1.Influence of systematic prenatal education on outcome of childbirth

Objective To study the effect of systematic prenatal education on the outcome of childbirth. Methods Pregnant women (200 cases) were randomly divided into 2 groups, 100 pregnant women who were willing to attend the maternity club to accept the systematic prenatal education were set as the experimental group, while 100 pregnant women who only accepted the regular prenatal health education as the control group. The experimental group not only received regular prenatal health education, but also had classes about prenatal gymnastics training exercise, the simulant delivery process, Lamaze pain relieving delivery, various postures of 3 delivery processes, the knowledge of husband assistance, the physical and psychological change and relevant coping method during gestation and postpartum, etc. The control group only accepted the regular prenatal health education, which was watching breastfecding VCD, having classes about the basic health knowledge and the physical process of pregnant period, delivery period, and puerperal period, the breasffecding knowledge, etc. Results The cesarean section rate and the total stage of labor of the experimental group was significantly less and shorter than that of the control group(P<0.05 and P<0.01). Conclusions The systematic prenatal e-ducation can improve pregnant women's knowledge of pregnancy and parturition, improve pregnant women's confidence of natural delivery, help to regulate nerve and the function of different organs, shorten the childbirth process, and reduce the cesarean section rate.
2.Evaluation of Sysmex Hematology Analyzers with Fresh Blood

Objective To investigate comparability between Sysmex different hematology analyzers.The consistency of all he-matology analyzers with the same fresh sample.Methods Selected Sysmex XE-2100 as reference instrument which attend external quality assessment.Fresh high,medium and low blood were detected in Sysmex XS-800i,Sysmex XT-4000i and Sys-mex XT-1800i,compared results of WBC,RBC,HGB,HCT,MCV and PLT.Results The bias of XS-800i and XE-2100 was WBC 2.85%,RBC 1.44%,HGB 0.75%,HCT 2.11% and PLT 5.53%.The bias of XT-4000i and XE-2100 was WBC 1.26%,RBC 0.95%,HGB 0.68%,HCT 1.35% and PLT 2.68%.The bias of XT-1800i and XE-2100 was WBC 5.21%, RBC 1.96%,HGB 1.60%,HCT 1.96% and PLT 4.95%.It had good compatability.All various parameters was in the al-lowed range.Conclusion Should maintenance and compare the hematology analyzers periodically,found the problem then calibration instruments in time,ensure the consistency of measurement results between different instruments and guarantee accuracy of the results.
3.Clinical study on the treatment of senile choledocholithiasis with improved approach and bile duct approach of laparoscopic biliary tract exploration

Objective To investigate the improved approach and bile duct approach of laparoscopic biliary tract exploration for the treatment of the elderly patients with common bile duct stones. Methods Ninety-two elderly patients with common bile duct stones were enrolled and randomly divided into the improved group and the conventional group ,with 46 cases in each group. The time of hemorrhage ,the time of common bile duct suture,the recovery time of gastrointestinal function,the time of hospitalization,body temperature,albumin and total bilirubin levels,complications and recurrence of stones were recorded and compared between the two groups. Results The time of hemorrhage,the time of common bile duct suture,the recovery time of gastrointestinal func-tion and the time of hospitalization in the improved group were lower than those in the conventional group(P<0.05, respectively). The body temperature of patients in the improved group was lower than that in the conventional group on 3 days after surgery(P<0.05). The overall incidence of postoperative complications in the improved group was lower than that in the conventional group (P < 0.05). No recurrence of stones was observed in both two groups. Conclusion The improved approach for laparoscopic common bile duct exploration surgery results in less bleed-ing,short common bile duct suture time,rapid postoperative recovery,less body temperature fluctuations and less complications,deserving popularization.
4.Appearance of MRI in Tornwaldt′s cyst (report of 18 cases)

Objective To evaluate Tornwaldt′s cyst using MR imaging. Methods 1 452 cases with head MR imaging were reviewed. The signal intensity, shape, and size of the lesions were observed. Results Tornwaldt′s cyst were found in 18 of 1 452 cases (1 24%), all of the 18 cases were isointense to CSF on T 2WI and hyperintense to muscle on T 1WI.Tornwaldt′s cyst were round ( n =13)and ovoid ( n =5). The size of lesions were from 2 mm to 15 mm. There were 15 cases with adenoid hyperplasia. Conclusions Tornwaldt′s cysts occupied 1 24% in routine head MR imaging and were hyperintense to muscle on T 1WI and isointense to CSF ON T 2WI.
5.Toll like receptor 4-a potential transmembrane receptor contributes to the cardiac remodeling of hypertension

AIM:To observe the expression of Toll like receptor 4 (TLR4) in the left ventricle of Goldblatt rats and to explore the role and mechanism of TLR4 in left ventricular remodeling of hypertension.METHODS:Goldblatt model of Two-kidney,one-clip (2K1 C) renovascular hypertension was induced in twenty-five rats(H group),and twenty rats served the sham-operated group(sham group).The tail cuff blood pressure was detected every week and echocardiogram was observed every other week.After eight weeks of operation,the rats were killed and the samples of the left ventricle were collected.The concentration of Ang Ⅱ in left ventricle was assessed by tadioimmunoassay.Western blotting and RT-PCR were used to exam the mRNA and protein expression of TLR4 in the left ventricle.Immunohistochemistry was adopted to exam the location of TLR4 in the myocardium.RESULTS:TLR4 mRNA and protein expression were consistently upregulated in the left ventricle of H group compared with sham group.In H group, predominantly sarcolemmal staining was observed,especially focal areas of intense TLR4 staining were found in juxtaposed regions of two or more adjacent myocytes;However,in sham group,TLR4 expression was diffuse and presumably cytoplasmic.Considerable correlation was found between blood pressure,MESS,LVMI,RWT,the concentration of Ang Ⅱ in left ventricle and the protein expression of TLR4 in myoeytes.CONCLUSION:During the development of left ventricular remodeling of Goldblatt rats,expression of TLR4 increases significantly.Enhanced expression of TLR4 locates in the sarcolemma,especially in juxtaposed regions of two or more adjacent myocytes.These indicate that TLR4 transmembrane receptor which is closely relative with inflammation and immunity probably contributes to the development of ventricular remodeling.[KEY WORDS]Hypertension;Ventricular remodeling;Receptors,Toll-like;Goldblatt rats
6.Emergency interventional therapy for massive hemobilia

