1.Low frequency electric stimulation and balloon dilatation therapy for treating dysphagia in nasopharyngeal carcinoma patients after radiation therapy

Objective To study the effect of low frequency electric stimulation and urethral catheter balloon dilatation therapy on dysphagia in nasopharyngeal carcinoma(NPC)patients after radiation therapy. Methods Eighteen dysphagic NPC patients who had received radiation therapy were studied.They included 15 males and 3 females with a mean age of 49.5 years.Thirteen were being fed with a nasogastric tube and 5 with a gastmstomy tube.Low frequency electric stimulation was applied to the swallowing muscles and urethral catheter balloon dilatation therapy was used for the crieopharygeus muscle.Before and after treatment the effects were evaluated with a videofluoroscopic study of swallowing.Results Most swallowing function improved after treatment.After treatment the pharyngeal transit time of paste iodine and fluid iodine(diatriazoate)through the mouth and hypopharynx showed significant improvement.Displacement of the hyoid bone was significantly greater after treatment.Before treatment the incidence of pyriform sinus stasis and aspiration were 100%:after treatment these were 44.4%and 16.6% respectively.Videofluoroscopy showed that 7 cases had been cured(38.9%),9 cases had improved(50.0%)and 2 cases showed no effect(11.1%).In 15 cases the nasogastric or gastrostomy tube could be removed. Conclusion Low frequency electric stimulation and urethral catheter balloon dilatation can improve swallowing in NPC patients with dysphagia after radiation therapy.This treatment was easy to administer,safe and reliable.
2.Expressions of Toll like receptor 3 (TLR3) and TLR9 in lesions of condyloma acuminatum with different prognosis

Objective To investigate the location of TLR3 and TLR9 protein as well as the expressions of TLR3 and TLR9 protein and mRNA in the lesions of condyloma acuminatum (CA) with different prognosis. Methods Immunocytochemical staining with streptavidin-peroxidase (SP) and reverse transcription (RT)-PCR were conducted to detect the expressions of TLR3 and TLR9 in tissue specimens from lesions of 10 patients with recurrent CA, 14 patients with non-recurrent CA as well as from the foreskin of 10 normal human controls. Results TLR3 and TLR9 were mainly distributed in spinous layer and granular layer in non-recurrent CA lesions, but in basal layer and spinous layer in recurrent CA lesions. A significant elevation was observed in the expression of TLR3 mRNA in non-recurrent CA lesions compared with the control tissue and recurrent CA lesions (P < 0.01, 0.05), and in the expression of TLR9 mRNA in non-recurrent CA lesions compared with the control tissue (P < 0.05). No significant difference was observed between the recurrent and non-recurrent CA lesions in the expression of TLR9 mRNA. Conclusions The changes in the distribution and expressions of TLR3 and TLR9 in epidermis may be associated with the prognosis of CA, and TLR3 may exert a greater impact.
3.Correlation analysis on MN-RET from peripheral blood and MN-PCE from bone marrow in mice following exposure to irradiation

objective To study the changes of reticulocyte micronueleus(MN-RET)from peripherai blood and polychromatic erythrocyte mieronucleUS(MN-PCE)from bone marrow in mice following exposure to X-rays in order to provide an experimental basis for exploring possible hish-throughput radiation biodosimeter.Methods Male ICR mice were whole-body irradiated with 0,0.5,1,2,4 and 5 Gy at a dose rate of 0.488 Gy/min.MN-RET from peripheral blood wag scored with FCM and MN-PCE from bone marrow was scored with manual microscopy at 24,48 and 72 h post-irradiation.Results Both MN-RET and MN-PCE rates increaged with doses in the range of 0-5 Gy at 24,48 and 72 h after WBI.The dose-response relationship can be fit with linear equations(t=10.26-25.77,P<0.05).The correlation coeffcients between MN-RET from peripheral blood and MN-PCE from bone mallow were highly significant(r=0.986-0.996,P<0.05).Conclusions In view of its simplicity,accuracy and high throughput capacity,FCM scoring of peripheral blood MN-RET may be a candidate for radiation biodosimetry,More work should be carried out on human specimens to investigate this possibility.
4.MRI Diagnosis and Differential Diagnosis of Multiple Sclerosis in the Spinal Cord

