1.Expression of Hepcidin in rats after subarachnoid hemorrhage

Objective To examine the iron content and the expression of hepcidin in early period after subarach-noid hemorrhage (SAH) in rats, and to explore the role of hepcidin in dysregulation of brain iron metabolism after SAH. Methods Totally 90 adult male SD rats were randomly divided into two groups:the sham-operation group and SAH group. The SAH model was established by single blood injection to prechiasmatic cistern. Immunohistochemical and Western Blotting were used to examine the expression of hepcidin at 12, 24, 48 and 72h after SAH. Meanwhile, Atomic Absorption Spectrometer was used to detect the iron content. Results Immunochemistry showed that hepcidin expression in rats in SAH group began to rise at 12 h(0.30±0.06)and gradually increased over time until 72 h(0.56±0.07)compared with the sham group(0.19±0.05). The expression of hepcidin was significantly higher in SAH group than in the sham group(F=31.911, P<0.05). Western blot showed that hepcidin expression in rats in SAH group began to rise at 12h(0.481±0.065) and gradually increased over time until 72h(1.627±0.143)Compared with the sham group(0.238±0.047). The expression of hepcidin was significantly higher in SAH group than in the sham group after SAH(F=147.314,P<0.05). Iron content in SAH group began to rise at 12h after SAH(58.50±9.19)and gradually increased until 72 h(99.34±7.68). The iron con-tents in SAH group were higher at every time points than those in sham group(43.51±4.59)(F=28.799,P﹤0.05). The ex-pression of hepcidin was correlated with the iron content in SAH group(r=0.914,P﹤0.01). Conclusion The increase in iron content following SAH is associated with the increased hepcidin expression.
2.Treatment and prognosis of differentiated invasive thyroid carcinoma

Objective To evaluate the clinical characters, management and prognostic factors of patients with differentiated invasive thyroid carcinoma (DITC). Methods The data were analyzed retrospectively for 114 DITC patients treated at Department of Head and Neck Surgery of Sun Yat-sen University Cancer Center. Survival analysis was performed by Kaplan-Meier method, comparison among/between groups was performed using log-rank test, and multivariate analysis was carried out using Cox proportional hazard model. Results After surgery, 68 patients were with tumor residue. The 5-year and 10-year overall survival rate were 91.9% and 80.1% respectively in all patients, while the 10-year overall survival rate were 88.5% 、78.5% and 53.1% in no tumor residue group, micro-residue group and grossresidue group respectively. This study failed to prove that radiotherapy might improve the survival rate in patients with postoperative tumor residue. Multivariate analysis indicated that age, invasion to esophagus and recurrence predict the prognosis. Conclusion DITC may be treated mainly by surgical operation. Radical resection is the key factor in the treatment of DITC. Patients with DITC have a relatively poor prognosis.Age, esophagus invasion and status of tumor residue are the most important factors affecting the prognosis.
3.Bending strength and stress distribution of the interlocking intramedullary nails with high-thread wooden club-shaped nails versus traditional nails

BACKGROUND:Interlocking intramedullary nailing is a main method for bone fractures,but traditional static intramedullary nails usually lead to nail breakage and loosening.Thereafter,we design a novel high-thread wooden club-shaped screw (HTWCSS) and explore its mechanical properties.OBJECTIVE:To measure the bend strengths of HTWCSS,analyze its stress distribution,and to evaluate its biomechanical properties,thereby providing theoretical basis for its clinical application.METHODS:The bend strength of HTWCSS and traditional nails were measured via three-point bending experiments.Transverse fractures of the middle tibia were simulated using finite element method,and then the force and stress distribution of the two different nails were analyzed.RESULTS AND CONCLUSION:(1) The average maximum load of HTWCSS was 3.52 kN and 1.81 kN at span of 20 mm and 30 mm,respectively,which were larger than those of the traditional nails.(2) The average maximum displacement and the stress of HTWCSS were smaller than those of the traditional screws in finite element analysis,and the stress distribution was relatively dispersed.(3) The average maximum axial displacement and stress HTWCSS,traditional interlocking screws,wooden club-shaped screws and traditional screws were 131 MPa and 3.27 mm,162 MPa and 4.07 mm,26.5 MPa and 0.323 mm,and 34.3 MPa and 0.407 mm,respectively.(4) These results suggest that HTWCSS has relative high bend strength,and it is able to disperse stress and improve fatigue strength bend strength,further reduce screw broken.
4.Surgical treatment of anorectal fistula in patients of acute leukemia

Objective To evaluate treatment and operation timing of anal fistula in acute leukemia patients.Methods Clinical data of 42 acute leukemia patients with anal fistula from 2010 to 2015 in First Hospital of Xinjiang Medical University were evaluated according to the choice of operation time.Patients were divided into two groups undergoing surgery before chemotherapy, (group A).and surgery after chemotherapy (group B).Clinical analysis was performed usingx2 or t test and correlation index.Results Neutrophil count in group B was lower than that in group A (x2 =8.077, P =0.004).Chemotherapy before surgery regime reduces the incidence of infection.Conclusions Chemotherapy before surgery may improve the prognosis of anorectal fistula patients of acut leukemia.
5.Abdominal electroacupuncture for post-stroke constipation:a randomized controlled trial

Objective:To observe the effect of abdominal electroacupuncture(EA)on intestinal function in patients with constipation after stroke. Methods:A total of 100 patients with post-stroke constipation were divided into an observation group and a control group by the random number table method,with 50 cases in each group.The control group was treated with oral lactulose,10 mL each time,3 times a day,for 2 consecutive weeks.The observation group was treated with additional abdominal EA at Tianshu(ST25),Fushe(SP13),and Daheng(SP15)on the basis of oral lactulose,30 min for needle retention each time,once a day,for 2 consecutive weeks.The total effective rate after treatment,constipation symptom score,the score of patient assessment of constipation quality of life scale(PAC-QOL),serum motilin(MTL)level,and the score of mini-mental state examination(MMSE)were compared between the two groups. Results:After 2 weeks of treatment,the total effective rate was 93.5%in the observation group and 76.1%in the control group,and the difference between the two groups was statistically significant(P<0.05).The constipation symptom score,PAC-QOL score,MMSE score,and MTL level changed significantly after treatment in both groups(P<0.05).After treatment,the constipation symptom score and PAC-QOL score in the observation group were lower than those in the control group,while the MMSE score and MLT level were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05). Conclusion:On the basis of oral lactulose,EA at abdominal points can significantly improve the intestinal and cognitive function of stroke patients simultaneously.

7.Preliminary exploration of reserved talents training in thoracic surgery

Based on the Nanshan class of clinical medicine in Guangzhou Medical University, the center established an interest-oriented thoracic surgery learning collaboration group. All recruited students received the full-range cultivation, including "MDT" learning collaboration with characteristics of early clinical practice, early scientific research, English training and thoracic surgery collaboration group as thecore, accelerated clinical skills training under the naked-eye 3D thoracoscopic system, and scientific research thinking which recommended by new media — "WeChat public platform". A total of 10 undergraduate students from 2013 to 2017 batch were recruited into the this group and they showed advantages in clinical technique, scientific research and higher education enrollment after cultivation, demonstrating that part of the clinical teaching and scientific research thinking of thoracic surgery gave to undergraduates in advance can pave the way for the training of thoracic surgeons and formulate more detailed and individualized programs to teach students in accordance with their aptitude in the future.
