1.Effects of astragalus membranaceus parenteral solution combined with recombinant human erythropietin on erythrocyte immunity function in patients with hemodialysis

Objective To investigate the effects of astragalus membranaceus parenteral solution combined with recombinant human erythropietin (rHuEPO) on erythrocyte immunity function in patients with hemodialysis. Methods Fifty-one patients of chronic renal failure(CRF) with hemodialysis were divided into 2 groups randomly according to whether treated with astragalus membranaceus parenteral solution or not(therapeutic group, was 26 cases and control group was 25 cases). IgG, IgA, IgM, RBC-C3b,RR,RBC-ICR, erythrocyte percentage of CD35 positive and SOD was measured before treatment and 8 weeks after treatment. Results After 8 weeks' treatment, IgG,IgA,IgM levels in therapeutic group and IgG,IgA levels in control group were higher than those before treatment(P < 0.01 ). IgM [ (2.03±0.73 ) g/L ] was higher than that in control group (P< 0.05). RBC-C3B,RR, erythrocyte percentage of CD35 positive and SOD were higher and RBC-ICR was lower than those before treatment in two groups (P < 0.05 ) ;compared with that in control group, RBC-C3b, RR was higher [(10.85±2.31 )% ] and RBC-ICR was lower [(5.34±1.91 )% ] (P < 0.05 ).Conclusion Astragalus membranaceus parenteral solution combined with rHuEPO is superior to improve CRF patients' levels of immune globulin and erythrecyte immunity function.
2.A clinical observation of simethicone, mosapride combined with flupentixol-meiltracen for treatment of patients with functional dyspepsia

ObjectiveTo observe the efficacy of simethicone, mosapride combined with flupentixol-meiltracen in the treatment for functional dyspepsia(FD). MethodsThe diagnosis of functional dyspepsia patients treated according to Zung Self-rating Anxiety Scale (SAS) > 40, Hamilton Depression Scale(HAMD) > 17 for inclusion criteria,a total of 64 patients were selected and randomly divided into two groups. The treatment group( simethicone,mosapride combined with flupeentixol-meiltracen) in 33 cases and the control group ( single flupenthixol melitracen) in 31cases. 2 groups of the period of treatment was 4 weeks. Evaluation before and after treatment with mood disorder scale score changes and syndrome efficacy and adverse reactions. ResultsThe treatment group after treatment, regardless of scale score change of mood disorders, syndrome treatment efficacy were significantly better than the control group,especially in the syndrome efficacy difference was significant( P <0.05 ,P <0.01 ) ;after the treatment without obvious adverse reaction. ConclusionThe simethicone, mosapride combined with flupentixol-meiltracen treatment with anxiety and depression in patients with functional dyspepsia could effectively relieve the symptoms of functional dyspepsia,and relieve the patient's psychological disorder;certainly result in fewer adverse reactions.
3.Antibiotics Resistance Analysis of Pathogens Causing Nosocomial Infections in Patients after Surgery

OBJECTIVE To investigate the distribution of nosocomial infection(NI) in patients after surgical procedures and antimicrobial resistant change of pathogens. METHODS A total of 195 strains of pathogenic bacteria isolated from the samples after surgical procedures from Jan 2006 to Dec 2006 were analyzed by WalkAway-40 system with NC31 system. RESULTS Among the 195 strains of pathogenic bacteria,there were Gram-negative bacilli(74.4%) and Gram-positive cocci(25.6%).The dominating microorganisms were Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa and Staphylococcus aureus.The result showed that vancomycin and imipenem still had better activities to the bacteria. CONCLUSIONS Surgical incision infection should be monitored by some effective measures,and it is very important to prevent surgical infection and use antibiotics reasonably.
4.Analysis of risk factors of metabolic syndrome in obese subjects: a follow-up study

ObjectiveTo investigate the risk factors of metabolic syndrome ( MS ) in obese subjects.Methods A seven-year follow-up study was conducted in 413 simple obese subjects and 196 subjects with normal body weight who were recruited from community residents during physical examination in 2000.There was a 7 years follow-up.Anthropometrics,blood pressure,lipid profile,fasting blood glucose,and 2 h blood glucose after glucose loading were measured.Endothelium-dependent dilatation (EDD) test was also performed.Results Among 553 of 609 subjects who were followed up in 2007,there were 381 simple obese subjects ( simple obese group) and 172 normal weight subjects( normal weight group).Seven-year cumulative incidence of MS was 35.17% in simple obese group and 8.14% in normal weight group.In simple obese group,subjects with MS showed greater or higher levels of waist circumference( WC ),waist-hip ratio ( WHR ),triglyceride ( TG ),fasting plasma glucose ( FPG ),fasting insulin (FINS),and homeostasis model assessment for insulin resistance index (HOMA-IR) (all P<0.05 ),and also decreased EDD( P<0.05 ) as compared with those without MS.WC,WHR,and FINS were higher( all P<0.05 ) and EDD was lower( P<0.05 ) in subjects with MS of normal weight group than those without MS.Logistic analysis showed that the male gender,WC,WHR,FPG,HOMA-IR,and EDD were major risk factors of MS.Conclusion Central obesity,insulin resistance,and endothelial dysfunction are important independent risk factors for development of MS.
5.Effects of nurse-led case management intervention on arm function and uncertainty for women with breast cancer

