1.Research in effect of nursing intervention on rehabilitation promotion in patients with pulmonary tuberculosis and diabetes mellitus

Objective To discuss effect of nursing intervention on rehabilitation promotion in patients with pulmonary tuberculosis and diabetes mellitus. Methods 170 patients with pulmonary tuberculosis and diabetes mellitus were randomly divided into the experimental group and the control group with 85 cases in each. In the experimental group, nurses gave them detailed assessments upon admission, according to their disease condition to formulate corresponding diet and psychological nursing measures, in the control group, conventional care was given. The rehabilitation of the two groups was compared. Results The recovery time of the experimental group shortened, and more patients restored to normal blood glucose and negative urine glucose compared with the control group. Conclusions Diet and psychological nursing intervention contribute a lot to glucose control and disease rehabilitation for patients with pulmonary tuberculosis and diabetes mellitus, and have important significance to improve the quality of life of these patients.
2.Correlation of serum TNFR level with renal function injury in patients with T2DM and normal albumin ;excretion

Objective To discuss the correlation of serum TNFR level with renal function injury in patients with type 2 diabetes mellitus (T2DM)and normal albumin excretion.Methods 90 T2DMpatients with normal albu-min excretion(24h urine albumin excretion rate <20μg/min)were divided into three groups according to the esti-mated glomerular filtration rate level:eGFR >90mL · min -1 · (1.72m2 )-1 ,eGFR 60 -90mL · min -1 · (1.72m2 )-1 and eGFR <60mL·min -1 ·(1.72m2 )-1 ,30 cases in each group.The serum TNFRs level and related clinical data were measured and analyzed statistically.Results The levels of serum TNFR1,TNFR2 were negatively correlated with eGFR(r =-0.428,-0.335,all P <0.05),and positively correlated with Scr(r =0.476,0.225,all P <0.05)in patients with T2DM and normal albumin excretion.The course of disease and the levels of BUN,Scr, CysC,α1 -MG,β2 -MG were increased as the degree of renal function injury.All these index had statistically signifi-cant differences in three groups(F =3.758,5.851,71.738,25.751,7.530,13.735,all P <0.05).Logistic regres-sion analysis showed that the levels of serum TNFR1,TNFR2 were independent risk factors for renal impairment in patients with T2DMand normal albumin excretion(OR =2.009,1.143,all P <0.05).Conclusion TNFRs may be involved in the development and progression of renal impairment,and may become a new conventional indicator for DN because of reflecting the degree of renal function injury in some extent in T2DMpatients with normal albumin excretion.
3.The clinical significance of TNFR1 and TNFR2 in early diabetic nephropathy of type 2 diabetes

Objective To explore the relationship of serum tumor necrosis factor receptor 1 (TNFR1),and TNFR2 with early diabetic nephropathy in type 2 diabetes patients.Methods A total of 70 patients with type 2 diabetes and 35 healthy person were selected from December 2013 to November 2014 in the Fifth Hospital of Datong,the urinary albumin excretion rate (UAER) was determined,and were divided into three groups:normal control group (n =35),simple diabetes group (n =35),and early diabetic nephropathy group (n =35).Enzyme-linked immuno sorbent assay (ELISA) method was used to determine TNFR1 and TNFR2.Results Compared to normal control group [TNFR1 (302.5 ± 60.0) pg/ml,TNFR2 (62.1 ±9.8)pg/ml],simple diabetes group [TNFR1 (515.1 ±78.1)pg/ml,TNFR2 (178.5 ±38.9)pg/ml],and early diabetic nephropathy group [TNFR1 (1 066.8 ± 205.6) pg/ml,TNFR2 (279.6 ± 37.1)pg/ml] were all increased;and early diabetic nephropathy group was higher than simple diabetes group (P< 0.05).The levels of TNFR1 and TNFR2 were positively correlated with age,course,glycated hemoglobin (HbA1c) levels(P <0.01),and were negatively correlated with high density lipoprotein cholesterol (HDL-C) and estimated glomerular filtration rate (eGFR) (P <0.01).The multiple stepwise regression analysis in eGFR had significant effects on TNFR 1 and TNFR 2 levels (P < 0.01).Conclusions The levels of TNFR1,and TNFR2 in early diabetic nephropathy were increased.TNFR1 and TNFR2 may become new markers to evaluate early diabetic nephropathy.

