1. Chemical constituents from Isodon amethystoides distributed in Libo

Objective: To investigate the chemical constituents from Isodon amethystoides distributed in Libo. Methods: Chemical constituents were isolated and purified by chromatography with silica gel, MCI gel CHP 20P, and Sephadex LH-20. The structures of all compounds were elucidated by physicochemical properties, comprehensive spectral data and references. Results: Eighteen known compounds were isolated from the methanol extract of I. amethystoides, which were elucidated as lophanic acid (1), rubesanolide D (2), fladin A (3), 3,4-secoisopimara-4(18),7,15-triene-3-oic acid methylester (4), 3,4-secoisopimara-4(18),7, 15-triene-3-oic acid (5), isopimara-7,15-dien-3-one (6), isopimara-7,15-dien-3β-ol (7), alpiniol (8), (-)-clovane-2,9-diol (9), ursolic acid (10), oleanolic acid (11), epimaslinic acid (12), 7α-hydroxysitosterol (13), β-sitosterol (14), stigmasterol (15), sesamin (16), paulownin acetate (17) and dibutyl phthalate (18). Conclusion: Compound 4 is a new natural product, and compounds 2-9, 15-18 are isolated from I. amethystoides for the first time.
2.Expression of interleukin-18 in liver tissue of patients with chronic hepatitis B and its clinical significance

Objective To investigate the expression of interleukin 18 (IL-18) in liver tissues of patients with chronic hepatitis B (CHB) and its clinical significance.Methods Totally 172 CHB patients admitted in the Second Affiliated Hospital of Anhui Medical University from January 2012 to December 2012 were enrolled.The tissue specimens were obtained by liver biopsy from all patients,and 22 normal liver specimens were also collected as controls.Liver function,serum markers of HBV and HBV DNA load were measured and liver inflammation grades and fibrosis stages were assessed.Immunohistochemical method was used to detect the expression of IL-18 in liver tissues.Spearman rank correlation was used to analyze the correlations between IL-18 expression and the above indicators.Results IL-18 was mainly expressed in cytoplasm of liver cells.It was positive or strong positive in liver cells of healthy people,but weakly positive in liver cells of CHB patients.The difference of IL-18 expression between CHB patients and healthy people was of statistical significance (x2 =42.124,P < 0.05).In CHB patients,the expression of IL-18 was gradually enhanced with the increasing of ALT,inflammation grades and fibrosis stages (r =0.182,0.173 and 0.068,P <0.05),but it was not correlated with HBV DNA load and HBeAg (r =-0.05 and 0.041,P >0.05).Conclusion In CHB patients,the expression of IL-18 may be involved in liver injury and fibrosis formation.
3.Quantitation value of HBsAg in different clinical stages of chronic hepatitis B virus infection

Objective To investigate the levels of HBsAg and HBV DNA in different clinical stages. Methods Serum samples of 246 patients, without accepting antiviral therapy, from the second affiliated anhui medical univrsity were collected from 2012.1 to 2013.5. Among the enrolled patients, 41 cases with chronic HBV-carrier (ASC), 99 cases with chronic hepatitis B patients (CHB), 70 cases with liver cirrhosis (LC), and 36 cases with primary liver cancer (HCC) were included. Results Levels of HBsAg in the group of ASC, CHB, LC, HCC were 135 280.00 (1 788.80 ~ 51 820.00) IU / mL, 8 218.00 (1 644.60 ~ 266 700.80) IU / mL, 1 280.38 (265.70~2 375.00) IU / mL, and 925.10(135.05 ~ 2 381.8) IU / mL, respectively. Positive correlations were observed between HBsAg tire and HBV DNA levels (rs = 0.332, P = 0.000), and negative correlations were doserved between HBsAg leve and age(rs = -0.496,P = 0.000). Conclusions The level of HBsAg in serum decreased progressively from the group of ASC to CHB, LC and HCC, the positive correlation between levels of HBsAg and HBV DNA, and the negative correction between HBsAg level and age were demonstrated.
4.Efficacy and Safety of Eplerenone in the Treatment of Essential Hypertension :Reevaluation of Systematic Review/Meta-analysis

