1.Advantages of cemented dynamic hip screw for osteoporotic intertrochanteric fracture

BACKGROUND:Dynamic hip screws have been considered as a classic method for intertrochanteric fracture. However, migration of the hip screw resulting in cut-out of the femoral head remains the most common mechanical failure, which requires secondary surgery. OBJECTIVE:On the basis of the traditional dynamic hip screw, we modified the lag screw, designed new internal fixation system as cemented dynamic hip screws to explore the feasibility and the biomechanical properties for the treatment of osteoporotic intertrochanteric fracture. METHODS:A total of 24 hip samples were harvested from fresh cadavers with osteoporosis. Bone density was measured. According to the standards, the specimens were made into intertrochanteric fracture models of AO31-A1 type. Of each specimen, one side implanted with cemented dynamic hip screws served as experimental group, and another side implanted with traditional dynamic hip screw served as control group. Axial compression, twisting and fatigue load biomechanics were tested. Their biomechanical functions were compared. RESULTS AND CONCLUSION:Stress intensity, axial stiffness and horizontal shear stiffness were higher in the experimental group than in the control group (P<0.05). At torsion angle of 3°, torsional moment was higher in the experimental group than in the control group (P<0.05). Fatigue load was higher in the experimental group than in the control group (P<0.05). Results suggested that cemented dynamic hip screw internal fixation system in the treatment of senile osteoporotic intertrochanteric fracture can effectively enhance the hip screw holding force and prevent femoral cutting. Biomechanical property, stiffness, endurance resistance and stability of cemented dynamic hip screw are better than traditional dynamic hip screw.
2.Effect of FK506 on functional recovery of peripheral nerve after end-to-side anastomosis

BACKGROUND: Injured nerve can regenerate through end-to-side anastomosis and obtain partial physiological function in animal experiment. As an immuno-suppressivedrug, FK506, which is characterized by promoting neural growth and functional recovery, is paid widely attention to.OBJECTIVE: To investigate the effect of FK506 on functional recovery of peripheral nerve after end-to-side anastomosis.DESIGN: Randomized controlled animal study.SETTING: Department of Operation Surgery, Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology.MATERIALS: The experiment was completed in the Department of Operation Surgery, Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology in March 2005. Totally 26female Wistar rats were selected and divided into experimental group and control group with 13 in each group.METHODS: Peroneal nerve was transacted on the right side of all the animals, the proximal end of peronieal nerve was turned back and ligated, and the distal end was sutured to the side of tibial nerve with perineurial window. No operation was done on the left side. FK506 was injected in the right tibial muscle (2 mg/kg· d), once a day within two weeks in the experimental group, and saline was used in another group as contrast.MAIN OUTCOME MEASURES: Three months afteroperation, the bilateral common peroneal nerve and the tibial muscle were taken for electrophysiological and histological examination, and the number of common peroneal nerve fiber, area of tibialis anterior muscle fiber of transverse section and muscle wet weight of tibialis anterior muscle were measured. The results were expressed with the ratio between right side and left side (i.e.recovery rate).RESULTS: Totally 26 rats entered the final analysis. ① Results of histological examination: Both ratio of number of nerve fiber and ratio of area of muscle fiber in the experimental group were higher than those in the control group (0.734±0.143, 0.412±0.119; 0.628±0.125, 0.432±0.135; P < 0.01,0.05). ② Results of electrophysiological examination: Recovery rate of action potential, recovery rate of single muscular contractility and recovery rate of tetanus contractility in the experimental group were higher that those in the control group (P < 0.05). ③ Ratio of wet weight in the experimental group was higher than that in the control group (0.765±0.101,0.513±0.116, P < 0.05).CONCLUSION: FK506 injected into targeted muscle can promote growth and functional recovery of peripheral nerve in end-to-side anastomosis.
3.Effect of stellate ganglion block on heart rate variability during thyroidectomy performed under cervical plexus block

