1.Effect of epigenetic modification of maspin on extravillous trophoblastic function.

This study investigated the effect of epigenetic modification of maspin on extravillous trophoblastic function. The mRNA expression of maspin in placentae from normotensive and preeclamptic pregnant women was detected by RT-PCR. TEV-1 cells, a human first-trimester extravillous trophoblast cell line, were cultured and treated with CoCl(2) (300 μmol/L) to induce chemical hypoxia and with 5-aza (500 nmol/L) to induce demethylation. The mRNA expression of maspin in TEV-1 cells subjected to different treatments was determined by RT-PCR, and the proliferative and migratory abilities of TEV-1 cells were assessed by cell counting kit-8 (CCK-8) and Transwell assays. Our results showed that the maspin mRNA expression level in placentae from preeclamptic women was much higher than that from normotensive women. CoCl(2) or 5-aza could up-regulate the mRNA expression of maspin and significantly suppress the proliferation and migration of TEV-1 cells. It was concluded that the epigenetic modification in promoter region of maspin contributes to incomplete trophoblast invasion, which offers a novel approach for predicting and treating placental dysfunction.
2.The application effect of process management in critical patients with PICC

Objective To explore the application effect of process management in critical patients with PICC.Methods A total of 160 critical patients with PICC were divided into experi-mental group (80 cases)and control group (80 cases).The experimental group applied PICC pro-cess management and patients in the control group were given routine nursing.The one-time suc-cess rate of catheter indwelling,incidence of catheter related thrombosis and mechanical phlebitis on the 60th day after placement,abnormal catheter removal,satisfaction rate of nursing were ob-served and recorded.Results The significant differences were found in two groups in one-time success rate of catheter,incidence of catheter related thrombosis and mechanical phlebitis,abnor-mal catheter removal and satisfaction rate of nursing.Conclusion The application of process man-agement in critical patients with PICC can promote one-time success rate of catheter indwelling and satisfaction of nursing,lower incidence of catheter related thrombosis,mechanical phlebitis and abnormal catheter removal.

4.Effect of Fructus Corni extract on B7-H6 expression in primary liver cancer of rats

Objective:To investigate the effects of Fructus Corni extract on the B7-H6 expression in primary liver cancer cells of rats. Methods:Sixty SD rats were randomly divided into three groups, namely, model, matrine, and Cornus officinalis. The rat model bear-ing the primary liver cancer was induced by diethylnitrosamine, except for the rats in the control group. The rats in both the matrine and Cornus officinalis groups were fed with matrine and Cornus officinalis. The rats in model groups were fed with 0.9%sodium chlo-ride solution. The number of hepatocellular carcinoma nodules was calculated, and the tumor growth inhibition rate was also calculat-ed. The pathological changes of hepatic tissues in rats of each group were observed by hematoxylin and eosin staining. The expression levels of B7-H6 in these three groups were determined by immunohistochemistry and Western blot. Results:The number of liver nod-ules of the matrine and Fructus Corni group rats was lower than that of the model group (P<0.05). The tumor inhibition rate of the Cor-nus group was significantly higher than that of the matrine group (P<0.05). The tumor growth inhibition rate of the Cornus officinalis group was significantly higher than that of the matrine group (P<0.05). Immunohistochemistry showed that the positive expression of B7-H6 in the Cornus officinalis group and the matrine group was significantly higher than that in the model group (P<0.05), and the positive expression of B7-H6 in the Cornus officinalis group was significantly higher than that in the matrine group (P<0.05). Similarly, the protein expression of B7-H6 in the Cornus officinalis and matrine groups was significantly higher than that in the model group (P<0.05) by Western blot, while the protein expression of B7-H6 in the Cornus officinalis group was significantly higher than that in the matrine group (P<0.05). Conclusion:Fructus Corni extract may inhibit the growth of hepatocellular carcinoma through upregulating the B7-H6 expression.
5.The impact of CYP2A6 polymorphisms on adjuvant S-1 chemotherapy outcomes in pa-tients with curatively resected gastric cancer

Objective:Oral fluoropyrimidine S-1 contains tegafur,gimeracil,and oteracil;among them,tegafur is the major active pre-cursor,which is metabolized to 5-fluorouracil by cytochrome P4502A6(CYP2A6).We examined the associations between CYP2A6 poly-morphisms and the treatment outcomes of adjuvant S-1 in patients with gastric cancer.Methods:Two hundred patients diagnosed with pathological stageⅡ-Ⅲgastric cancer were included in this study,and they received adjuvant S-1(40 mg/m2,bid,days 1-28,ev-ery 6 weeks for eight cycles)after curative surgery.Additionally,we analyzed the wild-type allele(W)(CYP2A6*1)and four variant al-leles(V)(CYP2A6*4,*7,*9,and*10).Results:Two hundred patients were enrolled in this study between November 2007 and July 2013.With a median follow-up of 46.4 months(range:12.5-80.1),the 3-year relapse-free survival(RFS)and overall survival(OS)rates were 83.1%(95% confidence interval(CI),77.7%-88.5%)and 94.8%(95% CI,91.6%-98.0%),respectively.However,RFS differed signifi cantly according to the CYP2A6 genotype.The 3-year RFS rates were 95.9% for W/W,83.1% for W/V,and 72.5% for V/V(P=0.032)gen-otypes.Grades 3 and 4 overall toxicity did not differ according to genotype for any grade(P=0.628 and P=0.227,respectively).Conclu-sions:CYP2A6 genotypes correlate with the outcome of S-1 chemotherapy,wherein patients with the variant genotypes show worse prognosis.Additionally,polymorphism detection may be used as a biomarker to guide clinical chemotherapy choices for adjuvant ad-ministration of gastric cancer therapy.
6.Efficacy, safety and common adverse reactions of Apatinib monotherapy in elderly patients with advanced colorectal cancer failing standard regimens

