A male patient, 55 years old, suffered from intermittent sound,voice depression and shortness of breath for one year,and from dysphagia for 3-4 months. Through fiber laryngoscopy,we could see tumor in the left posterior aryepiglottic fold. The tumor's surface was smooth. A portion of the tumor protruded to the laryngeal cavity and the aryepiglottic fold external,it also covered most of the glottis. Bilateral vocal cord were smooth and had good mobility. Throat CT demonstrated an irregular soft tissue mass on the left side of the aryepiglottic fold in supraglottic area with obscure normal boundary from adjacent structure. The left side of pyriform sinus became shallow without obvious bone destruction. The pathological report showed pleomorphic adenoma. The diagnosis was laryngeal pleomorphic adenoma.

OBJECTIVE: To Study the personality characteristics in patients with sudden deafness. METHOD: Thirty-eight sudden deafness patients and 45 healthy volunteers were assessed by Eysenck personality questionnaire (EPQ). RESULT: The standard scores of P, E, N dimensions of patients with sudden deafness were greater than healthy volunteers. The standard score in L dimensions of patients with sudden deafness was less than healthy volunteers. The difference of the standard score of P dimension revealed statistical significance (P < 0.05). The difference of the standard score of N dimension revealed statistical significance (P < 0.01). The difference of the standard scores of E, L dimensions revealed no statistical significance (P > 0.05). CONCLUSION Sudden deafness patients have emotional instability and psychoticism personality characteristics.

This research aims to construct a lentiviral expression vector carrying the extracelluar domain (ED) of human hepatocyte growth factor receptor (C-Met), and to express it in transfected 293T cells. The extracellular domain of C-Met was amplified by RT-PCR, ligated with lentiviral expression vector p RRL-CMV-ED, and then expressed in 293T cell line. The expressed protein was purified and identified by RT-PCR and Western blot. The enzyme digestion and sequence analysis showed that the lentiviral expression vector p RRL-CMV-ED was constructed correctly. The size of amplified genes was about 2 700 bp. The purified protein with Ni-affinity column was about 105 kD analyzed by SDS-PAGE. The Western blot and ELISA results showed that the expressed protein which could bind to HGF specifically was the extracelluar domain of human hepatocyte growth factor receptor. This research may lay a foundation for further study of anti-C-MET monoclonal antibody and neutralizing antibody.

Objective: To compare the accumulative scores of cerebrovascular hemodynamic parameters (CVHP) in different populations and to analyze the relationship between the change of accumulative scores and the risk of stroke. Methods: A total of 10 565 participants aged 40 years and above were selected from a cluster sampling community-based population. Their risk factors were investigated and CVHP was evaluated. The CVHP scores were accumulated by a unified approach, and the incidence of stroke was followed up. The participants were grouped into health adult, single factor exposure, multiple-factor exposure, and newly developed stroke during the follow-up period. The differences of CVHP accumulative scores and the change law among the 4 groups were compared respectively according to age and sex. Results: The accumu-lative scores of the mean (SD) CVHP in the health adult, single factor exposure, multiple-factor exposure, and newly developed stroke groups were 86.44 (20.69), 72.07 (28.10), 60.82(34.64), and4.01 (29.36) in men respectively, and they were 83.95 (22.19), 67.97 (29.73),59.91 (31.34), and 42.64 (28.00) in women respectively, which had a tendency to gradually decrease. The accumulative scores of CVHP and their distributions at the same age stage in all the 4 groups had significant differences (P ＜ 0.01 ). The accumulative scores of CVHP and their distributions for the same factors between all age groups also had significant differences (P ＜0. 01). Conclusions: The accumulative scores of CVHP had a tendency to gradually decrease from the health adults, risk factor exposure to high risk status before stroke. The decrease of the accumulative scores is closely associated with the increase of stroke risk, which can be used as a predictor of stroke.

ObjectiveTo investigate clinical characteristics and perioperative management of abdominal aortic aneurysm in elderly patients. MethodsThe clinical data of 24 patients aged 60 years and over with abdominal aortic aneurysm in our hospital were analyzed retrospectively. ResultsThe average age was 75.5 years and the proportion of male and female was 5 : 1. The course of disease was 2 days to 15 years and the median course was 2.8 months. 17 patients were complicated with hypertension, 5 patients with coronary heart disease, 4 patients with type 2 diabetes mellitus, 3 patients with chronic bronchitis, 2 patients with lacunar cerebral infarction, 3 patients with abdominal aortic aneurysm rupture, 13 patients with endovascular stent grafts, 10 patients with abdominal aortic aneurysm resection and artificial vascular replacement, 1 patient with endovascular stent grafts, endarterectomy of right femoral artery and right deep femoral artery,right deep femoral artery plasty and the application of artificial blood vessel in right femoro-popliteal arterial bypass. Postoperative complications happened in 15 cases (62.5%, 15/24) and the postoperative mortality rate was 20.8%.ConclusionsThe elderly patients with abdominal aortic aneurysm are always complicated with manyother chronic diseases such as hypertension, coronary heart disease , diabetes mellitus, chronicbronchitis. The operation for abdominal aortic aneurysm in elderly patients has high risk. Reasonablesurgical procedure and active perioperative management are the key for successful treatment.

