1.MRI Features and Diagnostic Value in Adenomyosis

Objective To analyze MRI features and diagnostic value in adenomyosis.Methods Clinical and MRI data of 31 cases of adenomyosis confirmed by pathology were retrospectively analyzed.Turbo FSE T2WI,SE T1WI,and sagittal Turbo FSE T2WI examination were performed in all cases,10 of them were added to anatomic and sagittal fat-suppression(FS) T2WI scan.MR features of adenomyosis were analysed.Results In 19 cases of diffuse uterine adenomyosis,11 cases mainly showed diffusely thichened junctional zone of posterior wall.12 cases were focal uterine adenomyosis,the lesions located at posterior wall in 7 cases,front wall in 4 cases and bottom part in 1 case.28 cases displayed hypointensive masses like the intensity of junctional zone on T2WI and vesicular hyperintensive foci were found within the hypointensive lesions.The lesions in 21 cases were similar to that of myometrium on T1WI,9 of them displayed vesicular hyperintensive foci.Focal round-like junctional zone low signal intensity lesion in myometrial layer was found in 3 cases.Conclusion MRI can clearly determine the size and site of adenomyosis.FS is the valuable examination way.
2.Determination of loratadine in human plasma by LC-MS and its pharmacokinetic studies

AIM:To establish an LC-MS method for determining the concentrations of loratadine (LOR) in human plasma and to evaluate its pharmacokinetic characteristics. METHODS: A ZORBAX Eclipse XDB-C8 (5 μm, 150 mm×4.6 mm) column was used, atmospheric pressure electronic spray ionization (AP-ESI) and ion mass spectrum (m/z) of 388.2 (M+H)+ were selected to quantify LOR, and 275.1 (M+H)+ for ropivacaine (internal standard, IS). RESULTS: The linear range of LOR standard curve was 0.5-50 ng·ml-1, and the determination limit was 0.5 ng·ml-1. The pharmacokinetic parameters of LOR after a single dose of 20 mg tablet (T1), capsule (T2) and reference (R) were as follows, the half life (t1/2) 13.52±1.35, 13.14±0.98 and 14.00±1.25 h, the time to peak concentration (Tmax) 1.24±0.06, 1.18±0.12 and 1.17±0.12 h, the peak concentration (Cmax) 21.72±7.70, 21.49±8.34 and 20.50±8.65 ng·ml-1, the area under time-concentration curve (AUC0-48 and AUC0-∞) 137.24±47.84 and 146.61±51.03 ng·ml-1·h, 139.65±45.69 and 148.04±48.10 ng·ml-1·h, 134.19±49.03 and 143.70±52.08 ng·ml-1·h, the relative bioavailability of LOR tablet and capsule were (105.49±8.08)% and (102.90±10.02)%, respectively. CONCLUSION: The LC-MS method for determining the concentration of LOR in human plasma is sensitive and accurate and can be used for LOR bioavailability and pharmacokinetic studies. LOR tests and reference are bioequivalent.
3.Chemical constituents of Helicia nilagirica seeds (Ⅰ)

Objective To study the chemical constituents of Helicia nilagirica. Methods The ethanol extract was seperated by petroleum ether, dichloromethane, and n-butanol in sequence, then isolated by silica gel column chromatography. The structures were identified and elucidated by physicochemical pro-(perties) and spectral analysis. Results Four compounds were isolated from the petroleum ether extract and identified as n-hexadecane acid (Ⅰ), ?-sitosterol (Ⅱ), ?-sitosterol-3-O-?-D-glucoside-6′-acetate (Ⅲ), and daucosterol (Ⅳ). Conclusion All the compounds are isolated from the plant for the first time. Compounds Ⅰ, Ⅲ, Ⅳ are isolated from the plants of Helicia Lour. for the first time, and compound Ⅲ is a new natural product.
4.Determination of betamethasone in human plasma by liquid chromatography with tandem mass

A sensitive and selective liquid chromatography-tandem mass spectrometric (LC-MS/MS) method was developed and validated for the determination of betamethasone in human plasma. The analyte was isocratically eluted on a Venusil XBP C8 column (200 mm ×3.9 mm ID, 5 μm) with methanol-water with a triple quad LC-MS/MS using ESI with positive ionization. Ions monitored in the multiple reaction monitoring (MRM) mode were m/z 393.3→355.2 for betamethasone and m/z 361.3→343.2 for prednisolone (IS). Betamethasone was extracted from 0.5 mL human plasma with ethyl acetate. The validation study demonstrated excellent precision and accuracy across the calibration range of 0.5 - 80.0 injection in healthy Chinese volunteers.
5.Prognostic value of endothelial dysfunction assessment in patients on peritoneal dialysis

Objective To explore the value of endothelial dysfunction assessment in predicating major adverse cardiac and cerebrovascular events (MACCE) in peritoneal dialysis (PD) patients.Methods A prospective cohort study included 136 end stage renal disease (ESRD) patients from Jan 1,2009 to Dec 31,2011 was conducted.Endothelial function was assessed by flowmediated dilation (FMD) of brachial artery.Kaplan-Meier method was used to estimate survival rate.The survival difference between the two groups was compared by the log-rank test.Multivariate Cox proportional hazards regression was used to determine the independent risk factors of MACCE.Results In the follow-up period,18 patients in low FMD (FMD≤2.7%) group occurred MACCE,and 13 patients in high FMD (FMD > 2.7%) group occurred MACCE.Compared with high FMD group,MACCE-free survival rate in the low FMD group had a significantly decreased (x2=4.190,P=0.041).Multivariate Cox proportional hazards regression analysis showed that higher level of total cholesterol,lower FMD,longer PD time and higher levels of hs-CRP were all independent predictors of MACCE.Conclusion Reduced brachial artery FMD is an independent risk factor of MACCE,and the application of FMD contributes to the risk stratification of cardiac and cerebrovascular disease in PD patients.
6.Determinations of mifepristone and its metabolites and their pharmacokinetics in healthy female Chinese subjects.

