1.Combination of vascularized fibular head epiphysis and superficial peroneal neurovascular reverse flap repair defects of bone and skin in lateral malleolus in children

Objective To explore the clinical effect of combination of vascularized fibular head epiphysis and peroneal neurovascular reverse flap repair defects of bone and skin in lateral malleolus in children.Methods Eight children with defects of bone and skin in lateral malleolus were recruited from May,2009 to September,2014.The age of children was ranging from 4 to 12 years old.Four children were injured by wheel twist,2 children were injured by traffic accident,1 was injured by machinery,and 1 was crashed with heavy object.The size of soft-tissue defect was ranging from 3 cm × 5 cm to 7 cm × 9 cm.The flap and bone healing along with ankle joint function scoring were evaluated.Patients were reviewed every 4 to 6 weeks during one year after their surgery,and they were reviewed every 6 months one year later.Results Secondary repair were conducted in all children.The curative effect got well after follow up for 6 months to 2 years(mean,12 months).The flap and fibular head epiphysis were well developed.The ossification of fibular head epiphysis did not found.According to American Orthopaedic Foot and Ankle Society (AOFAS) ankle function evaluation criteria,the results were excellent in 5 cases,good in 2 cases and poor in 1 at 10 months after operation.The shape and function of ankle joint were good,with one exception.The average motion degree of ankle joint was (68±5.12)°.Conclusion Combination of vascularized fibular head epiphysis and peroneal neurovascular reverse flap repair defects of bone and skin in lateral malleolus in children is effective.
2.Use of partial nail carried by great toe fibular flap for repairing the defect of fingertip soft tissue and nail bed: 12 cases report

Objective To investigate the method and clinical outcome of partial nail carried by great toe fibular flap for repairing the defect of fingertip soft tissue and nail bed.Methods From June,2016 to October,2017,12 cases suffered the defect of fingertip soft tissue and nail bed.The injury fingers included 5 index fingers,6 mindle fingers and 1 ring finger.All cases were complicated with nail bed defect of different degrees.The nail matrix was intact.The area of fingertip defect ranged from 1.0 cm×1.0 cm to 1.5 cm×2.5 cm.The area of nail bed defect ranged from 0.2 cm×0.8 cm to 0.5 cm×1.5 cm.Great toe fibular flap combine with partial nail was harvested.Donor site was directly sutured or skin grafting according to the size.The regular preoperative followed-up was performed.Results All flaps survived with donor sites healing good.The average followed-up time was 7 (ranging from 2 to 10) months,and cosmetically acceptable results were achieved for all patients.The mean static 2-PD in the flaps was 7.5 (range,6.0 to 8.0) mm.No obvious deformity of the great toe nail.Conclusion Findings proved that using partial nail carried by great toe fibular flap is a beneficial microsurgical alternative for reconstructing defect of fingertip soft tissue and nail bed.
3.Design and clinical application of postoperative marker system of perforator flap

Objective To investigate the design of the postoperatively marker system based on the perforator flap and its clinical application.Methods According to the basic anatomy and blood supply characteristics of perforator flap,the observation points were marked on the flap surface and recipient area postoperatively,and judge flap blood flow changes with hand-held Doppler(HHD) and clock-face measurement based on perforators.From November,2015 to November,2017,prospectively conducted a control study evaluating the incidence of vascular crisis,accuracy rate,reoperation rate and survival rate of flaps between simple traditional flap monitoring group (n=100) and traditional flap monitoring combine with postoperatively marker system monitoring group (n=100).Results Vascular crisis was mostly concentrated with in 48 h postoperative in traditional flap monitoring group and combined group.There was no significant difference in the incidence,positive rate and negative rate of vascular crisis in the two groups (P>0.05).The reoperation rate in the traditional flap monitoring group (87.5%) was higher than the combined group (27.8%)(P<0.01).The survival rate of flap of combined group (99%) was higher than the traditional falp monitoring group (90%) (P<0.05).Conclusion The postoperatively marker system of perforator flap can effectively guide the medical staff to observe the blood supply of the flap,detect and judge the vascular crisis in time,and It is simple and easy to do without increasing the patient's trauma and medical expenses as well as no additional equipments.
4.Safety and efficacy of levosimendan in patients with acute heart failure: a prospective, multicenter, and observational study

