1.Optimization of Semi-bionic Extraction Technology for Rhubard by Uniform Design

Objective: To optimize semi-bionic extraction(SBE)of rhubard. Methods: The best extraction conditions of semi-bionic extraction for rhubard were optimized by uniform design with the total content of loeemodin, rhein, emodin, chrysophanol, and anthraquinone and the dried extract weight as the indices. Results:According to the optimization and the industrial production condi-tion, the pH values of water in three times of extraction were 2. 0,6. 5 and 9. 0,and the extraction time was 2, 1, 1 h, respectively. Conclusion:This method provides a theoretical basis for the optimization of rhubard extraction.
2.Fingerprint Chromatogram of Forsythia Suspensa Dispensing Granule by HPLC

Objective To establish the fingerprint of Forsythia suspensa dispensing granule by HPLC. Methods The total of 10 samples of Forsythia suspensa were analyzed by HPLC on a Waters SunFire C18 column(4. 6 mm×250 mm,5 μm) with mobile phase of A:acetonitrile and mobile phase B:0. 4% acetic acid solution as gradient elution, with flow rate at 1. 0 mL·min-1 , wavelength at 277 nm and column temperature at 30 ℃. Results There were 13 characteristic common peaks in the Forsythia suspensa dispensing granule, in which two peaks identified as forsythoside A and forsythin, the similarities were more than 0. 999. Conclusion The HPLC fingerprint of Forsythia suspensa dispensing granule is stable, and is easy to manipulate, which can provide more information for the quality control of Forsythia suspensa dispensing granule.
3.Laparoscopic surgery in the treatment of early uterine malignancies: a report of 8 cases

Objective To explore the feasibility of laparoscopic operation as treatment for early malignant uterine tumors Method Six patients with endometrial carcinoma underwent laparoscopic extensive total hysterectomy and bilateral adnexectomy, and 2 with cervical cancer underwent additional pelvic lymphadenectomy Results The mean operating time of the 6 laparoscopic extensive total hysterectomy and bilateral adnexectomy was 220 min, and mean blood loss was 200 ml The mean operating time of 2 cases underwent additional pelvic lymphadenectomy was 240 min No organ trauma occurred, and the average post operative hospital stay was 8 days Conclusion These results suggest that laparoscopic operation is safe and feasible in treating early stage malignant uterine tumors

Conclusion: The technology provides a theoretical basis for the extraction optimization of Forsythia suspensa.
5.The comparative study of perfusion MR imaging in primary central nervous system lymphomas and high grade astrocytomas

Objective To investigate the value of perfusion MR imaging in differential diagnosis between primary central nervous system lymphomas(PCNSL)and high grade astrocytomas.Methods Twelve patients with PCNSL and 23 patients with high grade astrocytomas were preoperatively examined using a 1.5T MR unit.Routine MR sequences were performed followed by dynamic susceptibility contrastenhanced MR perfusion imaging.The perfusion color images and the time-signal intensity curves of the two tumor groups were compared.The relative cerebral blood volume(rCBV)within the tumor parenchyma was measured and the data were analyzed with unpaired Student's t-test.Results The rCBVs within the tumor parenchyma of the PCNSL and high grade astrocytomas were 1.78±0.5 1 and 3.87±0.87 respectively.The rCBV in the PCNSL was significantly lower than that of the high grade astrocytoma(P<0.05).When the time-signal intensity curves were compared,the PCNSL showed a trend towards the baseline after the first pass and the curves even overshot above the baseline in 7 out of 12 cases,whereas the high grade astrocytoma showed a trend to be close to the baseline but couldn't return to the baseline completely.Conclusion The MR perfusion imaging can be very useful in distinguishing the PCNSL from high grade astrocytomas.
6.A comparative study on the outcomes between ileal neobladder and orthotopic ileal neobladder

Objective To compare the recent clinical efficacy between ileal neobladder and orthotopic ileal neobladder.Methods The clinical data of 62 patients with bladder cancer who were performed with cystectomy plus urinary diversion were retrospectively analyzed.Among them,32 patients were performed with orthotopic ileal neobladder(orthotopic ileal neobladder group)and 30 patients were performed with ileal neobladder(ileal neobladder group).The operation time,intraoperative bleeding,intestinal function recovery time,hospital stay,early postoperative complications were compared between two groups.Results All the patients were successful,and there was no operative deaths occurred.There was no significant difference in the postoperative intestinal function recovery time between two groups(P > 0.05).The operation time,intraoperative bleeding,hospital stay in orthotopic ileal neobladder group was higher than that in ileal neobladder group [(463.59 ± 50.24)min vs.(436.07 ± 44.91)min,(1081.16 ± 320.49)ml vs.(867.53 ± 224.61)ml,(46.88 ± 4.67)d vs.(20.37 ± 5.24)d],but the incidence of early postoperative complications in orthotopic ileal neobladder group was lower than that in ileal neobladder group [25.0%(8/32)vs.53.3%(16/30)],and there were significant differences between two groups(P <0.05).Conclusions Two kinds of urinary diversion surgical urinary diversion are clinically more mature manner,in clinical practice.Clinicians should be based on the patient's comprehensive situation,combined with the clinical experience to select the most appropriate surgical procedures.
7.Effect of shenmai injection on the expression of hippocampal c-fos gene of rats with ischemic cerebral injury

