1.Postoperative care to patients with pancreaticoduodenectomy combined with vascular allotransplantation

Objective To probe into methods for postoperatively nursing the patients undergoing pancreaticoduodenectomy combined with vascular allotransplantation. Method The clinical data of postoperative care to 2 patients with pancreatic head carcinoma treated with pancreaticoduodenectomy combined with vascular allotransplantation at the First Affiliated Hospital of Sun Yat-sen University between August 2014 and October 2014 were retrospectively analyzed. Results The two patients lived through the operation, with the operation time of 480 mins, volume of intraoperative blood loss volume of 3000 mL and 600 mL, no complications, hospital stay of 42 days and 27 days. Conclusions The patients undergoing pancreaticoduodenectomy combined with vascular allotransplantation are susceptible to postoperative complications. The nursing care including close observation of disease conditions , drainage tubes care, nutritional care and prevention of complications are critical for the success of operation.
2.Research on human epidermal stem cells differentiating into glandular epithelium-like cells in vitro

Objective To investigate the influencing conditions of human epidermal stem cells(ESCs) differentiating into gland-like cells and to identify the induced tubular structure.Methods The ESCs were seeded onto compound polysaccharide shell dermal matrix and collagen gel,adding different concentrations of epidermal growth factor(EGF) and culturing by vertical shaking in vitro,the three dimensional culture and induced directional differentiation were performed.The means of HE staining,immunofluorescence and confocal laser scanning microscope were adopted to observe the conditions,morphology and phenotype change of ESCs directionally differentiating into glandular epithelium-like cells.Results 15-20 ng/mL EGF could induce ESCs in tissue-engineering dermis to grow into dermis and appear the gland-like structure.The HE staining in this structure showed its profile as a single layer with lacune in the middle compartment,eosinophilic cytoplasm,lightly stained.Under CLSM,by CK19 staining,the luminal structure in the middle of the cell mass was observed,while CK18 and CEA were expressed in this structure.Conclusion Under the induction of particular concentration of EGF,the in vitro cultured human ESCs seeded on the tissues engineering dermis could form into the tubular structure,which is similar to the in vivo sweat gland secretory cells in morphology and histology.
3.The establishment of the three-dimensional culture model of the human skin squamous-celled carcinoma and their biological evaluation of the high-invasive squamous carcinoma cells

Objective To establish the three-dimensional culture model of the human skin squamous-celled carcinoma ,and isolate the highly invasive cell subsets ,and compare their biological characteristics .Methods By using the composite chitosan tissue engi-neering skin ,the three-dimensional culture model of the human skin squamous-celled carcinoma was successfully established , through digestion ,we divided and got the high-invasive squamous carcinoma cells subsets .After that ,the different invasive proper-ties of subpopulations of cells in nude mice tumor effects were compared .Results Different concentrations of cinnamaldehyde and interferon on skin squamous-celled carcinoma cell proliferation inhibition had statistical significance (P<0 .01) ,the different invasive characteristics of skin squamous cells carcinoma between subpopulations of cells proliferation inhibition also was statistically signifi-cant(P<0 .01) ,and the two different invasive properties of subpopulations of cells in the tumor characteristics also had statistical significance(P<0 .01) .Conclusion We established the three-dimensional culture model of the human skin squamous-celled carcino-ma successfully ,by using this model ,we could screen the higher invasive cells subsets .
4.Improvement of high-sensitivity mechanical test device for detecting properties of small tension membranaceous biomaterials

5.The inhibitory effect of isoquercitrin onRaf/MEK/ERK signaling pathway in HepG2 cells

Aim To study the inhibitory effect of isoquercitrin on Raf/MEK/ERK signaling pathway in HepG2 cells.Methods MTT was used to detect the proliferation of human liver cancer HepG2 cells after the treatment of isoquercitrin.The morphology and growth of cells were observed under inverted microscope after the different concentrations of isoquercitrin(0, 40, 80, 160, 320 μmol·L-1) to treat HepG2 cells for 24 and 48 h.Cell cycle was assessed by flow cytometry.Ras, Raf, MEK, ERK expression was assayed by Western blot, and mRNA expression was detected by quantitative fluorescence PCR.Results Isoquercitrin could inhibit the growth of HepG2 cells in a concentration-and time-dependent manner.Typical morphological changes of apoptosis were observed by inverted microscopy after HepG2 cells were treated with different concentrations of of isoquercitrin for 24 h or 48 h.The cell cycle assay showed that with the increasing concentration of isoquerditrin, the number of cells that was arrested in G1 phase gradually increased.Compared with the blank group, the expressions of Ras, Raf, MEK, ERK mRNA were down-regulated, and related proteins expression were also down-regulated(P<0.05), and these results had statistical significance.Conclusion Isoquercitrin can induce the apoptosis of HepG2 cells, which may be related to the Raf/MEK/ERK signaling pathway.
6.Expression of miR-339-5 p in diffuse large B-cell lymphoma and its significance

