2.Six Cases of Pulmonary Cavity in Children

Objective To investigate the clinical feature,image of CT scan pulmonary,diagnosis and treatment response in children with pulmonary cavity,and discuss the method of diagnosis and the tactics of treatment for pulmonary cavity in children.Methods A retrospective study of 6 patients with pulmonary cavity,who were diagnosed and treated from Jul. 2003 to Oct. 2009 in Department of Pediatrics of the First Hospital Affiliated to China Medical University.The clinical manifestations,laboratory tests,image of CT scan pulmonary,microbiological evidence,diagnostic procedure and treatment response were collected and evaluated.Results Six patients all didn′t have history of lung di-sease,there were 4 boys and 2 girls,8-15 years old,average age was 10.5 years old.Two cases of them had unrelated pulmonary underlying diseases,1 case had hyperthyroidism,and the other had juvenile idiopathic arthritis and had complication of macrophage activation syndrome,the other 4 cases had no obvious history.All cases had fever (38-40 ℃),3 cases had cough and 1 case had chest pain.Staphylococcus aureus were cultured in 2 cases,no bacteria was cultured in other 4 cases;the count of white blood cell decreased in 2 cases and increased in 4 cases;C-reactive protein increased in 5 cases and was normal in 1 case;plasma IgE level increased in 2 cases and was normal in other 4 cases;plasma 1,3-beta-D-glucan of all 6 cases were negative.Pulmonary cavities were found in the first CT scan of the lungs in 5 cases and only 1 case of patient′s pulmonary cavities was found in the second CT scan of the lung.Five cases were diagnosed infective causes,1 case was diagnosed noninfectious cause,5 cases of infective causes had been treated with anti-microbial drugs for at least 1 week,1 case of noninfectious cause were treated with methylprednisolone cobined cyclosporin A for 2 weeks.Pulmonary CT scan was rechecked in all cases,and the state of the cases were improved before discharged from hospital.Conclusions The causes of pulmonary cavity in children are not only infective factors,but also some non-infective disease,especially some changes of image of pulmonary CT scan has diagnostic value,detailed past medical history and appropriate rechecking of chest radiographic check are very necessary for diagnosis,according to the result of microbial inspection and evaluation of treatment effect in time and then adjust the treatment protocols.
4.Effect of Shuanglong pill on expression of vascular endothelial growth factor(VEGF) and basic fibroblast growth factor(bFGF) on aortic atherosclerotic plaque

ObjectiveTo study the effect of Shuanglong pill on expression of vascular endothelial growth factor(VEGF) and basic fibroblast growth factor(bFGF) on aortic atherosclerotic plaque.MethodsAs atherosclerotic(AS) models of rabbits were made by immune injury plus hyperlipid diet, all rabbits were divided into blank control group(group Ⅰ,6), experimental control group(group Ⅱ, 6), captopril control group(group Ⅲ, 6; 10 mg/kg·d,12 weeks), and Shuanglong pill group(group Ⅳ, 6; 2.52 g/kg·d,12 weeks). Quantitive and qualitative analysis of expression of VEGF and BFGF on aortic atherosclerotic plaque were performed.ResultsExpressive area, density and density index of VEGF and bFGF on aortic atherosclerotic plaques were significantly decreased in group Ⅲ and Ⅳ than in group Ⅱ(P<0.01). There were significant decrease for VEGF expression in group Ⅳ than in group Ⅲ(P<0.01).Pathologic staining showed that there were no foam cells formation and no stained yellow in group Ⅰ, but mass foam cells formation and widespread stained yellow in group Ⅱ. There were a small quantity of foam cells formation and little stained yellow in group Ⅲ and group Ⅳ.ConclusionShuanglong pill can delay/reverse AS progression by inhibiting VEGF and bFGF expression in atherosclerotic plaque as well as captopril.
5.Alteration of Multiple Systems in Offspring After Prenatal Stress

This paper, from the aspects of neuroendocrine, emotion, learning etc, analyzed comprehensively the characteristics of the influence of prenatal stress (PS) on offspring, including intensity of PS, timing of exposure and individual differences. Despite the variety in methodology, most studies on it indicate that the prenatal stressful events lead to offspring's increased plasma glucocorticoid (GC) level, more depressed-related behaviors and impaired learning abilities. Although mechanism of prenatal stress is still unclear, most studies show that it is concerned with hypothalamio-pituitary-adneral (HPA) axis,dopaminergic (DA-ergic) system, neuropeptide Y (NPY)and serotoninergic (5-HT ergic) system. Moreover, further studies are proposed to pay more attention to the relationship and interaction of various related substances.
6.Initial investigation of magnetic resonance perfusion-weighted imaging in leukoaraiosis

