3. Molecular characteristics of Acinetobacter baumannii and the changes of efflux pump expression under tigecycline pressure

Objective Acinetobacter baumannii (A. baumannii) is a commonly infective bacterium in the hospital. This study aims to analyze its molecular epidemiological characteristics, detect the carrying rate of efflux pump and regulatory protein genes, and investigate the effects of tigecycline on the efflux pump and expression of regulatory protein genes. Methods A total of 183 A. baumannii strains were collected from inpatients of the affiliated hospital of Jiangsu University from May 2017 to March 2019. They were divided into an antimicrobial-resistant group (one or more antimicrobial-resistant strains, 139 strains) and a sensitive group (the drugs in the drug sensitivity test were all non-resistant strains, 44 strains). Repeated sequence PCR was used for homology analysis of the strains, and pulse-field gel electrophoresis (PFGE) was used as the gold standard for homology analysis to verify and compare some strains. PCR was used to detect the occurrence of drug resistance-related genes. Based on homology analysis, efflux pump carrying rate detection and antibiotics sensitivity test results, 6 clinical strains carrying all efflux pump genes but different resistance phenotypes were selected as experimental strains, including sensitive strains (SAB), the multidrug resistance strain (MDRAB) and the extensively drug-resistant strain (XDRAB). All strains were induced in vitro with the minimum inhibitory concentration (MIC) of tigecycline. The induced strains were categorized as induction group, and the same strains cultured in LB agar without tigecycline was used as a control group. MIC was used to analyze the tigecycline susceptibility, and RT-qPCR was used to detect the gene expression of efflux pumps, such as TetB, AbaQ and regulatory proteins (AdeS and BaeS), in drug-resistant strains. Results Homology analysis showed that there were 45 clonal groups in the detected clinical isolates, with no obvious outbreak of epidemic clonal groups. Efflux pumps and regulatory proteins were widely distributed in the clinical isolates, and the expression of AdeB, TetB, AbeS, AdeS in MDRAB and XDRAB is significantly higher than that insensitive group SAB. Continuous in vitro induction with tigecycline could increase the antimicrobial resistance of some clinical strains and even significantly increase the expression levels of efflux pumps and regulatory proteins. Conclusion A. baumannii is widely distributed in the clinic, and efflux pumps and regulatory proteins might play an important role in drug resistance process. The unreasonable use of tigecycline could enhance the tolerance of A. baumannii by up-regulating the expression of some bacterial efflux pumps.
4.Effect of yiqi huoxue formula on healing quality and recurrence rate of peptic ulcer.

OBJECTIVETo study the effect of yiqi huoxue formula (YHF) on healing and recurrence of peptic ulcer.
METHODSAdopting randomized and controlled principle, the 46 patients with active peptic ulcer were randomly allocated into the treated group and the control group, they received respectively the treatment of YHF + Omeprazole and Omeprazole alone for 5 weeks. If exhibiting positive Hp, the Trigeminy one-week eradicating treatment, using Omeprazole, Clarithromycin and Amoxicillin, was applied firstly, after then, followed with the prescriptive treatment. Gastroscopy was performed before and after treatment to assess the status of Hp eradicating and the endoscopic and histological maturity of regenerated mucosa. Meanwhile, the changes of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) levels were detected with ELISA. Patients with ulcer healed were followed up for 6 months to observe the condition of recurrence.
RESULTSThe percentage of Sc phase maturity and excellent histological maturity of regenerated mucosa obtained in the treated group were more than those in the control group respectively (P < 0.05), and levels of VEGF and bFGF increased significantly in the treated group after treatment (P < 0.05). But the difference of ulcer recurrence rate between the two groups in the 6-month follow-up study showed no statistically significance. However, the recurrence rate in patients with excellent histological maturity of regenerated mucosa was lower than that with good histological maturity (P < 0.05); and it was decreasing along with the progressing of regenerated mucosa endoscopic maturity, i.e., Sc < Sb < Sa (P < 0.01).
CONCLUSIONYHF can improve the quality of ulcer healing, and its mechanism is possibly related to the increase of VEGF and bFGF expressions. The higher the quality of ulcer healing, the lower the recurrence rate.

5.Study on the soil fertility changes in planting base to develop the special fertilizer for cultivation of Chrysanthemum morifolium.

