2.Application of Yinertong antibacterial fluid combined with Yintshu antibacterial gel on puncture wound associated infection in pleural catheter drainage

Objective:To investigate the effect of Yinertong antibacterial fluid combined with Yintshu antibacterial gel on puncture wound infection in pleural catheter drainage.Methods:A total of 80 patients who underwent pleural catheter drainage in Gulou Hospital Affiliated to Medical College of Nanjing University from January 2018 to December 2019 were enrolled. They were divided into the control group and the experimental group by random digits table method, with 40 cases each. The control group was given alcohol disinfection and traditional iodophor. The experimental group was given Yinerong antibacterial fluid combined with Yintshu antibacterial gel. The infection of puncture wound, degree of edema, incidence of catheter displacement, colonization rate of catheter pathogens, pain, and nursing satisfaction were observed and compared between the two groups.Results:The wound infection rate was 15% (6/40) in the control group, and 0(0/40) in the experimental group, the difference was statistically significant ( χ2=4.50, P<0.05). There was no significant difference in colonization rate of catheter pathogens between the two groups ( P>0.05). There were 28, 6, 4 and 2 cases of no edema, mild, moderate and severe edema in the control group and 38, 1, 1 and 0 cases in the experimental group, the difference was statistically significant ( Z=8.19, P<0.05). There was no significant difference in the scores of Visual Analogue Scale (VAS) of immediate pain of puncture between the two groups ( P>0.05). The score of VAS after 24 hours of puncture and at the time of extubation was (3.10 ± 1.34), (1.50 ± 1.36) points in the experimental group, and (3.83 ± 1.28), (2.38 ± 1.28) points in the control group, the differences were statistically significant ( t=2.48, 2.97, both P<0.05). The satisfaction rate after discharge was 97.5% (39/40) in the experimental group, and 80.0% (32/40) in the control group, the difference was statistically significant ( χ2=8.31, P<0.05). Conclusions:The combined application of Yinertong antibacterial fluid and Yintshu antibacterial gel can significantly reduce the wound infection rate of pleural catheter drainage, reduce wound edema and pain, and improve patients′ satisfaction with nursing work, which is worthy of popularization and application.
3.Effects of home care on patients with pulmonary alveolar proteinosis

Objective To explore the effects of home care on pulmonary alveolar proteinosis patients. Methods The 24 cases of pulmonary alveolar proteinosis ( PAP) patients were divided into observation group and the control group on average. The patients of observation group received after discharge home care including health education, outpatient visits, telephone follow-up and on-site visits while the patients of control group had no this kind of interventions. Dyspnea index, six-minute walk test, pulmonary function tests and body mass index were compared between two groups after discharge. Results The dyspnea evaluation scores were (1. 31 ± 1. 07), (1. 29 ± 0. 62) and (1. 21 ± 0. 83) at 4, 8,12 weeks after discharged in the observation group and all were better than that of the control group (t=6. 438, 6. 897, 8. 572;P<0. 01). Six-minute walk test scores of observation group outbalanced scores in the control group at 4, 8,12 weeks after discharged (t=3. 214, 4. 316, 5. 673;P<0. 05). At 12 weeks after discharged,lung function of observation group were better than that of the control group(P<0. 05). The BMI differences between the two groups were not statistically significant(P>0. 05). Conclusions The implementation of home care can improve the PAP patients′ six-minute walk test distance and lung function, and reduce the patients with dyspnea index.
4.Effect of transitional care model on health recovery in discharged elderly patients with angiocardiopathy

Objective To explore the effect of transitional care model on health recovery in discharged elderly patients with angiocardiopathy.Methods A group of transitional care of geriatric department was formed by physicians and nurses of geriatric department, physicians and therapists of rehabilitation department and so on. A total of 81 discharged patients with angiocardiopathy of geriatric department in the Second People's Hospital of Nantong were selected as research group from July 2015 to February 2016 using purposive sampling. They accepted transitional care. A total of 85 elderly patients with angiocardiopathy were recruited as control between December 2015 to June 2016 with routine health education. The effect and the patients' satisfaction were compared between two groups.Results The compliance rate of medication of patients was 93.6% in research group and 65.8% in control group in the second week after leaving hospital with a significant difference (χ2=20.447,P<0.05). There was a statistically significant difference in satisfaction between research group [(95.21±1.98)%] and control group [(88.34±1.84)%] (t=32.7470,P<0.05).Conclusions The transitional care can improve the medication compliance in discharged elderly patients with angiocardiopathy, help them build up health lifestyle and improve the quality of life of patients.
