1.Targeting Effect of Curcumin Gelatin Microspheres in Rats in vivo

Objective To observe the targeting effect of curcumin gelatin microsphere in rats in vivo. Methods Injections of curcumin gelatin microsphere and curcumin were injected via tail vein, respectively. HPLC was used to determine the content of curcumin in different organs. The pharmacokinetic parameters were calculated on the basis of compartment models by using DAS 2. 0 program. Targeting efficiency was used to evaluate tissue distribution of curcumin. Results Targeting efficiency of curcumin gelatin microsphere in heart, liver, spleen, lung and kidney was 0. 875, 0. 121, 1. 182, 5. 834 and 0. 896, respectively. Conclusion Curcumine gelatin microspheres can improve lung-targeting efficiency, and benefit for study on lung targeting therapeutic effect.
2.Allergen Specific Serum IgE Antibody in Patients with Intestinal Polyp and Irritable Bowel Syndrome:A Clinical Analysis

Background:Eosinophilic gastroenteritis is the most common gastrointestinal allergic disease,however,allergy also accounts for some gastrointestinal functional disorders. The correlation between allergic factor and intestinal polyp is not clarified. Aims:To investigate the correlation of allergic factor with intestinal polyp and irritable bowel syndrome(IBS). Methods:Ninety-five inpatients including 60 cases of intestinal polyp and 35 cases of IBS admitted from May 2012 to April 2014 were recruited;95 healthy volunteers were served as controls. Serum samples were collected to test specific IgE antibodies derived from 20 common inhalant and alimentary allergens by EUROIMMUN blotting test. Results:The positivity rates of allergen specific serum IgE antibodies in intestinal polyp group,IBS group and both two groups were 56. 67% ,74. 29% and 63. 16% ,respectively,all were significantly higher than that in control group(6. 32% ,P all <0. 05),but no statistically significant difference was seen between intestinal polyp group and IBS group(P > 0. 05). In all the 20 allergens tested,the positivity rates of Dermatophagoides,roach,crab,sea fish,freshwater fish,shrimp,and egg albumin specific serum IgE antibodies were significantly higher in patients group than in control group( P < 0. 05), especially the crab,see fish and Dermatophagoides specific IgE antibodies. Conclusions:Allergic factor might have some association with intestinal polyp and IBS.
3.Improvement of the cholinergic function by melatonin in amnesic rats induced by amyloid ?-peptide 25~35

AIM To investigate whether melatonin improve the learning and memory dysfunction in the amnesic rats induced by amyloid ? peptide 25~35 (A? 25~35 ) via cholinergic nervous system or not.Methods The amnesic model in adult rats was induced by injection of A? 25~35 into hippocampus; Morris water maze was used to determine the effects of A? 25~35 and melatonin on the learning and memory. The activity of the choline acetyltransferase and acetylcholinesterase were determined by immunohistochemistry and spectrophotometry respectively. RESULTS Injection of A? 25~35 20 ?ginto the adult rats hippocampus induced learning and memory dysfunction, and a decrease in the number of ChAT immunoreactive neurons in hippocampus. Melatonin (0 1, 1, and 10 mg?kg -1 , ig?10 d) improved the A? 25~35 treated rats cognitive function and increased the number of ChAT immunoreactive neurons in hippocampus. CONCLUSION Improvement of the cholinergic dysfunction by melatonin in adult rats induced by amyloid ? peptide 25~35 may be via cholinergic nervous system.
4.Pathologic diagnosis and differential diagnosis of bizarre parosteal osteochondromatous proliferation

Objective To study the clinicopathological features and differential diagnosis of bizarre parosteal osteochondromatous proliferation (BPOP). Methods The clinicopathological features of 3 patients with BPOP were analyzed , and related literature was reviewed. Results 3 patients included 2 males and 1 female with an average age 40 years old (24-53 years old). The lesions in three patients were located in the left side of the distal radius, the left side of the little finger inside and the right side of ulna coracoid, respectively. Histologically, characterized by unusual mineralized cartilaginous matrix (`blue bone'), the lesion was composed of a heterogeneous mixture of bone, cartilage and fibrous tissue. Conclusion BPOP is a rare benign surface osteocartilaginous lesion, and its diagnosis and differential diagnosis mainly depend on radiographic and histopathologic features.
5.Common risk factors for daytime sleepiness and hypertension in patients with obstructive sleep apnea syndrome

