2.Analysis of DNA Ploidy Pattern of Hepatocellular Carcinoma with Comparison between Flow Cytometric and Image Cytometric Study.

Fifty cases of hepatocellularcarcinoma were studied using retrospective flow cytometric(FCM) and image cytometric(ICM) DNA analysis to determine the prevalence of aneuploid cell population and whether they were associated with any particular clinico-pathologic findings. At the same time, we compared the difference between FCM and ICM. The materials were prepared from 50 micrometer cut of paraffin embedded blocks. The DNA modal values, which could be defined in 42 cases by FCM with 74% aneuploidy and in 50 cases by ICM with 76% aneuploidy. So 95% of the cases had concordant DNA ploidy results by both techniques of FCM and ICM. Abnormal DNA pattern was correlated with age(<50), presence of cirrhosis, pathologic grade and some pathologic types(p<0.05), but was not correlated with presence of HBsAg, sex, alphafeto protein, and alcohol history(p>0.05). Also we found that ICM technique was easier to perform and interprete.

3.Latent Multiple Noncontiguous Pyogenic Spondylitis: a case report.

Spinal disc surgery has been the most common cause of iatrogenic disc infection, resulting in pyogenic spondylitis. The diagnosis is usually difficult because of the infrequency of occurrence of pyogenic spondylitis. If three or more vertebrae are involved, the diagnosis of osteomyelitis is unlikely to be correct. Especially involvement of multiple non-contiguous vertebrae indicates metastatic tumor. Therefore, we report a rare case of latent multiple non-contiguous pyogenic spondylitis following spinal surgery. The actual cause of infection in this case is unknown. CT guide biopsy may be needed to rule-out tuberculosis spondylitis and metastatic tumor in such case of multiple spinal involvement. In conclusion, the possibility of latent spinal infection must be considered even though surgery had been done several years ago.

4.Nephropathy related to computed tomography in emergency department patients with serum creatinine <1.5 mg/dL.

BACKGROUND: Contrast-induced nephropathy (CIN) can cause serious adverse effects. To reduce the occurrence of CIN related computed tomography (CT) in emergency patients, we assessed the respective roles of serum creatinine (SCr) alone and estimated glomerular filtration rate (eGFR) as an early predictor for CIN related CT. METHODS: For patients with SCr <1.5 mg/dL who underwent CT in emergency department (ED) between September 2012 and October 2013, we assessed the prevalence of CIN and its adverse effects. The Modification of Diet in Renal Disease Study (MDRD) and Cockcroft-Gault (CG) formula was used for the calculation of eGFR. Practical calculation was performed by electronic medical record (EMR) system for MDRD and internet calculating service for CG. And we investigated the prevalence of CIN in eGFR <60 mL/min/1.73 m2 before CT. RESULTS: A total of 1,555 patients were enrolled. The prevalence of CIN after CT was 4.6% and it showed correlation with renal deterioration, increased in-hospital mortality, and prolonged hospitalization. Despite baseline SCr <1.5 mg/dL, among enrolled patients, 11.3% as MDRD equation and 29.5% as CG formula were <60 mL/min/1.73 m2 and in this condition, the prevalence of CIN was significantly high (odds ratio was 2.87 [1.64-5.02] as MDRD equation and 2.03 [1.26-3.29] as CG formula). CONCLUSION: Just SCr <1.5mg/dL was not appropriate to recognize preexisting renal insufficiency, but eGFR using MDRD equation was useful in predicting the risk of CIN related CT in ED. Using EMR, calculation of eGFR can be easier and more convenient.

5.A Case of Wegener's Granulomatosis with Acute Renal Failure and Pneumothorax Developed during Treatment with Immunosuppressive Agent.

Wegener's granulomatosis is characterized by necrotizing granulomatosis lesion of the respiratory tract, glomerulonephritis and frequently vasculitis involving other organs. The basic pathophysiologic mechanism of Wegener's granulomatosis is not defined yet. However, it may be suspected an autoimmune disease. We experienced a case of Wegener's granulomatosis which are associated with acute renal failure and pneumothorax. The patient suffered from hemoptysis, fever and cough. Despite antibiotic therapy, symtoms did not improved and multiple varying sized nodules were aggravated on chest roentogenogram and serum creatinine elevated 3.4mg/dl. After diagnosis using video associated thoracoscopic surgery, the patient was treated with cyclophsphamide, glucocorticoid and sulfamethoxasole-trimethoprime. With the combination therapy, the patient felt completely well and chest roentogenogram showed lungs were improved and serum creatinine was normal. The patient was readmitted because of right pleuritic pain and dyspnea 15 day after discharge. The patient was developed a right pneumothorax. The lung was easily expanded by intercostal tube drainage with a one way valve. The patient has been treated as an out- patient with immunosuppressive agents continously.

