Objective To study rational method for managing recurrent ventricular fibrillation during aortic valve replacement in patients with valvular aortic stenosis.Methods One hundred consecutive patients with valvular aortic stenosis who scheduled to receive aortic valve replacement were enrolled into the study.Three steps method was applied when ventricular fibrillation occurred after aortic unclamping.The first step was to correct disturbance of internal environment,intravenous lidocaine (100mg) and defibrillation (20-30 W/s); the second step was to inject norepinephrine to maintain mean arterial pressure 70-90 mm Hg (1mm Hg =0.133 kPa),then repeated defibrillation; the third step was infusion of magenisum sulphate (1g) and/or amiodarone (150 mg),then repeated defibrillation.Preoperative left ventricular mass index,mean arterial pressure,nasopharyneal temperature,serum potassium level and hematocrit after aortic unclamping were collected and compared.ResultsAfter aortic unelamping,56 patients recovered spontaneous rhythm without defibrillation,after the first step,16 patients could be defibrillated successfully,other 10 patients recovered spontaneous rhythm after the second step,still other 18 patients remained unresponse untill the third step.Patients who recovered spontaneous rhythm without defibrillation or needed the first step management had significant lower left ventricular ragas index when comparing with patients who entered the second step and third step management respectively [(184±43),(178±51)g/m~2 vs(237±61),(242±46) g/m~2,P＜0.05 ].Patients who needed the second step or third step management also had significant higher mean arterial pressure than the other patients.Conclusion Three steps method can be used to manage recurrent ventricular fibrillation during aortic valve replacement in patients with valvular aortic stenosis.

Objective To study the prevalence and subtype of hemorrhagic transformation (HT ) among patients with different subtypes of acute cerebral infarction ,and investigate its clinical significance .Methods 977 patients with acute cerebral infarction from January 2008 to December 2011 were divided into groups according to the stroke subtypes :large-artery atherosclerosis(LAA) , cardioembolism(CE) ,small-artery occlusion(SAO) ,undetermined cause(UND) ,and other determined cause(OC) .HT included hemorrhagic infarction (HI) and parenchymal hematoma (PH) .The baseline data were registered and the prevalence and subtype of HT between different groups were compared .Results The rates of hypertension were the highest in LAA ,The rate of diabetes was the highest in SAO ,The rate of atrial fibrillation was the highest in CE .The rates of hyperlipidemia had no significant difference a-mong different subtypes of CI .The HT incidence of LAA ,CE ,SAO ,OC ,UND were 12 .8% ,31 .1% ,6 .6% ,4 .5% ,5 .5% respec-tively ,the difference was statistically significant (χ2 =61 ,P<0 .01) .As to the subtype of HT ,PH was more common in CE group (χ2 =31 ,P<0 .01) .Conclusion Distribution of risk factors ,HT prevalence and classification are different in different subtypes of acute cerebral infarction ,the differences might be related to the distribution of different risk factors .

OBJECTIVE　To establish a quality standard for Tiaogu tablet.METHODS　TLC was used to make a qualitative determination for 4 drugs in the formulary.The content of bomeol was determined by GC, the contents of strychnine and brucine were determined by TLC.RESULTS　4 drugs in the formulary were determined under stable condition with high sensitivity and precise results.CONCLUSION　This study may be helpful to develop the quality standard for Tiaogu tablet.

Urinary albumin is a biomarker for kidney damage and is widely applied in clinical practice.With more evidence on urinary albumin, it is also recognized as a risk factor for cardiovascular disease and all-cause mortality.This paper focuses on thecurrent status of pre-analytical, mid-analytical and post-analytical process and discusses the progress in standardization of measurement for urinary albumin and the issues which call for the attention of clinical laboratorian.