Objective To evaluate angiography and interventional embolization in diagnosis and treatment of massive hemobilia.Methods During 10 years,9 patients with massive hemobilia underwent emergency selective hepatic artery angiography to find the bleeding points,and then embolized the feeding branches.Results All cases demonstrated clearly on angiography about the arterial hemorrhage,and extravasation of contrast-medium.Hemorrhage was stanched immediately after embolization of the feeding branches of the hepatic artery.Conclusion Selective hepatic angiography and interventional embolization for massive hemobilia is a safe and efficient diagnostic and therapeutic method.(J Intervent Radiol,2006,15:213-214)
7.Emergency Interventional Therapy for Ureter Invasion by CACX, Uremia complicated with Hemorrhoea

Objective To discuss the emergency interventional therapy for ureter invasion by CACX, obstructive bilateral hydronephrosis and uremia complicated with hemorrhoea. Methods 5 cases with ureter invasion by CACX, obstructive bilateral hydronephrosis and uremia suffered from hemorrhoea during hemodialysis, who were performed with emergency interventional therapy. Results After therapy, hemostasis were realized in all cases, and all symptoms were alleviated, such as vaginal fluid and fall-swell in pelvis. The short-term total effective rate was 100% . Conclusion Interventional chemoembolization can be used in the treatment of CACX with acute hemorrhoea.
8.Splenic Abscess′s Continuous Drainage Treatment by Percutaneous Puncture

Objective To study the continuous drainage treatment by percutaneous puncture of Splenic Abscess (SA), which is due to double-embolization for treatment of liver cancer with hypersplenism. Methods 158 patients with liver cancer with hypersplenism were adopted as the subjects, of which, 7 ones were complicated with SA. Results The symptoms of the 7 ones were all alleviated, such as fever, abdominal pain, left interior pain. Conclusion Continuous drainage treatment by percutaneous puncture, combined with flushing abscess cavity by antibiotics solution, is very effective for SA due to double-embolization.
9.Identification of Six Trichinella Isolates from China by Multiplex PCR

Objective To identify and classify six isolates of swine-originated Trichinella from China. Methods Five specific pairs of primers were synthesized based on DNA sequence of expansion segment V region and internal tran-scribed spacers (ITS1 and ITS2) of ribosomal DNA repeat from Trichinella. International reference strains of five Trich-inella species [Trichinella spiralis (T1), T. nativa (T2), T. britovi (T3), T. pseudospiralis (T4) and T. nelsoni(T7)] were used as control. Six swine Trichienlla isolates from Henan, Yunnan, Harbin, Tongjiang of Heilongjiang, Hubei and Tianjin were identified by multiplex PCR and its effecting factors of PCR amplification were observed. Results Electrophoresis results of multiplex PCR products of Trichinella larvae showed that the band (173 bp) of the six isolates was the same as T. spiralis(T1). The specific band (173 bp) was detected by multiplex PCR through amplification from issues of single T. spiralis larva, the larvae conserved in 80% ethanol for 6 months, the larvae stored in 10% formaldehyde, in 0.05% formaldehyde, 0.2% sodium azide or 0.05% merthiotate for 2 weeks,or fresh mouse muscle with larvae. Conclusion All the six swine Trichinella isolates are identified as T. spiralis (T1) by multiplex PCR.
10.Analysis of risk factors associated to malnutrition in elderly inpatients

Objective To investigate the risk factors associated to malnutrition in elderly inpatients for supplying clinical evidences for the prevention and therapy of malnutrition. Methods A one-to-one matched case-control study was used to investigate the risk factors associated to malnutrition in elderly inpatients over 60 years old. Results Eight-five pairs were included. Single factor analysis suggested that serum total protein. hemoglobin, serum total cholesterol, non auto -selective food, anorexia, dysphasia, confinement in the bed and infectious diseases might be associated with malnutrition in elderly inpaticnts (all P< 0.05 ). Logistic regression analysis demonstrated that serum total protein, hemoglobin, anorexia, dysphasia and confinement in the bed were the major risk factors (P<0.05). Hosmer-Lemeshow test showed the predicable results and practical results had better similarity (x2 =4. 957, P= 0. 762). Receiver operation characteristic curve ( ROC curve) also revealed the area under curve of serum total protein and hemoglobin were 0 760 and 0.621(P=0.000 and P=0.006 respectiuely), therefore, 56.8 g/L of serum total protein and 108 g/L of hemoglobin might be appropriate reference points for evaluating the malnutrition. Conclusions Serum total protein, hemoglobin, anorexia, dysphasia and confinement in the bed may play crucial roles for malnutrition in elderly inpatients, the status of malnutrition will be improved if the reversible risk factors are amendable.
