1.Predicting value of ischemia grade on arrhythmia in patients with acute ST-segment elevation myocardial infarction

Objective To evaluate clinical significance of ischemia grade to predict severe arrhythmia in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods A total of 225 patients with STEMI admitting to emergency department were enrolled.All enrolled patients were divided into two groups based on the QRS complex electrocardiogram on admission:grade 1Ⅱ ischemia group(135 cases) and grade Ⅲ ischemia group (90 cases).All patients received thrombolytic therapy.The incidence rate of ST segment resolution (STR) and severe arrhythmia in hospital stay was observed.Results The ST segment elevation (Σ.ST) on admission and 2 h after thrombolysis in grade Ⅲ ischemia group was significantly higher than that in grade Ⅱ ischemia group [(0.84 ± 0.57) mV vs.(0.44 ± 0.35) mV,(0.50 ± 0.23) mV vs.(0.11 ± 0.06) mV] (P < 0.01).The backing rate of ST segment ≥ 50% 2 h after thrombolysis in grade Ⅲ ischemia group was significantly lower than that in grade Ⅱ ischemia group [56.7% (51/90) vs.83.7% (113/135)] (P <0.01).The creatine kinase MB (CK-MB) peak value in grade Ⅲ ischemia group was significantly higher than that in grade Ⅱ ischemia group [(363 ± 105) U/L vs.(212 ± 97) U/L] (P < 0.01).There was no significant difference in the incidence of severe arrhythmia between two groups (P > 0.05).Multiple Logistic regression analysis demonstrated that the independent predictors of severe arrhythmia were duration from symptom to thrombolysis and initial ΣST,whereas grade Ⅲ ischemia remained a strong predictor of severe arrhythmia.Conclusion Grade m ischemia on admission is associated with lower incidence of STR in patients with STEMI after thrombolysis and a strong predictor of severe arrhythmia.
2.The study of atorvastatin combined with hydration to prevent contrast-induced nephropathy

One hundred patients with chronic renal failure(CRF) undergoing contrast diagnosis and/or interventional therapy were randomly divided into two groups: patients in hydration alone group( n = 49 )received intravenous infusion of 0.9% sodium chloride 12 h before injection of contrast media till 12 h after examination (hydration), patients in atorvastatin plus hydration group (n = 51 ) received atorvastatin 80mg/d 24 h before examintion for 3 d in addition to hydration.The serum creatinine concentrations were measured before and 48 h after injection of contrast media.In atorvastatin plus hydration group, the incidence rate of contrast-induced nephropathy (CIN) was 8% (4/51), which was significantly lower than that of hydration group (24%,12/49 )(P<0.05).Short-term and large dose administration of atorvastatin plus hydration can decrease the incidence of CIN in patients with chronic renal failure undergoing contrast diagnosis and/or interventional thrapy.
3.Clinical study of warfarin anticoagulation effect in elderly patients based on vitamin K epoxide reductase complex 1 and cytochrome P450 2C9 genetic polymorphisms

Objective To investigate the efficacy and safety of warfarin anticoagulation in Chinese elderly patients based on vitamin K epoxide reductase complex 1 (VKORC1) and cytochrome P450 2C9 (CYP2C9) genetic polymorphisms.Methods Clinical data of 41 elderly patients with initial anticoagulation therapy in our emergency department and respiratory department were collected.Patients were divided into observation group (n=20,patients treated with warfarin based on genetic polymorphisms) and control group (n =21,patients treated based on clinical experience).The international normalized ratio (INR),the time of INR stabilized within target range (2.0-3.0) and the incidence of bleeding episodes in 6-month follow up were compared between groups.Results INR within target range at day 3,4,5 and 7 were 0.0%,42.1%,52.6%,68.4% in observation group and 0.0%,10.0%,25.0%,35.0% in control group,respectively.There were significant differences in INR within target range at day 4,7 between the two groups (both P<0.05),while no significant difference was found in INR within target range at day 5 (P>0.05).The time of INR stabilized within target range was shorter in observation group than in control group [(9.5±2.4) d vs.(12.3± 4.8) d,P<0.05].Bleeding complication occurred in 3 patients in observation group and 5 patients in control group,and there was no significant difference between the two groups.Conclusions Warfarin therapy based on VKORC1 and CYP2C9 gene polymorphisms may shorten the time of first INR reaching the target value and INR within target range in elderly patients.However,the risk of bleeding complications should be alerted.
4.Application of MR fast scanning techniques in fetal congenital diaphragmatic hernia