OBJECTIVETo study the changes of soil fertility in Sheyang county where Chrysanthemum morifolium has been cultivated for more than 30 years and to develop the special fertilizer for cultivation of C. morifolium.
METHODThe pH values, organic matter and the contents of total and available N, P, K and Zn in the soil layer of 0 to 40 cm, as well as the total N, P, K and Zn contents in the flowers, roots, stems and leaves of the plants, were analysed. The balanced fertilization plan for cultivation of C. morifolium was put forward. In addition, the formula of special fertilizer for cultivation of C. morifolium was determined according to flower yield and utilization rate of N, P, and K.
RESULTS AND CONCLUSIONThe soil had high pH values and high soil salt contents, with unbalanced application of N, P, and K fertilizers and a shortage of available Zn after cultivation of C. morifolium. The contents of soil organic carbon, N and P declined with increasing cultivation time of C. morifolium, which resulted from the improper rotations and fertilization. The balance fertilization practice and the special fertilizer utilization are effective ways to improve soil fertility for C. morifolium.

6.IDH1 Gene Mutation and Its Clinical Significance in patients with Acute Myeloid Leukemia.

OBJECTIVETo evaluate the incidence rate of IDH1 in acute myeloid leukemia and analyze its effect on clinical characteristics and prognosis.
METHODSMononuclear cells in bone marrow samples were collected from 192 adult patients with newly diagnosed AML. Polymerase chain reaction (PCR) and direct sequencing were used to amplify exon 4 of IDH1 gene, the gene sequencing was used to analyze the gene mutations, at same time, the detection of NPM1, FLT3-TKD, FLT3-ITD, C-KIT, CEPBA, TET2 and JAK2V617F and MLL mutations were carried out, the follow-up was used to determine its therapeutic efficacy and outcomes of patients. The clinical and laboratory data of these cases were collected, and their clinical characteristics and prognosis were then analyzed.
RESULTSAmong the 192 AML patients, 13 cases were detected with IDH1 gene mutation, the mutation rate was 6.77% [95% CI (5.70%-13.38%)]. The sequencing chart of IDH1 gene showed double peaks, the mutations were heterozygous, out of them c.G395A (p.R132H) was found in 8 cases, c.C394T was found in 4 cases (p.R132C), c.C394A (p.R132S) was found in 1 cases, R132H and R132C are common, 13 cases showed missense mutation. The median age in mutation group was 52 years old, the median age in unnutration group was 40 years, there was significant difference between them (P = 0.010). Mutation rate of IDH1 gene in M1 and M2 was significantly higher than that in other FAB subtypes. There were no significant difference in sex, newly diagnosed peripheral white blood cell count, hemoglobin, platelet count, peripheral blood and bone marrow original cell proportion of primitive cells between them. Mutation of IDH1 gene had certain correlation with NPM1 gene mutation, but no correlation with FLT3-TKD, FLT3-ITD, C-KIT, TET2 and JAK2V617F and MLL natations was found. In addition, the IDH1 mutation easily occurred in patients with normal karyotype or in patients with middle prognostic risk karyotype, IDH1 mutation occurred in 11 cases with normal karyotype, the mutation rate was 10.28%, IDH1 mutation were observed in 2 cases with abnormal karyotype, the mutation rate was 3.50%, there was significant difference. In AML patients with middle prognostic risk karyotype. The complete remission (CR) and the 3 year survival (OS) rate of IDH1 mut patients were less than that in IDH1 wt, there was significant difference (P < 0.05).
CONCLUSIONSThe IDH1 mutation more easily occurr in older AML patients and mutations effect of IDH1 on clinical characteristics may represent a molecular marker for poor prognosis in AML.

7.Distribution of Type 9 T Helper Cells and Expression of Transcriptional Factors in Acute Myeloid Leukemia.