Objective To increase the ability to recognize the characteristic MRI features of multiple sclerosis (MS) in the spinal cord. Methods Fifteen patients with MS underwent MR examination of the cervical spine. The MS plaques were evaluated for lesion location, length,size on cross-section and lesion enhancement. MRI features of intraspinal tumor of spinal cord, cervical spondylopathy of myeloid form and transverse myelitis were analyzed and compared. Results The lesion mainly developed in cervical cord in the 15 patients. The lesions were less than five vertebra body in length. MRI features showed that involved cord segment was mild swollen with smooth periphery in sagittal section;the plaques showed iso-mild hypointense on T 1WI and hyperintense on T 2WI; the plaques were located peripherally and occupied less than 50%of the cross-sectional area of the cord; the lesions in active stage were enhanced by Gd-DTPA or no enhancement, the enhanced size of plaque was smaller than the extent showing hyperintense on T 2WI. The case of several onset showed various enhancement. Conclusion MS of spinal cord has characteristic feature on MRI, this can provide reliable proof for clinical diagnosis.
5.A preliminary study on relevant factors of the metacognition of perimenopausal women

Objective This preliminary study aimed to investigate relevant factors of the metacognition of peri?menopausal women. Methods Total 66 perimenopausal women voluntarily participated in this study from October 2012 to July 2013. The Metacognitions Questionnaire 30-item version (MCQ-30) was used to assess metacognition from 5 di?mensions including cognitive confidence (F1), positive beliefs (F2), cognitive self-consciousness (F3), uncontrollability and danger (F4), and need to control thoughts (F5). Eysenck Personality Questionnaire was utilized to measure the person?ality characteristics such as the extraversion/introversion (E), neuroticism/stability (N), psychoticism/socialization (P), and lie (L). Depression and its 4 symptom components including core, cognitive, anxiety, and somatic symptoms were deter?mined by Zung Self-rating Depression Scale (SDS). The linear multiple stepwise regression were performed to analyze the relevant factors of each MCQ dimension. Results The education level (β’=-0.229, P=0.035), N score (β’=0.255, P=0.042), and L score (β’=-0.292, P=0.021) were related to F1. The education level (β’=-0.260, P=0.031) and N score (β’=0.248, P=0.039) were predictors of the dependent variable F2. The core depression symptom (β’=-0.251, P=0.037) and anxiety symptom (β’=-0.248, P=0.039) of SDS were negatively related to F3. Predictors of F4 were the body mass in?dex (β’=0.211, P=0.048) and L score (β’=0.511, P<0.0001). Only P score (β’=0.299, P=0.015) was related to F5. Con?clusion The metacognition level of perimenopausal women is affected by a variety of factors such as personality character?istics and education level, and low self-consciousness and lack of confidence to the cognitive process may be involved in the increased susceptibility to depression.
6.Clinical characteristics and prognosis of different subtypes ofbreast cancer with bone metastasis

Objective To analyze the clinicopathological characteristics and prognosis of different subtypes of breast cancer patients with bone metastasis.Methods For this study, we recruited 300 primary breast cancer patients with bone metastasis treated at the Department of Oncology, the First Affiliated Hospital of Xi`an Jiaotong University, between September 1, 2007 and September 1, 2011.We also retrospectively analyzed their clinical and follow-up data.Results The percentage of Luminal A, Luminal B, human epidermal growth factor receptor-2 (HER-2) overexpression and triple negative subtypes in all the bone metastatic breast cancer patients was 59.0%, 16.0%, 13.7% and 11.3%, respectively.Age, tumor size and histologic grade significantly differed among the four subtypes (P<0.05).However, there were no significant differences in menopausal status, lymph node metastasis, histological type or lymphovascular invasion among different subtypes (P>0.05).The median survival time of Luminal A breast cancer patients with bone metastasis was 28.6 months, longer than Luminal B (26.9 months), HER-2 overexpression (20.9 months) and triple negative breast cancer patients (12.0 months) with bone metastasis.The overall survival significantly differed among the patients with four subtypes of breast cancer.Conclusion Different subtypes of breast cancer patients with bone metastasis have different clinical characteristics and prognosis.Luminal A breast cancer patients with bone metastasis have better prognosis whereas triple negative subtype has poorer prognosis.
7.Effect of sitagliptin on the expression of NO/iNOS in type 2 diabetic nephropathy