Objectives:To investigate the common risk factors for excess daytime sleepiness (EDS) and hypertension in obstructive sleep apnea-hypopnea syndrome(OSAHS) patients.Methods:Between January 2020 and February 2021, a total of 103 OSAHS patients diagnosed in the Department of Sleep Medicine Center, the Affiliated Huaian No.1 People′s Hospital of Nanjing Medical University were enrolled as the study population. During polysomnography (PSG) monitoring, noninvasive continuous blood pressure (BP) and heart rate variability (HRV) were monitored simultaneously. Low/high frequency components (LF/HF) were used to reflect sympathetic-vagal balance in frequency domain analysis. According to Epworth Sleepiness Scale (ESS) and BP levels, patients were divided into four groups: simple OSAHS group (ESS<10 scores and BP<140/90 mmHg, n=30)(1 mmHg=0.133 kPa), OSAHS+hypertension group (ESS<10 scores and BP≥140/90 mmHg, n=23), OSAHS+EDS group (ESS≥10 scores and BP<140/90 mmHg, n=26) and OSAHS+hypertension+EDS group (ESS≥10scores and BP≥140/90 mmHg, n=24). The clinical and PSG parameters were analyzed and compared among the four groups. Regression analyses were used to explore the common causative factors for EDS and hypertension. Results:The LF/HF in OSAHS+hypertension+EDS group was significantly higher than the other three groups [3.2% (2.6%, 4.2%) vs 1.4% (1.2%, 1.6%), 2.2% (1.8%, 2.9%), 2.5% (1.6%, 3.1%), all P<0.05]. No difference was observed between OSAHS+hypertension group and OSAHS+EDS group ( P=0.779), but both higher than simple OSAHS group. The linear regression equation showed that LF/HF was most correlated with the percentage of sleep time with oxygen saturation<90% (T90) as compared to the other parameters of sleep disordered breathing (β=0.201, P=0.006). In addition, Pearson correlation analysis showed that LF/HF was significantly correlated with ESS scores and asleep BP levels ( r=0.536, r=0.456, all P<0.05). The logical regression equation showed that LF/HF was a causative risk factor for both EDS and hypertension in OSAHS (β=0.164, 95% CI: 1.018-1.364, P=0.028). Conclusion:The sympathetic-vagal imbalance is a common risk factor for EDS and hypertension in OSAHS patients
6.Effectiveness analysis of self-care behavior intervention for elderly patients with diabetes mellitus in Yizhou rural community

Objective To explore the effectiveness of self-care intervention for elderly patients with diabetes mellitus in rural community. Methods We randomly selected 100 elderly patients with diabetes from January 2013 to December 2013 in rural community health service station in Yizhou, Guangxi Zhuang Autonomous Region. Individualized intervention of diabetes-related knowledge was delivered to patients. Fasting blood glucose and glycated hemoglobin were compared before and after the intervention. The effectiveness of intervention was investigated by diabetes self-care scale. Results Levels of fasting blood glucose [(5. 91 ± 0. 10)mmol/L] and glycated hemoglobin [(4. 62 ± 0. 14)%] from 100 patients were lower than those before the intervention after 1 year intervention (t=11. 840,12. 379;P<0. 01). After intervention, the total score of diabetes self-care scale was improved to (89. 66 ± 1. 56) from (75. 32 ± 1. 50) (t = -7. 597,P <0. 01). Conclusions Blood glucose can be effectively controlled by helping the rural elderly diabetic patients to learn about self-care.
7.Reconstruction of thumb degloving injury with hallux nail flap combined with 3D printing: a report of 16 cases

Objective:To explore the effect of hallux nail flap design assisted with 3D printing in reconstruction of thumb defect in degloving injury.Methods:From January 2020 to March 2022, 16 patients with thumb defect caused by degloving injury with intact digit flexor and extensor tendons were treated. The patients were 11 males and 5 females, aged 20 to 52 years old, with an average age of 31 years old. The injured patient's hands were firstly scanned by CT and the 3D models were created to map the profile of the tissues required for reconstruction. Photopolymer templates for the defects in thumb were then 3D printed. The printed templates were put on the great toes and accordingly the hallux nail flaps were designed and harvested. The degloving wound of thumb was repaired by the hallux nail flap. Donor sites were repaired with artificial dermis in 6 patients and with ilioinguinal flaps in 10 patients. The effect of surgery was observed at outpatient clinic during postoperative follow-up. The survival of the hallux nail flap and the recovery of the donor site were observed. Function recovery were evaluated according to the Evaluation Standard of Finger Replantation and Reconstruction of Hand Surgery Society of Chinese Medical Association.Results:All the harvested hallux nail flaps matched with the profiles of recipient sites. All the hallux nail flaps survived over 4 to 30 months of follow-up, in an average of 16 months. Appearance of all hallux nail flaps was similar to normal thumbs, with good fingerprint and nails. Sensation recovery were S 3-S 4, with TPD at 4-7 mm. According to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 15 thumbs were excellent and 1 was good. Conclusion:Application of 3D printing assisted hallux nail flap transfer in reconstruction of defect of thumb in degloving injury can precisely design and harvest the required tissue and minimise a damage to the donor great toe. It improves the appearance of thumb as well as patient satisfaction. It is practical in reconstruction of the defect of thumb in degloving injury.
