1.Preparation of Breviscapine Microemulsion for Parenteral Injection

Objective To study the prescription and preparation technology of breviscapine microemulsion for parenteral injection,and to evaluate its quality.Methods The prescription was selected and optimized through single-factor test and the pseudo-ternary phase diagram method.The preparation technology was investigated,and the particle diameter,drug content,encapsulation efficiency and haemolyticus were evaluated.Results The prescription composition of breviscapine microemulsion was soybean oil:phospholipid:HS15:PEG400:water=1:0.1:0.55:0.55:6.64 (m/m),with the drug content of 4.01 mg·mL-1,the acquired breviscapine microemulsion exhibited light yellow,uniform and transparent,with average particle diameter of 92.1 nm and encapsulation efficiency of 96.8%.The compatibility test showed no drug precipitation and the preparation was no hemolytic crisis.Conclusion The preparation of breviscapine microemulsion injection is correspond to the main index of parenteral injection.
2.Analysis of clinical characteristics and influencing factors of prognosis in elderly patients with heart failure in ICU

Objective:To investigate the clinical features and influencing factors of prognosis in elderly patients with heart failure(HF)in ICU.Methods:Clinical data of 148 elderly HF patients(≥60 years old)admitted to ICU of our hospital from January to December 2021 were retrospectively analyzed.Disease history,etiology and related ex-amination data were analyzed in these patients,and predisposing factors and clinical features of HF were summa-rized.According to incidence of adverse cardiovascular events within 6 months,they were divided into event group(n=70)and no event group(n=78).General clinical data were compared between two groups.Univariate and multivariate Logistic regression were used to analyze influencing factors of poor prognosis in elderly HF patients in ICU.Results:Predisposing factors of elderly HF mainly included upper respiratory tract infections and pneumonia 34 cases(22.97%),20 cases(13.51%)of rapid atrial fibrillation and fatigue respectively.There were 91 cases(61.49%)with typical cardiac insufficiency as initial symptom;24 cases(16.22%)complicated with neuropsychiatric symptom;44 cases(29.73%)complicated with anxiety and depression of different extent.Univariate and multivariate Logistic regression a-nalysis indicated that age,NYHA class Ⅲ~Ⅳ,Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score,N terminal pro brain natriuretic peptide(NT-proBNP),growth differentiation factor 15(GDF-15)and high sensitive C reactive protein(hsCRP)levels were independent risk factors for poor prognosis in elderly HF patients(OR=1.021~1.393,P<0.05 or<0.01).Conclusion:For elderly HF patients in ICU,clinical manifestations are not typical,and the predisposing factors are complex.It is found that APACHE Ⅱ score,GDF-15,hsCRP and NT-proBNP levels possess important significance for diagnosis and prognosis of these patients.

4.Early reconstruction of the periorbital injuries.

OBJECTIVETo study the clinical characteristics and the treatment of periorbital injuries.
METHODS61 cases were treated, including 30 cases orbitozygomatic fracture, 6 cases of frontal-orbital fracture, 8 cases of naso-ethmoid-orbital fracture, 7 cases of blow -out fracture and 10 cases of complicated fracture. The patients were diagnosed after physical examination and other examination, like CT. Through bicoronal or local mini incision at the end of eyebrow, combined with subciliary incision and local wound approach, the fractured sites were exposed completely. Then the fractured fragments were repositioned and fixed rigidly. The orbital wall was reconstructed with titanium net and Medpor.
RESULTSThe wounds healed primarily. Good cosmetic and functional results achieved in most of the patients. 4 cases underwent second-stage ophthalmectomy. 2 patients had diplopia after operation, but improved gradually. 3 cases of blepharoptosis needed further treatment.
CONCLUSIONSEarly diagnosis and treatment is very important for periorbital injuries. Fracture reposition and orbital wall reconstruction should he performed at early period.

5.Relationship between pharmacokinetics and pharmadynamics of mycophenolic acid in liver transplant recipients

Objective To investigate the relationship between pharmacokinetics and pharmadynamics of mycophenolic acid(MPA)in liver transplant recipients.Methods Plasma MPA concentration were determined EMIT method before MMF dose and after dosing in 20 liver transplant patients received mycophenolate mofetil(MMF).The inhibitory capacity of serum MPA on proliferation of CEM cells was also monitored at 0,1 h and 2 h.Results The rate of proliferation of CEM cells before MMF administration was higher significantly than those at 1 h and 2 h after dosing (P<0.05).The concentrations of MPA at 1 h and 2 h were conversely related with the rate of proliferation of CEM cells(P<0.05).Conclusion After MMF dose,the inhibitory capacity of serum MPA on proliferation of CEM cells was enhanced significantly.This effect was related greatly to MPA concentrations.
6.Relationship between the polymorphisms of UGT1A6 genes and anti-tuberculosis drug induced hepatic-injury.