Objective To investigate the expression of miR-199b in colorectal cancer and the effect of miR-199b on metastasis of colorectal cancer.Methods The expression of miR-199b in colorectal cancer was assessed by quantitative reverse transcription polymerase chain reaction (qRT-PCR).The miR-199boverexpressed human colon tumor-116 (HCT-116) stable cell lines was constructed by lentivirus assay.The effects of miR-199b on the migration and invasion of colorectal cancer were confirmed by Transwell assay.Bioinformatics,luciferase reporter assay,and enzyme linked immunosorbent assay (ELISA) were performed to verify the potential target genes of miR-199b.Results miR-199b was at lower level in cancerous tissues comparing to the corresponding noncancerous tissues.Up-regulation of miR-199b significantly blocked the migration and invasion of colorectal cancer cells.Co-transfection of miR-199b and VEGFA wild type (WT)3'untranslated region (3'UTR) could inhibit the luciferase activity,while there was no significant difference when co-transfection with mutant (MT) 3'UTR.The protein level of vascular endothelial growth factor A (VEGFA) after overexpression of miR-199b was repressed,which was validated by ELISA.Conclusions miR-199b was a new colorectal cancer suppressor miRNA,which inhibited tumor migration and invasion by negatively regulating VEGF.

Objective:To establish a specialized detection method for quinolone drugs illegally added in health foods and Chinese traditional patent medicines. Methods: LC-MS/MS was used to simultaneously screen pipemidic acid, norfloxacin, ciprofloxacin, lomefloxacin, enoxacin, fleroxacin, pefloxacin, sparfloxacin, gatifloxacin and ofloxacinan in health foods and Chinese traditional patent medicines. Qualitative and quantitative analysis and methodology validation were studied as well. Results:Under the conditions of the analysis method, the 10 quinolone drugs added illegally obtained a good separation on a chromatographic column. With the two grade mass spectrum for the qualitative analysis and the peak area of multiple reactions monitoring mode for the quantitative calculation, the method was specific and sophisticated. The recovery and linear relationship met the demand of screening. Conclusion:The method is simple and accurate in the detection of quinolone drugs in health food and Chinese traditional patent medicines.

Objective To analyse the nutritional compontents of Luidia quinaria Von Martens collected from the Yellow Sea,and to provide the theoretical bases for exploitation and utilization of its resource,.Methods The contents of total suger,crude fat and protein were determined by anthorone reagent,improved Soxhlet extraction and micro-Kjeldahl method,respectively.The analysis of fatty acid composition was determined by gas chromatography.Results The contents of total suger,reducing suger,crude fat and protein from Luidia quinaria Von Martens were 2.64%,1.68%,2.71% and 22.20%,respectively.Unsaturated fatty acid was rich in the Luidia quinaria Von Martens,especially EPA was 45% of the total(li-)pid,and chemical elements were rich.Conclusion Luidia quinaria Von Martens was rich in nutritional constituents and was worthy to be utilized.

Objective To study the mechanism of extracellular signal-regulated kinase (ERK) pathway inhibitor U0126 on cell cycle of K562 cell lines. Methods The mRNA and protein expressions of ERK, Cyclin D2, Cyclin E in and P27 K562 cell lines after 10 ?mol/L U0126 treatment for 12, 24 and 72 h were detected by RT-PCR and Western blot, respectively. The cell cycle was determined by flow cytometry. Results The mRNA and protein expressions of ERK, Cyclin D2 and Cyclin E in K562 cell lines after U0126 treatment were decreased, while those of P27 were increased. The percentage of cells in G0/G1 phase was increased and that in S phase was decreased. Diabetic Osteoporosis is a general metabolic bone disease having fracture tendency because of osteopenia in unit volume,microstructural changes of bone tissue,decreased bone strength and increased friability,which is one of important complications of diabetes mellitus in skeletal system.Studies on bone involvement in patients with diabetes mellitus have generated conflicting results,largely because of the pathogenetic complexity of the condition.There are many factors,including advantageous and disadvantageous factors,influence the probability of osteoporotic fractures in diabetics.Adequate glycemic control,and calcium and vitamin D intakes,and the prevention and treatment of diabetic complications are key elements in the elimination of osteoporosis in diabetes mellitus.Patients with osteoporosis and diabetes should be offered the same pharmacological treatments as non-diabetics.
