2.EEG Abnormalities in Henoch-Schonlein Purpura.

Schonlein-Henoch purpura is a generalized small vessel vasculitis characterized by nonthrombocytopenic purpura, arthritis, abdominal pain and nephritis. In 1914 Osler described an allergic purpura associated with hemiplegia. After then Lewis et al. reported the cases of Schonlein-Henoch purpura associated with convulsion, coma, confusion, intracranial hemorrhage, and chorea, CNS complication has been reported in 1-8% of children and subsided spontaneously in most cases. Headache is a remarkable sympton and appears nonspecific nature. In 1991 Ostergaard and Storm reported that headache occured during the first week following skin rash and frequently showed abnormal EEG findings. We investigated prospectively the presence of a possible cerebral and renal involvement in the case of Schonlein-Henoch purpura. EEG abnormality demonstrated in 52.6% of all cases, and headache or irritability in 47.4% of all cases. A significant association was found between abnormal EEG finding and presence of headache, but was not found between EEG findings and presence of renal involvement and hypertension. Patients with abnormal EEG had no Past or famity history of febrile convulsion or ididopathic epilepsy.

3.The Analysis of the Dietary Factors Related to Climacteric Symptoms in Middle-aged Women.

The purpose of this study was to investigate the dietary intakes in middle-aged women (pre and postmenopausal) and relation to climacteric symptoms. We conducted the anthropometric measurements, climacteric symptoms survey, and dietary intake to assess the nutrient intakes. Also, dietary quality indices were evaluated. The subjects, over 40 years old, were 168 middle aged women ( pre = 74, postmenopausal = 94), mean age was 48.9 +/- 3.6yr. The anthropometric measurements showed that 59% of subjects were overweight and obese. Climacteric symptoms that were answered "yes" in 50% plus in the subjects, were dry eyes, forgetfulness, difficult concentration, large joint pain (shoulder), fatique, backache, dry skin, joint pain (ankle and knee), dry mouth, dizziness, depression and lonesomeness, snore, morning stiffness, and hot flash. Some symptoms showed significant difference between pre and postmenopausal groups. The average energy intake was 1602.1 Kcal, which was 82% of the Korean EER. The subjects had lower vitamin A, riboflavin, folic acid, Fe, Zn and Ca intake than Korean RI. The lowest nutrient intake was Ca. The mean of food intake was 1294.2 g and MAR of diet quality indices was 0.83. In relation to dietary factors with climacteric symptoms, significant correlations have been found between total protein, animal protein, lipid, cholesterol and niacin intake and "fatique", energy, carbohydrate, thiamin, vitamin B6, folic acid, vitamin C, Na, K and Fe intake and "dry skin", Ca intake and "difficult concentration". Our results indicate that dietary factors (food and nutrients intake, INQ, NAR, MAR) may effect the prevention and reduction of some climacteric symptoms in middle aged women.

4.Painless Colonoscopy: Available Techniques and Instruments.

During colonoscopy, air insufflation to distend the lumen and facilitate careful inspection and scope insertion can induce pain and cause discomfort. Carbon dioxide (CO₂) insufflation can decrease abdominal pain and discomfort during and after colonoscopy. The advantage of CO₂ insufflation is the rapid absorption of the gas across the intestine. Another painless option is water-assisted colonoscopy. Two methods for water-assisted colonoscopy are available: water immersion and water exchange. In a recent direct comparison, the water exchange method was superior to water immersion, CO₂ insufflation, and air insufflation with respect to pain during colonoscopy, although it still had the disadvantage of being a time-consuming procedure. Cap-assisted colonoscopy is a simple technique involving the use of a small transparent cap attached to the tip of the scope. Three studies showed an advantage of this technique in terms of reduced patient discomfort compared with the conventional method. Three robotic colonoscopy systems (Endotics System [Era Endoscopy], NeoGuide [NeoGuide Systems Inc.], and Invendoscope [Invendo Medical]) have been introduced to evaluate pain reduction during colonoscopy, but none has been widely adopted and used in practice. In this review, clinical trials of several techniques and new devices for painless colonoscopy are described and summarized.

5.A Case of Myocardial Bridge in a Patient with Hypertrophic Cardiomyopathy.

Although myocardial bridge is not thought to have any hemodynamic significance in most cases, some have suggested that when it produces severe systolic narrowing, ischemia or infarction may result. Myocardial bridge in adults with hypertrophic cardiomyopathy may be associated with a higher incidence of sudden death, myocardial wall-motion abnormalities, and perfusion defects on thallium-201 scintigraphy. When myocardial bridge is associated with left ventricular hypertrophy, it is known to affect longer segment and cause more severe compression during systole. We report a case of hypertrophic cardiomyopathy with myocardial bridge at the middle part of the left anterior descending coronary artery, who also showed reversible perfusion defect on the thallium scan at the same coronary territory.

6.A Case of Acute Tubular Necrosis Induced by Paraquat (Gramoxon) Intoxication.