OBJECTIVETo investigate the distribution of T helper (Th9) cells and its relationship with clinical characteristics of patients with acute myeloid leukemia (AML), and to analyze the activating levels of different transcriptional factors in Th9 cells.
METHODSThe peripheral blood specimens of 102 AML patients and 83 healthy persons as controls were collected, then the T cells of peripheral blood in AML patients and controls were isolated by using CD3 magnetic beads, the mRNA expression of IL-9 was detected by real-time quantitative PCR, the Th9(CD4IL-9) cell levels in diffrent stages and activating level of Th9 coexpression with IL-9 were detected by flow cytometry.
RESULTSThe mRNA expression of IL-9 in peripheral blood of AML M2 and M3 patients was significantly higher than that in control groups (P<0.01), at same time the CD4IL-9cell rate was significantly higher than that in control group also(P<0.01). The results of dynamically monitoring the distribution of Th9 cells in AML-M2 and M3 patients showed that the Th9 cell rate and the mRNA expression of IL-9 in newly diagnosed M2 and M3, and relapsed M2 groups were significantly higher than those of M2 and M3 in remission (P<0.01); the detection results of IL-9co-expression with transcriptional factors (SMAD3, IRF-1and IRF-4) indicated that the percantage of Th9 pSMAD3cells in peripheral blood of newly diagnosed and relapsed M2 and M3 patients was significantly higher than that in M2 and M3 patients in remission (P<0.01); on the contrary, the percentage of Th9 IRF-1cells in peripheral blood of M2 and M3 patients in remission was significantly higher than that in newly diagnosed M2 and M3 patients (P<0.01).
CONCLUSIONThe distribution of T helper cells in peripheral blood of AML-M2 and M3 patients significantly increases, moreover, correlates with disease status. The prediction of Th9 cell functions should be performed in combination with it transcriptional factors which have inmportant significance for microenvironment of tumors in AML patients.
8.Classification and management of the tandem ossification of the posterior longitudinal ligament and flaval ligament.

OBJECTIVETo provide appropriate guidelines for treatment of tandem ossification of the posterior longitudinal ligament (OPLL) and flaval ligament (OFL). Data sources Published articles about OPLL and OFL were selected using Medline and Embase electronic databases. Study selection An English literature search from January 1980 to December 2006 was conducted. Because many reported cases were incorporated in OFL studies, the key words for search were OFL or OFL and OPLL. The first step revealed 93 studies of which 13 reports of tandem OPLL and OFL (tandem ossification) were selected.
RESULTSAll studies were case series or case report and advocated that the primary therapy for tandem ossification should be operative. The clinical outcomes of surgery were evaluated in most reports, predominantly using the JOA scores. Gender is the only factor which has prognostic value. A higher proportion of women was found in the failure group. A two-stage classification of tandem ossification was developed to relate diagnosis to outcome.
CONCLUSIONSAll patients with suspected ossification of the spinal ligaments should undergo routine MRI screening of the whole spine. The correlation of the classification with surgical treatments needs further studies to validate its usefulness.

9.Overexpression of resistin affect 3T3-L1 adipocyte lipid metabolism.

OBJECTIVETo investigate the effect of resistin overexpression on 3T3-L1 adipocyte lipid and glucose metabolism.
METHODSExpression vector for rat resistin gene was constructed and transfected into 3T3-L1 adipocytes. Cell differentiation and lipid accumulation was determined by Oil Red O staining. Differentiation marker genes (pref-1, C/EBPalpha, FAS) and glucose transporter 4 (GLUT4) gene mRNA expressions were evaluated by reverse transcription-PCR (RT-PCR). Triglyceride (TG) and free fatty acids (FFAs) in adipocytes were measured by colorimetric kit.
RESULTS(1) In resistin-overexpressed adipocytes, the lipid droplets presented at the second day which was earlier than the control cells. (2) The expression of C/EBPalpha and FAS genes in resistin-overexpressed adipocytes were up-regulated and the pref-1 was down-regulated compared with that of the control cells. (3) In resistin-overexpressed adipocytes, cellular TG and FFAs levels were significantly increased (P<0.05). (4) There was no difference in the expression of GLUT4 gene between 3T3-L1 adipocytes and resistin-overexpressed adipocytes (P> 0.05).
CONCLUSIONOverexpression of resistin can affect 3T3-L1 adipocyte lipid metabolism and thereby result in obesity and insulin resistance, but have no effect on GLUT4 gene expression.

10.Pelvic vas deferens abscess: a case report and review of the literature.

OBJECTIVETo investigate the clinical characteristics, diagnosis, misdiagnosis and treatment of pelvic vas deferens abscess.
METHODSThe clinical data of one case of pelvic vas deferens abscess were reported and the relevant literature was reviewed.
RESULTSThe patient underwent incision and drainage of pelvic vas deferens abscess. After the catheter was removed, he could urinate easily. With a follow-up of one year, he was found free of dysuria and recurrence.
CONCLUSIONPelvic vas deferens abscess tends to be misdiagnosed when enwrapped. Surgical procedures are the most effective treatment for this disease.