5.Expressions of p63 mRNA and murine double minute 2 in esophageal cancer and molecular mechanisms involved

Objective To explore the expressions and the roll of p63mRNA and murine double minute 2(MDM-2) protein in esophageal squamous cell cancer and the prognosis of the cancer.Methods The expressions of p63mRNA and MDM-2 protein in 38 cases of esophageal squamous cell carcinoma tissues and the corresponding esophageal normal tissues were detected using reverse transcriptase polymerase chain reaction(RT-PCR) and immunohistochemistry. Results The positive rates of deltaNp63 mRNA in all malignant and normal tissues were 63.5% and 52.63% respectively,without significant difference.The expression of the DeltaNp63 mRNA was significantly higher in low differentiated group than in well and moderate differentiated group,without significant difference in age,gender,lymph node metastasis and clinical stage.Only 1 of 38 cases of esophageal squamous cell carcinoma tissues presented TAp63 mRNA expression and all esophageal normal tissues had no TAp63 mRNA expression.The positive rates of MDM-2 protein in esophageal squamous cell carcinoma tissues and normal tissues were 65.79% and 5% respectively,with significant difference.The expression level of the MDM-2 protein was higher significantly in low differentiated group than in well and moderate differentiated group,with significant difference in clinical stage and without significant difference in age,gender and lymph node metastasis.The DeltaNp63 mRNA expression in the esophageal squamous cell carcinoma tissues was positively related to MDM-2 protein expression.Conclusion The major isotype expressed in the esophageal cancer and epithelia is DeltaNp63,which may play an important role in the carcinogenesis,development and prognosis of the esophageal squamous cell carcinoma.TAp63 is not the major isotype expressed in the esophageal cancer and epithelia,and not related to the carcinogenesis of esophageal squamous cell carcinoma.High expression of MDM-2 protein is associated with the carcinogenesis of esophageal squamous cell cancer and related to the prognosis of esophageal squamous cell carcinoma.DeltaNp63 regulates the cell cycle and apoptosis through the pathway of MDM2 and causes the carcinogenesis of esophageal squamous cell cancer.
6.Clinical observation of diabetic foot ulcer treated with Shuxuetong plus external application of deproteinised ;calf blood serum injection

Objective To evaluate the clinical effect of Shuxuetong plus external application of deproteinised calf blood serum injection on diabetic ulcer foot.Methods 60 patients with diabetic foot ulcer were enrolled:30 cases (treatment group)were randomly assigned to receive deproteinised calf blood serum injection(external use)plus Shuxuetong(iv gtt),and the other 30 cases (control group)received routine western medicine therapy.One course of treatment lasted one month.In addition,both two groups received insulin to control blood sugar and antibiotics to control infection,with foot ulcer treated with debridement,drainage,and removal of necrotic tissue etc.Results Com-pared with the control group,the effective rate was higher in the treatment group(86.7% vs.60.0%,χ2 =5.455,P =0.020).The occurrence time of granulation tissue[(6.60 ±1.14)d vs.(10.80 ±2.17)d,t =-3.834,P =0.008] and wound healing time[(23.62 ±6.14)d vs.(35.65 ±4.45)d,t =-4.405,P =0.002]were shorter in the treat-ment group.The wound area in the two groups reduced after treatment,and the wound area in the treatment group reduced significantly[(3.48 ±1.16)cm2 vs.(8.60 ±2.51)cm2 ,t =-4.143,P =0.007].Conclusion As com-pared with the routine therapy with western medicine,the combination therapy of deproteinised calf blood serum injec-tion plus Shuxuetong has better efficacy for diabetic ulcer foot.
8.Clinical observation of the effect of safflower yellow combined with kallidinogenase on early diabetic nephropathy