OBJECTIVE:To reeval uate the systematic review/Meta-analy sis of efficacy and safety of eplerenone in the treatment of essential hypertension. METHODS :Retrieved from PubMed ,Embase,Cochrane Library ,Web of Science ,Wanfang database,CNKI,VIP,systematic review/Meta-analysis about eplerenone in the treatment of essential hypertension were collected from the inception to June 24th,2021. After literature screening and data extraction ,the quality of included literatures were evaluated with PRISMA statement ;methodology quality of included literatures were evaluated with AMSTAR 2 scale;GRADE method was adopted to evaluate the evidence quality of outcome measures. Efficacy and safety index evaluation of included literatures were summeried. RESULTS :A total of 8 systematic reviews/Meta-analyses were included ,involving 5 systematic reviews and 3 Meta-analysis,including 73 outcome indicators. PRISMA scores ranged from 7.5 to 23.5,including 6 literatures (75.0%)with≤15 points,1(12.5%)with >15-<21 points and 1(12.5%)with ≥21 points. The results of AMSTAR 2 evaluation indicated that the methodological quality of 2 studies was low ,and that of 6 studies was very low . GRADE quality evaluation results showed that there were 3 high quality indicators ,24 medium quality indicators and 46 low or very low quality indicators;the factors contributed to downgrading evidence quality were limitation ,inconsistency,imprecision and publication bias. In terms of efficacy ,compared with placebo ,eplerenone could significantly reduce clinical blood pressure (CBP)and 24-hour ambulatory blood pressure (ABP). Its effect in reducing CBP was significantly better than other antihypertensive drugs or equivalent to other antihypertensive drugs. The effects of eplerenone on reducing clinical systolic blood pressure was not as good as spironolactone and enalapril ,or bett er than calcium channel blocker ,enalapril and angiotensin receptor antag onist,or equivalent to calcium channel blocker and enalapril ;the effect of eplerenone on reducing clinical diastolic blood pressure was not as good com as spironolactone ,calcium c hannel blocker and enalapril ,or as good as enalapril ,but better than angiotensin receptor antagonist. In terms of safety ,there was no significant difference in the incidence of ADR ,serious ADR or hyperkalemia caused by eplerenone ,compared with placebo ,or the incidence of ADR was higher than that of placebo. There was no statistical significance in the incidence of ADR or serious ADR ,compared with other antihypertensive drugs. CONCLUSIONS :Efficacy and safety of eplerenone in the treatment of essential hypertension was good ,but in view of the poor methodological quality of systematic reviews or Meta-analysis and the low or very low level of outcome indicator evidence ,the authenticity and effectiveness of the conclusion will be reduced ,so that those indcaters should be interpreted carefully.
5.Clinical observation on therapeutic effect of combination of acupuncture and ginger-partition moxibustion for treatment of patients with cardiac arrhythmia.

OBJECTIVETo compare the therapeutic effect of the combined method of acupuncture and ginger-partition moxibustion with that of routine western medicine on the patients with cardiac arrhythmia.
METHODSSeventy-five patients with cardiac arrhythmia were randomly divided into an observation group (38 cases) and a control group (37 cases). In the observation group, the patients were treated at Neiguan (PC 6) with acupuncture, and Xinshu (BL 15), Pishu (BL 20), Gongsun (SP 4) etc. with ginger-partition moxibustion. In the control group, the patients were given the routine western medicine for cardiac arrhythmia, such as Metoprolol, Propafenone and Aspirin (enteric solubility). Through the 2-4 courses of treatment and 1 year follow-up survey, the therapeutic effect, recurrence and complication on the patients were observed and compared between the both groups.
RESULTSThe total effective rate of 97.4% in the observation group was better than that of 81.1% in the control group (P < 0.05). The serious recurrence rate and complication occurred in the observation group were lower than those of the control group (both P < 0.05).
CONCLUSIONThe therapeutic effect of the combined method of acupuncture and ginger-partition moxibustion on the patients with cardiac arrhythmia is obviously better than that of routine western medicine.

6.Gene Mutation Types and Ethnic Distribution Characteristic of Thalassemia in Guiyang.

OBJECTIVE:
To investigate the genetic mutation types and ethnic distribution characteristics of thalassemia in Guiyang.
METHODS:
The population underwent physical examination, antenatal examination and pre-pregnancy examination in our hospital from January 2019 to November 2019 was selected, and the thalassemia gene was detected by RDB-PCR.
RESULTS:
Among the 4 572 samples, 346 were positive , and the total carrying rate was 7.57%. The carrying rate of α-thalassaemia gene was 5.42% (248 cases), while β-thalassemia was 1.99% (91 cases), and α+β-compound thalassemia was 0.15% (7 cases). α-thalassaemia genotype with the lack of right side -α
CONCLUSION

7.Study on Chemical Constituents of Diterpenoids from Buyi Medicine Isodon coetsa

