2.Measurements and analysis of breast morphology changes after breast augmentation by 3D scanning

Objective To evaluate the application of three-dimensional (3D) scanning in the measurements and analyses of breast morphology changes that occur after breast augmentation. Methods Pre- and postoperative breast 3D images were obtained in 18 patients (36 breasts) undergoing augmentation mammaplasty by 3D scanning. All implants were round sillicone and planted under pectoral muscle. Average implant volume was 232.2 mL, and average projection was 3.46 cm. The number of implants in low profile implant group was 12( h < 3.0 cm), and 16 in moderate group ( 3.0 ≤ h < 4.0 cm), 8 in high group ( h ≥ 4.0 cm). The following parameters were measured by Geomagic 10.0: volume, projection, angle and distance of key landmarks, and symmetry degree. Results The increased volume of breast was corresponding with the implant volume (P >0.05). Postoperative breast projection was 12.9 %, less than expected projection (preoperative + implant projection). The values in low, moderate, and high profile group were 5.2 %, 15.1%, and 20.2 %,respectively. Conclusion 3D scanning is a quick, convenient, accurate and non-contact method for measurement of breast morphology, and it is promising in breast plastic surgery in the future.
3.Efficacy Observation and Pharmacokinetic Study of Dexmedetomidine Combined with Propofol for Sepsis In-fection Deliration in Patients Underwent Continuous Intravenous Blood Replacement

OBJECTIVE:To observe therapeutic efficacy of dexmedetomidine combined with propofol for sepsis infection deliration in patients underwent continuous intravenous blood replacement,and to study pharmacokinetic study. METHODS:60 patients with sepsis infection deliration underwent continuous intravenous blood replacement were selected and randomly divided into observation group and control group with 30 cases in each group. Control group was given propofol 0.5 mg/kg,and observa-tion group given dexmedetomidine and propofol. Therapeutic efficacy of 2 groups were analyzed comparatively,and pharmacoki-netic parameters of propofol were studied. RESULTS:The pharmacokinetic parameters of propofol as elimination half-life period (t1/2β),slow distribution half-life period (t1/2α) were shorter than literature reference value,with statistical significance (P<0.05),while the distribution rate constants(K21,K13,K10),the central compartment volume of distribution(VC),systemic clear-ance(CL),rapid distribution half-life period(t1/2pi)had no statistical significance,compared with literature reference value(P>0.05). After treatment,ICDSC score of 2 groups were decreased significantly,with statistical significance(P<0.05),while the observation group was significantly higher than the control group,with no statistical significance before and after treatment(P>0.05);the average hospital stay of observation group was significantly shorter than that of control group,with statistical signifi-cance(χ2=6.263 4,P=0.000);the incidence of ADR in control group was significantly higher than the observation group,with statistical significance(P<0.05). CONCLUSIONS:Dexmedetomidine combined with propofol have good effect on sepsis infec-tion deliration in patients underwent continuous intravenous blood replacement,and can reduce the length of hospital stay and the incidence of ADR.
4.Construction and expression of eukaryotic expression vector for recombinant human plasminogen Kringle 5 gene

Objective To construct baculovirus expression system for the human plasminogen kringle 5(hK5) gene,and detect its expression in Spodoptera frugiperda 21(sf21). Methods Baculovirus Bacmid-rhK5 was prepared with bac-to-bac baculovirus expression system,and sf21 cells were infected.The expression of rhK5 in sf21 cells was determined by Western blot.The activity of purified protein was detected by vascular endothelial cell inhibition test. Results The pFastBac HTB-rhK5 was successfully constructed,and sf21 cells were transfected with the constructed vector.The output of rhK5 obtained was 90 ug/L,and the protein possessed the inhibitory activity of endothelial cell proliferation.The median effective dose(ED50) was 4 ug/mL. Conclusion The rhK5 baculovirus expression vector is highly expressed in sf21 cells,which lays a foundation for the the expression of rhK5 protein with biologic activities.