Purpose To investigate the relevance between the expression of miR-339-5p and the clinicopathological characteristics in diffuse large B-cell lymphoma (DLBCL). Methods The level of miR-339-5p expression was detected in 123 cases of diffuse large B-cell lymphoma tissues and 20 cases of reactive lymphoid hyperplasia tissues by chromogenic in situ hybridization ( CISH) technique. The expression of Ki-67 and BCL-6 protein was examined in diffuse large B-cell lymphoma tissues by immunohistochemical technique (IHC) (EnVision two-steps), and the correlation between the expression of miR-339-5p and BCL-6 and the clinicopathological param-eters was also analyzed. Results The positive rates of miR-339-5p were 39. 8% (49/123) in DLBCL tissues, which was significantly lower than that in RH tissues (90%, 18/20). The positive rates of miR-339-5p were 31. 0% (22/71) for germinal center B-cell-like (ABC type) DLBCL, which was significantly lower than that in activated B-cell-like (GCB type) DLBCL (27/52, 51. 9%). The low-er expression of miR-339-5p in DLBCL was correlated with late Ann Arbor staging and high-risk group of international prognostic index (P<0. 05). The survival rates of miR-339-5p negative patients of ABC type and GCB type of DLBCL were significantly lower than that of the positive patients (P<0. 01). The levels of miR-339-5p expression in DLBCL were negatively correlated with the levels of BCL-6 expression in DLBCL (P<0. 01). Conclusion The low expression of miR-339-5p might be relatived with the progression and poor prognosis of DLBCL.
7.An experimental study on dexamethasone combined with puerarin in treatment of acute paraquat-poisoned rats

ObjectiveTo explore the therapeutic effect and mechanism of dexamethasone combined with puerarin for treatment of paraquat (PQ) poisoned rats.Methods Thirty Sprague Dawley (SD) rats were divided by random number table into 5 groups: control, model, dexamethasone, puerarin and combined groups, 6 rats in each group. The PQ poisoned rat model was established by intraperitoneal injection of PQ 25 mg/kg (1 mL), while in the rat of the control group, the same volume of normal saline was injected intraperitoneally. After 2 hours, in the rat of dexamethasone group, 20 mg/kg dexamethasone in 1.5 mL was injected intraperitoneally, and in the rat of puerarin group, 100 mg/kg puerarin in 1.5 mL was injected intraperitoneally. In the rat of combined group, 20 mg/kg dexamethasone combined with 100 mg/kg puerarin in a total amount of 1.5 mL was injected intraperitoneally. In the control group and model group, 1.5 mL of normal saline was injected intraperitoneally. The above mentioned treatments were repeated once more 26 hours later. In 26-50 hours after modeling, urine was collected, and urine N-acetyl-beta-D-amino glycosidase enzymes (NAG) was tested; at the 50th, abdominal aortic blood was collected to test oxygen partial pressure (PaO2). The lung index, kidney index, and the levels of malonaldehyde (MDA), superoxide dismutase (SOD) and myeloperoxidase (MPO) were measured in the left lung and kidney tissue homogenates separately; the right lung and kidney were
harvested to observe pathological changes under light microscope. Another 30 SD rats were treated the same as above measures but no sacrifice to observe 30-day survival rate in each group.Results The lung index, kidney index, NAG in urine, MDA and MPO levels in lung and kidney tissue homogenates in model group were significantly higher than those in the control group [lung index: 9.80±1.83 vs. 4.97±1.14, kidney index: 9.40±1.32 vs. 7.01±0.32, NAG (U·kg-1·day-1): 1.93±0.18 vs. 0.41±0.03, MDA of lung (nmol/mg): 1.04±0.15 vs. 0.28±0.10, MDA of kidney (nmol/mg): 1.39±0.16 vs. 0.66±0.13, MPO of lung (U/g): 1.14±0.08 vs. 0.81±0.06, MPO of kidney (U/g): 0.88±0.08 vs. 0.52±0.12]; while PaO2 [mmHg (1 mmHg = 0.133 kPa): 59.83±4.40 vs. 97.00±2.83] and SOD (U/mg): lung was 27.38±3.48 vs. 86.88±5.88; kidney was 24.18±3.74 vs. 57.86±6.14) were obviously lower than those in control group (allP < 0.05). After drug treatment was given, lung index (7.21±1.87), urine NAG (1.01±0.21) and MDA (lung was 0.49±0.09, kidney was 0.85±0.08), MPO (lung was 0.97±0.07, kidney was 0.68±0.10) in the puerarin group were significantly lower than those in model group, while PaO2 (82.17±5.38), SOD (lung was 68.99±6.51, kidney was 37.90±3.62) were remarkably higher than those in model group (allP < 0.05). The improvement in the indexes related to the kidney injury was not obvious in dexamethasone group and in the dexamethasone combined with puerarin group, while the lung and kidney pathological changes were lesser in extent in each of the above two treatment groups than those in model group. The 30-day survival rate in model group was significantly lower than that in control group (0% vs. 100.0%);the 30-day survival rates in control group, dexamethasone group, puerarin group and combined group were remarkably higher than those in model group (100.0%, 16.7%, 50.0% and 33.3% vs. 0%, allP < 0.05). But there were no statistical significant differences in the survival rates among those treatment groups (P > 0.05).Conclusions Dexamethasone can improve the prognosis of rats with acute paraquat intoxication, it can provide lung protection markedly, but cannot provide significant protective effect on kidney; puerarin has therapeutic effect on rats with acute PQ poisoning, it can provide not only lung protection but also kidney protection. The effect of treatment with dexamethasone combined with puerarin is not superior to that by using dexamethasone or puerarin alone.
8.Comparison of proliferation and differentiation capacity of tissue-engineered skin built by different passages of keratinocytes

