1.Endovascular treatment of chronic total occlusions of the iliac vein

Objective To present clinical results of endovascular treatment of total iliac vein occlusions and to discuss the technique details of this treatment. Methods From Feb 2006 to Aug 2010,15 patients with chronic total occlusive lesions of the iliac vein and adjacent vein segments underwent endovascular treatment. Average age was (62 ±7) years (range 35 to 81 years), the male/female ratio was 12: 3. Clinical score of CEAP was grade 3 in 33. 3%, grade 4 in 40%, grade 5 in 13. 3%, and grade 6 in 13.3%. Venography showed left iliac vein was occluded in all 15 patients, common femoral vein was occluded in 14 patients, and superior femoral vein was occluded in 9 patients, however profounder femoral vein was patent in these patients with superior femoral vein being occluded. Results No postoperative major morbidity or mortality was seen. The technique success rate was 93. 3%. Treatment failure was caused in one case for a wrong selection of the femoral vein approach site. The average length of stent was 18. 4 cm.In 12(80. 0% ) stents crossed the inguinal ligament. The average follow-up time was 11.6 ± 2. 4 months.The primary patency rates of the stents at 6 months were 92. 9%. 10 (66. 7% ) patients were asympotomatic, 3(20% )were improved, 1 (6. 7% ) was unchanged, and 1 (6.7%) was worse, compared to before intervention. Conclusions Endovscular recanalization and stent placement is a safe and effective treatment for chronic total occluded iliac veins, with good patency, significant symptom resolution, and minimal morbidity in the short term follow-up.
2.Clinical effect analysis in the treatment of acute limb arterial critical ischemia

Objective To discuss the therapeutic effect in the treatment of the acute limb arterial critical ischemia.Methods Collect thirty-nine cases of acute limb arterial critical ischemia in Renji Hospital from Janary 2014 to July 2016.According to the patients' manifestation,these operations were porfermed including thrombectomy,cathetery-directed thrombolysis,mechanical suction bolt,percutaneous angioplasty and stenting.The effect and complications were observed.Results The eighteen patients in 39 cases (46.2%) were dead,including 5 cases without operation,13 operation.The eight cases were amputated during 34 cases of operations.In the 21 out-patients safely,2 cases were not followed up.The time of follow-up was from 3 to 27 months,on average 14.3 months.During the 21 patients,5 cases died from heart cerebrovascular or tumor diseases,3 cases with footdrop,2 cases with toe amputations,3 cases with distal leg and foot anesthesias.Conclusions The patiens with acute limb arteries critical ischemia must be treated as early as,and reinforced the management of multiple organ function,which may improve the diseases' therapeutic effect.
3.The clinical significance of dynamic changes of serum D-dimers in esophageal carcinoma pre-and post-operation

Objective To investigate the relationship between dynamic changes of plasma D-dimer and survival rate of the esophageal carcinoma patients pre-and post-operation.Methods 30 cases of normal control group,160 cases of esophageal cancer group( including operation cases n =112),with the gold standard method for the determination of plasma D-dimer.Results There was a link between the level of D-dimer,TNM staging,lymph node metastasis and tumor size in esophageal carcinoma patients.Compared with the preoperation,the plasma D-dimer is significantly elevated 2 years later( t =7.35,P > 0.05 ).Conclusion Before or after the operation,dynamic changes of plasma D-dimer had a relationship with short-term survival rate.
4.Experimental study on characteristics of craniocerebral injury caused by handgun bullet impacting on bulletproof helmet

Objective To study the characteristics of craniocerebral injury caused by the handgun bullet impacting on the bulletproof helmet.Methods Fourteen healthy landrace pigs were involved and randomly divided into injury group(n =9)and control group(n =5).The landrace pigs of the injury group were shot vertically on the head under the protection of helmet plate with 9 mm handgun bullet at velocity of 360 m/s.While the landrace pigs of the control group were dealt with the same process as the injury group except for use of unarmed handgun bullet of the same ammunition dose.The changes of vital sign in the early period and the retina injury at two hours after injury were observed.Porcine cerebrospinal fluid (CSF)at pre-injury and at three hours post-injury were obtained for investigating the levels of neuron specific enolase(NSE)andαⅡ-spectrin protein.Then,the landrace pigs were sacrificed and dissected to examine the general morphological changes of the brain.The brain cortex was taken,fixed and stained with hematoxylin-eosin for microscopic observation.Results The landrace pigs in the injury group manifested decrease of the blood pressure and increase of the heart rate and respiratory rate in the early stage after injury.General morphological observation found retinal hemorrhage(3/9),skull fracture(3/9)and brain surface damage including local impact lesion(9/9)and contrecoup lesion(9/9),with severe impact lesion than contrecoup lesion.Optical microscopic observation revealed acute injury of the cerebral cortex neuron both on the impact and contrecoup locations.The concentrations of NSE and αⅡ-spectrinwere significantly increased in CSF three hours after injury(P < 0.05).Conclusions The handgun bullet impacts on the pig head protected by the bulletproof helmet may induce blunt craniocerebral injury in the early period,with severe impact lesion than contrecoup lesion.Therefore,traumatic brain injury of the soldiers armed with the helmets should be stressed and managed early.
5.Discussion on the development of hospital library and its function of hospital's scientific research management based on texts analysis