Objective To investigate whether perfusion-weighted imaging (PWI) can be used to evaluate blood perfusion of patients with leukoaraiosis (LA), its relationship to clinical features, and the therapy effect of LA. Methods The 44 patients with LA were recruited in the study. All the subjects were examined with both conventional and dynamic susceptibility contrast-enhanced perfusion MR imaging, 13 of them repeated the examinations after the treatment. The shapes and sizes of the biggest lesions in each patient were confirmed by conventional MR imaging, then the values of relative cerebral blood volume (rCBV), relative cerebral blood flow (rCBF) and relative mean transit time (rMTT) in each lesion were calculated respectively. Furtherly, a Pearson's correlation analysis was performed to show whether these values were correlated with mini-mental state examination (MMSE)scores or activities of daily living (ADL) scores. Results The 44 lesions of all the subjects showed significant decreases in rCBV (0. 797 ± 0. 160) and rCBF (0. 779± 0. 164) but increase in rMTT (1. 029±0. 073). There were positive correlations between rCBV and MMSE (r = 0. 524, P =0. 000), between rCBV and ADL (r=0. 621, P=0. 000), between rCBF and MMSE (r=0. 555, P=0. 000), and between rCBF and ADL (r= 0. 690, P= 0. 000), and negative correlations between rMTTand MMSE (r=-0.307, P=0.043), and between rMTT and ADL (r=-0.434, P=0. 003). The blood perfusion was enhanced in 10 out of 13 patients who received the repeated examinations after the treatment, most of whom showed the improvement of clinical symptoms.Conclusions Perfusion MR imaging can assess hemodynamic alterations in LA, which could reflect the clinical symptoms. Moreover, the changes of blood perfusions can be used to evaluate and monitor the therapy effect of LA.
7.Genetic Engineering Reconstruction of Klebsiella pneumoniae Producing 1,3-propanediol by the Gene yqhD Encoding 1,3-propanediol Oxidoreductase Isoenzyme

In the reductive branch, glycerol is first dehydrated to 3-hydroxypropionaldehyde that is then reduced to 1,3-PD under the consumption of reducing power (NADH). If over-expression of the gene dhaB encoding glycerol dehydrase is achieved,the reducing power will be scarce and 3-hydroxypropionaldehyde will be accumulated,which is disadvantage to produce 1,3-propanediol.The structure gene yqhD from E.coli encoding 1,3-propanediol oxidoreductase isoenzyme[under the consumption of reducing power (NADPH)]and the gene dhaB encoding glycerol dehydrase from Klebsiella pneumoniae was amplified using PCR method.The two gene were transferred into expression vector pEtac to construct a novel recombinant Klebsiella pneumoniae (pEtac-dhaB-tac-yqhD).Over-expression of yqhD and dhaB in Klebsiella pneumoniae was achieved with pEtac-dhaB-tac-yqhD.The fermentation result on aerobic conversion showed the increase of 20% of 1,3-propanediol yield by Klebsiella pneumoniae(pEtac-dhaB-tac-yqhD) was obtained compared with Klebsiella pneumoniae.The main by-products,acetic acid and butanediol decreased estrogen receptors 30% obviously.
8.Clinical and pathological analysis of primary biliary cirrhosis-autoimmune hepatitis overlap syndrome

Objective:To explore clinical and histopathological characteristics of primary biliary cirrho-sis-autoimmune hepatitis overlap syndrome.Methods:Clinical data and pathological findings of 10 pa-tients were reviewed.Results:Serum glutamine transpeptidase,alkaline phosphatase levels,alaninetransaminase,aspartate transaminase,serum IgG and IgM were elevated in all the patients.They were allpositive for anti-mitochondrial antibody and AMA-M2.Nine patients were positive for anti-nuclear anti-body and one patient was positive for anti liver-kidney microsome antibody.Liver biopsies in these pa-tients revealed:ten patients had bile duct lesion,hepatitis activities ranged from moderate to severe,andfibrosis ranged from S1 to S3.Conclusion:PBC-AIH overlap syndrome is mostly found in middle-agedwomen.It has the clinical and histopathological characteristics of both PBC and AIH.Accurate andprompt diagnosis of overlap syndrome patients should be based on the clinical presentation,biochemicaland immune indexes,and hepalic pathological changes.
9.Dynamic changes of pulmonary lesions on CT in patients with severe acute respiratory syndrome treated by non-invasivc positive pressure ventilation

Objective To study the dynamic changes of pulmonary lesions of severe acute respiratory syndrome(SARS)treated by non-invasive positive pressure ventilation(NIPPV)on CT. Methods Serial chest CT images in 18 patients with SARS were retrospectively analyzed before and after NIPPV(4—12 days),and a long-term follow-up(3—36 months)examination by CT was carried out.And 27 SARS cases without NIPPV were enrolled as the control group.Results(1)There were dynamic changes in 15 cases of 18 cases:partial multilobar consolidations of bilateral lungs changed into ground-glass opacities in 7 of 18 cases,a circular pulmonary emphysema around bronchial vascular strains of the lower lobes was seen in 5 of 18,subpleural pulmonary emphysema was seen in 8 of 18,pulmonary barotraumas were showed in 2 of 18.Residual of subpleural curvilinear shadow related to NIPPV was found in 2 cases on the follow-up scan.(2)Above changes on CT were not found in 27 cases without NIPPV.Conclusion There were changes of pulmonary lesions of SARS on CT related to NIPPV treatment,It is beneficial to assess the CT images in the NIPPV treatment of the related pneumonia in SARS.
10.Comparative study of EEG and CT in early youth with cerebral cysticercosis

Objective To investigate the diagnostic value of EEG and CT in early youth with cerebral cys- ticercosis.Methods The EEG and CT manifestations were studied in 240 early youth with cerebral cysticercosis.