Objective To compare the epidermal shape built by different passages of keratinocytes and its ability of proliferation and differentiation in three-dimensional conditions.Methods Different passages of keratinocytes were used to construct tissue-engineered skin.The morphology of the tissue-engineered skin was observed with HE and PAS staining,while CK1/CK10,CK5/CK14,Ki67 were detected by immunohistochemical assays.Results All the tissue-engineered skin had a significant dermoepidermal structure.The stratification of 1st and 2nd passage skins were better,and 2nd passage epidermis was thicker than that in other passages (P<0.05).Dermoepidermal structure in collagen type Ⅳ group binded more tightly,but collagen type Ⅳ had little effect on the thickness of the epidermis (P>0.05).In collagen type Ⅳ group PAS stain was negative,indicating type Ⅳ collagen was unable to promote the reconstruction of BM in vitro.The Ki-67 proliferation index of the 2nd keratinocyte was similar to the normal skin,the remaining passages keratinocyte proliferation gradually decreased (P<0.05) ; the 1st and 2nd passage skins expressed CK1/CK10 and CK5/CK14.Conclusions Keratinocytes before the 3rd passage have a better ability in the proliferation and differentiation,and so they are more suitable as seed cells for tissue-engineered skin.
9.The experience of adult inpatients with obstructive sleep apnea-hypopnea syndrome after surgery: a qualitative research

Objective:To explore the experience of the postoperative adult patients with obstructive sleep apnea-hypopnea syndrome from the patients′ angle and to provide references for improving the postoperative comfort of the patients.Methods:The phenomenology research method was adopted in this study. Thirteen postoperative adult patients with obstructive sleep apnea-hypopnea syndrome from Department of Otolaryngology, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology participated in the in-depth interview with the interview outline. Data were analyzed by using Colaizzi′s phenomenological method.Results:Three level-1 themes and ten level-2 themes were extracted. The physical discomfort included throat discomfort, lacking of enough sleep at night, restricted diet, limitation in speaking. The negative emotion included the sense of worry, helplessness, fidget and regret. The wish of support included the wish of coping strategy and being understanded.Conclusions:The main uncomfortable experience of the patients is the throat discomfort which has the chain reaction and causes the other discomfort. So the nurses should inquire patients initiatively, cooperate with doctors and patients′ family members to relieve patients′ discomfort particularly at night, and then the patients′ quality of life, nurse-patient relationship and patients′ satisfaction may be improved.
10.Analysis of the Implementation Status of the Classification Management Policies for Retail Pharmacies in Guangdong Province Based on Thematic Framework Analysis

OBJECTIVE:To provide assistance for the smooth implementation of classification management policies of retail pharmacies in Guangdong province and the scientific supervision of retail pharmacies by relevant departments. METHODS :In this study,key interviews were conducted among 68 interviewees,involving experts from Guangdong drug regulatory department ,head of retail pharmacies and research experts on relevant policies of universities. The current situation ,problems and suggestions of the classification management policy of retail pharmacies in Guangdong province were summarized. RESULTS & CONCLUSIONS : The implementation of the classification management policies for retail pharmacies in Guangdong province operated well. Drug regulatory authorities at all levels could conduct daily supervision in accordance with policy requirements. Retail pharmacies operated in strict accordance with the classification management policy and established a relatively complete quality management system. However ,there were still problems such as difficulty in policy implementation ,insufficient policy clarity ,increased regulatory pressure and regulatory risks from government regulatory agencies ,rising operating costs and increasing pressure in retail pharmacies,the large gap of licensed pharmacists ,difficult to investigate and deal with “affiliation of certificate ”behavior,and difficult survival for remote pharmacies ,and the impact on the convenience of drug use of the public. It is suggested to improve the content of the policy ,introduce related supporting measures ,strengthen the construction of the supervisory team ,increase policy publicity,improve the ways and methods to investigate and deal with the “affiliation of certificate ”behavior of licensed pharmacists, and help retail pharmacies to diversify their operations and chain operations. It is recommended that relevant government departments should further improve the content of the policy and actively adjust the supervision methods to make the policy better implemented.