Objective To investigate the effect of stellate ganglion block(SGB)on heart mte variability (HRV)during thyroidectomy performed under cervical plexus block.Methods Forty-five ASA Ⅰ or Ⅱ pafients of both sexy8(8 males,37 females) aged 20-60 yr,weighing 40-70 kg undergoing elective thyroidectomy under cervical plexus block were randomly divided into 3 groups(n=15 each):group Ⅰ control,group Ⅱ left SGB and group Ⅲ right SGB.The parameters of HRV included low frequency(LF),high frequency(HF),LFnu,HFnu and LF/HF ratio.SBP,DBP,HB and HRV parameters were recorded before anesthesia(To,baseline),immediately(T1) and at 5 and 10 rain after cervical plexus block(T2,T3),at the beginning and end of operation (T4,T5).Results SBP and DBP were significantly increased at different time points after cervical plexus block in all 3 groups.Compared with the baseline at T0,LFnu and LF/HF ratio were significantly increased while HR,HF and HFnu decreased during operation in group Ⅰ,LF,HF and HFnu were significantly decreased while HR increased during operation in group Ⅲ(P<0.05),but no significant change in HRV parameters was found during operation in group Ⅱ(P>0.05).Compared with group Ⅰ,HR and LFnu were significantly decreased while HF and HFnu increased in group Ⅱ(P<0.05),but no significant change was found in the indexes mentioned abovein group Ⅲ (P>0.05).Conclusion SGB can adjust cervical plexus block-induced sympathetic excitability and maintain the balance between sympathetic and parasympathetic tension.Left SGB outweighs right SGB.
4.One-staged coverage and revascularisation of traumatised fingers by neighboring digital artery flow-through flaps

Objective To investigate the clinical application of neighboring digital artery flow-through flaps for one-staged skin coverage and revascularisation after severe finger injuries.Methods From May 2008 to July 2010,there were ischemic fingers had severe injuries,aged 19 to 52 years old,which were male in 5 cases,female in 2 cases,with skin and arterier defected.One-staged coverage and revascularisation of traumatised fingers were used with neighboring digital artery flow-through flaps.Functional exercise and physical therapy were done 3 weeks later.All cases had more than 12 months' fellowed-up periods.The sensibility testing,Questionnaire of the flap aesthetic satisfactory and TAM evaluations were all performed in all patients.All the results were measured and evaluated.Results All flaps survived completely and digit blood circulations were good in 7 patients.The flaps had good appearance and aesthetic satisfactory (100%).At 12 months after operations,the pain and touch perception reinnervated until 6 months and two-point discrimination testing were 6.1 to 9.6 mm,mean was (7.1 ± 1.3) mm,and the flap sensibility assessment were S3 * and TAM evaluation ratio were 0.81 ±0.16.Conclution Neighboring digital artery flap ideally suited as a thin and pliable flow-through flap in digital replantation/revascularization.It is helpful for salvage of the ischemic fingers after injury with skin and arteries defects.
5.Monostotic fibrous dysplasia of the sphenoid sinus: two cases report and literature review

objective:To study the etiopathogenisis、clinical features、diagnosis and treatments of monostotic fibrous dysplasia of the sphenoid sinus. Method:Two cases of monostotic fibrous dysplasia of the sphenoid sinus without any symptoms was reported with relevant literature review.Result: No aggravation was found after 6 months-follow-up.Conclusion:The cranial fibrous displasia has low incidence rate with non-specific symptoms and high rate of misdiagnosis. The monostotic fibrous dysplasia of the sphenoid sinus without any symptom is rarely seen clinically. Imageological examination,for example,CT and MRI,is valuable for the diagnosis of this disease. The histopathological evidence is absolutely necessary to make definite diagnosis. The conservative treatment may be chosen for the asymptomatic cases. Nasal Endoscopic surgery can not only remove the diseased region but also make diagnosis.The long-term follow-up should be carried out in all of these patients.
6.Hand hygiene status and effect of hand-drying measures on hand-washing of health care workers in an intensive care unit

Objective To realize hand hygiene status and effect of hand-drying measures on hand-washing of health care workers(HCWs)in an intensive care unit(ICU).Methods From February to April 2013,210 HCWs in an ICU were selected and randomly divided into three groups,group A dried hands with paper towel,group B with hand drier,and group C with personal towel,specimens from hands before hand-washing,after hand-washing,and after hand-drying were taken and performed detection.Results Hand microorganism count in group A,B and C before washing hands after contacting patients was (29.10±15.08)CFU/cm2 ,(31.42±14.76)CFU/cm2 and (30.36±15.52) CFU/cm2 respectively,the difference was not statistically different(F=0.048,P >0.05);After six-step hand-washing, hand microorganism count before hand drying in group A,B and C was (3.26 ±0.61 )CFU/cm2 ,(2.98 ±0.59) CFU/cm2 and (3.87±0.67)CFU/cm2 respectively,compared with hand microorganism count before hand-washing, the difference was statistically different(all P <0.01 ).After adopting different hand-drying methods,microorgan-ism count in three groups was statistically different(F =1 .892,P <0.05),group A ([1 .29±0.58]CFU/cm2 )was significantly lower than group B and C,group B ([9.51 ±0.73 ]CFU/cm2 )was significantly lower than group C ([22.76±4.11]CFU/cm2 );the qualified rate in group A (90.00%)was significantly higher than group B and C, group B (68.57%)was significantly higher than group C (47.14%).The top 5 pathogens isolated from HCWs’ hands were Pseudomonas aeruginosa ,Acinetobacter baumannii ,Klebsiella pneumoniae ,Escherichia coli ,and Coag-ulase negative Staphylococcus ,these strains were highly consistent with the top 5 multidrug-resistant organisms (MDROs)isolated from ICU patients in this hospital in 2013.Conclusion HCWs’hands are seriously contamina-ted after all kinds of medical performance;if hands are improperly dried,secondary contamination may occur;patho-gens isolated from HCWs’hands are highly consistent with MOROs from patients,timely and correct hand-washing and hand-drying is the key link to ensure the quality of hand hygiene,and is of great significance to reduce the occur-rence of MDROs infection in ICU patients.

