1.Metabolic Characteristics and Its Effects of Metronidazole and Tinidazole in Human Bile

Objective To investigate the regulation of metabolic characteristics of metronidazole and tinidazole in human bile,and provide a theoretic basis for physician selecting antibiotic rationally when biliary tract was infected.Methods Samples of hman bile were obtained by ERCP plus ENBD,contents of metronidazole and tinidazole were simultaneously measured by HPLC method in the human bile and plasma.Results 0 25 hour after metronidazole and tinidazole were given through iv,contents of two antibiotics had reached the effective bactericida.Concentraction,content of tinidazole in the bile was relatively stable,but its C max was lower as compared with the metronidazole,contents of the two antibiotics in bile were continuously higher than those of the effective bactericidal in 24 hours,and after 16 hours and re-raise was observed in contents of the two antibiotics in bile.Conclusion As maintaining a higher concentration level for long time in the bile by iv,metronidazole and tinidazole were recommended in the patients with an infection in biliary truct.
2.The Evaluation of the Efficacy of Intraperitoneal Chemotherapy on Advanced Stage of Liver Cancer with Tension Ascites

Objective To explore curative effect of intraperitoneal chemotherapy to treat patients with tension ascites caused by advanced stage of liver cancer. Methods 61 patients with tension ascites caused by advanced stage of liver cancer were divided randomly into three groups, patients in group A (n=26) were treated by ultrafiltration ascites and peritoneal reinfusion (A-P) combined with intraperitoneal chemotherapy; patients in B group (n=18) were treated by paracentesis and intravenous albumin infusion combined with intraperitoneal chemotherapy; patients in group C (n=17) were treated by paracentesis and intravenous albumin infusion. Clinical situation, biochemical alteration, sodium metabolism and RAAS metabolism were observed before and after treatment.Results The findings revealed that definite clinical amelioration in all patients. Patients in three groups, biochemical examinations were improved during 24h after treatment, including of endogenous creatinine clearance rate and urine sodium output. But in group B and C, the biochemical examinations returned to the former levels and the levels of serum albumin was markedly reduced during 72h after treatment. The short-term effective rate on postoperative ascites in group A, B, C were 57.7%, 44.4% and 11.9% respectively.Conclusion A-P associated with intraperitoneal chemotherapy is an effective and safe therapy for patients with tension ascites caused by advanced stage of liver cancer.
3.Clinical analysis of 151 cases of controlled premature

Objective To explore the effect of controlled premature delivery and its termination method of pregnancy.Methods The clinical data of 151 cascs of controlled premature delivery were retrospectively analyzed.Results The incidence of premature delivery was 7.5% and the incidence of controlled premature delivery was 44.8%.The cesaran section rate of controlled premature delivery Was 93.4%.Pregnancy induced hypertension(PIH)was the first place reason of controlled premature delivery.The second was antepartum hemorrhage,intrauterine fatal diatress and placenta previa,pregnancy associated with cardiac disease.The complications and mortality of premature infants were reduced if antenatal care is regularly and glucocorticoids could be used.Conclusion Antenatal care and proper treatment can increase the survival rate of premature infants in the inevitable controlled delivery.
4.Cost-effectiveness Analysis of Three Pharmacotherapeutic Schemes for Biliary Tract Infections

OBJECTIVE:To evaluate the3pharmacotherapeutic schemes for biliary tract infections by pharmacoeconomic method.METHODS:Scheme A:cefazolin+metronidazole;Scheme B:ampicillin+gentamicin+metronidazole;Scheme C:lome?floxacin injection+metronidazole.These three schemes were retrospectively analysed with cost-effectiveness analy?sis.RESULTS:Scheme C had the lowest cost-effectiveness ratio with the highest total effective rate.CONCLUSION:Scheme C is the best one.
5.Clinical Study on Deep Needling at Tianshu (ST 25) and Fujie (SP 14) plus Electroacupuncture for Functional Constipation

