The Chinese version of the OKAT in the elderly is proved to be reliable and valid.It can be used to assess osteoporosis for the aged people in China.
4.Correlation between self-efficacy and fall efficacy in patients with stroke

Objective To explore the relationship between self efficacy and fall efficacy in patients with stroke.Methods The falls efficacy scale-international (FES-I) and the stroke self-efficacy questionnaire(SSEQ) were applied to 300 patients with stroke.Results The FES-I score of the participants was 39.13± 16.40,the scores of the indoor activity dimension and outdoor activity dimension were (26.01 ± 11.13) and (18.33 ± 7.76),respectively.The total mean score of SSEQ was (40.11 ± 22.37),the scores of the activity function dimension and self-management dimension were (24.97± 14.40)and (15.14±8.35),respectively.FES-I scores were positively correlated with SSEQ scores (r=0.550-0.802,P<0.05).Falls efficacy could explain 34.3% and 36.8% variation amount for each dimension of self-efficacy in patients with stroke.Conclusion Falls efficacy is closely related to self-efficacy in patients with stroke.In order to improve the quality of medical care,medical staff should pay attention to the occurrences of reduction of self-efficacy and selfesteem in patients with stroke when they avoid falling during some activities.
5.Correlations between Fetal Congenital Cardiovascular Anomalies and Chromosomal karotypes

Objective To investigate the distribution of congenital cardiovascular malformations in fetuses with chromosomal abnormalities.Method Congenital cardiovascular malformations of fetuses were diagnosed by prenatal ultrasonic cardiography from Jan 2011 to Sep 2013,and whose chromosomal karotype were tested by amniocentesis or cordocentesis.The association between chromosomal karyotypes and distribution of congenital cardiovascular malformations was analyzed.Result In 173 Fetuses with chromosomal abnormalities,20(11.56%) cases had congenital cardiovascular malformations,including seven 21-trisomies,eight 18-trisomies,three 13-trisomies and two 45,X.64% (16/25) fetuses with congenital cardiovascular malformations accompanied with other malformations had chromosomal abnormalities.Only 1.87% (52/4379) fetuses with normal karotype had congenital cardiovascular malformations.Conclusion Chromosomal abnormality is the most reason of complicate CHD.Chromosomal karotype test should be detected in fetus with complicate CHD.
6.Bone marrow mesenchymal stem cells transplantation for the treatment of sclerodermatous chronic graft-versus-host disease: Immunologic mechanism changes in 4 cases

BACKGROUND:The immunomodulatory ability of bone marrow mesenchymal stem cells(BMSCs)gives it a promising future in treating graft-versus-host disease(GVHD),especially with previous success in treating patients with acute GVHD.However,there are fewer reports concerning BMSCs in treating chronic GVHD,particularly for sclerodermatous chronic graft-versus-host disease(ScGVHD).OBJECTIVE:To evaluate the efficacy and safety of treatment of BMSCs for ScGVHD,and to primarily explore the immunological mechanism of clinical efficacy.METHODS:Four ScGVHD patients at the Affiliated Hospital of Academy of Military Medicine Science,between September 2006 and August 2008,were enrolled for this trial.The median patient age was 41 years,1 female and 3 male.The patients received BMSCs infusion at a dose of(1.0~2.0)×10~7 cells every time by intrabone marrow injection from the anterosuperior iliac spine and BMSCs from the same donor for the same patient were infused more than once.Concomitant medications for ScGVHD were individualized for each patient,but all were current standard medicines and the doses were significantly tapered.RESULTS AND CONCLUTION:After BMSCs infusion,the ratio of Th1 to Th2 was dramatically overturned,with an increase of Th1 and a decrease of Th2 reaching at a new balance.Correspondingly,symptoms of all the four patients gradually improved.During the course of BMSCs treatment,the life signs and laboratory results from the recipients remained normal.By the time of this report,there has been no recurrence of leukemia in the four patients.Although this study alone cannot guarantee the application of BMSCs in ScGVHD,the results are strongly in favor of the idea that the BMSCs treatment for ScGVHD patients is therapeutically practical without any detectable side effects,which may provide a new insight into the matter of treating ScGVHD clinically,thus will greatly increase the survival rate of leukemia after allogeneic bone marrow transplantation.
7.Correlation analysis of anxiety, depression and quality of life in in vitro fertilization and embryo transfer patients before treatment

Objective To investigate the correlation between anxiety, depression and quality of life in patients undergoing in vitro fertilization and embryo transfer. Methods The psychological status of patients undergoing in vitro fertilization and embryo transfer in Nanjing General Hospital of Nanjing Military Command from November 2016 to March 2017 was investigated with the method of purposive sampling. Results The survey released a total of 300 questionnaires with effective recovery of 280, the effective recovery rate was 93.33%. The score of anxiety scale was (55.16 ± 3.92) points, the score of depression scale was (55.62 ± 8.45) points. Nearly 87%(244/280) patients showed anxiety symptom, and 48%(134/280) patients showed depression symptom. The score of quality of life was (62.55±12.09) points, and the anxiety and depression were negatively correlated with the quality of life (r=-0.581,-0.544). Multivariate regression analysis showed that factors affecting the fertility quality of life scores including anxiety, depression, age, educational level, family income, infertility years, type of infertility, previous receiving assisted reproductive history. Conclusions The patients with in vitro fertilization and embryo transfer show different degrees of anxiety and depression, and is correlated with quality of life. Medical staff should pay more attention to the psychological status of patients, adopt targeted interventions to improve anxiety and depression status of patients, help patients to actively take effective coping style to improve the quality of life of patients, improve the clinical pregnancy rate.