The aim of this study is to establish an HPLC method for simultaneous determinations of mifepristone and its metabolites, mono-demethylated mifepristone, di-demethylated mifepristone and C-hydroxylated mifepristone in plasma and to evaluate the pharmacokinetic characteristics of mifepristone tablet. Twenty healthy female Chinese subjects were recruited and a series of blood samples were collected before and after 0.25, 0.5, 1.0, 1.5, 2.0, 4.0, 8.0, 12.0, 24.0, 48.0, 72.0 and 96.0 hours administration by a single oral dose of 75 mg mifepristone tablet. Mifepristone and its three metabolites were extracted from plasma using ethyl acetate and determined by high performance liquid chromatography. The main pharmacokinetic parameters of mifepristone and its metabolites, including Cmax, tmax, MRT, t(1/2), V, CL, AUC(0-96 h) and AUC(0-infinity), were calculated by Drug and Statistical Software Version 2.0. The simple, accurate and stable method allows the sensitive determinations ofmifepristone and its metabolites in human plasma up to 4 days after oral administration of 75 mg mifepristone tablet and the clinical applications of their pharmacokinetic studies.
7.Association of HOTAIR genetic variation and gene-environment interaction with clinical features of liver cancer

ObjectiveTo investigate the association of the genetic variation of the long non-coding RNA HOX transcript antisense RNA (HOATIR) and gene-environment interaction with prognosis-related clinical features of liver cancer. MethodsA total of 923 patients with primary liver cancer Shunde Hospital of Southern Medical University were admitted to a hospital from October 2010 to October 2016 were enrolled in this study. TaqMan quantitative PCR was used to detect HOTAIR rs17105613 T>C, rs12427129 C>T, and rs3816153 G>T genotypes. The chi-square test was used to analyze the difference in the distribution of clinical features of liver cancer, and the logistic regression model was used to analyze the influence of the genetic variation of HOTAIR on the TNM stage of liver cancer, portal vein tumor thrombus, and age of onset. ResultsAfter the adjustment for environmental factors, rs17105613 and rs3816153 were significantly associated with TNM stage in the recessive mode (P<005), and there was a statistically significant multiplicative interaction between rs12427129 and smoking on the age of onset of liver cancer (P=0.029), as well as an additive interaction with critical statistical significance (P=0.092). ConclusionHOTAIR rs17105613 and rs3816153 may be associated with TNM stage of liver cancer. The interaction between rs12427129 and smoking may influence the age of onset of liver cancer. Therefore, the genetic variation of HOTAIR may promote the invasion and metastasis of hepatoma cells.

Objective To improve the synthesis process of chlorine f (1).Methods A "one-pot"method was applied to prepare Photosensitizer component (1),using pheophorbide a (3) as raw material by oxidating and cracking of the E-ring of (3) with bubbling oxygen in alcoholic solution of potassium hydroxide at 0 ℃ followed by refluxing in nitrogen atmosphere.In order to obtain the optimal synthetic procedure,the orthogonal experimental design of L9 (34 ) was adopted to investigate three different levels of four main factors i.e.ring opening reaction time,alcoholic variety,alkali concentration and refluxing reaction time.Results The target compound (1) was optimizedly synthesized through treatment of raw material (3) with bubbling oxy-gen in 25% ethanol solution of potassium hydroxide at 0℃ for 30 min,followed by refluxing in nitrogen atmosphere for 20 min in yield of 40.8%.Conclusion The procedure developed has some advantages of simple and safty operation,and high synthetic yield.

Objective To design and prepare 13 ,15-cycloimides chlorin p6 (1) ,a class of chlorin related antitumor photo-sensitizers ,which contain a more stable six-membered cyclic imide comparing to the exocyclic anhydride ring of purpurin-18 (2) .Compounds (1) exhibit strong absorption at long wavelengths near λmax 700 nm to take full advantage of greater tissue penetration .Methods Pheophorbide a (3) was obtained by acid hydrolysis of chlorophyll a ,which was from crude chlorophyll extracts of Chinese traditional herb named Silkworm excrement .Purpurin-18 (2) was prepared by air oxidation and alkali open loop simultaneously on five-membered beta-keto carboxylic ester ring of pheophorbide a (3) .Finally ,the target compounds 1a~1j were synthesized via condensation of its anhydride ring with various amines including carboxyl-protected amino acids . Results Target compounds 1a~1j were successfully synthesized in yields ranged from 32 .6% to 65 .2% .Their structures were confirmed by elemental analysis ,ESI-MS and 1 H NMR spectra .Conclusion Treatment of purpurin-18 (2) with amines can produce target compounds 1a~1j .The starting raw material was inexpensive and readily available .The reaction conditions were mild and workup was convinient .
