1.Preparation of a new cationic liposome and determination of its gene transfection rate

Objective To preparation a new cationic liposome of which the positive component is a new cytofectin as a gene delivery vehicle for laboratory research of gene transfection and gene therapy in lung diseases. Methods The new cationic liposome was prepared by film. Electron microscopy and gel electrophoresis were employed to define the condition for transfection. The prepared cationic liposome was used in the transfection of a fluorescence plasmid to lung cancer cell line SPC and the rate of the transfection was evaluated. Results The prepared new cationic liposome was global microcapsule in size of 100 nm-1 ?m. The highest transfection rate of 60% was attained when the liposome was in a ratio of (6-8)∶1 with plasmid. Conclusion The new cationic liposome is prepared with cytofectin N 4-spermine cholesteryl carbamate as its positive component, and sound transfection activity is observed under a certain circumstance. It provides a basis for gene transfection and gene therapy.
2.Expression of serum microRNA-92 in patients with liver cancer and its relationship with poor prognosis

Objective:To investigate the expression of serum microRNA (miR)-92 in patients with liver cancer and its relationship with poor prognosis.Methods:The clinical data of 70 patients with liver cancer in the First People′s Hospital of Huzhou City, Zhejiang Province and the Central Hospital of Huzhou City, Zhejiang Province from May 2015 to May 2018 were retrospectively analyzed. The miR-92 expression level was detected by real-time quantitative polymerase chain reaction method and compared with that of 80 healthy subjects. Receiver operating characteristic (ROC) curve was drawn to evaluate the efficacy of serum miR-92 in diagnosing liver cancer and early stage (stage Ⅰ to Ⅱ) liver cancer. The relationship between serum miR-92 expression level and clinicopathological characteristic was analyzed. Independent risk factors affecting the prognosis in patients with liver cancer were analyzed by multivariate Cox regression model.Results:The expression level of serum miR-92 in patients with liver cancer was significantly higher than that in healthy subjects (4.10 ± 1.74 vs 1.88 ± 0.78), and there was statistical difference ( P<0.05). ROC curve analysis result showed that the area under curve (AUC) of serum miR-92 for the diagnosis liver cancer was 0.874, the best cut-off value was 2.43, the sensitivity was82.86%, and the specificity was 86.25%; the AUC of serum miR-92 for the diagnosis early liver cancer was 0.755, the best cutoff value was 2.38, the sensitivity was 68.57%, and the specificity was 84.42%. The serum miR-92 expression level was related to TNM stage and lymph node metastasis ( P<0.01), but not related to gender, age, tumor diameter, history of hepatitis, liver cirrhosis, degree of differentiation and portal vein tumor thrombus ( P>0.05). The 5-year overall survival rate in liver cancer patients with high serum miR-92 expression was significantly lower than that in liver cancer patients with low serum miR-92 expression (17.1% vs. 31.5%), and there was statistical difference ( χ2 = 5.561, P<0.05). Multivariate Cox regression analysis result showed that miR-92 was an independent risk factor affecting the prognosis in patients with liver cancer ( HR = 0.282, 95% CI 1.179 to 3.562, P<0.05). Conclusions:The expression level of serum miR-92 in patients with liver cancer is significantly increased, which plays an important role in the progression of the disease. Serum miR-92 can be used as an indicator for early diagnosis and prognosis evaluation of liver cancer.
3.Characterization and identification of three kinds of wood class medicine by fourier ;transform infrared spectroscopy

Objective Using infrared spectroscopy to analyze three kinds of lignum Chinese medicine to provide a theoretical basis for the establishment of medicinal overall quality evaluation system. Methods Functional groups identification, Fourier Transform-infrared spectroscopy(FT-IR) and the second derivative infrared spectroscopy (SD-IR) could be applicable to analyze and identify the overall chemical composition of three kinds of lignum Chinese medicine. Results The main chemical components of Aquilariae Lignum Resinatum (ALR), Dalbergiae Odoriferae Lignum (DOL) and Sappan Lignum (SL) were cellulose, lignin and aromatics. The main chemical compositions of Sappan Lignum (SL) were cellulose, lignin and aromatics. The differences in FT-IR and SD-IR of DOL and SL indicated that they had different aromatic compounds. Conclusion Infrared spectroscopy can be used as a simple and accurate quality control method of three kinds of lignum Chinese medicine.
4.Standardization of gestational diabetes diagnosis and treatment of the impact on pregnancy outcome

