1.Current status and advances of therapy for anal canal cancer

Objective To summarize the current status and development of therapy for anal canal cancer. Methods Application of Medline database and CHKD database, with "anal canal cancer", "chemotherapy", "radiotherapy", "abdominoperineal resection" as key words search literature about anal canal cancer published from January 1996 to June 2009. Retrieval criteria: the epidemiology of anal canal cancer; the etiology of anal canal cancer; the clinical manifestation and classification of anal canal cancer; strategies and development of therapy for anal canal cancer. Sixty-seven of them met the retrieval criteria, while finally 22 related papers were analyzed. Results Over the last 30 years, treatment of anal canal cancer has undergone a profound evolution. Combined-modality treatment combined with chemoradiotherapy (CRT), including three-dimensional conformal radiation therapy (3D-CRT), intensity modulated radiation therapy (IMRT), novel anti-tumor agent and salvage surgery, had resulted in a 5-year survival rate of approximately 80 %. Conclusion Anal canal cancer is a rare tumor. CRT has replaced surgery as the first choice of anal canal cancer. The development of molecular biology will likely lead to further proposals for trials of anal canal cancer.
2.Analyse the relation of the stage and type with the lab diagnosis guideine in the patients with PM and DM

Objective To analyse the lab diagnosis guideline in the patients with PM and DM,and guide the clinical diagnosis. Methods Stage the controlling condition and examine the serologic zymogram,electromyography and pathology.Results In the and Hospital of Hebei medical Vnirersity,from Jan.2003 to Jan.2005 CK,CK-MB,AST,LDH and HBDH was higher in 82.9%patients of aetive stage than the stable stage,especially changed obviously in CK.The occurance ritio(75.9%,48.3%)of the inside electricity and multiphase wave were much higher than the stable stage(46.7%,20.0%).Lots of organic fibre shrank in PM,so did the inflammation of lymohocyte soakage.In DM,the fascicle began to shrink in different degrees.Conclusion CK could become the guideline to watch the activity of illness.The number of the electromyography inside electricity and multiphase wave are related to the illness stage of patients with PM and DM.In PM pathology,muscle fibre had some infected cell soakage,with the differential recessive change.The pathology character in DM was the atrophy around the facicle and the change of tiny blood vessel.
3.Cognitive behavioral intervention in the nursing care for chronic obstructive pulmonary disease

Objective To explore the application effect of cognitive behavioral intervention on chronic obstructive pulmonary disease nursing care.Methods 80 patients with chronic obstructive pulmonary disease were randomly divided into an experimental group and a control group(n=40).Both groups were given medical treatment and conventional nursing care,and the control group was given the cognitive behavioral intervention nursing care additionally.The average days of stay,the ratio of readmission,the treatment adherence,and smoking cessation rate in one year were recorded.Moreover,the life quality of the two groups was analyzed with a CAT assessment test.Results Compared with the control group,the experimental group achieved shorter average days of stay,higher medication adherence and smoking cessation rate,lower readmission rate and admission rate.The experimental group achieved better life quality with lower CAT value than the control group.Conclusion Cognitive behavioral intervention can obviously improve the treatment effectiveness of patients with chronic obstructive pulmonarv disease.
4.Study on the Protective Effect of Shuanghuanglian Sterilized Powder for Injection on Hepatic Injury of Model Mice Induced by Human Cytomegalovirus Infection

OBJECTIVE:To study the protective effect of shuanghuanglian sterilized powder for injection on hepatic in?jury of model mouse induced by human cytomegalovirus(HCMV)infection.METHODS:The mice were randomized into nor?mal control group,infection model group,ganciclovir group and shuanghuanglian sterilized powder for injection group(high,middle and low dosage),all of which except the normal control were vaccinated with HCMV5d to establish the hepatic injury model,each group was administered with different drugs;10days later,the enzyme activity of ALT and AST in the eye sockets blood of each group was determined;The mice were put to death by cervical vertebra dislocation and the necrosis degree of livers from the dead mice was evaluated and the pathological observation was conducted.RESULTS:The activity of ALT and AST were normal in the normal control,and no significant changes were found in the livers of this group;Compared with the normal control,the plasma ALT and AST levels have been increased significantly and there was a significant pathological hepatic injury in the infection model group;Compared with the infection model group,significant decreases were found in the activity of ALT and AST in the high and middle dosage groups of shuanghuanglian sterilized powder for injection,and there were obvious alleviated pathologic hepatic injury while no significant differences were found in the plasma ALT and AST ac?tivity in the low dosage group;Compared with the infection model group,the ALT and AST activity of the ganciclovir group were lowed significantly;Compared with the ganciclovir group,no significant differences were found in the ALT and AST ac?tivity in the high and middle dosage groups of shuanghuanglian sterilized powder for injection.CONCLUSION:Shuanghuan?glian sterilized powder for injection does have protective effect on hepatic injury of model mouse induced by HCMV.
5.Relation between dampness-heat syndrome of acute period of GBS and neuroendocrine-immunoregulatory network

