OBJECTIVE: To explore the changes of pulmonary hemodynamic parameters in pneumoconiosis patients examined by echocardiography and its clinical significance. METHODS: Using simple random sampling method,102 male patients with pneumoconiosis were chosen and divided into simple pneumoconiosis group( 70 cases) and pneumoconiosis combined with chronic pulmonary heart disease( CPHD) group( 32 cases). Forty healthy male volunteers were chosen as the control group. The right heart morphological indexes including the diameters of main pulmonary artery( MPA),right ventricular outflow tract( RVOT),right ventricle( RV),the tricuspid regurgitation velocity and the time velocity integral of the right ventricular outflow tract were measured with echocardiography. The right atrial pressure was estimated and the pulmonary artery systolic pressure( PASP),the pulmonary vascular resistance( PVR) and the ratio of pulmonary artery acceleration time / right ventricular ejection time( PAAT / RVET) were calculated. RESULTS: The diameters of MPA,RVOT and RV in pneumoconiosis combined CPHD group were longer than those of the control group and simple pneumoconiosis group( P <0. 05). The RVOT diameter of simple pneumoconiosis group was longer than that of the control group( P < 0. 05). The indexes of PASP and PVR in pneumoconiosis combined CPHD group were higher than those of the control group and simple pneumoconiosis group, respectively( P < 0. 05). The indexes of PASP and PVR in simple pneumoconiosis group were higher than those of the control group( P < 0. 05). The indexes of PAAT / RVET in pneumoconiosis combined CPHD group and simple pneumoconiosis group were lower than that of the control group, respectively( P < 0. 05). There was no statistical significance difference found in MPA and RV diameters between simple pneumoconiosis group and the control group( P > 0. 05). CONCLUSION: Our results indicate that monitoring the pulmonary hemodynamic parameters of pneumoconiosis patients by using echocardiography can provide basis for planning early intervention measures in clinic.

OBJECTIVE: To explore the exercise capacity of patients with occupational pneumoconiosis with varying degrees of pulmonary dysfunction. METHODS: A total of 488 hospitalized occupational pneumoconiosis patients were selected as study subjects using the judgment sampling method and examined for pulmonary function test and cardiopulmonary exercise test(CPET). Among them, 272 patients with normal lung function were assigned as the control group, and 216 patients with abnormal lung function as the case group. The case group was divided into mild, moderate and severe pulmonary dysfunction subgroups according to the forced expiratory volume in one second/predicted value ratio(FEV_1%pred).RESULTS: The FEV_1%pred, maximal voluntary ventilation(MVV), maximum exercise tidal volume(VT_(max)), breathing reserve(BR), maximal Watt(W_(max)), maximum oxygen uptake(VO_(2max)) and anaerobic threshold(AT) in patients of the case group were lower than that in the control group(all P<0.05). The FEV_1%pred, MVV, VT_(max), W_(max), and VO_(2max) in patients in the 3 subgroups of abnormal lung function were decreased(all P<0.05) compared with the control group. The VO_(2max) and AT decreased in the case group with the increase of the degree of pulmonary dysfunction(P<0.05). The FEV1%pred, MVV, maximal exercise minute ventilation and VT_(max) of the study subjects were positively correlated with VO_(2max) and AT(all P<0.01), but the BR had no correlation with VO_(2max) and AT(all P>0.05). CONCLUSION: The more serious the abnormal degree of pulmonary function in the patients with occupational pneumoconiosis, the more obvious the decline of their exercise ability, showing a dose-effect relationship.

Chinese Medical Journal 2019;132(6):664-670
BACKGROUND:
Peritoneal fibrosis is the primary reason that patients with end-stage renal disease (ESRD) have to cease peritoneal dialysis. Peritonitis caused by Gram-negative bacteria such as Escherichia coli (E. coli) were on the rise. We had previously shown that matrine inhibited the formation of biofilm by E. coli. However, the role of matrine on the epithelial-mesenchymal transition (EMT) in peritoneal mesothelial cells under chronic inflammatory conditions is still unknown.
METHODS:
We cultured human peritoneal mesothelial cells (HPMCs) with lipopolysaccharide (LPS) to induce an environment that mimicked peritonitis and investigated whether matrine could inhibit LPS-induced EMT in these cells. In addition, we investigated the change in expression levels of the miR-29b and miR-129-5p.
RESULTS:
We found that 10 μg/ml of LPS induced EMT in HPMCs. Matrine inhibited LPS-induced EMT in HPMCs in a dose-dependent manner. We observed that treatment with matrine increased the expression of E-cadherin (F = 50.993, P < 0.01), and decreased the expression of alpha-smooth muscle actin (F = 32.913, P < 0.01). Furthermore, we found that LPS reduced the expression levels of miR-29b and miR-129-5P in HPMCs, while matrine promoted the expression levels of miR-29b and miR-129-5P.
CONCLUSIONS
Matrine could inhibit LPS-induced EMT in HPMCs and reverse LPS inhibited expressions of miR-29 b and miR-129-5P in HPMCs, ultimately reduce peritoneal fibrosis. These findings provide a potential theoretical basis for using matrine in the prevention and treatment of peritoneal fibrosis.

