1.Clinical evaluation of 7 cases of elderly Kienbock's disease

ObjectiveTo investigate the characteristics of Kienbock's disease in patients older than 60 years of age.MethodsGender, age at onset, affected side, dominant hand, occupation, and clinical results in 7 wrist of 7 elderly patients with Kienbock's disease were investigated.ResultsKienbock's disease in elderly patients commonly was observed in the dominant hand of manual works and was found more often in women than in men. The age at onset was from 60 to 70. Although radiographic examination revealed development of carpal collapse in all patients.ConclusionThe etiologic factors different from conventional ones might have played a role in aged patients with Kienbock's disease, surgical and rehabilitation treatments are good or excellent.
2.One-step methylation variable position analysis technology in single-tube.

OBJECTIVE:
To develop the single-tube one-step methylation variable position (MVP) analysis technology-single-tube post-digestion PCR-melting curve analysis (PDP-MCA).
METHODS:
Based on differentially methylated region (DMR) reported previously as the model, a set of primers with different melting temperatures of products in the two sides of MVP were designed. By using the FastDigest methylation-sensitive restriction enzyme (MSRE), DNA digestion, multiplex amplification, MCA detection and MCA profiles were performed in a single reaction tube. Same samples (peripheral venous blood, semen, and vaginal fluid, 5 samples each type) were tested by single-tube one step MVP and traditional MSRE-PCR MCA technology. To verify the feasibility of this method, the results were compared with that of the traditional technology. The MCA/HRM profiles of different samples were analyzed and compared.
RESULTS:
When the melting temperature of the fragments had a differential of 2 degrees C, the MCA melting peaks separated well, and MCA detection after multiplex amplification was successful. The single-tube PDP-MCA assay was developed, which integrated multiple reactions (digestion, amplification and detection) into one tube. By this method, the sample-specific profiles and data were analyzed in 2 h, which is similar to that of the traditional method. The rapid classifications of the samples were also realized.
CONCLUSION
Multiplex MVPs can be analyzed in a single closed-tube. The single-tube PDP-MCA technology is a simple, fast, and automatable method. It can be used for detection of DNA methylation variations.

3.The rectus abdominis musculoperitoneal composite flap for tongue reconstruction following extensive resection of cancer: a report of 2 cases.

OBJECTIVETo explore the possibility of tongue reconstruction following cancer resection by a rectus abdominis musculoperitoneal composite flap.
METHODSHalf tongue defect was reconstructed using the rectus abdominis musculoperitoneal composite free flap in 2 patients.
RESULTSThe postoperative period was uneventful and complete survival of the flap was occurred in the 2 cases. The reconstructed tongue showed excellent color match and natural shape.
CONCLUSIONSThe rectus abdominis musculoperitoneal composite flap can be used for tongue defect reconstruction, especially when the defect was not more than half of the tongue.

4.Design, synthesis and antalgic activities of aralkyl-ketone-4-piperidol derivatives.

A series of aralkyl-ketone-4-piperidol derivatives were synthesized and tested for their analgesic activities. All of the novel 30 compounds were prepared from 4-piperidone and alpha-halo-aralkyl-ketone through five steps, including Boc protection, nucleophilic addition in presence of CeCl3/NaI catalyst, deprotection, condensation and salification. Their structures were confirmed by 1H NMR and HRMS. Preliminary in vivo pharmacological trials showed that most of the synthesized compounds revealed analgesic effects. Among the tested compounds, 8, 13 and 22 exhibited potent analgesic activities in both mice writhing and mice hot plate model. The three compounds have low affinity for mu, delta, kappa receptors, which is a chance to find a better precursor of non-opioid analgesic for further optimization.

5.Comparison of the Effects of Single Sperm Cryopreservation and Conventional Cryopreservation on Embryo Culture and Pregnancy in Patients Undergoing Micro-TESE