Objective:To investigate the efficacy, safety and common adverse reactions of Apatinib monotherapy in elderly patients with advanced colorectal cancer(CRC)who failed to respond to standard regimens.Methods:This was a retrospective study.A total of 106 elderly patients with advanced CRC who had failed standard regimens from January 2015 to December 2019 were included.Patients enrolled in this study received Apatinib with an initial dosage of 500 mg or 250 mg.The objective response rate(ORR)and disease control rate(DCR)were assessed after treatment with apatinib.The progression-free survival(PFS)and overall survival(OS)were evaluated during the follow-up period.Additionally, adverse reactions during treatment with apatinib were recorded.Results:The efficacy was assessed by using the best overall response during apatinib treatment.Of 106 patients, there were 9 patients with partial response(PR), 68 patients with stable disease(SD)and 29 patients with progressive disease(PD). The ORR was 8.5% and the DCR was 72.6%.The median PFS was 3.6 months and the median OS was 10.1 months.Relatively common adverse reactions in these patients were hypertension(63 patients, 59.4%)and hand-foot syndrome(HFS)(51 patients, 48.1%)during apatinib treatment.The median PFS of patients with hypertension and of patients without hypertension were 5.0 months and 3.0 months, respectively( P=0.008). The median PFS of patients with and without HFS were 5.4 months and 3.0 months, respectively( P=0.013). Conclusions:Preliminary evidence suggests that Apatinib monotherapy has good efficacy and safety in elderly patients with advanced CRC who have failed standard regimens, and patients with adverse reactions such as hypertension and HSF still have a good prognosis.
7.The comparison of retention of diferent post and core systems.

To study the effects of different post design on retention. 27 simulated tooth roots made of PMMA were divided into three groups, then manufactured the cement-expanded composite screw post-core system (CECSP), cement screw post-core system (CSP) and cast metal post-core system (CMP). The root and post-core system was tensiled by the speed of 10 mm/min on the Instron 4302 and recorded the retention force. It appeared significantly different between retention force of CECSP and CSP and CAM (P < 0.05). As a result, the retention mechanical properties of CECSP was much better than those of cement screw post-core system and cast metal post-core system. Therefore, it is possible to find a new way for restoration of endodontically treated teeth.

8.Clinical application of atrial ECG guided the three directional valve PICC tip positioning method

Objective To investigate the clinical application effects of atrial electrocardiogram guided the three directional valve PICC tip localization method. Methods After application of instant location technology of arterial electronic monitoring in the experimental group, the patients received X ray examination on chest, and double check the top catheter location of PICC;the control group utilized routine location method to ensure the top catheter position, which inserted the fitted length of tube ( from the puncture position to right chest lock joint then to the third intercostal space) then sent to do X ray examination to ensure the top catheter position. The incidences of location accuracy, operation time and cost were compared between two groups. Results The accuracy of locations were 98. 7% in the experimental group and 91. 0% in the control group (χ2 =8. 01,P<0. 05);the operation time was (16. 29 ± 0. 35) min in the experimental group compared with (13. 58 ± 0. 52) min in the control group (t=7. 12,P<0. 05);the cost was (2 364. 00 ± 0. 52) and (2 375. 00 ± 0. 35) in the experimental and control group (t=0. 98,P>0. 05). Conclusions The position control of atrial electrocardiogram technology was applied to place PICC, which can judge catheter top position promptly and accurately, improve the accuracy rate of PICC catheter positioning effectively, and increase work efficiency.
9.Analysis of ultrasonographic features of pulmonary ultrasound in children′s community acquired pneu-monia

Objective To investigate the lung ultrasonography characteristics of community acquired pneumonia ( CAP) in children and to compare the value of lung ultrasonography in the diagnosis of CAP in children. Methods A total of 173 patients with CAP ( CAP group) were diagnosed by chest radiograph in our hospital,and 64 healthy children were treated with lung ultrasonography as the control group. The sono-graphic features of two groups were compared,and the positive rates of diagnosis of lung ultrasonography and chest radiograph in CAP group were also compared. Results It showed A-line disappearance ( 100%) , B-line presence ( 88. 4%) , pleural line abnormalities ( 98. 3%) , pulmonary consolidation ( 46. 2%) , bronchial sign(42. 8%),pleural effusion(9. 2%),lung sliding signs(97. 7%) in CAP group (P<0. 001). In the con-trol group,except the A-line disappearance,scattered in the B-line (34. 4%),the others were not present. The difference of the two groups was statistically significant (P<0. 001). A-line disappearance,B-line (or even fusion) ,pleural line abnormalities were the main signs of ultrasound in children with CAP, sensitivity and specificity were both 100%. The sensitivity of chest X radiograph was 100% and the specificity was 78. 0%. Conclusion The lung ultrasonic imaging features of CAP in children include A-line disappearance, with varying numbers and lengths of B-line, pleural line abnormalities, consolidation, bronchial signs, pleural effusion and so on. LUS can diagnose CAP accurately and reliably,with high sensitivity and specificity. It is simple and noninvasive, and has the advantage of being detected at any time and avoiding the damage of X-ray. So it is worth clinical application for children with respiratory tract diseases.