8.Clinical study on repairing huge soft tissue defect in foot with a extensive reverse peroneal artery flap

Objective To introduce the clinical application of a extensive revense peroneal artery flap in repairing big soft tissue defect in foot, especially in repairing a big defect in distal foot and the exposure of one or more metatarsophalangeal joint. Methods The skin flaps were used in 11 cases to repair the big soft tissue defect in foot, including the distal part of foot. The flap upper boundary may surpass the capitulum fibulae; The lower boundary may reach the back line between external malleolus and internal malleolus; Lat-eral boundary could overrun anterior margin of fibula about 2 cm; Inboard boundary could get to medial mar-gin of gastrocnemius muscle. In attention: the two branch vessels from peroneal vessel in the site of 11-13 cm and of 5-7 cm above the outer anker would be included in the flap. Results Ten of 11 skin flaps survived satisfactory. A small, marginal portion of one flap in one case presented necrosis. Conclusion The reverse peroneal artery flap is easy to elevate and safety, which is an ideal flap for a huge soft tissue defect in foot, especial in the distal part of foot.
9.Skeletal muscle-derived stem cells exhibit cardiocyte competences.

Adult stem cells from skeletal muscle cells were induced to differentiate into cardiocytes to see if stem cells from another different but histologically-comparable tissues can differentiate to the target cells. Skeletal muscles-derived stem cells (MDSCs) were isolated from adult skeleton muscle tissues by differential adhesion, and immunocytochemically identified by using Sca-1. In order to induce the proliferation but not differentiation of MDSCs, the cells were cultured in Dulbecco's modified Eagle's medium/F12 (DMEM/F12) supplemented with 1:50 B27, 20 ng/mL basic fibroblast growth factor (bFGF), 20 ng/mL epidermal growth factor (EGF) in a suspension for 6 days. Then these stem cells were treated with 5 mumol/L 5-azacytidine for 24 h in an adherence culture. The characteristics of induced cells were examined by immunocytochemistry, quantitative real time RT-PCR and morphological observation of cell phenotype. Our results showed that the appearance of some cells gradually changed from spindle-shape into polygonal or short-column-shape. Some of these post-treated cells could contract spontaneously and rhythmically. The expression of GATA-4 and cTnT was increased 1 and 2 week(s) after the treatment. And about 16.6% of post-treated cells were cTnT-positive. Therefore, we are led to conclude that skeletal muscle-derived stem cells could differentiate into cardiocyte-like cells, which exhibited some characteristics of cardiocytes.
10.Flow-through deep inferior epigastric perforator flaps transplantation for reconstruction of large defects at the extremities.

OBJECTIVETo investigate the clinical application of flow-through deep inferior epigastric perforator flaps for reconstruction of large defects at the extremities.
METHODSThe deep inferior and superior epigastric arteries were designed as the axial vessel and the arterial supply to the flap was the paraumbilical perforator artery. Free deep inferior epigastric perforator flaps were harvested in flow- through manners to reconstruct associated arterial defect in the wound. The sensation assessment,Enneking score,and questionnaire of the flap aesthetic were all performed during follow-up period.
RESULTSFrom December 2011 to September 2012, 5 patients with large defects at extremities were treated. The deep inferior and superior epigastric arteries were designed as the axial vessel and the arterial supply to the flap was the paraumbilical perforator artery. The wound defects ranged form 11 cm x 5 cm to 30 cm x 11 cm. And the flap size ranged from 13 cm x7 cm to 33 cm x 13 cm. All flaps survived completely. The recipient arteries were all bypassed well documented by color Doppler examinations. All cases had 12-24 months' follow-up period. The flaps had good appearance and high aesthetic satisfactory(100%). 12 months after operations, sensation assessment were all S3+, and the Enneking score ratios were 82%-95% ,with 87.2% in average.
CONCLUSIONSFlow-through deep inferior epigastric perforator flaps are reliable and effective for reconstruction of large defects at the extremities with maintenance of the vascular status of the extremities. The flaps can also be designed in transverse or oblique mode for clinical application.
