AIM: To explorethe therapeutic efficacy of treating cataract phacoemulsification remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.
METHODS:Twenty-seven cases ( 27 eyes ) of glaucoma combined cataract surgery(3mo) in the early failure were treated with that the phacoemulsification auxiliary hook into anterior chamber under the original sclera flap, open from the inside have adhesion of the sclera flap. Postoperative follow-up 12mo, the intraocular pressure and best corrected visual acuity were observed.
RESULTS: The postoperative follow- up was 12mo. There were functional filtering bleb in 14(52%) eyes; and the IOP was below 21mmHg in 17 eyes(63%) without any anti- glaucoma drug, and in 7 eyes ( 26%) with 1 - 2 categories of lower intraocular pressure( IOP) drugs, and 3 eyes(11%), failure and the success rate was 89% after 12mo. Average intraocular pressure after 1wk, 1mo, 3mo, 6mo and 12mo were(7. 1±4. 3) mmHg,(10. 5±5. 1) mmHg,(15. 1±4. 8 ) mmHg, ( 16. 8±5. 2 ) mmHg, ( 17. 3±5.1 ) mmHg, significantly lower than the IOP preoperatively(30. 2±6. 8) mmHg (P<0. 01). Average visual acuity ( all of the following were best corrected vision) after 1wk, 1mo, 3mo, 6mo and 12mo was(0. 32±0. 52),(0. 52±0. 42),(0. 55±0. 39),(0. 53±0. 47),(0. 54±0. 42 ). Postoperative 1wk visual acuity deeper than preoperative( 0. 46 ± 0. 44 ) ( P < 0. 05 ), more than all improved compared with preoperative ( P < 0. 05 ). Postoperative anterior chamber bleeding in 5(19%) eyes, all absorbed itself in 3 to 5 days; ciliochoroidal detachment in 7 ( 26%) eyes, except one case need operate, others were conservative treatment to heal. Postoperative continuous low intraocular pressure concurrent macular edema was 1 eyes(4%), 4mo healed after treatment.
CONCLUSION: It is a safe and effective method thattreating phacoemulsification cataract remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.

6.Dynamical observation of the expression of TGF-?_1 and MAPK_(1/3) in the renal tubules of rats with diabetes

AIM: To observe the expression of transforming growth factor ? 1 (TGF-? 1), MAPK 1/3 and fibronectin (FN) in the development of renal tubulointerstitial disease. METHODS: Wistar male rats were randomly divided into normal control group, diabetic group of 1week, 2 weeks, 4 weeks and 8 weeks. Diabetic model was induced by peritoneal injection of streptozotocin. Immunohistochemistry was employed to detect the expression of TGF-? 1, MAPK 1/3 and FN in the kidney. TGF-? 1 protein in the renal cortex was checked by Western blot. BG, Scr and UP were analysed by biochemical methods, and the morphological changes in renal tubulointerstitium were also examined under microscopy on sections stained with HE and PAS. RESULTS: The expression of MAPK 1/3 and FN was observed, but not the expression of TGF-? 1 in normal renal tissue. Positive staining of TGF-? 1 was observed in the renal tubulo-interstitium in 1-week diabetic group and thereafter it increased in the course of diabetes. A continuous increase in the expression of MAPK 1/3 and FN was also observed in two - week diabetic rats. Chronologically the expression of TGF-? 1,MAPK 1/3 and FN and the ratio of KW/BW were positively correlative with each other in diabetic animals except one -week diabetic rats. There was also a positive correlation between MAPK 1/3 and FN in l -week diabetic rats. CONCLUSION: Our data suggest that TGF-? 1 appears in the renal tubulointerstitium in early period of diabetes and then its signal is mediated by MAPK 1/3 cascades to accelerate production of FN ,and in turn leads to renal hypertrophy and tubulointerstitial fibrosis. 7.Clinical and Pathological Analysis of 116 Children with Lupus Nephritis