Objective: To discuss the relation between dampness-heat syndrome of GBS and neuroendocrine-immunoregulatory network.Methods: 34 patients with GBS were divided into dampness-heat syndrome group and nondampness-heat syndrome group by syndromes differentiation of TCM.The autonomic nerve function,the level of immunoglobulin,cortisol(COR) and adrenocorticotropin(ACTH) of these two groups were detected.The datas were compared with that of normal control group.Results:① There were significant difference in function of autonomic nerve between the two groups(damp-heat syndrome group and nondamp-heat syndrome group) and normal control group.There is difference between dampness-heat syndrome group and nondampness-heat syndrome group,too.Dampness-heat syndrome group mainly showed sympathetic nerve excitement(60.0%).In nondampness-heat syndrome group,the incidence of sympathetic nerve excitement is 35.7%;parasympathetic nerve excitement is 28.6%.② In dampness-heat syndrome group the level of IgG was higher than that in nondampness-heat syndrome group and normal control group.The level of IgA was higher than that in normal control group(P
6.Preparation and in vitro Dissolution Behavior Investigation of Frovatriptan Succinate Film-coated Tablet

OBJECTIVE:To prepare Frovatriptan succinate film-coated tablet,and investigate its in vitro dissolution behavior. METHODS:Using lactose monohydrate,microcrystalline cellulose,dioxide,silica,sodium carboxymethyl starch and magnesium stearate as accessories,Frovatriptan succinate tablet was prepared. Using opadry premix spray-coating liquid,Frovatriptan succinate film-coated tablet was prepared. Single factor test was used,using moisture,angle of repose,rigidity,friability,disintegration time and dissolution rate as indexes,to screen the formulation;using dissolution degree as index,coating material dosage was screened. The dissolution curves in vitro of self-made tablets and imported tablets in water,0.1 mol/L HCL,pH of 5.5,6.8 phos-phate buffer solutions were compared. RESULTS:The optimal formulation of Frovatriptan succinate uncoated tablet was as follow as frovatriptan succinate 3.91 mg,lactose monohydrate 99.18 mg,microcrystalline cellulose 33.06 mg,magnesium stearate 1.40 mg,sodium carboxymethyl starch 1.05 mg,silica 1.40 mg;optimal coating weighed quality was 2.0%-4.0%. In the 4 mediums, the dissolution behavior of self-made tablets and imported tablets were similar. CONCLUSIONS:Frovatriptan succinate film-coated tablet is prepared successfully,and its in vitro dissolution behavior is similar to the imported preparations.
7.Effects of orientation transplantation of human bone marrow mesenchymal stem cells on behavioral tests of focal cerebral ischemia in rats

Objective To explore the effect of mesenchymal stem cells(MSCs) on nervous function in rats with focal cerebral ischemia.Methods The MSCs were cultivated,purified,and proliferated in vitro by density gradient and adherence to plastic dishes method.The models of Wistar rats were prepared after middle cerebral artery occlusion(MCAO) of right lasted 90 min and reperfusion 1 h.Wistar rats were randomly divided into normal control group(A,n=10),sham operation group(B,n=10),no-handle group after cerebral ischemia/reperfusion (C,n=10),free-serm DMEM transplantation group after cerebral ischemia/reperfusion(D,n=10),MSCs transplantation group after cerebral ischemia/reperfusion(E,n=10).After identified by flow cytometry,5 ?L 5-bromo-2-deoxyuridine(BrdU) labeled MSCs(4?105? ?L-1) and 5 ?L serum-free DMEM were respectively injected intracerebraly into ischemic boundary zone of right in D and E groups.Immunohistochemical method was used to detect the expression and survival of BrdU-labeled MSCs in vivo.Nervous function behavioral tests were performed on 1st,3th,7th and 28th day after transplantation by forelimb use asymmetry test and postural reflex test.Results MSCs were successfully purified and proliferated in vitro.The MSCs expressed CD29,CD44,but didn't expressed CD34,CD45,CD31 identified by flow cytometry.transplanted MSCs survived and were localized to the ischemic boundary zone.Behavioral tests of every group were improved with time prolonged.However,MSCs transplantation group was significantly better than any other groups(P