Objective To investigate the diagnostic value of MR fast scanning techniques in fetal congenital diaphragmatic hernia. Methods 12 cases diagnosed or suspected fetal congenital diaphragmatic hernia by ultrasound with gestational age (>20 weeks) were underwent MR examination.The fast scan sequences were as follows:Half-Fourier acquisition single-shot fast spinecho se-quence(HASTE)、True fast imaging steady-state precession sequence (True FISP)、Turbo FLASH T1 WI sequence.The axial,co-ronal and sagittal scanning were focused on fetal head and body.The results of prenatal MR examination was compared with neonatal surgical findings or autopsy results after inducing labor.Results All of 12 cases,9 cases were left diaphragmatic hernia,2 cases were right diaphragmatic hernia and 1 case was hiatal hernia.2 cases of them were complicated with other congenital malformations. The features of MRI were as follows:Some portions of bowels and stomach crossed into the thorax were visible in the HASTE and TRUE FISP sequence.The herniated stomach and bowels showed high or scattered high signals.The intestinal types were visible as well.The lung tissue intruded at the apex of the lung and the heart deviated to the opposite side were demonstrated.T1 weighted im-ages obtained from ultra-fast FLASH (Turbo FLASH)T1WI sequence showed characteristic high signals in the meconium,which delineated clearly the shape of the colon and small intestine.The prenatal MRI results of all cases were consistent with surgical (10 cases)and autopsy (2 cases)findings.Conclusion MR fast scanning techniques can clearly show fetal congenital diaphragmatic her-nia.Multiple sequences scanning can comprehensively map the organizational structure and anatomical relationship of fetal congenital diaphragmatic hernia with the surrounding tissues.MR fast scanning techniques play an important role in the diagnosis of fetal con-genital diaphragmatic hernia.
5.Peripheral blood immune cells in long-term survival patients after liver transplantation

Objective To explore the features of peripheral blood immune cells in long-term survival recipients after liver transplantation.Methods The expression of T subsets (Th1,Th2,Th17,Th22,Tregs),NK cells,NKt cells,Bregs,MDSC in long-term survival recipients (postoperative follow-up time ≥5 years,30 cases),short-term survival recipients(postoperative follow-up time ≤1 year,15 cases) and healthy control (15 cases) were determined by flowcytometry.Results Th17 cells were significantly higher in the long-term group compared with short-term group and healthy control group(P <0.01).Tregs in long-term group compared with short-term group were significantly higher (P < 0.01),but the difference was not statistically significant compared with healthy control group (P > 0.05).NK cells were significantly higher in long-term group compared with short-term group and healthy control group (P < 0.01).MDSC were significantly higher in long-term group compared with short-term group and healthy control group (P <0.01).Conclusions Th17,Tregs,NK cells and MDSC were significantly higher in long-term survival of liver recipients,which may be related to immune tolerance.
6.Clinical application of nurse-led catheter extraction assessment model in children

Objective:To evaluate the application effect of the nurse-led catheter extraction assessment model for children in PICU.Methods:From January to May 2020, 100 children with short-term catheter in PICU of Guangzhou Women and Children Medical Center were selected by convenient sampling method as the experimental group, the need for urethral catheter indwelling was assessed daily using an evidence-based assessment scale in PICU children, and the unnecessary indwelling catheters were removed timely, and 109 children with indwelling urethral catheters from August to December 2019 were collected as the control group, the catheter was removed by the nurse on medical advice, recorded and compared days of indwelling of catheters, the incidence of patients with catheter-associated urinary tract infection , resetting of catheters and the length of stay in ICU between the two groups.Results:The median and interquartile spacing of the days with indwelling catheter were 5.0 (6.0) days in the experimental group and 6.0 (6.0) days in the control group ( Z=-2.01, P<0.05) . In the experimental group, the incidence of catheter-associated urinary tract infection was 1.000 percent (1/100), and in the control group, the incidence of catheter-associated urinary tract infection was 1.835 percent (2/109); in the experimental group, 2 cases of urethral catheter were reset, and in the control group, 2 cases of urethral catheter were reset; the median and interquartile spacing of the length of stay in ICU was 6.5 (7.0) days in the experimental group and 7.0 (8.0) days in the control group. The differences of the above three indexes between the two groups were statistically significant ( χ2=0.26, 0.01, Z=-0.96, all P>0.05). Conclusions:The nurse-led catheter extraction assessment model for children can effectively shorten the catheter indwelling days for children in PICU, which has certain clinical practice significance for reducing the incidence of catheter-associated urinary tract infection.
7.Correlation between immune related adverse reactions in patients treated with pembrolizumab and Traditional Chinese Medicine constitution

Objective To explore the correlation between the immune related adverse(irAEs)reactions in patients treated with pembrolizumab and traditional Chinese Medicine(TCM)constitution.Methods A total of 110 patients diagnosed with non-small cell lung cancer for the first time were selected.When receiving pembrolizumab immunotherapy for the first time,a general information questionnaire,a TCM constitution classification and judgment scale,an immune related adverse reaction follow-up record book,and a patient's self-perception diary were used to investigate and analyze the TCM syndrome and adverse reactions of the patients.Results Among non-small cell lung cancer patients,there were more than four TCM constitutions,with 48 cases(43.64%)having a calm constitution,20 cases(18.18%)having a biased constitution,30 cases(27.27%)having a yang deficiency constitution,and 12 cases(10.91%)having a yin deficiency constitution.Qi deficiency and Yang deficiency were more prone to fatigue,while Yang deficiency was more prone to rash;Qi deficiency and Yin deficiency were more prone to itching;Yang deficiency was more prone to diarrhea;Non-small cell lung cancer patients with mild constitution were less prone to immune related adverse reactions.Conclusion TCM constitution is related to irAEs,which could predict the occurrence of immune related adverse reactions from the perspective of TCM constitution and intervene in adverse reactions early.
8.Status quo and role expectations of clinical teachers for master of nursing specialist in traditional Chinese medicine universities