Objective To observe the effects of standardized treatment to pregnancy and perinatal outcome of children gestational diabetes (GDM).Methods Forty-five cases of GDM glycemic control satisfied patients and 36 cases of GDM glycemic control dissatisfied patients were enrolled in this study who in the General Hospital of Jizhong Energy Feng Feng Group hospital from January 2010 to October 2012,and 50 cases of the same period of hospitalization single fetal blood sugar normal pregnant women were randomly selected as control group.Pregnancy outcome and perinatal outcome of three groups were analyzed and compared.Results (1) Pregnancy outcome of three groups:the incidence of concurrent gestational hypertension (27.8%),polyhydramnios(22.2%),premature delivery (16.7%),cesarean section rate (80.5%) and puerperalism (13.9%) in GDM glycemic control dissatisfied patients were higher than those of GDM glycemic control satisfied patients (8.9%,6.7%,2.2%,55.6%,2.2%) and control group (6.0%,4.0%,2.0%,46.0%,2.0%),and the differences were statistically significant (P< 0.05).(2) Perinatal outcome of three groups:the incidence of huge child (27.8%),neonatal hypoglycemia (41.7%),neonatal asphyxia rate (22.2%) of GDM glycemic control dissatisfied patients were higher than those of GDM glycemic control satisfied patients (4.4%,17.8%,6.7%) and control group (6.0%,2.0%,4.0%),and the differences were statistically significant (P<0.05).Conclusion Gestational diabetes increases the incidence of infant and maternal complications,early diagnosis,standardized diagnosis and treatment can reduce infant and maternal complications,improve pregnancy outcomes.
5.Approach to the patient with primary adrenal lymphoma combined with empty sella

[Summary] To discuss the clinical features and treatment of primary adrenal lymphoma( PAL) with empty sella(ES).The lymphoma of the patient was nonspecific,the levels of serum sodium, cortisol, adrenocorticotropic hormone, and gonadotropin were decreased.MRI confirmed ES, CT showed bilateral adrenal enlargement.CT-guided fine-needle aspiration biopsy of the adrenal gland revealed a cytological diagnosis of diffused large B cell lymphoma( DLBCL).The patient had received R-CHOP chemotherapy, which results in a complete functional recovery and the tumoral masses disappeared.PAL combined with ES was rarely reported, which is difficult to diagnose due to complex hormone levels.
6.A case of pachydermoperiostosis caused by a deletion mutation in the HPGD gene

Objective To confirm a case of pachydermoperiostosis (primary hypertrophic osteoarthropathy) at the molecular level by gene sequencing.Methods Peripheral blood samples were obtained from a 26-year-old male patient with pachydermoperiostosis and his parents,and DNA was extracted from these blood samples.Polymerase chain reaction (PCR) was performed to amplify all the exons of HPGD and SLCO2A1 genes,and gene sequencing to identify gene mutations.According to sequencing results,the spatial structure of relevant proteins was predicted.Results Gene sequencing showed a homozygous frame-shifting mutation c.310_31 1delCT (p.L104AfsX3) in exon 3 of the HPGD gene in the patient.His mother was a heterozygous carrier of the mutation,but no mutation was identified in his father.The prediction of spacial structure of proteins revealed that the above gene mutation could shorten the length of the encoded peptide by about 60%.Conclusion Typical clinical manifestations and imaging findings are helpful for the primary diagnosis of pachydermoperiostosis,while mutation analysis of HPGD and SLCO2A1 genes is a main approach to its final diagnosis.
7.Effect of age on the association between normal thyroid hormone level and coronary artery disease