7.Relation of self-esteem, implicit self-esteem and perfectionism in the depressed patients

Objective To explore the relationships of self-esteem, implicit self-esteem and perfectionism in the depressed patients. Methods Both of the depression group (n=50) and control group (n=45) completed the self-esteem scale (SES), the self-acceptance questionnaire (SAQ), the Chinese Frost multidimensional perfectionism scale (CFMPS), the implicit association test and the Raven’s Standard Progressive Matrices. The implicit association test were performed before (pre-IAT) and after (later-IAT) the Raven’s Standard Progressive Matrices. Results The SES score was lower in depression group than in control group (P=0.002). To the IAT score, the interaction effect of group and time achieved no statistically different (P=0.735). The group main effect of IAT was significant (P=0.001). The IAT score was higher in de?pression group than in control group (P=0.013). The time main effect was significant (P=0.033). The pre-IAT was higher than later-IAT (P=0.007). The depression group had higher scores of concern over mistakes (P=0.007) and doubt about action (P=0.006) dimension of CFMPS, and lower score of parental expectations (P=0.038) dimension than the control group. The later-IAT was negatively related with parental expectations dimension and the total score of CFMPS in the de?pression group (P<0.05). Depressed patients’SES and SAQ scores were negatively related with concern over mistakes and doubt about action dimension and the total score of CFMPS (P<0.05). Conclusion The instability of implicit self-es?teem of depressed patients is associated with negative perfectionism.
8.Acupuncture intervention combined with assisted reproductive technology: its different effects at different time points during the in vitro fertilization-embryo transfer course.

Recently the combination of acupuncture with assisted reproductive technology (ART) to increase the outcomes of ART is being widely studied. In this article, the literatures concerning random controlled clinical trials since 2002 are reviewed and the designs of the trials, especially the timing of acupuncture, are evaluated. Over the past 5 years, the related clinical trials have primarily showed that acupuncture done immediately before and after embryo transfer might increase the assisted reproduction rates, but still requiring further high quality trials with large samples; in addition, different stimulation modes could produce different result, and so far there has not been a consensus as to the optimal time-point for the acupuncture intervention during the in vitro fertilization-embryo transfer (IVF-ET) course. Since the effects of acupuncture change with women's endocrine cycles, it is important and possible to make a breakthrough in ART outcomes if acupuncture is performed at a suitable time point during the cycle of IVF/ET combined with ART.
9.Exploring the effects of Chinese medicine in improving uterine endometrial blood flow for increasing the successful rate of in vitro fertilization and embryo transfer.

Endometrial blood flow is directly related to endometrial receptivity thereby affecting in vitro fertilization and embryo transfer (IVF-ET) outcomes. In recent years a growing number of studies have shown that traditional Chinese medicine (TCM) can play a role to improve endometrial blood flow and embryo transfer. Studies have confirmed that formulas based on reinforcing kidney and activating blood can promote the formation of uterine endometrial blood vessels by adjusting expressions of a variety of vessel growth factors, and regulating nitric oxide level for inhibition of vascular smooth muscle contraction of the uterus. Treatments based on differentiation of syndromes are key to the theory of TCM. Differentiation of syndromes should be combined with biomedical disease diagnosis. It is also necessary to further clarify other endometrial blood flow disorders using TCM diagnostic methods. In these cases, drugs for reinforcing kidney and activating blood are relevant, but other medicines for smoothing liver qi and nourishing blood, as well as tonifying the spleen and generating blood may also be effective. Future clinical studies should focus on the observation of different types of TCM syndromes and the research on compatibility ratio and dose-dependent relationship of Chinese medicines. Acupuncture has been used during IVF-ET for more than 10 years. Electro-acupuncture can control the expansion of the uterine arteries by inhibiting the sympathetic nerves. However, acupuncture has not been proved to regulate endometrial arteries. To sum up, TCM can be applied to improve endometrial blood flow so as to increase birth rates in IVF-ET. The combination of Chinese and Western medicines and acupuncture application will increase their combined effect, thereby obtaining greater clinical benefits.
10.Packing effects of different hemostatic materials during functional endoscopic sinus surgery: histocompatibility comparison

BACKGROUND:At present, the most common materials for clinical hemostasis are absorbable tampons and Vaseline gauze. OBJECTIVE:To compare the packing effects of absorbable tampons and Vaseline gauze in functional endoscopic sinus surgery. METHODS:Totaly 100 sinusitis patients, 57 males and 43 females, aged 16-61 years, with a history of 5 months to 23 years, were randomized into two groups: patients in the observation group were treated with absorbable tampons and patients in the control group treated with Vaseline gauze during functional endoscopic sinus surgery. Patient's discomfort, bleeding when the filer was extracted at 48 hours, and nasal bleeding within postoperative 24 hours were compared between the two groups. RESULTS AND CONCLUSION: No adverse reaction related to absorbable tampons occurred in the observation group, indicating that the absorbable tampon has better histocompatibility. Patient's discomfort, bleeding when the filer was extracted at 48 hours after operation, and nasal bleeding within postoperative 24 hours were significantly improved in the observation group compared with the control group (P < 0.05). These findings suggest that the absorbable tampons has a better hemostatic function, which can effectively reduce postoperative nasal bleeding and relieve postoperative discomfort in patients with sinusitis.
