With the intensification of population floating in China, there has been a growing demand for ecdemic medical care. Because basic medical insurance in China conduct localized management, the differentiation between reimbursement policy and level in different areas result in the difficulty of immediate settlement. The great quantity of floating population are faced with rather heavy personal burden when need medical care at different locations, which to some extent restrict in-time healthcare for floating population. On the basis of analyzing the features of endemic medical care for floating population, it described three mode of typical immediate settlement on ecdemic medical care, discussed the main problems in restricting the flowing population get immediate settlement at different locations and put forward suggestions on improving the immediate settlement at different locations.

In order to improve the optimum temperature of lipases, The Penicillum expansum lipase (PEL) gene was mutated by site-directed mutagenesis using overlap extension PCR technique. The recombinant plasmid pPIC3.5K-lip-E83V containing matant gene was expressed in Pichia pastoris GS115. The comparison experiments of the mutant PEL-E83V-GS with the wild-type PEL-GS showed that: the optimum temperature of the mutant (45℃) was higher by 5℃ than that of the wild type. The thermostability of the mutant was similar to the wild type. The enzymatic activity of the mutant was 188 U/mL at 37 ℃, which was 80% that of the wild type at the same conditions. The hydrophobic interaction may be enhanced in the surface region by the hydrophilic amino acid, Glu substituted with the hydrophobic amino acid, Val, and be responsible for the improvement of optimum temperature.

· Mesenchymal stem cells ( MSCs ) are a population of multipotent stem cells with various neurotrophins from bone marrow which are widely used in tissue, cell repair and alternative research.Recently, some researches have shown that MSCs could enhance the viability of neurons under a pathological circumstance by secreting some neurotrophins. So the neuroprotection of MSCs can provide a new method of the treatment on retinopathy that it possible to promote cells survial and functional recovery.Here we make a brief review on the secretory function of neurotrophin and neuroprotective effects of MSCs on retinal cells and its application in the treatment of retinal disease.

Objective To evaluate the efficacy and safety of HyperCVAD/HDMTX-L regimen as salvage treatment for patients with diffuse large B-cell lymphoma. Methods A retrospective study was made to analyze the efficacy and adverse reaction of HyperCVAD/HDMTX-L regimen as salvage treatment for 54 patients with diffuse large B-cell lymphoma. 27 cases were divided in CHOP class scheme ineffective group, 24 cases in recurrence group, HyperCVAD/MA regimen were as the control group. The efficacy in the treatment of 4 cycles, side effects of all cycles and 3-year progression free survival rates of the two regimens were compared. Results In CHOP class scheme ineffective group, efficiency rate of HyperCVAD/HDMTX-L was 59.3%(16/27), 3y PFS rate was 73.3%, efficiency rate of HyperCVAD/MA was 61.9 % (13/21), 3y PFS rate was 61.5%. There was no significant difference in efficiency and PFS rate between the two groups (P>0.05). In recurrence group, efficiency rate of HyperCVAD/HDMTX-L was 47.8 % (11/23), 3y PFS rate was 54.5 %, efficiency rate of HyperCVAD/MA was 45.5 % (10/22), 3y PFS rate was 50.0 %, there was no significant difference in efficiency and PFS rate between the two groups (P> 0.05). The incidence rates of nausea and vomiting, aleukocytosis, thrombocytopenia, hemorrhage associated with chemotherapy in HyperCVAD/HDMTX-L group were significantly lower than those in the control group (all P < 0.001). Conclusion HyperCVAD/HDMTX-L regimen maybe replace HyperCVAD/MA as the salvage treatment of diffuse large B-cell lymphoma.

Carcinogenesis is a multistep process that a couple of genes involve in the initiation of this disease.As a pivotal hallmark of cancer, the genomic instability ensues in which genetic alterations when this process was initialized and developed. With the increasing number of studies on genomic instability, the scientists pay more attentions on its biological function at post-genomic era. Genomic instability in various carcinomas is associated with broken telomere, gene modification, and contributes to the dysfunction of DNA damage and repair pathways.This review focus on the factors involved in genomic instability, as well the current detection methods of genomic instability and their clinical application.

Objective To investigate the hepatitis B liver cirrhosis complicated with hepatic encephalopathy (HE),relevant risk factors in the development of strategies for the prevention and treatment of the disease,and make for the clinical experience accumulation.Methods A retrospective analysis of 64 cases of liver cirrhosis patients complicated with HE,for the observation group.In addition,53 patients of hepatitis B cirrhosis without HE patients were selected as control group.The two groups of patients with objective physiological indicators were recorded and compared.Results (1) Compared with the control group,blood ammonia,serum total bilirubin,blood urea nitrogen level and infection,incidence of digestive tract hemorrhage of the study group increased,blood sodium,alanine amin otrans ferase,decrease the serum albumin level,the differences were statistically significant (t =3.0 8 5,5.5 2 8,x2 =9.174,7.126,t =4.102,4.337,8.675,5.323,all P ＜0.05).(2) For the patients in the study group,age,more than 3 kinds of incentives and C grade of liver function were the high risk factors of death.(3) In the cause of deaths in the study group,the infection was the most common,the incidence rate was 75.9%,the second above digestive rare tract hemorrhage,the incidence rate was 65.5%.Conclusion The elderly,the number of risk factors and severity of liver function are the occurrence and development of related risk factors of hepatitis B related cirrhosis with hepatic encephalopathy,to remove the incentive is the key of HE with prevention and treatment of hepatitis B cirrhosis.

Glia maturation factor(GMF)detected in the adult brain is an acidic proteinthat has the ability to promote the morphological and chemical differentiation ofastroblasts,but this effect is reversible.In the present study we have establishedneuron-enriched dissociated primary cultures from 7-day-old rat cerebellar cortexin which about 97% of the cells are neurons especially granule cells using differential count of indirect immunofluorescence.The addition of purified GMF to the culturesmarkedly enhances neuronal survival,while control cultures grown in the absence ofGMF exhibit a significant decrease in neuronal number.These GMF effects aredose-dependent,with optimal stimulation occurring at a concentration of 250 ng/ml.Although the mechanism by which purified GMF influences cerebellar corticalneuronal survival is not known,these results suggest that GMF not only affectsglial cells,but also may function as a neurotrophic agent in the central nervousmetsys
