2.The clinical significance of C-peptide for assessing the prognosis of non-proliferative diabetic retinopathy

Background Diabetiretinopathy(DR) icommon ophthalmivasculadisease which can lead to blindness,and itpathogenesiicomplex.C-peptide iby-producin insulin synthesis,and loof studieindicate thaC-peptide iclosely related with diabetimicrovasculacomplications.Objective Thistudy wato explore the clinical significance of the detection of C-peptide to assesthe prognosiof non-proliferative DR(NPDR).Methodretrospective case-observational study wadesigned.The clinical datof 205 casewith NPDwere collected in Wuhan Puren Hospital from January 1,2009 to January 1,2010.The patientwere diagnosed by fundufluorescein angiography (FFA) and the stageof theidiseasewere determined based on the Dstaging developed by the Chinese Ophthalmological Society fundustudy group.Twenty-seven-month follow-up waperformed fothe patientto record the parameterof theiblood biochemistry,changein theiphysical conditions,and changein theiblood pressure and DM.The patientwere assigned to the PDgroup and NPDgroup athe end of the follow-up.The independenrisk factofoNPDwaanalyzed using the univariate analysis,and multivariate logistiregression analysiwaused to evaluate the correlation of varioufactorwith pooprognosiof NPDR.Receiveoperating characteristi(ROC) curve wadrawn to assesthe prediction efficiency of C-peptide fothe prognosiof NPDR.ResultThe clinical datof 205 patientwith NPDwere reviewed,and 15 patientfailed to show up fovisitarate of 7.3%.One hundred and ninety patientwith NPDfinished follow-up with mean age of (41.54± 8.15)years.Of the 190 patients,69 patient(36.3%) advanced to PDR,and 121 patient(63.7%) were NPDduring the follow-up.Significandifferencewere seen in age,hypertension history,smoking history,DM course and C-peptide levelbetween the PDgroup and PDgroup (age:=13.78,P<0.05 ; hypertension history:x2 =21.57,P<0.05 ;smocking history:x2 =26.96,P<0.05;DM course:t=6.89,P<0.05;C-peptide:t=20.50,P<0.05).pooprognosiwacorrelated with age (OR:1.04,95% CI:1.01-1.07),hypertension history (OR:1.05,95% CI:1.02-1.08),smokinghistory (OR:1.07,95%CI:1.04-1.11),DM course (0R:1.18,95%CI:1.13-1.25) oC-peptide conten(0R:1.09,95% CI:1.06-1.12).The areundethe ROcurve wa0.835 (P<0.05,95% CI:0.818-0.843) when the C-peptide waincluded,buwalarge than 0.769 (P < 0.05,95% CI:0.754-0.780) when Cpeptide waexcluded.ConclusionThe levelof blood C-peptide are lowein the PDpatientthan those in the NPDpatients,and the lowelevel of blood C-peptide in the NPDpopulation increasetheirisk of developing PDR.Testing foblood C-peptide level can benefithe prognosiof NPDR.
3.Effect of scraping therapy on Th1/Th2 balance in rats with lumbar disc herniation

Objective: To observe the effect of scraping therapy (ST) on the immune balance of serum helper T (Th) 1/Th2 cells in autologous nucleus pulposus transplantation non-compressive lumbar disc herniation (LDH) model rats, and explore the immune mechanism of ST in reducing LDH pain. Methods: Seventy-two male Sprague-Dawley (SD) rats were randomly divided into a model group, a sham operation group and a ST group, with 24 rats in each group. Autologous nucleus pulposus transplantation non-compressive LDH models were established in the model group and ST group, while the rats in the sham operation group underwent sham operation without model establishment. On the fifth day after the model was successfully prepared, rats in the ST group received ST, once every other day, 3 times as a course for a total of 3 courses. Six rats in each group were randomly selected to observe their pain thresholds, and peripheral blood of the rats was collected before the first scraping treatment and at the end of the first, second, and third courses of treatment. The serum was isolated and enzyme-linked immunosorbent assay (ELISA) was applied for the detection of rat serum interferon (IFN)-γ, interleukin (IL)-4 and IL-10. Results: Compared with the model group, the pain threshold in LDH rats in the ST group increased (P<0.05), the serum IFN-γ level was significantly reduced (P<0.05), but the levels of IL-4 and IL-10 did not change significantly (both P>0.05). At the end of the second and third courses of treatment, the IFN-γ/IL-4 ratio was negatively correlated with the pain threshold in the rats, and the IFN-γ/IL-4 ratio was significantly reduced in the ST group (P<0.01). Conclusion: ST can help suppress the Th1 immunity in LDH rats triggered by the autologous nucleus pulposus, restore the immune balance of Th1/Th2, and reduce the pain of LDH.