Objective To observe the clinical efficacy of deep needling at Tianshu (ST 25) and Fujie (SP 14) plus electroacupuncture in treating severe functional constipation (FC).Method Sixty-three patients with severe FC were randomized into a treatment group of 33 cases and a control group of 30 cases. The treatment group was intervened by deep needling at Tianshu (ST 25) and Fujie (SP 14) plus electroacupuncture, while the control group was by sham electroacupuncture. The Complete Spontaneous Bowel Movement (CSBM), Bristol Stool Scale (BSS), and Patient Assessment of Constipation Quality of Life (PAC-QOL) were evaluated before and after intervention.Result In the treatment group, the CSBM after 2-week, 4-week, and 8-week treatments and CSBM of the 4-week and 12-week follow-up study were significantly different from that before intervention (P<0.05). In the control group, the CSBM after 8-week treatments was significantly different from that before intervention (P<0.05). The CSBM after 2-week, 4-week, and 8-week treatments and CSBM of the 4-week and 12-week follow-up study in the treatment group were significantly different from that in the control group (P<0.05). The BSS scores were significantly changed after 2-week, 4-week, and 8-week treatments in both groups compared to that before intervention (P<0.01,P<0.05). The BSS scores after 2-week, 4-week, and 8-week treatments in the treatment group were significantly different from that in the control group (P<0.01,P<0.05). The item scores of PAC-QOL (physical discomfort, psychosocial discomfort, worries and concerns, and satisfaction scores and global score) were significantly changed after intervention in the treatment group (P<0.05). The item scores of PAC-QOLA including physical discomfort, worries and concerns, and satisfaction and global score were significantly changed after intervention in the control group (P<0.05). After intervention, the PAC-QOL scores of the treatment group were significant different from that of the control group (P<0.05).Conclusion Deep needling at Tianshu (ST 25) and Fujie (SP 14) plus electroacupuncture is an effective method in treating severe FC.
6.Expression of calpain and calpastatin in arthritis and antigen epitopes of human calpastatin recognized by rheumatoid arthritis sera

Objective To compare the expression level of calpain and calpastatin mRNA in rheumatoid arthritis (RA) and osteoarthritis (OA) synovial tissues (ST) and analyze the antigen epitopes of human calpastatin recognized by RA sera.Methods Total RNA of three RA and two OA patients was isolated from ST and expression levels of calpain and calpastatin mRNA were detected by reverse transcription polymerase chain reaction (RT PCR).According to hydrophilicity of human calpastatin (Accession,U 31346), overlapping oligopeptides were synthesized by auto spot Robot in N terminal (L domain) and C terminal (Ⅳ domain) of calpastatin.The epitopes of synthesized peptides were analyzed by RA sera using dot ELISA.Results mRNA expression of calpain showed higher than that of calpastatin in both OA and RA.Meanwhile,the mRNA expression level of calpain increased in RA more than that in OA.Three antigen epitopes of human calpastatin (with the sequence DKDLDDALD,DTIPPEYRH and QDPIDALSG) were identified by RA sera,while control sera failed to react with synthetic peptides.Conclusion The calpain calpastatin system may participate in pathogenic mechanism of RA,and the calpastatin may be a target for autoantibody of RA,which implies that it is not only possible to treat RA patients with synthetic peptide of human calpastatin,as a chemotherapeutant,but also a materials for immunodiagnosis of RA.
8.Analysis of effects of recombinant human growth hormone in the treatment of liver cirrhosis with hypoproteinemia

Objective: To investigate the effect of recombinant human growth hormone(rhGH) in the treatment of liver cirrhosis with hypoproteinemia. Methods: 28 patients with liver cirrhosis,whose albumin level was below 35 g/L,were randomly divided into two groups:rhGH treatment group and control group.Serum albumin,ALT and other items were measured 30,60 and 90 days after the start of treatment. Results: After 30 days,an increase in plasma albumin was observed in the two groups,but the change in the treatment group was more obvious than that in control group,An increase in plasma protein in treatment group and the decrease in control group were observed with significant difference between groups. Conclusion: Recombinant human growth hormone can increase the level of ambumin in liver cirrhosis patients and improve the function of liver.
9.Investigation and analysis of status of research and research utilization in practice among nurses of hospital of Traditional Chinese Medicine

Objective To investigate the status of research and research utilization in practice among nurses of hospital of Traditional Chinese Medicine.Methods Two hundred and sixty nurses of hospital of Traditional Chinese Medicine were selected to fill out the questionnaire.Results The rates of involvement of research activities and research utilization among nurses of hospital of Traditional Chinese Medicine were low.The score of every factor ranged from the higher to the lower were communication,organization,research and nurse.Conclusions Involvements of research activities and research utilization among nurses of hospital of Traditional Chinese Medicine are insufficient.Most factors associated with nurses' low extent of research utilization are objective factors.It is pivotal for nursing managers to construct supportive professional environment,improve nurses' research and information skills and the capacity of the research utilization.
10.Observation of Therapeutic Effects of Needle Warming Moxibustion for Chronic Diarrhea

Objective: To observe the therapeutic effects of needle warming moxibustion for chronic diarrhea. Methods: Two-hundred cases with chronic diarrhea were divided randomly into a treatment group and a control group. 100 cases in the treatment group were treated with needle warming moxibustion on specific acupoints. 100 cases in the control group were treated by oral administration of Smecta. Results: The therapeutic results showed cure in 52 cases, remarkable effect in 34 cases, improvement in 14 cases and the total effective rate in 100.0% in the treatment group, and cure in 5 cases, remarkable effect in 23 cases, improvement in 33 cases, failure in 39 cases and the total effective rate in 61.0% in the control group. The comparison of the total effective rate between the two groups was statistically significant (P<0.05). Conclusion: The needle warming moxibustion on specific acupoints is better than oral administration of Smecta in the therapeutic effects.