Objective To construct and evaluate a mouse model of traumatic brain injury (TBI)that simulates both motor and cognitive impairment.Methods Twenty-four healthy male C57BL/6 mice were randomly divided into a sham group and a TBI group (n=12/group).The TBI model was prepared by referring to the compression injury model with some modifications.The sham group underwent an identical process without mechanical trauma.Motor function was evaluated using the rotarod and beam-walking tasks at 1,3,7,14,21,28 days post-injury.Spatial learning and memory capacities were assessed at 28,29,30,31,32,33 days post-injury by the Morris Water Maze (MWM) test.Nissl staining was performed to observe pathological changes and immunofluorescence staining to detect the expression of glial fibrillary acidic protein (GFAP) and ionized calcium binding adaptor protein (Iba-1) in the mouse brain on the 34th day after modeling.Results The latency for the TBI group was significantly lower than that for the sham group,and the frequency of slipping off the beam by the right hindlimbs for the TBI group was significantly higher than that for the sham group at 1,3,7,14,21,28 days post-injury (P＜0.05).The escape latency for the TBI group was significantly longer than that for the sham group in the MWM test at 30,31 and 32 days after modeling (P＜0.05).The times of crossing the platform for the TBI group were significantly less than those for the sham group at day 33 after TBI (P＜ 0.05).The lesion volume for the sham group was significantly smaller than that for the TBI group [(0.55± 0.06)％ vs.(16.90±1.14)％,P＜0.05].The numbers of astrocytes in the TBI and sham groups were respectively 101.40±6.18/mm2 and 20.17±1.55/mm2,and the numbers ofmicroglia in the 2 groups were respectively 119.20±6.28/mm2 and 23.58±1.72/mm2,showing statistically significant differences between the 2 groups (P＜0.05).Conclusion Since the TBI model we constructed is simple and reproducible with stable motor deficits and cognitive impairments which are consistent with the pathological changes of moderate TBI.it can be used in animal TRI experiments.

OBJECTIVETo evaluate efficacy and optimal cut-off-point through cerebral vascular hemodynamic indexes (CVHI) examination to predict stroke.

METHODSA number of 20,333 people at 35 years old and over were checked by CVHI and accumulative score was calculated according to the value of detected indexes. Risk factors of stroke were investigated simultaneously. One hundred and sixty-eight patients with stroke occurred during 4-year following up. Typical syndromes and signs stroke were used as golden standard to evaluate screening efficacy of CVHI.

RESULTSScore of optimal cut-off-point of cerebral vascular hemodynamic indexes was under 75 in ROC curve analyses. Sensitivity, specificity, accuracy, positive and negative predictive values, positive and negative likelihood ratios as well as Youden's index for predicting stroke within 4 years after examination were found to be 87.50%, 67.70%, 67.86%, 2.21%, 99.85%, 2.71, 0.18 and 0.55 respectively. Sensitivity and positive predict values for predicting cerebral vascular thrombosis were superior to predicting cerebral hemorrhage. Positive predicting value in risk exposure population was higher than that of overall population. Coefficiency of variation of cerebral vascular hemodynamic examination was 4.03%. The agreement rate of examination between two physicians was 97.62% and Kappa value was 0.94.

CONCLUSIONThe score of optimal cut-off-point of cerebral vascular hemodynamic indexes examination was 75. Both Efficacy and reliability for predicting stroke seemed to be good, especially for predicting cerebral vascular thrombosis.

OBJECTIVETo investigate the myeloperoxidase (cMPO) expression pattern by flow cytometry (FCM) in patients with acute myeloid leukemia (AML) and its role in classifying AML.

METHODSEight- color multiparametric FCM with CD45/SSC gating was used to determine the cMPO expression in 502 AML patients.

RESULTSThe positive rate of cMPO in all patients was 58.0%, in which the proportion of normal positivity, dim positivity and partial positivity was 21.5%, 34.1% and 2.4%, respectively. The remaining case (42.0%) were all negative. In AML with t (15;17）(q22;q12)/PMLRARα, the positive rate was the highest (100%) and the intensity was similar to that of the normal granular leukocytes, followed by AML with t (8;21（q22;q22/RUNX1-RUNX1T1, the positive rate was 91.4% and the intensity was mostly dim. AML with minimal differentiation and acute megakaryoblastic leukemia were all cMPO negative. The positive rates of cMPO in the remaining subtypes were between 22.7% and 76.2%.

CONCLUSIONThe positive rate and intensity of cMPO were significantly different among different subtypes of AML.

Surgical operation is the main treatment of oral and maxillofacial tumors.Dysphagia is a common postoperative complication.Swal-lowing disorder can not only lead to mis-aspiration,malnutrition,aspiration pneumonia and other serious consequences,but also may cause psychological problems and social communication barriers,affecting the quality of life of the patients.At present,there is no systematic evalua-tion and rehabilitation management plan for the problem of swallowing disorder after oral and maxillofacial tumor surgery in China.Combining the characteristics of postoperative swallowing disorder in patients with oral and maxillofacial tumors,summarizing the clinical experience of ex-perts in the field of tumor and rehabilitation,reviewing and summarizing relevant literature at home and abroad,and through joint discussion and modification,a group of national experts reached this consensus including the core contents of the screening of swallowing disorders,the phased assessment of prognosis and complications,and the implementation plan of comprehensive management such as nutrition management,respiratory management,swallowing function recovery,psychology and nursing during rehabilitation treatment,in order to improve the evalua-tion and rehabilitation of swallowing disorder after oral and maxillofacial tumor surgery in clinic.