Objective:To explore the current status of clinical teaching staff for the graduate program of master of nursing specialist (abbreviated to nursing masters) in traditional Chinese medicine (TCM) universities and compare it with the role expectations of students, aiming to provide a reference for the subsequent construction and improvement of clinical teaching staff.Methods:Totally 172 nursing masters from Jiangsu, Zhejiang, Henan provinces and Tianjin and 86 clinical teaching staff from the hospitals where they practiced were selected using convenient samping and investigated with the self-made questionnaire from October to December 2018. The differences between clinical teaching staff's resources, abilities and students' expectations were compared. A total of 258 questionnaires were distributed among 86 clinical teachers and 172 nursing masters, and 258 valid questionnaires were recovered, with an effective rate of 100%. SPSS 22.0 software was used for statistical analysis of data.Results:There was a certain gap between the clinical teaching staff and students' expectations, especially in terms of the graduate school and academic qualifications of the clinical teachers. Totally 34.88% (30/86) of the clinical teachers graduated from TCM universities, while 71.51% (123/172) of the students expected to be taught by teachers graduating from TCM universities. Among the clinical teachers, 60.47% (52/86) were undergraduates, and fewer were masters or graduates with higher education. There was a certain difference between the teaching teachers' abilities and students' expectations. There were statistically significant differences in 17 secondary indicators (χ 2 were 12.027-130.149, P<0.05) , except for the professional attitude in clinical nursing abilities, decision-making abilities in clinical management abilities, communication abilities in interpersonal abilities, and collaboration abilities ( P>0.05) . There were statistically significant differences between students' current status and their learning needs for TCM nursing (χ 2 were 14.722-165.339, P<0.01) . TCM nursing skills and practical application of TCM nursing research were the contents that most students wanted to learn, accounting for 83.72% (144/172) and 70.35% (151/172) , respectively. Conclusions:There were differences between the current status of the teaching staff for nursing masters at TCM universities and the expectations of the students. The universities should fully consider the graduates' own expectations and needs based on the training objectives for graduates, actively explore the admission system for clinical teachers, improve the training system for teachers, and standardize the teaching evaluation mechanism based on the status quo of nursing talents in China.
9.Scoping review of factors influencing chemotherapy-related cognitive impairment in cancer patients

Objective:To provide a scoping review of the assessment tools and influencing factors of chemotherapy-related cognitive impairment (CRCI) in cancer patients and offer insights for developing personalized intervention methods.Methods:Guided by the scoping review methodology proposed by the Joanna Briggs Institute (JBI) in Australia, a comprehensive search was conducted in databases such as CNKI, Wanfang, VIP, SinoMed, PubMed, CINAHL, Embase, Cochrane Library, and Web of Science. The type of literature searched include cross-sectional studies, cohort studies, and case-control studies from the inception of the databases to July 2023, focusing on the factors influencing CRCI in cancer patients. Data extraction, collection, summarization, and reporting of research results were performed for the included studies.Results:A total of 14 studies were included. The influencing factors of CRCI in cancer patients identified were age, gender, fatigue level, anxiety and depression, nutritional status, educational level, number of chemotherapy sessions and exercise.Conclusions:CRCI is a common issue among cancer patients and is influenced by multiple complex factors. Healthcare professionals can develop risk screening tools for CRCI based on these factors, allowing for timely and precise preventive and therapeutic measures for patients at risk, ultimately improving their quality of life and self-efficacy.
10.Surgical techniques of organ procurement from donation after cardiac death

Objective To investigate the surgical techniques and effect of organ procurement from donation after cardiac death (DCD). Methods Clinical data of 64 DCD donors implementing organ donation in the Third Afifliated Hospital of Sun Yat-sen University from July 2012 to December 2014 were retrospectively analyzed. There were 52 males and 12 females, aged 25 d to 57 years with a median age of 31 years. Forty-two donors were with cerebral damage, 18 with cerebral vascular accidents and 4 with brain nervous system diseases. The agreement statement of organ donation signed by the family relatives of all donators were obtained and the local ethical committee approval had been received. Organ procurement was performed by in situ rapid perfusion of abdominal aorta-superior mesenteric vein and rapid retrieval and separation of liver-kidney. Results All the organ procurement operations were performed successfully. The median operative time was 30(22-48) min. Sixty-four livers and 128 kidneys were procured and successfully applied to 58 hepatic, 118 renal and 2 liver-kidney transplantations. Twelve organs including 4 livers and 8 kidneys were discarded. The graft function recovered well after operation except one developed primary graft failure. Conclusion In DCD organ procurement, rapid perfusion and retrieval of liver-kidney play a pivotal role in ensuring high-quality donated organs and enhancing surgical therapeutic effect.
