1.Comparison of China-and Germany-made Electromagnetic Lithotripters for Upper Urinary Tract Calculi:Preliminary Report of Perspective Study

Objective To perspectively compare the clinical outcomes of China-and Germany-made electromagnetic lithotripters in the treatment of upper urinary tract calculi.Methods Between January 2005 and August 2005,60 cases of upper urinary tract calculi were randomly divided into two groups:China-made and Germany-made groups(30 in each).Extracorporeal shock wave lithotripsy(ESWL)was performed by a same group of surgeons.Results The stone-free rate was 73%(22/30)in China-made group,and was 90%(27/30)in Germany-made group 3 weeks after ESWL(?2=2.283,P=0.095);and was 100% in both the groups 3 months postoperation.The retreatment rate was 27%(8/30)in China-made group,and 10%(3/30)in Germany-made group(?2=2.283,P=0.095).The rate of auxiliary procedure in both the groups was 0.The efficacy quotient(EQ)was 0.79 in China-made group,and was 0.91 in Germany-made group.Totally,39 ESWLs were performed on the patients in China-made group,while 33 ESWLs were carried out in Germany-made group.The rates of macroscopic hematuria,skin injury,and renal colic were 87%(34/39),64%(25/39),and 8%(3/39)respectively in China-made group,and were 91%(30/33),6%(2/33),and 6%(2/33)in Germany-made group(?2=0.016,P=0.900;?2=25.693,P=0.000;and ?2=0.000,P=1.000;respectively).One patient in China-made group developed fever after ESWL,while no one in Germany-made group had the symptom.In both the groups,no patients had hematuria,renal edema,or subcutaneous perinephric hemorrhage.Conclusion Both China-and Germany-made electromagnetic lithotripters are effective,safety,and non-invasive.
2.Construction of linker for activation of T cells gene eukaryotic expression vector in mouse

Objective To construct the eukaryotic expression vector of linker for activation of T cells(LAT) gene in mouse. MethodsLAT cDNA of mouse mononuclear cells was amplified with polymerase chain reaction.The fragment was inserted into the eukaryotic expression vector of pCMV-HA plasmid.The recombinant plasmid was verified by DNA sequencing and used to transfect lymphocytes of the asthmatic model. Results The length of amplified fragment was 729 bp.The sequence of LAT gene was confirmed by Genbank.The LAT protein could be detected in the lymphocytes of asthmatic model.Conclusion The recombinant eukaryotic expression vector pCMV-HA-LAT can be successfully constructed.

4.The nursing care for patients of hepatocellular carcinoma treated with transcatheter hepatic arterial chemoembolization and high intensive focus ultrasound

Objective To discuss the nursing measures for patients of hepatocellular carcinoma treated with transcatheter hepatic arterial chemoembolization(TACE)and hish intensive focus ultrasound (HIFU).Methods During the period of Aug.2008-Aug.2009,TACE together with HIFU were performed in 40 patients with hepatocellular carcinoma.The perioperative nursing measures were summarized.Results During hospitalization no severe complications,such as dangerous infection,intestinal bleeding,etc.occurred in all patients.Conclusion Correct and proper periopemtive nursing care is of significant importance for the prevention of complications and for the therapeutic effectiveness.
5.CD36 expression in monocytes and ankylosing spondylitis

BACKGROUND:Ankylosing spondylitis is an autoimmune disease involved in chronic systemic inflammation. Tumor necrosis factor and interleukin-6 levels increased in patients with ankylosing spondylitis. Inflammatory factors such as tumor necrosis factor and interleukin-6 can suppress CD36 expression in monocytes. OBJECTIVE: To analyze the correlation between CD36 expression in monocytes and ankylosing spondylitis. METHODS:A total of 84 newly diagnosed ankylosing spondylitis patients and 111 healthy individuals were included in this study. CD36 expressions in monocytes in ankylosing spondylitis patients and healthy individuals were tested using flow cytometer; meanwhile, biochemistry, immunology, routine blood examination and related inflammatory markers were determined between the two groups. RESULTS AND CONCLUSION:Results of baseline data in both groups demonstrated that CD36 fluorescence intensity in monocytes was significantly lower in patients with ankylosing spondylitis compared with healthy controls (P < 0.01). CD36 fluorescence intensity in monocytes was negatively correlated with C-reactive protein, erythrocyte sedimentation, interleukin-6 and tumor necrosis factor. In addition, CD36 fluorescence intensity in monocytes was negatively correlated with BASDAI score. Logistic regression analysis showed that erythrocyte sedimentation, interleukin-6, tumor necrosis factor and CD36 fluorescence intensity in monocytes were associated with ankylosing spondylitis, and risk factors for ankylosing spondylitis (P < 0.05). These findings confirm that inflammatory cytokine in patients with ankylosing spondylitis weakened the expression of CD36 in monocytes. There was a remarkable association between low expression of CD36 expression in monocytes and ankylosing spondylitis. CD36 expression of monocytes clinicaly may be considered to be an effective indicator to evaluate inflammation and disease activity in patients with ankylosing spondylitis.
6.Holmium laser combined with choledochoscopy for the treatment of common bile duct stones

