1.A Case of Reticulum Cell Sarcoma(Microglioma) of the Brain.

A rare case of primary reticulum cell sarcoma(microglioma) of the brain in a 30-year-old female is reported. The tumor was located in the left parietal lobe. Brain CT scan showed a slightly high attenuated area with well demarcation in the left parietal lobe, and a tumor staining was appeared in the delayed arterial phase of the left carotid angiogram. Osteoplastic craniotomy on the left parietal area was performed for removal of tumor. Then the patient received radiation therapy with a good result.

2.CT Evaluation in Patients with Intraventricular Hemorrhage.

Ninety-three patients who had evidence of intraventricular hemorrhage(IVH) were reviewed. Primary and secondary IVH could be well distinguished by computerized tomographic(CT) brain scan. 11 patients had primary pure IVH and remaining 82 patients were secondary IVH from mainly hypertensive intracerebral hemorrhage. Four patients died and 7 survived in the pure IVH. Three of the 4 who died were aneurysmal rupture. Remaining one was unknown etiology. In CT scan hemorrhage was most frequently found in the occipital horns according to the supine position of the patient during the examination and least found in the temporal horns probably due to narrow crescentic shape. In the secondary IVH the distribution of the blood within the lateral ventricle was mainly ipslateral to the intracerebral hematoma. The point of penetration of the intracerebral hematoma into the lateral ventricle was the posterior body in the basal ganglia or thalamic hemorrhage, the frontal horn in the aneurysmal rupture and the trigone in the arteriovenous malformation. Although the level of consciousness on admission related to the size and location of primary intracerebral hemorrhage or IVH, the ventricular dilatation itself also seemed to contribute to the consciousness state. The degree of temporal horn dilatation correlated more with the clinical grade than the frontal horn dilatation.

3.The Nutrient Intakes of Rural High-Schoolers with Depressire Tendencies.

This study was carried out to evaluate high-school students' nutrient intakes according to the status of depressive trends, and to analyse the in physical and mental health status. The volunteers were 560 high school students selected from 8 provinces, but 511 students were analyzed. A was used for evaluation, The questions were composed in consideration of socio-demographic characteristics, physical health condition by CMI, mental condition assessed by a modified Zung's self-rating depression scale (SDS), and the evaluation of food consumption by a 24 hours recall method. The family situations of the subjects were mostly nuclear family(73%), agricultural work(53%), and having religion(53%). Only 3% of the students suffered depression where as 19% showed depression trendencies. The individual factors that affected their mental condition were family farming and consciousness of academic accomplishment. The health status counted with CMI was 6.2/20 scores. The non-depressed trend group of students had a lower score of CMI than others. The overall nutrient intakes of students were low compared with the Korean RDA for adolescents. The Mean Adequacy Ratio(MAR) of nutrient intakes was only 0.45 +/- 0.23. Iron, vitamin A, and riboflavin intakes were significantly low and correlated with depressive trend score. The result from the regression analysis showed that CMI, study accomplishment, and vitamin A were the most significant factors influencing mental condition. We concluded that Korean rural high-school students were healthy in mental condition and showed lower rates of depression academic. For there more we could say that the intakes of iron and vitamin A also influenced mental health. Findings confirmed the need of becoming hopeful job of farming and better nutritional status. These situations would be necessary for students to ameliorate the quality of rural living.

4.The Dietary Behavior and Nutrient Intake Status of the Yooth in Rural Areas of Korea.

The purpose of this study was to explore the relationship between dietary behavior and the nutrient intake status in the youth of rural areas, based on their personal characters such as their sex, age, family type, number of family members, mother's age, occupation, and school career. This study was made by use of a self-administrated questionnaire with a 24-hour recall for 5days as an instrument with 439 randomly selected students. Statistical methods applied to analyze the data were frequency, percent, Willcoxon Rank-sum test, Kruskal-wallis test, x2-test by contingence table, and Spearman's correlation coefficient in non parametric statistical cases. Some of the interesting results are as follows : 1) The correlation between sex and breakfast is highly significant. That is, over 80% of male students had breakfast every day, whereas only about 60% of female students had breakfast every day. Bad table manners and some other problems are insignificant with respect to the personal characters in the relations between the personal characters and eating behaviors. However, the regularity of every meal, the frequency of having meals with other family members, and the frequency of meals eaten out and snacks show significant differences. 2) The mother's occupation does not have any influence on the nutrient intake of their students when the relationship between personal characters and nutrient intake was analysed. On the other hand sex, the mother's school career, the numbers of family members, the type of family, and the income of the family showed significant differences nutrient intakes. 3) The regularity and the frequency of having meals, meals eaten out with their family members, and the table manners during meals showed significant differences in the intake of nutrient elements such as calorie, protein, fat, fiber, calcium, iron, phosphorus, vitamin B1, vitamin B2, niacin and vitamin C. However some consideration during meals and problems in eating habit are insignificant with regard to nutrient intake.