Aim To explore the mechanism by which total coumarins in Pileostegia tomentella (TCPT) inhibits the proliferation of small cell lung cancer (SCLC) H1688 cells via inducing ferroptosis. Methods The gradient concentrations of TCPT were used to treat H1688 cells. CCK-8 assay was applied for detection of proliferative inhibition of H1688 cells. Transmission electron microscopy was used to approach the morphological changes of H1688 cells under the treatment of TCPT. Additionally, dichlorofluorescein (DCFH-DA) probe was used to detect the intracellular reactive oxygen species (ROS) level. BODIPY 581/ 589 Cll probe was applied to examine the intracellular lipid peroxide formation. Western blotting was employed to detect the expression levels of glutathione peroxidase 4 (GPX4), kelch-like ECH-associated protein (KEAP1), nuclear factor E2 related factor 2 (NRF2), ferritin heavy chain 1 (FTH1) proteins in HI688 cells. Results The proliferation of small cell lung cancer cell H1688 was dramatically inhibited after TCPT intervention (P < 0. 05, P < 0. 01). The morphological characteristics of ferroptosis induced by TCPT were observed by transmission electron microscope. TCPT could also effectively elevate intracellular level of ROS and lipid peroxide. In HI688 cells the expression of ferroptosis markers GPX4, NRF2, and FTH1 was down-regulated, while the expression of KEAP1 was up-regulated, and there were statistically significant differences among the markers mentioned a-bove (P<0. 01). Conclusions Total coumarins in TCPT can significantly inhibit the proliferation of H1688 cells, possibly through increasing ROS and intracellular lipid peroxide levels and eventually inducing ferroptosis.

Journal of Environmental and Occupational Medicine 2024;41(3):330-334
Occupational exposure limits for hazardous agents in the workplace—Part 1: Chemical hazardous agents (GBZ 2.1—2019) has been implemented since April 1, 2020. The document redefines the evaluation of occupational exposure by introducing a "reduction factor" to adjust the occupational exposure limits of chemical hazardous agents for long working hour systems to ensure a protection level reasonable for the workers and equivalent to conventional occupational exposure limits. This paper discussed common problems encountered in the context of using this adjustment strategy, such as the adjustment being unable to cover all working hours, and the adjustment of values of terms associated with occupational exposure limits. Corresponding solutions were proposed. Working cycle and average exposure time of each working cycle were introduced to evaluate the actual working hours of workers. Regarding involved terms, adjustments of their corresponding values were clarified according to their definitions and application scenarios. Examples were provided for occupational health service providers so that their occupational health practices could be carried out in a more scientific and reasonable manner.

Journal of Clinical Hepatology 2019;35(3):530-534
ObjectiveTo investigate the value of Barcelona Clinic Liver Cancer (BCLC) staging system versus Hong Kong Liver Cancer (HKLC) staging system in predicting the prognosis of patients with hepatocellular carcinoma (HCC). MethodsA retrospective analysis was performed for the clinical data of 436 previously untreated patients with HCC who were admitted to The Affiliated Tumor Hospital of Guangxi Medical University from July 2008 to July 2013, and the staging score was determined according to the HKLC and BCLC staging systems. The Kaplan-Meier method was used to plot survival curves, and the log-rank test was used to compare the cumulative survival rate between the patients with different HKLC or BCLC stages. The Cox proportional hazards model was used to calculate the likelihood ratios (LR χ2) of the two systems. The area under the receiver operating characteristic curve (AUC) and C-index were used to analyze the discriminability and trend of the HKLC and BCLC staging systems. ResultsThere was a significant difference in survival rate between the patients with adjacent HKLC stages (all P<0.05), and there was also a significant difference in survival rate between all patients with adjacent BCLC stages (all P<0.001), except between those with stage 0 and stage A HCC. The Cox proportional hazards model showed that the HKLC system (LR χ2=131.14) had a better homogeneity than the BCLC system (LR χ2=100.69). The C-index of the BCLC and HKLC systems was 0.703 (95% confidence interval [CI]: 0.675-0.731) and 0.720 (95% CI: 0.692-0.748), respectively, and there was a significant difference in C-index between the two systems (P<0.001). There was a significant difference in the AUC of 1-year cumulative survival rate between the BCLC system and the HKLC system [0.783 (95% CI: 0.745-0.821) vs 0.811 (95% CI: 0774-0849), P=0.026]. ConclusionBoth BCLC and HKLC staging systems have a good value in predicting the prognosis of HCC patients, but the HKLC staging system has a higher value than the BCLC staging system.