Objective To evaluate the effect of a nurse-led case management intervention on arm function and uncertainty for women with breast cancer.Methods 90 women with breast cancer were divided into the intervention group and the control group according to their nurses,with 45 patients in each group.All participants were followed six months after surgery.The intervention group attended a 6-month nurse-led case management program,while the control group received the routine care and follow-up only.Questionnaires on arm function and uncertainty were administered 1 month,3 months and 6months after the surgery respectively.Score changes were compared by repeated-measure ANOVA and MANOVA.Results The arm function of the intervention group was better than the control group,except for the 10th day after the operation,there were significant differences at other three time points.The disease uncertainty level of the intervention group was better than that of the control group,except for the dimension of unpredictability,there were evident differences in other dimensions at other three time points.Conclusions The nurse-led case management intervention could improve arm function recovery and decrease the uncertainty to disease of breast cancer survivors.
6.Early assessment of the level of serum cystatin C to renal damage in patients with multiple myeloma

Objective According to the level of serum cystatin C (Cys-C)in patients with multiple myeloma (MM),to explore its significance of early assessment to renal damage.Methods To select 42 patients with MM(MM group) and 30 healthy subjects(control group),and measure serum Cys-C,creatinine (SCr) and β2-microglobulin (β2-MG).Results The level of serum Cys-C in MM group was significantly higher than that in control group [(2.34 ± 0.57) mg/L vs.(0.66 ± 0.12) mg/L],which increased with ISS stage[Ⅰ,Ⅱ,Ⅲ stage was(1.77 ± 0.38),(2.11 ± 0.46),(3.14-± 1.07) mg/L](P< 0.05).Cys-C and β 2-MG,renal function had a positive correlation (r =0.760,0.694,P < 0.05).Conclusion The increasing of Cys-C in MM patients is related to impaired renal function and tumor burden,which can be used as an early assessment indicators.

Background and purpose:Hepatocellular carcinoma (HCC) is a common malignant tumor of the digestive system in our country, with high fatality, development of HCC and the machine system research and treatment is a primary issue in current study of HCC. To explore the expression ofβ-catenin at different stages in the process of hepatocellular carcinoma carcinogenisis for SD rats induced by chemicals. Methods: The experimental group included 48 male SD rats mice with primary liver cancer induced by diethylnirtosamine/carbon tetrachloride/Ethanol, while 48 normal male SD rats mice were used as the control group. The rats were killed every 3 weeks to collect the specimens and observe the pathological changes by HE staining. The changes ofβ-catenin protein expressions were detected by immunohistochemistry and Western blot respectively. Results:SD rats liver cancer was conifrmed by HE staining after 21 weeks DEN/CCl4/Ethanol induction. Immunohistochemistry showed thatβ-catenin expression level was obviously higher in the experimental group(0.27±0.01) than that of the control group(0.21±0.02) after 3 weeks induction(P<0.05). As time progresses, the expression levels ofβ-catenin kept on rising, and at the 18th(0.30±0.02) and 21th weeks(0.32±0.02), it was significantly higher than that of the earlier liver tissues of the experimental group(P<0.05), Western blot consistent with immunohistochemical results. Conclusion:β-catenin protein expression is different in the normal liver tissue, cirrhosis, liver cancer,β-catenin and the occurrence of liver carcinoma development had close relationship.β-catenin protein in the cell with further accumulation, may active a series of target gene, leading to the formation of liver cancer..
8.Review on taxanes-induced peripheral neuropathy

As a tubulin stabilizer, taxanes are widely used in the treatment of breast cancer, ovarian cancer, non-small-cell lung cancer (NSCLC),and part of head and neck malignant tumors, and were confirmed to be significantly effective.However, taxanes-induced peripheral neuropathy(TIPN) still affects the quality of life and psychological status of patients to varying degrees, severe cases can lead to dose reduction, and even chemotherapy interruption.In this paper, we summarize the pathogenesis, risk factors, clinical assessment, the progress of prevention and treatment of TIPN, to provide the basis for the prediction and early prevention of TIPN in the clinical administration, thus we can improve patients’ quality of life while extending their survival.
9.Inhibition effect of curcumin on proliferation and invasion of breast cancer cell by decreasing expression of MT1-MMP

Objective To investigate whether MT1-MMP is involved in the inhibition effect of curcumin on the proliferation and invasion of breast cancer cell and the mechanism . Methods Firstly, MCF-7 cell lines transfected by MT1-MMP eukaryotic expression vector was established. We divided all cells into 3 groups,including null vector transfection group, non-transfected and transfected group with different concentrations of curcumin. The expression of MT1-MMP protein, the proliferation and invasion ability were respectively analyzed by western blot, transwell method, and cell counting kit-8 (CCK-8). Results The expression of MT1-MMP was inhibited by curcumin. Transwell and CCK-8 experiment indicated the proliferation and invasion abilities of MT1-MMP transfected MCF-7 cells were inhibited by curcumin in a concentration dependent manner. Conclusion The inhibition value of curcumin on proliferation and invasion is probably due to its ability to inhibit the expression of MT1-MMP.
10.Progress and implications of the DRG-based prospective payment system ( DRG-PPS ) reform in China

DRG-based prospective payment system ( DRG-PPS ) is one type of mature inpatient care payment mechanisms in most countries and pilot projects would be introduced to China in 2017. This article provides an overview of the previous typical DRG-PPS reform initiatives on provincial, municipal and county level respectively. It also summa-rizes the characteristics of the reform programs, describes the diversities of policy implementation, and discusses the suc-cessful experience and implications of the initiatives. The paper puts forward some suggestions on the upcoming DRG-PPS pilot projects based on the progress and problems of the initiatives, to promote the DRG-PPS reform in China.