Objective To discuss the value of holmium laser lithotripsy combined with choledochoscopy in the treatment of complicated common bile duct stones.Methods Under choledochoscopy,200 ?m fibre holmium laser lithotripsy of common bile duct stones was performed.The settings on the laser were 1.0 J and 10 Hz.The choledochoscope was used to access the common bile duct stones.The stones were fragmented and then extracted with irrigation or by using a basket. Results The stones were successfully removed in all the 17 cases.The duration of operation was 10~50 min(mean,20 min).Out of the 17 cases,there were 8 cases of primary closure of the common bile duct,6 cases of T-tube drainage, and 3 cases of stone removal via drainage passage.No bile duct bleeding or biliary leakage occurred after operation.Follow-up was conducted in 16 cases for 6~15 months(mean,10 months).B-ultrasonography or magnetic resonance cholangiopancreatography found no recurrent or residual stones,or bile duct stricture.Conclusions Holmium laser lithotripsy combined with choledochoscopy in the treatment of complicated common bile duct stones has advantages of minimal invasion,rapid recovery,satisfactory lithotripsy effects,simplicity of performance,and high reliability.

Aim To study cytotoxic effects and mechanisms of unsaturated fatty acid from mao er yan of gansu origin on BEL-7402 strain cells.Methods The essential components of unsaturated fatty acid from mao er yan of gansu origin were determined by gas chromatography-mass spectrometry and cytotoxic effects of un-saturated fatty acid on BEL-7402 strain cells were determined by using flow cytometry,methods of MTT [3-(4,5-dimethyl thiazol-2yl)-2,5-diphenyltetrazolium bromide] and electron microscope scanning after 48 hours of the unsaturated fatty acid from mao er yan of gansu origin treatment at concentrations of 0.2,0.8 and 3.2 mg?L-1,respectively.Results The unsaturated fatty acid from maoer yan of gansu origin had significantly cytotoxic effects on BEL-7402 strain cells and inhibitory rates for proliferation of BEL-7402 strain cells were 34.6%、44.8% and 70.7% at those three concentrations,respectively.50% inhibiting concentration(IC_ 50)was 0.812 mg?L-1.Apoptosis rates were 18.6%,22.7% and 24.2%,respectively,and occurred cell membrane microvilli breakdown and cell rupture,compared to those of the control group.Conclusion The unsaturated fatty acid from mao er yan of gansu origin has significantly cytotoxic effects on BEL-7402 strain cells and their mechanisms may be related to inhibit cell division,destroy the structure of cell membrane and mitochondria.
10.Preparation of Vitamin B1 Sustained-release Tablets

Objective: To screen the optimal formula and preparation process of vitamin B1 ( VB1 ) sustained-release tablets. Methods:The single-factor tests were used to investigate the type of filler, type and amount of matrix material hydroxypropyl methycel-lulose(HPMC), type and amount of retarder, tablet weight and core hardness. The orthogonal tests were used to investigate the amount of HPMC and ethyl cellulose( EC) and the core hardness. Results:The optimum formula was as follows:HPMC accounted for 50% of the tablet weight, EC accounted for 20%, the filler was lactose, the tablet weight was 200 mg and the tablet core hardness was 50 N. Conclusion:The prepared sustained-release tablets have significant controlled-release property, which provides reference for the devel-opment of new dosage form for VB1 .