5.EEG Abnormalities in Henoch-Schonlein Purpura.

Schonlein-Henoch purpura is a generalized small vessel vasculitis characterized by nonthrombocytopenic purpura, arthritis, abdominal pain and nephritis. In 1914 Osler described an allergic purpura associated with hemiplegia. After then Lewis et al. reported the cases of Schonlein-Henoch purpura associated with convulsion, coma, confusion, intracranial hemorrhage, and chorea, CNS complication has been reported in 1-8% of children and subsided spontaneously in most cases. Headache is a remarkable sympton and appears nonspecific nature. In 1991 Ostergaard and Storm reported that headache occured during the first week following skin rash and frequently showed abnormal EEG findings. We investigated prospectively the presence of a possible cerebral and renal involvement in the case of Schonlein-Henoch purpura. EEG abnormality demonstrated in 52.6% of all cases, and headache or irritability in 47.4% of all cases. A significant association was found between abnormal EEG finding and presence of headache, but was not found between EEG findings and presence of renal involvement and hypertension. Patients with abnormal EEG had no Past or famity history of febrile convulsion or ididopathic epilepsy.

6.A Case of Puerperal Cerebral Hemorrhagic Infarction with Venoue Thrombosis.

A case of puerperal cerebral hemorrhagic infarction with venous thrombosis in a 36-year-old female is reported. Brain CT scan showed an isodensity area surrounded by ill-defind low density medially in the right frontal lobe, and the most anterior portion of superior sagittal sinus and its draining veins were prominent, but draining veins in the frontal high convexity were not filled in the venous phase of the right carotid angiogram. Osteoplastic craniotomy on the right frontal area was performed to remove the lesion which was mistakenly understood as a mass. Abnormal brain parenchyma was excised from the right frontal subcortical region. Microscopic and pathological evaluation confirmed the diagnosis of hemorrhagic infarction due to venous thrombosis.

7.A Case of Myocardial Bridge in a Patient with Hypertrophic Cardiomyopathy.

Although myocardial bridge is not thought to have any hemodynamic significance in most cases, some have suggested that when it produces severe systolic narrowing, ischemia or infarction may result. Myocardial bridge in adults with hypertrophic cardiomyopathy may be associated with a higher incidence of sudden death, myocardial wall-motion abnormalities, and perfusion defects on thallium-201 scintigraphy. When myocardial bridge is associated with left ventricular hypertrophy, it is known to affect longer segment and cause more severe compression during systole. We report a case of hypertrophic cardiomyopathy with myocardial bridge at the middle part of the left anterior descending coronary artery, who also showed reversible perfusion defect on the thallium scan at the same coronary territory.

8.A Case of Lumbar Extradural Meningioma.

A 27-year-old female with low back pain and paresthesia on the left lower extremity had been treated. Lumbar myelography showed total block of the dye column at the L3 vertebral body level with a brush border. After myelography, total laminectomy on the L3 and L4 vertebrae was done. The result of operation and biopsy revealed an extradural meningioma of angioblastic type. 9 days after the surgery all neurological deficits were completely recovered.

10.A Clinical Observation of Neonatal Hyperbilirubinemia Due to ABO incompatibility.

A clinical and statistical study was performed on 213 neonates of hyperbilirubinemia who were admitted to department of pediatrics of Korea University Hospital from Jan. 1989 to Jun. 1991. and we studied the difference between ABO compatible group and incompatible group, such as, day of apperance of hyperbilirubinemia, tendency of bilirubin concentration change, reticulocyte, hemoglobin and duration of hyperbilirubinemia. The following results were obtained. 1) There were no difference of initial body weight and gestational age between compatible (3.0 +/- 0.72kg, 38.2 +/- 3.2 weeks) and incompatible groups (3.2 +/- 0.54kg, 39.1 +/- 2.8 weeks). 2) Onset of hyperbilirubinemia within 2 days in the incompatible group (40.5%) was earlier than the compatible group (23.5%) significantly (p<0.05). 3) Mean bilirubin level in the incompatible group was higher than the compatible group significantly (p<0.05). 4) Peak bilirubin level in the incompatible group(18.7+/-4.1mg/dl) was higher than the compatible group (14.8+/-3.4mg/dl) significantly (p<0.05). 5) There were no difference of hemoglobin level, reticulocyte count, coombs positive finding and the ratio of exchange transfusion between two groups.