OBJECTIVETo investigate the relationship between the polymorphisms of UGT1A6 genes and anti-tuberculosis drug induced hepatic-injury (ADIH).
METHODS202 cases and 239 controls were collected and a case-control study was conducted. Information on related risk factors of tuberculosis was collected. The genotypes of UGT1A6-19T/G, UGT1A6-308C/A and UGT1A6-541A/G genetic polymorphisms were detected by polymerase chain reaction and restriction fragment length polymorphism technique (PCR-RFLP) in patients received anti-tuberculosis therapy. The Hha I, Dpn II and Nsi I enzyme were employed. Univariate and multivariate conditional logistic analyses were conducted using SPSS13.0 for windows software.
RESULTS
RESULTSThe allele frequency of gene UGT1A6-19T/T, UGT1A6-19T/G, UGT1A6-19G/G, GT1A6-308C/C, UGT1A6-308C/A, UGT1A6-308A/A, UGT1A6-541AA, UGT1A6-541A/G and UGT1A6-541G/G in ADIH group were 51.5%, 39.6%, 8.9%, 52.0%, 40.6%, 7.4%, 57.9%, 33.7%, 8.4% and 71.1%, 25.5%, 3.3%, 79.1%, 19.2%, 1.7%, 79.5%, 19.2%, 1.3% in control group, respectively. Univariate analysis demonstrated that the frequency of UGT1A6-19T/G, UGT1A6-308C/A and UGT1A6-541A/G genotype in cases were significantly higher than that in controls (P less than 0.05).
CONCLUSIONA positive association is found between UGT1A6 genotype and the occurrence of ADIH. The synergetic effect is proved on susceptibility to pulmonary tuberculosis between UGT1A6 mutant genotypes.

Column chromatographies over silica gel, Sephadex LH-20, reverse phase C18, and MCI, and semi-preparative HPLC were used for separation and purification of constituents from Inula cappa. The 22 compounds were obtained and their strutures were determined by NMR and MS spectra data as nine flavonoids: luteolin (1), apigenin (2), chrysoeriol (3), artemetin (4), 2', 5-di- hydroxy-3, 6, 7, 4', 5'-pentamethoxyflavone (5), chrysosplenol C (6), apigenin-5-0-β-D-glucopyranoside (7), luteolin-3-methyl, luteolin-3-methylether-4'-0-β-D-glucopyranoside (8), luteolin-4'-0-β-D-glucopyranoside (9); four triterpenes: darma-20, 24-dien- 3β-0-acetate (10), darma-20, 24-dien-3β-ol (11), epirfiedelanol (12), friedelin (13); three coumarins: scopoletin (14) , isosco- poletin (15) , scopolin(16) , and other types of compounds stigmasta-5, 22-dien-3β-0-7-one (17), stigmasterol (18), palmitic acid (19), linoleic acid (20), linoleic acid methyl ester (21), (E) -9, 12, 13-trihydroxyoetadee-10-enoie acid (22). Compound 5 is a new natural product. Compounds 3-9, 15, 17, 21, and 22 were isolated from this genus for the first time.

8.Interim report of prospective clinical study of two different digestive tract reconstruction after total gastrectomy.

OBJECTIVETo compare post-operative long-term complications and quality of life of two digestive reconstruction procedures after total gastrectomy.
METHODSA total of 109 gastric cancer patients in Tianjin Medical University Cancer Hospital from March 2012 to February 2013 were prospectively enrolled and randomly divided into functional jejunal interposition (FJI) group (52 cases) and Roux-en-Y (R-Y) group (57 cases). The post-operative complications, nutritional status, and the quality of life were compared between two groups.
RESULTSOne, 3 and 6 months after operation, the incidence of R-S syndrome in FJI group was lower as compared to R-Y group[13% (6/45) vs. 37% (18/49), 3% (1/30) vs. 42% (14/33), 5% (1/21) vs. 48% (11/23), all P<0.01], while 3 months after operation, the incidence of reflux and heartburn in FJI group was higher[53% (16/30) vs. 21% (7/33), P<0.01; 37% (11/30) vs. 12% (4/33), P<0.05]. There were no significant differences in quality of life questionnaire QLQ-C30 between R-Y and FJI groups. QLQ-STO22 stomach module revealed in FJI group, the eating score was better, but reflux score was worse as compared to R-Y group 3 months after operation (all P<0.01).
CONCLUSIONSFunctional jejunal interposition keeps intestinal continuity preserving and food duodenal passing, which is a reasonable digestive reconstruction procedure.

Objective To study the expressions of 1,25 (OH) 2 D3,vitamin D receptor (VDR) and 24-hydroxylase (CYP24A1) and investigate the effects of 1,25 (OH)2D3 and its related molecules in the pathogenesis of Henoch-Sch(o)nlein purpura (HSP).Methods 1.The levels in the plasma 1,25 (OH) 2 D3 of 35 HSP patients and 14 healthy children were detected by enzyme-linked immunosorbent assay (ELISA).2.Total RNA of peripheral blood were extracted and transcribed into cDNA.Sybr green dye based real-time quantitative PCR method was used to compare the expression levels (indicated as 2-△ct value) of VDR and CYP24A1 in patients with HSP and those in the controls.Results The level of 1,25 (OH)2D3 was (13.29 ± 10.12) μg/L in plasma of HSP patients,lower than that of the healthy control group[(29.51-± 23.06) μg/L] (P < 0.01).Compared with healthy control group,the level of VDR mRNA was higher but CYP24A1 mRNA was lower in HSP patients (P < 0.05).Conclusion The patients with HSP have lower ability to synthesize active form of vitamin D and respond to VDR-mediated vitamin D effects,enhancing the ability to degrade this hormone.