BACKGROUND: The c-fos gene is commonly expressed in neurons,which may act as one of the signs of activities and reflection of injured neural cells.OBJECTIVE: To investigate the expression of hippocampal c-fos gene of rats with ischemic cerebral injury and the protective role of shenmai injection at the molecular level.DESIGN: Randomized controlled study.SETTING: Department of Histology and Embryology, Luzhou Medical College.MATERIALS: The experiment was completed at the Department of Histology and Embryology, Luzhou Medical College from January to March 2002. Totally 30 male Wistar rats were randomly divided into control group, ischemia group and treatment group with 10 in each group.METHODS: Bilateral common carotid arteries of rats in the ischemia and treatment groups were splinted for 30 minutes and reperfused for 1 hour to establish models of ischemic cerebral injury. Rats in the treatment group were injected with 2 mL/kg of shenmai (constituted with hongshen, dwarf lilyturf tuber and other Chinese herbal medicines) 30 minutes before ischemia. Rats in the ischemia group were not treated with any drugs, and rats in the control group were treated with sham operation but without splinting common carotid artery and giving any drugs. Hippocampal tissue of rats was obtained to make paraffin sections. In every group, one section from each rat was taken at random. Totally 100 neurons of every group were counted. Expression of c-fos gene in hippocampal neuron was observed according to the nuclear color of neurons (+++ as dark-brown mark;++ as brown mark; + as light brown mark; - as no brown mark).MAIN OUTCOME MEASURE: c-fos expression of hippocampal neurons of rats in each group.RESULTS: Totally 30 rats entered the final analysis. Expression of c-fos gene in hippocampal neurons was significantly more in the ischemia group than that in the control group (+++: 24/visions, 7/visions, P < 0.05); but that in the treatment group was less than that in the ischemia group (+++:13/visions, 24/visions, P < 0.05).CONCLUSION: Shenmai injection can reduce the expression of c-fos gene in hippocampal neurons of rats with ischemic cerebral injury, and can protect neural cells of ischemic cerebral injured tissue.
8.Technical Condition Optimization of Semi-bionic Extraction for Kanggan Liyan Syrups by Uniform Design

Objective: To optimize the conditions of semi-bionic extraction for Kanggan Liyan syrups. Methods: The samples were extracted by water with different pH values under the same conditions of formula composition and dosage, extraction temperature, filtration method, water voulme, centrifugal time and concentration multiple. The best conditions of the semi-bionic extraction for Kang-gan Liyan syrups were screened by uniform design with the content of forsythoside A, polydatin, baicalin and emodin and the dried ex-tract weight as the indices. Results:The best technical conditions were as follows:the pH value of water for the 1st, 2nd and 3rd de-coction was 4. 9, 7. 5 and 9. 0, respectively, and the total extraction time was 2h. Conclusion:According to the industrial production conditions, the pH value of water for the 1st, 2nd and 3rd decoction is 5. 0, 7. 5 and 9. 0 with the extraction time of 1h,0. 5h and 0. 5h, respectively.
9.Study on HPLC Fingerprints of Forsytiae Suspensa Fructus from Shiyan District

Objective:To establish the HPLC fingerprints of chemical constituents in Forsytiae suspensa Fructus from Shiyan dis-trict to provide scientific evidence for the quality control of Forsytiae suspensa Fructus. Methods: HPLC was performed on a Waters SunFire C18 (250 mm × 4. 6 mm, 5 μm) column and a Diamonsil C18 guard column, and the mobile phase consisted of acetonitrile-0. 4% acetic acid solution with gradient elution. The flow rate was 1 ml·min-1 , the detection wavelength was 277 nm, and the col-umn temperature was 30℃. Results:There were 14 common peaks in the HPLC fingerprints of chemical constituents in 12 batches of Forsytiae suspensa Fructus from Shiyan. The 14 characteristic peaks of Forsytiae suspensa Fructus from different habitats were under clustering analysis, and the similarity showed little difference. The contents of forsythoside A and forsythin exhibited significant differ-ence in the samples. Conclusion:The HPLC fingerprint method is simple, rapid and feasible in the quality control of Forsytiae suspen-sa Fructus.
10.Boundary sign and cortex sign on MR in differentiation between solitary brain metastasis and astrocytoma

Objective To investigate the value of the boundary sign and cortex sign in differential diagnosis of solitary brain metastasis and astrocytoma. Methods Thirty-one patients with high grade astrocytoma and 42 patients with solitary brain metastasis diagnosed either pathologically or clinically were included in this study. The differential diagnostic value of the boundary and the cortex sign were analyzed retrospectively. Results The boundary sigh was found in 39 of 73 patients, which included 32 patients with solitary metastasis and 7 patients with astrocytoma. The sensitivity and specificity of the boundary sign was 76.20% and 77.42%, respectively, and the positive predictive value for solitary metastasis was 82.05%. The cortex sign was found in 34 of 70 patients, including 25 with astrocytoma and 9 with solitary metastasis. The sensitivity and specificity of the cortex sign was 80.65% and 78.57%, respectively, and the positive predictive value for astrocytoma was 73.53%. The boundary sigh was singlely foundin 24 patients, including 21 with astrocytoma and 3 with solitary metastasis. The area under the curve for both two signs was 0.85. Conclusion The boundary sign and the cortex sign are specific signs in differentiation between solitary brain metastasis and astrocytoma on routine MR series. Combination of this two signs is very useful in distinguishing solitary brain metastasis from astrocytoma.