Objective To investigate the effect of age on the association between normal thyroid hormone level and coronary artery disease (CAD).Methods A total of 1163 euthyroid patients undergoing coronary angiography (CAG) from January 2013 to June 2014 were enrolled and categorized into 2 groups:the young and middle-aged group (<60 years,n=602) and the elderly group (≥60 years,n=561),and each group was sub grouped into the CAD and non-CAD group according to CAG.Height,body weight,and levels of serum lipid,fasting blood glucose,glycosylated hemoglobin,free triiodothyronine (FT3),free thyroxine (FT4) and thyroid-stimulating hormone (TSH) were measured.Results In the youth and middle-aged group,309 patients (51.3%) were diagnosed as CAD,and there was no significant difference in the levels of FT3,FT4 and TSH between CAD and non-CAD patients [(5.3±0.6)mmol/L vs.(5.3±0.5)mmol/L,(11.8 ±1.9)mmol/L vs.(11.8±1.8) mmol/L,(2.0±1.1) mIU/L vs.(2.0±1.0)mIU/L,all P>0.05].In the elderly group,357 patients (63.6%) were diagnosed as CAD,and the FT3 level was lower in the CAD patients than in the non-CAD patients[(5.1±0.6)mmol/L vs.(5.2±0.5) mmol/L,P<0.01].Logistic regression demonstrated that FT3 was an independent influencing factor for CAD in elderly patients (OR=0.564,P<0.01).Conclusions The association between normal thyroid hormones levels and CAD varies along with age.Thyroid hormones have no relationships with CAD in the young and middle-aged people.The decline of FT3 level may increase the risk of CAD in the elderly.
8.Dexamethasone thyroid follicular cell proliferation and apoptosis in mice and its mechanisms

Objective:To discuss dexamethasone on proliferation of mouse thyroid follicular cells and apoptosis. Methods:Taken BALB/c mice thyroid tissue to trypsin+Ⅱcollagenase digestion organizations get thyroid follicular cells,and expression of thyro-globulin determined whether or not the target cell. Then different concentrations of dexamethasone to stimulate target cells,and the use of MTT,flow cytometry cell proliferation rate,apoptosis rate and the apoptosis-related gene expression analysis. Results: Trypsin joint type Ⅱ collagenase treatment of thyroid tissue to obtain a stable passage of thyroid follicular epithelial cells,and cells stably expressing thyroglobulin. At the same time, different concentrations of dexamethasone on cell proliferation difference was statistically significant (F=8. 544, P<0. 05 ), and the suppression of drug action have interaction ( F = 4. 532, P<0. 05 );in addition, differently dexamethasone concentration 10-6 mol/L, 10-5 mol/L, 10-4 mol/L, the apoptosis rates were 13. 39% ± 0. 79%, 17. 43% ± 1. 38%, 26. 42%±1. 74%,both with 0 mol/L to plug betamethasone 4. 51%± 0. 06% apoptosis rate differences were statistically significant (P<0. 05,P<0. 01),while the difference in the expression of apoptotic genes trend still showed a dose-dependent manner. Conclusion:Dexamethasone can effectively inhibit thyroid follicular cell proliferation and induce apoptosis through a variety of apoptotic pathways.

10.Awareness of nutrition knowledge and effects of nutrition management among colorectal cancer patients

Objective? To investigate the awareness of nutrition knowledge among colorectal cancer patients and to explore the effects of nutrition management on patients' nutrition. Methods? From January 2016 to December 2016, we selected 80 colorectal cancer inpatients of internal and surgical department of oncology in Xuzhou Central Hospital as subjects by convenience sampling. The self-designed nutritional knowledge questionnaire for colorectal cancer patients was used to investigate their awareness of nutrition knowledge. All of the patients were divided into observation group (n=40) and control group (n=40) with the random number table. Control group received routine nutrition guidance. Observation group carried out whole-course nutrition management. And then, the nutrition of patients in two groups was compared before and after intervention. Results? The awareness rate on nutrition knowledge of colorectal cancer patients was 60.0% (48/80). There were statistical differences in the awareness rate on nutrition knowledge among patients with different ages, education, family history of colorectal cancer, whether it was the first case and whether they received nutrition education (P< 0.05). Patients' total serum protein, serum albumin and hemoglobin of observation group were higher than those of control group; the score of nutrition risk screening-2002 (NRS 2002) of observation group was higher than that of control group; the differences were all statistical (P<0.05). Conclusions? The awareness rate on nutrition knowledge of colorectal cancer patients is low. Strengthening nutrition management for patients will contribute to improving patients' nutrition and nutrition risk.