ObjectiveTo compare the effects of single sperm cryopreservation and conventional cryopreservation on embryo culture and pregnancy in patients undergoing microdissection testicular sperm extraction (micro-TESE). MethodsA retrospective analysis was done on the patients who underwent micro-TESE at the Reproductive Medicine Center in the Sixth Affiliated Hospital of Sun Yat-Sen University between January 2018 and December 2021. The single sperm cryopreservation group included 39 patients undergoing single sperm cryopreservation and 307 MII oocytes. The conventional cryopreservation group included 39 patients undergoing conventional cryopreservation and 410 MII oocytes. Propensity score matching (PSM) was performed to balance the selection bias. The fertility rate, embryo culture and pregnancy of these two groups were compared. ResultsThere was no statistical difference in age, body mass index (BMI), years of infertility, basal FSH, basal LH, basal E2, AMH, AFC, number of oocytes retrieved and number of transferred embryos between the two groups (P>0.05). No significant difference was found in fertilization rate (65.8% vs. 60.49%), available embryo rate (67.82% vs. 58.87%) and high-quality embryo rate (70.80% vs. 75.34%). The single sperm cryopreservation group had significantly lower clinical pregnancy rate than conventional cryopreservation group (45.45% vs. 70.0%, P=0.049). ConclusionIf the patients undergoing micro-TESE have very few sperms, single sperm cryopreservation could be an effective choice to increase the utilization of each sperm and ensure the subsequent sperm retrieval after thawing, but the clinical pregnancy rate is decreased. Due to the limited number of cases, we need a large additional number of cases to observe and analyze.
6.Clinical Features and the Mode of Genetic Transmission in Vitiligo Vulgaris

Objective To analyze the clinical features and to explore the potential mode of genetic transmission of vitiligo vulgaris in Chinese Han population. Methods Information on the clinical features and the familial history of patients with vitiligo vulgaris was collected by a uniform questionnaire. The data were inputted into a data base, then analyzed by Epi Info 5.0 and SPSS 10.0 software packages. A complex segregation analysis was conducted using the REGTL program in SAGE 3.1 package in order to propose the putative mode of genetic transmission for vitiligo vulgaris. Results The mean age of onset of vitiligo vulgaris of the males and females was about 19 years old. In the severity of the disease and the season of onset, there was no significant difference between males and females. For the patients with vitiligo vulgaris, there was a female preponderance of complicating with autoimmune disorders, especially hyperthyroidism. The prevalence rates of vitiligo in patients′ relatives were significantly higher than that in general population. The mode of transmission of vitiligo vulgaris was one of the multifactorial inheritance according to the results obtained by the complex segregation analysis. Conclusions There are no differences in the prevalence and the age of onset between males and females patients. There is a marked familial aggregation in the patients with vitiligo vulgaris. The familial clustering might result from the polygenic additive genetic background and common environmental factors. It is suggested that vitiligo vulgaris be a multifactorial complex disease.
7.Longbishu Capsule combined with mesylate doxazosin: an efficacious therapy for benign prostatic hyperplasia.

OBJECTIVETo assess the clinical effect and safety of the Chinese medicine Longbishu Capsule combined with mesylate doxazosin in the treatment of benign prostatic hyperplasia (BPH) of the kidney deficiency and blood stagnation type.
METHODSThis was a randomized, double-blind, double-simulation control study. We equally assigned 60 men diagnosed with BPH of the kidney deficiency and blood stagnation type to an experimental and a control group, the former treated with mesylate doxazosin plus Longbishu Capsule and the latter with mesylate doxazosin plus placebo. We compared the International Prostate Symptom Score (IPSS), quality of life (QOL), Chinese symptom score (CSS), maximal urinary flow rate (Qmax), and prostate volume between the two groups of patients before and after 6 months of medication.
RESULTSAfter treatment, there were 5 cured cases, 13 markedly effective cases, 9 effective cases, 1 ineffective case, and 2 eliminated cases in the experimental group, as compared with 2 cured cases, 8 markedly effective cases, 10 effective cases, 7 ineffective cases, and 3 eliminated cases in the control group. The total effectiveness rate was obviously higher in the former (96.4%) than in the latter (74.1%). IPSS, Qmax, and CSS were improved in both of the groups after medication, even more significantly in the experimental than in the control group (IPSS: 15.22 ± 2.98 vs 18.15 ± 5.88, P <0.05; Qmax: [13.56 ± 2.26] ml/s vs [11.78 ± 2.97] ml/s, P <0.05; CSS: 6.18 ± 2.13 vs 9.52 ± 3.15, P <0.05). Because of the difference in the QOL score between the two groups at the baseline (P = 0.038 <0.05), no more comparison was made in this aspect after treatment.
CONCLUSIONThe combination of Longbishu Capsule with mesylate doxazosin is safe and effective for the treatment of BPH.

8.Blockades of angiotensin and aldosterone reduce osteopontin expression and interstitial fibrosis infiltration in rats with myocardial infarction.