Paraquat (1, 1'-dimethy-14, 4' dipyridium chloride: Gramoxon(r)) was first discovered in England in 1955. It has been widely distributed for use in Korea since 1970 as a pesticide and also often the cause of accidental intoxications. The authors would like to report the following case of paraquat intoxicatio in a 13 year old female child admitted via emergency room with complaints of epigastric pain, vomiting and hematuria of four days duration. On admission the data of laboratory evaluation were as follows: BUN/Cr 126/11.6 mg/dl; sGOT/sGPT 641/641 IU/L; total bilirubin/direct bilirubin 3.9/2.4 mg/dl; alkaline phosphatase 396.1 U/L; fecal occult blood test-positive; pH 7.428, pCO2 26.2 mmHg, pO2 131.4 mmHg, base excess -5.0 mmol/L on arterial blood gas analysis (ABGA). Gastroendoscopy revealed esophageal ulcers, and as symptoms of renal failure could not be relieved by hemodialysis, kidney biopsy was performed on the otherwise previously healthy child to establish the cause of renal failure. The result of biopsy revealed tubular necrosis with eosinophilic infiltrations. A more detailed retrograde probe of the child's past medical history evoked that the patient had ingested paraquat in a suicidal attempt. Supportive therapy and hemodialysis were continued thereafter, but the symptoms of renal failure could not be reversed, and the patient eventually died due to acute respiratory distress syndrome.

7.A Clinical Observation of Neonatal Hyperbilirubinemia Due to ABO incompatibility.

A clinical and statistical study was performed on 213 neonates of hyperbilirubinemia who were admitted to department of pediatrics of Korea University Hospital from Jan. 1989 to Jun. 1991. and we studied the difference between ABO compatible group and incompatible group, such as, day of apperance of hyperbilirubinemia, tendency of bilirubin concentration change, reticulocyte, hemoglobin and duration of hyperbilirubinemia. The following results were obtained. 1) There were no difference of initial body weight and gestational age between compatible (3.0 +/- 0.72kg, 38.2 +/- 3.2 weeks) and incompatible groups (3.2 +/- 0.54kg, 39.1 +/- 2.8 weeks). 2) Onset of hyperbilirubinemia within 2 days in the incompatible group (40.5%) was earlier than the compatible group (23.5%) significantly (p<0.05). 3) Mean bilirubin level in the incompatible group was higher than the compatible group significantly (p<0.05). 4) Peak bilirubin level in the incompatible group(18.7+/-4.1mg/dl) was higher than the compatible group (14.8+/-3.4mg/dl) significantly (p<0.05). 5) There were no difference of hemoglobin level, reticulocyte count, coombs positive finding and the ratio of exchange transfusion between two groups.

8.Sputum Smear Conversion During mDOT (Modified Directly Observed Treatment).

BACKGROUND: To assess the effects of mDOT implementation on sputum smear conversion for AFB (Acid fast bacilli) positive pulmonary tuberculosis patients, modified Directly Observed Treatment (mDOT) was started on October 8th 2001 at a health center in Seoul. mDOT was defined through weekly interviewing and supervising of a patient by a supervisor (doctor, nurse, or lay health worker). The sputum smear conversion of a mDOT group was compared with that of a self-medication (self) group. METHODS: This study included 52 AFB positive pulmonary tuberculosis patients registered at a health center in Seoul between October 8th 2001 and April 23rd 2002. 24 and 28 patients were enrolled in the mDOT and self medication groups, respectively. Paired (1:1) individual matching, by gender, extent of disease, relapse and age-matching variables, was performed between the two groups, resulting in 20 paired matches. This prospective study was planned as an unblinded, non-randomized quasi- experimental pilot project. Outcomes were identified from results of sputum smear examinations for AFB in both groups at 2 weeks, and 1 and 2 months. The paired matching data were analyzed using the SAS program version 8.1 by McNemar test. RESULTS: At the end of 2 weeks of treatment, the sputum smear conversion of the mDOT group was somewhat higher than that of the self medication group (78.57 vs. 50%, p-value=0.289), and after 1 month of treatment no statistically significant difference was shown between the two groups (83.33 vs. 50, p-value=0.125). At the end of 2 months of treatment (initial intensive phase), the sputum smear conversions of the mDOT and self groups were 95 and 75%, respectively (p-value=0.219). CONCLUSIONS: The implementation of mDOT did not result in clinically significant increases in the sputum smear conversion at 2 weeks, and 1 and 2 months compared with that of the self medication group. However, the increases experienced might contribute to diminishing the infectious period of AFB positive patients, and this approach may act as a guide for a specific group of patients. In this study, mDOT was performed for one hundred percent of the intensive treatment phase. It can also be an effective treatment for pulmonary tuberculosis patients, and may be useful for some high risk tuberculosis patients.

10.Effect of Postpartum Breast-feeding Support by Nurse on the Breast-feeding Prevalence.

PURPOSE: The purpose of this study was to investigate the effects of the professional nurse's postpartum breast-feeding support on breast-feeding prevalence for mothers who delivered in Baby-Friendly Hospitals (BFH). METHODS: This quasi experimental study was designed with a nonequivalent control group post test. The subjects of this study were 55 mothers who were hospitalized in the delivery room of a university hospital which was selected as a BFH in Daegu from October 1, 2005 to June 30, 2006. Twenty nine mothers were assigned to the experimental group and 26 mothers to the control group. Postpartum breast-feeding support by nurses' telephone calls to the experimental group was provided once a week for 4 weeks postpartum and then once a month for 16 weeks postpartum. Four post tests were given at postpartum week 4, 8, 12, and 16. The control group was given a telephone call at postpartum week 4, 8, 12, and 16. RESULTS: The breastfeeding prevalence of the experimental group was significantly higher than that of the control group for each period. CONCLUSION: Postpartum breast-feeding support by nurses may be a useful intervention to increase breast-feeding prevalence.
