1.A Case of Gastro-Colic Fistula due to Ectopic Gastric Mucosa and its 99mTcO4 Scan Findings.

We report a case of gastro-colic fistula caused by ectopic gastric mucosa developed at transverse colon. Fistula was detected by colonofiberscopy. And fistulous tract was proved by barium enema. Meckel's diverticulum scan finding was similar to that of GI bleeding; e.g. injected radioactivity was secreted into the lumen and moved along the lumen. There was no bleeding. And there was no diverticulum in the colon. Absence of diverticular pouch may explain this unusuaal GI bleeding-like scan finding rather than focal collection of radioactivity, which is typical of ectopic gastric mucosa found in the Meckel's diverticulum. Ectopic gastric mucosa was confirmed by colonfiberscopic biopsy. We suggest GI bleeding-like pictures should be included differential diagnosis of Tc-99m-O4 (ectopic gastric mucosa or Meckel's diverticulum) scan.

2.A Case of Wasp Sting of the Eye.

We presented a case of wasp sting of 42 year old, farmer who got stung on the cornea directly. At the time of first examination, the patient had visual acuity of 0.02 in the right eye. Slit lamp examination revealed corneal epithelial desquamation and vesicle, striate keratitis, diffuse stromal edema and fine keratic precipitates on the corneal endothelium. The iris was depigmented and atrophic, and pupil was mydriatic and nonreactive. The lens was opaque at the center of anterior caspsule and fundus was not visualized. 4 months later, the corneal lesion was slightly improved. Clinical findings of iris and lens still unchanged.

3.The Study of Normal Visual Fields Using Normal Strategy and Dynamic Strategy in Octopus 101 Automated Perimetery in Korean.

Automated static perimeters, which are commonly used for early diagnosis and follow-up of glaucoma patients in Korea, are manufactured overseas, and their basic data were obtained from foreigners and normal values were set to meet them. Therefore, verification of validity of those data should precede their application to Koreans. We compared the mean normal values of standard indices of OCTOPUS 101 automated static perimeter with the values obtained from 140 eyes of 70 healthy Koreans using normal strategy(NS)and dynamic strategy(DS). Mean navlues of mean defect(MD), loss variance(LV), corrected loss variance(CLV), and short-term fluctuation(SF)were: 2.84+/-0.85dB, 2.15+/-0.54dB, 1.22+/-0.42dB, 1.43+/-0.22dBwith NS, and 2.05+/-0.81dB, 1.98 +/-0.58dB, 0.97+/-0.49dB, 1.43+/-0.24dB, repectively with DS. With NS, 114 of 140 eyes (81. 4%)were out of the normal range of MD. With DS, 67 of 140 eyes(47. 9%)were out of normal range of MD, and 2 of 140 eyes(1. 4%)were out of the normal range of SF. Values of the other indices were within normal range of both strategies. Statistical analysis of global indices showed that age was significantly correlated with LV, CLV and SF in NS(P<0.05), and that it was also correlated with LV and CLV in DS(P<0.05). These results suggest that both race and age should be taken into acoount in interpretation of visual fields on automated static perimetry.

4.Innominate Artery Rupture Caused by Blunt Chest Trauma: A Case Report.

Rupture of an innominate artery caused by blunt chest trauma is extremely rare because this artery is short and relatively well protected by the bony cage. This report describes a 37-year-old male who sustained a blunt chest injury that resulted in an innominate artery rupture, detected by chest CT and thoracic aortography. The patient underwent an urgent operation through median sternotomy. A 3 by 3 cm sized pseudoaneurysm of proximal innominate artery was found with a complete intimal tear. After the origin of the innominate artery was closed, the injured segment of artery was excised and an aorto-innominate artery bypass with a 10 mm Gore-tex graft was performed without use of a shunt. The patient was discharged 20 days later without neurologic complications and had equal blood pressure in both arms.

5.A Case of Hereditary Anhidrotic Ectodermal Dysplasia.

Hereditary anhidrotic ectodermal dysplasia is a rare condition characterized by underdeveloped ectodermal structure including the skin, teeth or skin appendages. The patient has characteristic feature of anhidrosis, hypotrichosis and defective dentition. We experienced a case of hereditary anhidrotic ectodermal dysplasia in a l-month-old male infant who had unexplained recurring fever, anhidrosis and characteristic facial feature, so we established the diagnosis with clinical feature and skin biopsy.

6.Is 99mTc-MDP Mammoscintigraphy Useful in Patients with Breast Mass Lesions ?.

PURPOSE: Piccolo et al. advocated the usefulness of Tc-99m-MDP breast scan in differential diagnosis of breast mass with high accuracy. But there are little or no follow-up studies about it as we know. We studied Tc-99m-MDP uptake pattern and lesions/background ratio in patients complaining palpable breast lesions to evaluate the clinical usefulness of Tc-99m-MDP scan. MATERIALS AND METHODS: Total 34 patients were studied with physical examination, mammogram and Tc-99m-MDP scan prospectively. Anteroposterior and both lateral view of breast were obtained 5 minutes after iv injection of 740 MBq Tc-99m-MDP. Breast uptake pattern of Tc-99m-MDP was analyzed by a grade system: 0=no uptake, grade 1=bilateral diffuse uptake, grade 2=asymmetric faint uptake, grade 3=focal hot uptake. 20 cases were pathologically confirmed by excision biopsy or aspiration biopsy. 14 cases were normal in physical examination and mammo-grain. RESULTS: Pathologic results showed 7 carcinomas, 6 benign solid tumors, and 7 fibrocystic changes. Grade 3 pattern of Tc-99m-MDP uptake was noted in 4/7 carcinomas, 3/6 benign solid tumors, and 1/7 fibrocystic changes. Grade 2 pattern was 2/7, 0/7, 3/7 respectively. The average L/B ratio was 1.66 in carcinomas, 1.68 in benign solid masses, 1.20 in fibrocystic diseases, 1.05 in normal patients. L/B ratio was higher in carcinoma and benign mass groups than in fibrocystic change and normal control groups(p=0.005). But there was no statistical difference between L/B ratio of malignant mass group and benign mass group. CONCLUSION: Tc-99m-MDP scan is not suitable to routine clinical use for breast mass diagnosis. It might be used in limited conditions when whole body bone scan is planned.