6.Squamous Cell Carcinoma Involving Lacrimal Apparatus.

Among 7,500 patients whom we have performed gastrofiberoscopy from July 1979 to June 1,982, 226 cases of stomach cancer were diagnosed. The frequency was 3. 0%. The ratio of male to female was 2: 1, the peak incidence, 69 cases(30.5%) was in 5th decade. 1) The frequency of stomach cancer among the age were 0. 5% in 1st decade, 0.4% in 2nd decade, 1.2% in 3rd decade, 2.5% in 4th decade, 5.9% in 5th decade, 12.6% in 6th decade 16.8% in 7th decade. 8.Circulating Lymphoma Cells in the Peripheral Blood from 4 Cases of Mantle and T Cell Types of Non-Hodgkin's Lymphoma: Light and Electron Microscopic Morphology.

It is difficult to distinguish between circulating lymphoma cells and non-lymphoma cells which are lymphocytes, activated lymphocytes, monocytes, and leukemic cells. The presence of circulating lymphoma cells in the peripheral blood is infrequent but the incidence is probably significantly higher than has been reported in morphologic studies of peripheral blood smears. So the morphologic evaluations of the circulating lymphoma cells and non-lymphoma cells are needed. We experienced that circulating lymphoma cells were found in the peripheral blood smears in the 4 cases of non-Hodgkin's lymphomas. They were consisted of mantle cell lymphoma (1 case), peripheral T cell lymphoma (1 case), adult T cell leukemia/lymphoma (1 case), cutaneous T cell lymphoma (1 case) and diagnosed from their lymph node biopsies. Those circulating lymphoma cells were morphologically observed by light and electron microscopic methods. Using those morphological features, we think that detection rate of circulating lymphoma cells can be improved in the patient with lymphoma.

9.Predictors of Vertebral Endplate Fractures after Oblique Lumbar Interbody Fusion

Background:
Cage subsidence after oblique lumbar interbody fusion (OLIF) induces restenosis and adversely affects patient outcomes. Many studies have investigated the causes of subsidence, one of which is endplate fracture (EF). This study aimed to identify predictors of EF after OLIF.
Methods:
This retrospective study reviewed consecutive patients who underwent OLIF at a single institute between August 2019 and February 2022. A total of 104 patients were enrolled. The patients’ demographic data and surgical details were collected through chart reviews. Radiographic variables were measured. Related variables were also analyzed using binomial logistic regression, dividing each group into those with versus without EF.
Results:
EF occurred at 30 of 164 levels (18.3%), and the binary logistic analysis revealed that sex (odds ratio [OR], 11.07), inferior endplate concave depth (OR, 1.95), disc wedge angle (OR, 1.22), lumbar lordosis (OR, 1.09), pelvic incidence (OR, 1.07), sagittal vertical axis (OR, 1.02), sacral slope (OR, 0.9), L3–4 level (OR, 0.005), and L4–5 level (OR, 0.004) were significantly related to EF.
Conclusions
OLIF in older Asian patients should be performed carefully after recognizing the high possibility of EF and confirming the factors that should be considered preoperatively.
10.Horizontal Ratio of the Korean University Student's Face and Facial Golden Mask.

PURPOSE: Many attempts have been made to describe ideal facial proportions for over two thousands year and constantly lasted till now. Dr. Marquardt has derived supposedly ideal facial proportions from the facial golden mask using golden ratio of 1:1.618. On the other hand, facial reducing surgeries such as mandible angle reduction are popularized in Asia because the width of mid and lower face of Korean is recognized to be wider. The purpose is to analyze characters of Korean university students' faces in horizontal plane and establish the objective data for facial width distributions and clinical applications. METHODS: We applied the facial golden mask to the photographs in 1000 cases, compared the width of mid and lower face between the facial golden mask and Korean university students' faces. And we first calculated the horizontal ratio(HR) of middle and lower face each for using comparative scale of width, facial golden mask. We divided 1,000 cases into 3 groups by degrees of HR and analyzed data of HR on each groups. Using calculated horizontal ratio, we newly invented the cumulative frequency of distribution graphs in Korean university students' faces. RESULTS: Mean data of HR were over 1.0 in all groups, which means that Korean university students' faces are typically wider than facial golden mask in horizontal planes. And this study was statistically significant(p- value<0.05). Clinically using the cumulative frequency distributions of Korean university students' face width, we can easily explain changes of facial width to patient after facial reducing surgery and describe the changes into objective data. CONCLUSION: This study concludes thatKorean university students' faces are wider than facial golden mask is significantly true and the cumulative frequency of distribution graphs are expected to be widely used for comparison of results in facial reducing surgery.