Objective:To analyze the situation of recent ten-year publications in Chinese hospital libraries, providing advice for the development and innovation of hospital libraries and the help to improvement of scientific research management.Methods:Publications in databases including CNKI, Wanfang, VIP were retrieved when " hospital library" is included as a key topic. The general information of the enrolled publications was statistically described. With respect to the key words with high frequency, co-word analysis and cluster analysis were performed to explore the research hotspots in hospital libraries.Results:The number of publications in Chinese hospital libraries shows an overall downward trend. By contrast, the total numbers of publications in hospital libraries from Shandong and Guangzhou rank top. In terms of the ratio of total number of publications to total number of hospitals, Liaoning and Chongqing rank top. The periodicals of interest are mainly grouped into three categories including intelligence information, library and hospital management; however, the cooperation with clinical or other departments should be strengthened. Discipline and personalized service, digital library and library innovation are the dominating research hotspots in hospital libraries from China. Studies of hospital library mainly focused on clinical and scientific research services, but paid less attention to the support to hospital scientific research management.Conclusions:The service of hospital library is involved in the whole process of scientific research management. To improve the role of hospital library in scientific management, embedded service is recommended to be applied into the whole process of hospital scientific research management; construction of institutional repository for scientific research management and transformation of the role of hospital library are also highly recommended.
6.Biocompatibility of rat olfactory ensheathing cells and fibrin glue

BACKGROUND: Fibrin glue has been demonstrated to function as a kind of biomaterial with high quality. It has been used in nerve tissue engineering and proved to be a kind of scaffold for some cells.OBJECTIVE: To explore the biocompatibility of fibrin glue and olfactory ensheathing cells (OECs).DESIGN, TIME AND SETTING: An in vitro control trial based on cytology was performed at the Institute of Neurobiology,Nantong University from August 2007 to February 2008.MATERIALS: Fibrin glue was made of fibrin and catalyst, and OECs derived from rats' olfactory bulb were normally primary-cultured.METHODS: OECs were divided into control (OECs clone spheres were cultured alone) and in fibrin glue (OECs clone spheres were cultured and combined with fibrin glue) groups. After 1 week of culture, the proliferation of OECs were observed by convert microscope and detected by S-100 immunofluorescence histochemical staining.MAIN OUTCOME M EASURES: OECs morphology, cell count, the area of the cell bodies and the perimeter of the cell were determined.RESULTS: OECs could survive, migrate in fibrin glue, and float in the fibrin glue in the lower layer. After 7 days of incubation, cell body exhibited fusiform or triangle, predominantly bipolar or bipolar. The number of the S-100 positive cells was more, and cell bodies were larger in fibrin glue group than control group (P < 0.05). However, there was no obvious difference between two groups in cell perimeter (P > 0.05).CONCLUSION: Fibrin glue has good biocompatibility with OECs, and OECs can survive and migrate in fibrin glue.
7.Circulating Pro-angiogenic miRNAs in Predicting the Risk and Severity of Coronary Artery Disease