Objective To investigate effect of sitagliptin ( SIT) on the expression of inducible nitric oxide synthase ( iNOS) and nitric oxide ( NO) in type 2 diabetic nephropathy.Methods 30 rats were randomly divided into normal group (NC group), diabetic nephropathy group (DN group) and sitagliptin treatment group (SIT group).The type 2 diabetes mellitus (T2DM) rats were induced by a high fat diet (HFD) plus repeated low dose streptozocin (STZ) injections.At the end of the 12th week in treatment,there were 6 rats in each group, the NO level was determined by Griess method.mRNA levels of iNOS RT-PCR was detect ed by.The expression of iNOS protein was detected by western blot and immunohistochemical method. ResuIts Compared with the NC group, the expression of iNOS and NO of DN group increased significantly.However, compared with DN group, the expression of iNOS and NO of SIT group decreased significantly.ConcIusion SIT can decrease the expression of iNOS and NO, which implies SIT may protect the type 2 diabetic kidney.
8.Diagnosis and treatment of pyelogenic cyst

The clinical data of 32 patients with pyelogenic cyst were reviewed retrospectively. The diagnosis of 25 cases was confirmed by intravenous pyelography (IVP) , in which the ultrasonography and computed tomography ( CT) failed to provide accurate diagnosis. Eighteen cases were treated by surgical operations, among them 16 cases were cured, one case lost follow-up, and one case recurred in 6 months after surgery. The data indicate that the diagnosis of pyelogenic cyst mainly depends on IVP examination and the laparoscopic technique can be effectively applied for treatment of pyelogenic cyst.
9.Endoscopic features and earcinoembryonic antigen expressions of colorectal serrated adenomas

Objective To explore the endoscopic features and careinoembryonic antigen (CEA) ex-pressions of colorectal serrated adenomas (SA). Methods From June 2005 to July 2008, 27 patients with colorectal polyps and 26 cases of advanced colorectal cancer (ACC) were enrolled in the study. The pit pat-tern of the suspected lesions were observed with 0. 4% indigo carmine sodium stain and classified according to Kudo classification. The polyps were removed by biopsy, high-frequency electrical excision or endoscopic mucosa resection and the samples of ACC were collected with biopsy forceps. All specimens underwent rou-tine pathological examination and CEA expression was detected by immunohistochemistry. Results There were 47 SAs and 27 other types of polyps in polyp group. Most SAs located in left-side colon and were char-acterized by the bulge semipedunculation. The diameters of 45 serrated adenomas ranged from 3 to 8ram, and the maximal diameter of other 2 were more than 10ram. The most common type of pit pattern in SA was mixed type Ⅱ and ⅢL, with type Ⅱ predominant in a certain percentage. Expression of CEA in SA glands was significantly higher than that in proliferative polyps (P < 0. O1). Candnsion SA is an unique type of colorectal adenoma, which is capable of expressing CEA, and displays malignant potential and deserves great attention.
10.Diagnosis and treatment of corpus cavernosum penis rupture

The clinical data of 16 patients with rupture of corpus cavernosum penis were retrospectively reviewed.The patients were referred to the Department of Urology from January 2005 to November 2009.The causes of penile injury were trauma due to sexual intercourse ( 12 cases), trauma due to masturbatory (2 cases), traffic accidents ( 1 case) and others ( l case).All patients received immediate surgical treatment; 14 out of 16 patients were followed up for a mean period of 3 months.Sexual function was recovered with satisfactory erection in 11 patients, 3 patients complained pain or uncomfortab]eness during erection.Sexual trauma was the main cause of penile fracture.Immediate intervention for penile fracture can achieve satisfactory early and late results.