Objective:To investigate the indication, effectiveness, tolerance, and safety of levosimendan in patients with acute heart failure (AHF) in 20 hospitals in Beijing, China.Methods:This prospective, observational, and multicenter study consecutively enrolled AHF patients who were treated with levosimendan at 20 hospitals in Beijing from April 2020 to March 2022. Baseline demographics, laboratory parameters, clinical presentation, concomitant diseases and medications were collected. After initiation of levosimendan, levosimendan administration, laboratory parameter pre- and post-administration, symptoms improvement, and adverse events were also collected.Results:Totally 800 AHF patients were included, 67% of whom were male, aged (65 ±17) years, 50% of whom had ischemic heart disease, and the left ventricular ejection fraction (LVEF) was (36±11)%. The dose of levosimendan was (11.84 ±2.11) mg and the mean infusion time was (1 450±307) min. Dyspnea was improved in 83.4% of AHF patients at 24 h after treatment. The level of B-type natriuretic peptide (BNP) significantly decreased from 689 (406-1509) pg/mL to 410 (156-697) pg/mL in all patients at 24-72 h after treatment ( P<0.001), and the level of N-terminal pro-brain natriuretic peptide (NT-pro BNP) decreased from 6910 (3 715-13 914) pg/mL to 2 851 (1 288-6 191) pg/mL ( P<0.001). Meanwhile, LVEF level also improved significantly [(40±11)% vs. (36±11)%, P<0.001]. During levosimendan administration, adverse events occurred in 74 (9.3%) patients, including hypotension (5.9%), arrhythmia (1.9%), and other symptoms (1.1%). Among them, 7 patients ( 2 patients with hypotension and 5 patients with ventricular tachycardia) interrupted levosimendan administration. Conclusions:The use of levosimendan is safe, and can improve symptoms reduce BNP or NT-pro BNP levels and increase LVEF level in AHF patients.
5.Influence of γ-H2AX expression on prognosis in patients with newly diagnosed multiple myeloma

Objective To investigate the expression of bone marrow γ-H2AX in the patients with multiple myeloma(MM) and its correlation with the prognosis.Methods The patients with newly diagnosed MM in this hospital were selected as the case group,and the patients with non-hemopoietic system tumor without obvious morphological abnormalities by bone marrow smear and biopsy served as the control group.The immunohistochemistry was adopted to detect the expression level of bone marrow γ-H2AX in the cases group and control group,the image-Pro Plus(IPP) semiquantitative analysis was performed.The expression differences were compared between the two groups,moreover the case group was re-divided into the strong expression group and weak expression group according to γ-H2AX expression level.Then the relation ship between γ-H2AX expression level and the prognosis in the patients with MM.Results The bone marrow γ-H2AX expression level in the case group was significantly higher than that in the control group (P<0.05);the level of γ-H2AX expression in the strong expression group was significantly stronger than that in the weak expression group (P<0.05).Conclusion The level of γ-H2AX expression was higher among MM patients,and the over expression of γ-H2AX predicts the shorter survival time.
6.MICM classification for a case of acute myelomonocytic leukemia M

Objective:
To describe the MICM (morphology, immunology, cytogenetics and molecular biology) characteristics of a case of acute myelomonocytic leukemia M 4C .
Methods:
The medical history data of the case of M 4C admitted to our hospital was reviewed. The results of bone marrow cell morphology, cytochemical stains, bone marrow biopsy, immunophenotype, cytogenetics, molecular test and NGS (next-generation sequencing) of the case were analyzed.
Results:
The bone marrow smear showed markedly active proliferation of bone marrow cells in which the myelomonocytic cells accounted for 85.6%. Cytochemical stains showed peroxidase (POX) stain partially and weakly positive; specific esterase AS-DCE partially positive; non-specific esterase α-NBE partially positive and smothered by sodium fluoride; non-specific esterase AS-DAE partially positive and smothered by sodium fluoride. Bone marrow biopsy showed hyperproliferative cells and diffused hyperplasia of blasts. Immunophenotype analysis showed that the abnormal cell population was positive for CD11B, CD64, CD56, cMPO, CD33, CD41, CD61, CD38 and CD58, but negative for CD13, CD34, CD117, CD7, CD123, HLA-DR, CD10, CD19, CD20, CD2, CD14, CD235, CD15, CD303, CD304, CD25, cCD79a, cCD3, cCD22, CD1a and TDT. Cytogenetic analysis showed 47, XY, t(9;11) (p22;q23),+mar. The molecular test for leukemia showed MLLT3/KMT2A gene rearrangement. NGS showed NRAS and TET2 mutation. The case was finally diagnosed as AML (acute myelomonocytic leukemia) M 4C with t(9;11)(p22;q23), MLLT3-KMT2A.
Conclusion
Leukemia M 4C may show the characteristics of both granulocytes and monocytes with complex morphological features. The combined examination of MICM should be necessary for the diagnosis of M 4C with great significance.