OBJECTIVE:To st udy the chemical constituents of diterpenoids from Buyi medicine Isodon coetsa ,and to provide reference for the development and utilization of the medicinal resources. METHODS :The 95% methanol extract of Buyi medicine I. coetsa were isolated and purified with silica gel column ,Sehadex LH- 20 gel column and MCI column. The structures of the compounds were obtained by spectral analysis (mass spectrum ,hydrogen spectrum and carbon spectrum ),and then compared with active components of Miao medicine “Isodon flavidus ”. RESULTS & CONCLUSIONS :Ten diterpenoids were obtained from I. coetsa,including rabdocoetsin B (Ⅰ),megathgrin B (Ⅱ),rabdocoetsin A (Ⅲ),enanderianin N (Ⅳ),rabdocoetsin D (Ⅴ), megathyrin A (Ⅵ),lophanic acid (Ⅶ),rubesanolide D (Ⅷ),excisanin D (Ⅸ),excisanin K (Ⅹ). The compounds Ⅶ,Ⅸ and Ⅹ were isolated from this specie for the first time. Compound Ⅶ(lophanic acid )is a common active component in the Buyi medicine I. coetsa and Miao medicine “I. flavidus ”.
8.Meta-analysis of the efficacy and safety of PD-1/PD-L1 inhibitors combined with bevacizumab in the treatment of advanced non-small cell lung cancer

OBJECTIVE To evaluate the efficacy and safety of programmed death-1/programmed death-ligand 1 (PD-1/PD-L1) inhibitors combined with bevacizumab in the treatment of advanced non-small cell lung cancer (NSCLC) based on platinum- containing dual therapy. METHODS Retrieved from CNKI, Wanfang, VIP, Web of Science, PubMed and other Chinese and English databases, cohort studies or randomized controlled trial studies on the treatment of advanced NSCLC with platinum- containing double agents in combination with PD-1/PD-L1 inhibitors and bevacizumab (trial group) versus platinum-containing double agents with or without PD-1/PD-L1 inhibitor or bevacizumab (control group) were collected from the inception to April 25, 2024. After screening literature, extracting data and evaluating quality, meta-analysis and sensitivity analysis were performed by using RevMan 5.4.1 software. RESULTS A total of 15 pieces of literature were included, involving 13 clinical studies with a total of 3 282 patients. Compared with the control group, partial response rate [RR=0.75,95%CI(0.68,0.82),P<0.000 01], complete response rate [RR=0.47,95%CI(0.29,0.76),P=0.002], progressive disease rate [RR=1.23,95%CI(1.11,1.37),P<0.000 1], objective response rate (ORR) [RR=0.72,95%CI(0.67,0.79),P<0.000 01] and disease control rate (DCR) [RR=0.85, 95%CI (0.77,0.95),P=0.003] were higher in the trial group. There was no statistically significant difference in the stable disease rate [RR=1.25, 95%CI (0.86, 1.83), P=0.25] or overall adverse drug reaction incidence rate [RR=0.95, 95%CI (0.90, 1.00), P= 0.07] between the two groups of patients. Sensitivity analyses showed robust and reliable results for all outcome indicators. CONCLUSIONS PD-1/PD-L1 inhibitors combined with bevacizumab based on platinum-containing dual therapy in the treatment of advanced NSCLC can improve patients’ clinical benefits, such as ORR and DCR, without increasing the risk of adverse drug reaction.
9.Artificial culturing of Cordyceps cicadidae on living silkworm.

METHODTwenty Paecilomyces cicadae were collected and isolated from 8 natural reserves in China and the Cheju Island of Korea. By the impregnation method, their infection to silkworm larvaes and silkworm chrysalises and the synneneta production were studied.
RESULTThe results showed that all P. cicadiae strains could infect silkworm larvaes and silkworm chrysalises, and some strains could produce synnenetas.
CONCLUSIONThe silkworm chrysalis was better than silkworm larvae to culture C. cicadidae.

10.The effect of technological parameters of wide-band laser cladding on microstructure and sinterability of gradient bioceramics composite coating.

The gradient bioceramics coating was prepared on the surface of Ti-6Al-4V alloy by using wide-band laser cladding. And the effect of technological parameters of wide-band laser cladding on microstructure and sinterability of gradient bioceramics composite coating was studied. The experimental results indicated that in the circumstances of size of laser doze D and scanning velocity V being fixed, with the increasement of power P, the density of microstructure in bioceramics coating gradually degraded; with the increasement of power P, the pore rate of bioceramics gradually became high. While P = 2.3 KW, the bioceramics coating with dense structure and lower pore rate (5.11%) was obtained; while P = 2.9 KW, the bioceramics coating with disappointing density was formed and its pore rate was up to 21.32%. The microhardness of bioceramics coating demonstrated that while P = 2.3 KW, the largest value of microhardness of bioceramics coating was 1100 HV. Under the condition of our research work, the optimum technological parameters for preparing gradient bioceramics coating by wide-band laser cladding are: P = 2.3 KW, V = 145 mm/min, D = 16 mm x 2 mm.