Objective To assess the clinical effect of safflower yellow combined with kallidinogenase on ear-ly diabetic nephropathy.Methods 80 early diabetic nephropathy patients were all given diet control, appropriate exerciseand positive controlled blood sugar,blood fat,blood pressure.They were randomly divided into two groups,the control group was intramuscularly given kallidinogenase 40U dissolved in sodium chloride,once daily.Based on this, the treatment group added safflower yellow 100mg into 5%glucose injection 250mL and 3 unit insulin by intravenous drip.The period of treatment was three weeks.24h urinary micro-albumin(U-ALB) and urinary albumin excretion rate( UAER) before and after treatment between the two groups were surveyed.Results Before treatment,24 h U-ALB and UAER of the control group were (100.44 ±65.72) mg/L,(117.31 ±68.32)μg/min,which of the treatment group were (123.57 ±63.28) mg/L,(138.57 ±50.82)μg/min.There were no significant differences between the two groups(t=1.098,1.056,all P>0.05).After treatment,24h U -ALB and UAER of the control group were (79.39 ±65.33)mg/L,(92.68 ±67.95)μg/min,which in the treatment group were (71.87 ±60.03)mg/L,(62.52 ± 38.64)μg/min.Statistical differences of 24h U-ALB and UAER were observed in the two groups compared with before treatment(the control group:t=5.903,6.069,the treatment group:t=6.779,12.612,all P<0.05).After treatment, there was no significant difference in 24h U-ALB between the two groups(t=-0.386,P>0.05),while UAER in the treatment group was lower than the control group(t=-2.029,P<0.05).Conclusion Safflower yellow com-bined with kallidinogenase can reduce UAER in early diabetic nephropathy,and what's more,can put off its progress.
9.Content Determination of Metacresol by GC

Objective:To establish a GC method for the determination of metacresol. Methods:The separation was performed on an Agilent DB-225 MS column (30 m × 0. 25 mm,0. 25 μm). The initial column temperature was set at 90℃,maintained 10 min, raised to120℃ with a rate of 2℃·min-1 ,and then raised to 150℃ with a rate of 10℃·min-1 and maintained for 5 min. The inlet temperature was 200℃. The detector was a flame ionization detector (FID) and the temperature was 250℃. The flow rate was 1. 8 ml ·min-1 split ratio was 1∶30 and the carrier gas was nitrogen. Results:The linear range was 0. 6-1. 8 mg·ml-1 . The average recovery was 96. 0% (RSD=2. 6%,n=9). The content of 3 batches of the samples was 99. 3%, 98. 7% and 98. 4%, respectively. Conclu-sion:The established quantitative analysis method is applicable in the quality control of metacresol.
10.Proteomic analysis of effects of indomethacin on colorectal cancer xenografts in nude mice

Objective To investigate the effect of indomethacin on the protein expression profile of colorectal cancer cell line HCT116 xenografts in nude mice, and to screen the indomethacin anticancer-related proteins. Methods The nude mice with colorectal cancer cell line HCT116 xenograft were treated with or without indomethacin, and then the total proteins extracted from xenografts were separated and visualized by immobilized pH gradient two-dimensional gel (2-DE) electrophoresis. The differential expression proteins between the experimental and control groups were analyzed using PDQuest image analysis software, and identified by peptide mass fingerprinting based on matrix-assisted laser desorption ionization time of flight mass spectrometry and database searching. Results The 2-DE patterns with high resolution and reproducibility were obtained, displayed about 1100 protein spots in each gel, and the matching rate in the two groups was 96. 0% and 93. 6% , respectively. Thirty-one differentially expressed protein spots were found between the two groups, of which 25 spots decreased and 6 increased in the expression levels in the experimental group than in the control group. Eighteen peptide mass fingerprinting maps were obtained, and twelve proteins including galectin-1, annexin and trancription factor, were primarily identified after database searching, most of which were involved in tumor cell proliferation, invasion, apoptosis and immunity. Conclusions Indomethacin exerts its inhibiting effects on colorectal cancer through regulating multiple proteins expression directly or indirectly. The data will be helpful to elucidate the mechanism inhibiting cancer by indomethacin, and to find new targets of chemotherapeutic drugs.