7.Reliability of Measuring Leg Segments and Joint Angles Using Smartphones during Aquatic Exercise

Objectives:
Aquatic therapy is a significant intervention method for both patients and healthy individuals. However, in clinical practice, quantitative measurements are rarely applied in aquatic therapy due to the disadvantages of submerging expensive instruments in water. In this study, we used readily available smartphones and armbands to measure leg segments and joint angles during aquatic gait and evaluated the reliability of these measurements.
Methods:
Waterproof smartphones were strapped to the trunk, thighs, and shanks of 19 healthy young adults using armbands. The angles of the trunk, thigh, and shank segments were measured during aquatic gait. The measurements were repeated 1 day later. The data were analyzed to obtain the angles of the hip and knee joints.
Results:
Measurement repeatability, calculated using the intraclass correlation coefficient (ICC), was the highest for the shank segment range of motion (ROM) (first 46.79° ± 5.50°, second 50.12° ± 9.98°, ICC = 0.78). There was high agreement in trunk segment ROM (first 6.36° ± 1.42°, second 4.29° ± 1.83°, ICC = 0.73), thigh segment ROM (first 33.49° ± 5.18°, second 37.31° ± 8.70°, ICC = 0.62), and knee joint ROM (first 52.43° ± 11.26°, second 62.19° ± 16.65°, ICC = 0.68) and fair agreement in hip joint ROM (first 34.60°±4.71°, second 37.80° ± 7.84°, ICC = 0.59).
Conclusions
Smartphones can be used to reliably measure leg segments and joint angles during aquatic gait, providing a simpler method for obtaining these measurements and enabling the wider use of aquatic motion analysis in clinical practice and research.
8.Muscle Activities of the Brachioradialis and Extensor Carpi Radialis Longus According to the Type of Backhand Stroke in Badminton

PURPOSE: The purpose of this study was to investigate the muscle activities of the brachioradialis and extensor carpi radialis longus according to the type of backhand stroke in badminton.METHODS: To measure the muscle activities, we used electromyography (EMG) equipment to measure EMG values by performing maximal voluntary contraction (MVC) of the forearm muscles, which depends on the type of backhand stroke. With these values, the %maximum voluntary isometric contraction values were obtained. The data were calculated using SPSS ver. 21.0 and one-way repeated measures analysis of variance with a post-hoc least significant difference test.RESULTS: In this study, the backhand clear achieved higher maximum muscle activity values than those by the backhand push, under clear, and drive in the brachioradialis. The backhand smash achieved higher maximum muscle activity values than those by the backhand push in the extensor carpi radialis longus.CONCLUSION: The backhand clear is associated with a higher injury rate than those associated with the backhand push, under clear, and drive. The backhand smash is associated with a higher rate of the occurrence of tennis elbow than that associated with the backhand push. To prevent injuries, it is considered that the forearm's strength, stretching before and after exercise, and sufficient time to rest are important.

9.Muscle Weakening Effect After Superior Rectus Z-myotomy in Rabbits.

PURPOSE: To experimentally investigate the effect for muscle weakness after superior rectus Z-myotomy and histological changes. METHODS: Superior rectus muscle fibers of rabbits (16 rabbits, 32 eyes) were cut transversely with scissors across 75% of the muscle in two different positions on opposite sides. In group 1 (16 eyes), myotomies were performed at 2 and 7 mm from the muscle insertion (5 mm gap) and in group 2, performed at 2 and 5 mm (3 mm gap). The change of mark, eyeball position, and muscle tension after myotomy and 4 weeks postoperatively was evaluated, the location of the mark was examined, and muscle tissue biopsy was performed. RESULTS: After Z-myotomy, the marks of the two groups moved significantly posteriorly from insertion within groups (p<0.05), with no significant differences between groups (p=0.469). Eyeball positions of the two groups moved significantly inferiorly (p<0.05); the amount of position change of group 1 was greater than group 2 (p<0.05). When the globe was pulled in opposite directions for muscle action, the degree of change decreased with significant difference within groups (p<0.05), but there were no significant differences between groups (p=0.32). CONCLUSIONS: Z-myotomy of the superior rectus muscle affected the recession of eyeball position and weakened the muscle action. Muscle weakening affected by the different gaps between myotomies did not show consistent results.

10.A Case of the Holt-Oram Syndrome with Unaffected Parents Diagnosed by Antenatal Ultrasonography.

Holt-Oram Syndrome is an autosomal dominant disorder characterized by the association of upper-limb abnormalities and congenital heart disease. A woman with no family history of genetic disease underwent antenatal sonography at 27 weeks' menstrual age to screen for fetal anomalies. Ultrasonography revealed abnormalities in the upper limbs. The limb abnormalities included abscence of bilateral thumbs and radius: the left humus was short. Pregnancy termination was performed. The postnatal chromosomal analysis revealed a normal 46XX karyotype and the autopsy finding confirmed the Holt-Oram syndrome. We report a case of Holt-Oram Syndrome in fetus with unaffected parents with brief of the literatures.