6.Preparation, crystallization and preliminary X-ray diffraction analysis of PH1948, predicted RNA methyltransferase from Pyrococcus horikoshii.

RNA methyltransferase is responsible for transferring methyl and resulting in methylation on the bases or ribose ring of RNA, which existed widely but mostly remains an open question. A recombinant protein PH1948 predicting RNA methyltransferase from Pyrococcus horikoshii OT3 has been crystallized. The crystals of selenomethionyl PH1948 belong to space group C2, with unit-cell parameters a=207.0 A, b=43.1 A, c=118.2 A, b=92.1 degrees , and diffract X-rays to 2.2 A resolution. The V(M) value was determined to be 2.8 A3/Da, indicating the presence of four protein molecules in the asymmetric unit.

8.Influence of rehabilitation training on heart rate of stroke patients in the early stage

ObjectiveTo observe the influence of rehabilitation training on heart rate of stroke patients in the early stage.MethodsThe heart rate (HR) of 30 patients within one week after the onset of stroke was evaluated by FUKUDA DS 880A teleelectrocardiograph and the rating of perceived exertion (RPE) was collected during practicing bridging, rolling and moving the arms.ResultsThe HR of 30 patients was only increased 7.57, 6.23 and 6.57 beats per minute during practicing bridging, rolling and moving the arms. RPE of all patients was less than 11.ConclusionAppropriate rehabilitation training is safety and unable to increase the loading of heart in the early stage after the onset of stroke.

In order to provide scientific basics for exploitation and sufficient application of Dendrobium officinale leaves resources, the phenol-sulfuric acid method was applied to determine the polysaccharide content. The monosaccharides were derivated by PMP and the derivatives were identified by HPLC-DAD-ESI-MS(n) and the contents of mannose and glucose were determined simultaneously. Similarity evaluation system for chromatographic fingerprint of traditional Chinese medicine (2004A) was employed to generate the mean chromatogram and similarity analysis of the samples was carried out. The results demonstrated that polysaccharide content, monosaccharide compositions and composition ratio had an obvious difference between stems and leaves. The polysaccharide content of stems was higher than that of leaves. Monosaccharide composition in leaf was significantly different from that in stem. The polysaccharide from stems was composed of mannose and glucose, however the polysaccharide of leaves was acid heteropolysaccharide and was mainly composed of five monosaccharides, including mannose, galacturonic acid, glucose, galactose and arabinose. The similarity value of the 14 batches was above 0.9, indicating that similarity of fingerprints among different samples was high. The study can provide evidence for expanding the medicinal parts of D. officinale.

10.Changes of collagen fiber during reattaching of the musseter muscle following different curved osteotomies of prominent mandibular angle: a study in goats

Objective To investigate the changes of the constitution and its ratio of collagen fiber in the process of masseter reattachment following different osteotomies of the prominent mandibular angle so as to offer guidance for the resection of mandibular angle. Methods Sixteen adult goats were randomized into four groups. In group A we performed unilateral curved osteotomy of the mandibular angle. In group B unilateral curved ostectomy was performed with partial masseter resection. In group C unilateral angle splitting ostectomy, while in group D unilateral dissection of the masseter muscle was conducted. The constitution and its ratio of collagen fiber in the interface were observed at 1-month, 2-month, 3-month, and 6-month after operation. Results On the changes of collagen fiber in the process of muscular reattachment, at 1-month post-operation, the constitution of collagen fiber (types Ⅰ and Ⅲ) in groups A and B were significantly different from that of control group (P<0.05). However, both groups C and D had no statistic difference from control group (P>0.05). At 2-month, 3-month and 6-month post-operation, those of all experimental groups had no statistic difference from control group. And with time, the percentage of collagen fiber type Ⅰ increased and type Ⅲ decreased gradually. Conclusion The recovery sequences of masseter muscle reattachment in this study are firstly group C, secondly group A and finally group B. It suggests that the recoveries of mastication and other oral activities are different. Group B turns out to be with a slow muscle reattachment. Thus, we recommend treating different kinds of mandibular hypertrophy with different ostectomies.