Objective To analyze the clinical and pathological characteristics of children with lupus nephritis(LN).Methods The data of 116 inpatients from Mar.2000 to Nov.2008 with LN were retrospectively analyzed.The clinical,immunochemical and pathological data were recorded.Renal tissue was observed by light microscopy and electron microscopy after HE,PAS,Masson and PASM staining according to WHO standards.Follow-up results showed complete remission,partial remission,disease activity,renal insufficiency and death.Results Of the 116 cases of LN,there were 27 male and 89 female with a ratio of male to female 1.03.3,and the mean age was(12.0?2.2) years.The incidence of nephrotic syndrome was 63.8 %(74 cases),and acute nephritis was 21.5%(25 cases).Class Ⅳ LN was the most frequent type(14 cases,50%) and classⅢ was next(25 cases,21.5%).In view of the outcome,rapidly progressive glomerulonephritis and class Ⅳ LN were the worst.LN was initially controlled in 96.5% of the patients.Relapses of LN were most caused by the intermittent treatment.Totally 32 cases showed different renal injury,and 6 cases progressed to death.Conclusions Renal biopsy is important to diagnosis,treatment and prognosis evaluation of LN.Long and regular treatment is important for children with LN.

10.Expression of interleukin-32 in peripheral blood of rheumatoid arthritis patients and its clinical significance

Objective To detect the expression levels of interleukin (IL)-32 and tumor necrosis factor (TNF)-α in peripheral blood of rheumatoid arthritis (RA) patients,and analyze the relations between IL-32 and level of erythrocytes (ESR),creactive protein (CRP),rheumatoid factor (RF),the disease activity index 28 (DAS28) scores in RA patients.The aim of this study is to identify the clinical significance of IL-32 in RA patient.Methods We collected 97 specimens of peripheral blood from RA patients,including 36 RA patients with high active disease,33 RA patients with moderate active disease,28 RA patients with stable disease,36 non-RA connective tissue diseases (CTD) patients and 30 healthy volunteers.The mRNA levels of IL-32,TNF-α in the peripheral blood mononuclear cells (PBMC) were determined by quantitative real-time PCR (QRT-PCR).The concentration of IL-32,TNF-αt in plasma were detected by enzyme linked immunosorbent assay (ELISA),one-way AN OVA and Spearman's corrclation and multiple linear regression were adopted for statistical analysis.Results ①The mRNA levels of IL-32 and TNF-α in active RA patients were higher than those in healthy control group (0.29±0.07,0.24±0.06 vs 0.11±0.06 for IL-32 in the high active RA group and moderate active RA group versus the healthy group; and 0.042±0.022,0.012±0.006 vs 0.004±0.002 for TNF-α in the high active RA group and moderate active RA group versus the healthy group,P<0.01),and than those in stable RA patients,and non-RA CTD patients.There was no significant difference among the stable disease groups,healthy control group and non-RA CTD group in the level of mRNA.② The concentration of IL-32,TNF-α in the plasma of RA patients with active disease was higher than that of healthy control group [(107±42) pg/ml,(66±27) pg/ml vs (20±10) pg/ml for IL-32 in the high active RA group and moderate active RA group versus the healthy group; and (261±77) pg/ml,(186±84) pg/ml vs (72±20) pg/ml for TNF-αt in the high active RA group and moderate active RA group versus the healthy group,P<0.01].③ The expression levels of IL-32 was positively correlated with the level of TNF-α,ESR,CRP,RF,DAS28 scores in the plasma of RA patients (r=0.568,0.321,0.185,0.325,0.303 respectively,P<0.01).④ The expression levels of IL-32 was positively correlated with the level of TNF-α (the regression coefficients and P values were b=1.721,P<0.01 and b=0.176,P<0.01 respectively).Conclusion The levels of IL-32 in peripheral blood may be used as an indicator for disease activity of RA.