Objective To assess the feasibility and short-term effect of transanal everted extraction of laparoscopic rectal excision without abdominal incision.Methods Eleven patients were operated by transanal everted extraction of laparoscopic rectal excision without abdominal incision.Results All the operations were accomplished successfully,the average operative duration was (189.5±25.2) min,the intra-operative blood loss volume was (25.4±9.4) ml,the average time to flatus was (38.9±8.6) h,the average postoperative hospital stay was (12.7±2.5) d,and the number of lymph nodes harvested was 13.5±4.1.There were no post-operative complications.Conclusion The technique of transanal everted extraction of laparoscopic rectal excision without abdominal incision is safe and feasible,but the long-term effects need more observation.
9.Prognostic value of carcinoembryonic antigen and carbohydrate antigen 19-9 in patients with stageⅡcolorectal cancer

Objective To evaluate the significance of the expression of carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) as prognostic indicators for patients with stage Ⅱ colorectal cancer. Methods A total of 285 patients with stage Ⅱcolorectal cancer who underwent potentially curative surgery were enrolled in the study. A high CEA level was defined as a level exceeding 5 ng/ml and a high CA19-9 level was defined as a level exceeding 37 U/ml. Patients were followed up every 3 months to review CEA and CA19-9 levels. Two years later, the chest and abdominal CT examination were performed every 3 months, and then every 6 months until 5 years. Results Out of these 285 patients, 99 (42.60%) patients had high CEA levels, 25 (8.77 %) patients had high CA19-9 levels, and 12 (4.21 %) patients had both high CEA and CA19-9 levels. The overall survival rates of patients with both high CEA and CA19-9 levels were significantly worse than those of others (P< 0.05). During the study, 51 recurrences were diagnosed. There were 22 hepatic recurrences, 10 pulmonary recurrences, 9 local recurrences, 8 lymph node recurrences, and 2 peritoneal recurrences. CT, CEA and CA19-9 were the first abnormal examinations in 30, 5 and 8 recurrent patients, respectively. In 27 % of recurrent patients, the recurrence was detected earlier by CEA and CA19-9 than that by CT. Conclusions The detection of preoperative CEA and CA19-9 levels is useful for predicting the prognosis after potentially curative surgery in patents with stage Ⅱ colorectal cancer. For early detection of occult recurrence of colorectal cancer, tumor markers are relevant.
10.Inhibitory effect of dianhydrogalactitol on DNA TopoⅡ in NCI-H460 cells

Aim To evaluate the antitumor activity of dianhydrogalactitol ( DAG) in vitro, and further clarify its underlying mechanisms. Methods The inhibitory effect of DAG in NCI-H460 cells was detected by CCK-8 assay and colony formation assay. The morphology of cells treated with DAG was observed under optical mi-croscope. Nuclear morphology was captured by fluores-cence microscopy after Hoechst 33342 staining. Real-time PCR was used to analyze the expression level of topoisomerase Ⅱ ( Topo Ⅱ) mRNA. The protein ex-pression level of Topo Ⅱ was detected by Western blot. Additionally, molecular docking approaches were used to predict the interaction between DAG and TopoⅡ. Results DAG exhibited potent antitumor activity in NCI-H460 cells, and inhibited cell proliferation per-sistently. DAG obviously induced nuclear morphologi-cal changes of NCI-H460 cells. Furthermore, DAG could down-regulate the mRNA and protein expression level of Topo Ⅱ detected by Real-time PCR analysis and Western blot, respectively. Molecular docking predicted that DAG could bind to Topo Ⅱ. Conclu-sion DAG can significantly inhibit the proliferation of NCI-H460 cells, and its underlying mechanisms may involve the down-regulation of Topo Ⅱ mRNA and di-rect binding to Topo Ⅱ, leading to cancer cell death.