Objective To evaluate the correlations of pro-angiogenic miRNAs with risk and severity in patients with coronary artery disease (CAD). Methods After angiographic documentation, 10 patients with CAD and 10 healthy controls (HCs) were enrolled for screening the differently expressed microRNAs (DEMs) (exploration stage). Blood samples of 142 patients with CAD and 140 age- and gender-matched HCs with the same eligibility were collected (validation stage). And the plasma levels of DEMs were measured by real-time qPCR. Results In the exploration stage, miR-19a, miR-92a, miR-126, miR-130a, miR-210, miR-296, and miR-378 expression levels were higher, but miR-221 expression was lower in patients with CAD than in HCs. In the validation stage, plasma expression levels of miR-126, miR-130a, miR-210, and miR-378 were decreased in patients with CAD. Multivariate logistic regression revealed that miR-19a, miR-126, miR-130a, miR-210, and miR-378 were independent protective factors for CAD. Receiver operating characteristic (ROC) analysis displayed that testing of a combination of miR-19a, miR-126, miR-130a, miR-210, and miR-378 was of good value in predicting the risk of CAD, with an AUC of 0. 768. Additionally, miR-126 and miR-210 were negatively associated with Gensini scores. Conclusion miR-126, miR-130a, miR-210, and miR-378 could serve as promising biomarkers for predicting the risk and severity of CAD.
8.Experimental research on the compatibility of self-assembly nanofiber hydrogel from the amphipathic peptide containing IKVAV with olfactory ensheathing cells of rats.

The present research was aimed to explore the biocompatibility of IKVAV self-assembling peptide nanofiber scaffold with olfactory ensheathing cells (OECs) of rats. The OECs were seeded onto the surface of coverslips covered with IKVAV self-assembling peptide nanofiber scaffold hydrogel (2D culture system), and implanted within IKVAV self-assembling peptide nanofiber scaffold hydrogel (3D culture system), respectively. The adhesion, viability of OECs were observed with inverted microscope. Then the characteristics for survival and adhesion of cells by image processing were observed, and statistical analysis on the number of S-100 positive cell, the area of the cell bodies and the perimeter of the cell and MTT method were carried out. It was found that the OECs could survive and migrate in IKVAV self-assembling peptide nanofiber scaffold. The result of the cell MTT exam, of the shape and quantity of cells had no significant difference compared to those of the OECs cultured with poly-L-lysine (PLL). It has been proved that IKVAV self-assembling peptide nanofiber scaffold has good biocompatibility with rat OECs.

9.Effects of case-based learning combined with clinical anatomy in neurosurgery internships

Objective:To investigate the effects of case-based learning combined with clinical anatomy in neurosurgery internships.Methods:We included students in grades 2014 and 2015 (test group, n=58) and those in grade 2013 (control group, n=48) doing internships in The first Affiliated Hospital of Shantou University Medical College. The test group adopted case-based learning combined with clinical anatomy in the form of neurosurgery intern learning groups. The group learning consisting of four sessions was held twice a week for a total of four times. The traditional teaching method was used in the control group. All the students filled in the Self-Directed Learning Readiness Scale before and after neurosurgery internships. The test group filled in the Satisfaction Survey on case-based learning combined with clinical anatomy after neurosurgery internships. SPSS 20.0 was used to perform t test. Results:Before neurosurgery internships, there was no difference in self-directed learning ability between the test group and control group ( t=0.25, P=0.807). After the internships, the total score of self-directed learning ability was significantly improved in both the test group ( t=-6.49, P<0.001) and the control group ( t=-4.68, P<0.001). The test group showed improvements in six learning factors, while the control group had no significant changes in love of learning and learning motivation. Compared with the control group, the test group showed a significantly higher total score of self-directed learning ability ( t=2.17, P=0.032) and significantly higher scores of efficient learning ( t=3.81, P=0.001) and learning motivation ( t=4.20, P=0.001). The students in the test group were generally satisfied with the new method combining case-based learning and clinical anatomy. Conclusion:The new method combining case-based learning and clinical anatomy has positive effects on students' self-directed learning ability, especially on learning efficiency and motivation, with a high degree of satisfaction from the students.
10.Study on antioxidant effect of Herba Lycopi and screening of active fractions.

OBJECTIVETo Study the antioxidant activity of Herba Lycopi, and screen the main active fractions.
METHODThe different polar fractions were isolated from the ethanol extract of Herba Lycopi. The scavenging activity to DPPH and superoxide anions, reducing effect on Fe3+, and inhibiting activity of lipid peroxidation of the fractions were studied in vitro by high-throughput screening technique.
RESULTThe ethanol extract, ethyl acetate fraction (LLE-A. E, MCCM2921) and n-buthanol fraction (LLE-A. B, MCCM2922) of Herba Lycopi showed the scavenging activity to DPPH and superoxide anions, reducing effect on Fe3+, and inhibiting activity of lipid peroxidation.
CONCLUSIONThe ethanol extract and different polar fractions showed antioxidant capacity in vitro. That may be explain that the effect of activating blood and dissolving stasis and anti-fibrosis of Herba Lycopi in traditional application. The results may be helpful for further development of Herba Lycopi as a low-toxic antioxidant drug.