BACKGROUNDIt has been reported that osteopontin has an important role in cardiac fibrosis and remodeling. However, its direct mechanisms remain unclear. The purpose of this study was to investigate the role of angiotensin and aldosterone blockades in cardiac osteopontin expression associated with cardiac remodeling in myocardial infarcted (MI) rats.
METHODSFifty SD rats that survived 24 hours after ligating left anterior descending coronary artery were randomly divided into three groups: MI-saline group (n = 15, 5 ml/d), MI-perindopril group (n = 18, perindopril 2 mgxkg(-1)d(-1)) and MI-spironolacton (n = 17, spironolacton 20 mgxkg(-1)xd(-1)). A sham operation group (n = 15) was selected as non-infarcted control. At 6 weeks after treatment, hemodynamic pararmeters and left ventricular function were measured with catheterization, interstitial fibrosis infiltration and cardiomyocyte diameters were evaluated histologically. Myocardium osteopontin protein expression level in the non-infarcted myocardium was detected by Western blotting.
RESULTSNo osteopontin protein was detected in the myocardium of sham-operation rats. High levels of osteopontin protein expression were detected in the MI-saline rats, but the levels were suppressed in the MI-perindopril and MI-spironolacton rats at 6 weeks following MI (P < 0.01, respectively). Compared with the sham operation group, all rats in the MI group showed marked interstitial fibrosis infiltration in the non-infarction area, higher ventricular weight/body weight ratio, significantly increased cardiomyocyte diameter (P < 0.01, respectively), and developed significant systolic and diastolic dysfunction as indicated by decreased left ventricular systolic pressure (LVSP) and +/-dp/dt, as well as increased left ventricular end-diastolic pressure (LVEDP) (P < 0.01, respectively). Angiotensin and aldosterone blockades partly prevented cardiac fibrosis and systolic and diastolic dysfunction (P < 0.01, respectively).
CONCLUSIONTreatment with angiotensin and aldosterone blockades inhibits expression of osteopontin in the non-infarcted myocardium and prevents cardiac remodeling following MI.

9.Correlation of IPSS with IL-8 and COX-2 levels in patients with benign prostatic hyperplasia and prostatitis.

OBJECTIVETo investigate the correlation of International Prostate Symptom Score (IPSS) with the levels of interleukin-8 (IL-8) and cyclooxygenase-2 (COX-2) in the prostate tissue and expressed prostatic secretion (EPS) in patients with benign prostatic hyperplasia (BPH) complicated by prostatitis.
METHODSWe divided 80 BPH patients to be treated by transurethral resection of the prostate (TURP) into a simple BPH group (n = 30) and a BPH with prostatitis group (n = 50) based on the pathologic features. We statistically analyzed IPSS and the levels of IL-8 and COX-2 in EPS before surgery and the IL-8 and COX-2 levels in the prostate tissue after surgery.
RESULTSIPSS was positively correlated with the IL-8 and COX-2 levels in the prostate tissue and EPS of the BPH patients, moderately in the simple hyperplasia group (r > 0.5) and highly in the other (r > 0.8). The levels of IL-8 and COX-2 in the prostate tissue and EPS were significantly higher in the BPH with prostatitis group than in the simple BPH group (P < 0.05).
CONCLUSIONThe levels of IL-8 and COX-2 in EPS indirectly reflect those in the prostate tissue. IPSS and the levels of IL-8 and COX-2 in EPS can help determine whether BPH is complicated by histological prostatic inflammation.

10.Effects of sirolimus on the growth of transplanted hepatocellular carcinoma.

OBJECTIVETo study the effects of sirolimus (SRL) on the growth of transplanted human hepatocellular carcinoma (HCC) in nude mice.
METHODSHepG2 cells were Implanted into the liver of nude mice. The implanted mice were then treated with SRL and tacrolimus (FK506). The expression of vascular endothelial growth factor (VEGF) and proliferating cell nuclear antigen (PCNA) was detected by immunohistology, microvessel density (MVD) was counted by immunostaining with anti-CD34 antibody for endothelial cells. Tumor apoptosis was detected by terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL) assay.
RESULTSThe tumor weight was (352+/-38) mg, (683+/-53) mg and (675+/-45) mg in SRL, FK506 and control group respectively. The tumor weight was significantly decreased in SRL group (P < 0.01), and there was no difference between FK506 group and control group. The expression of VEGF and PCNA protein was remarkably down-regulated in SRL group compared to control group (P < 0.05), and it was not significantly different between FK506 group and control group (P > 0.05). Compared to the control group, MVD was significanly decreased in SRL group, and the apoptosis index of tumor cell was significantly higher in SRL group (P < 0.01).
CONCLUSIONSRL inhibits transplanted HCC tumor growth by reducing tumor angiogenesis, inhibiting tumor proliferation and inducing tumor apoptosis.
